Optimizing surgical strategies for ruptured vertebral artery dissecting aneurysms: experience with perforator bypass and hyperacute management.

Ruptured vertebral artery dissecting aneurysms (rVADAs) are life-threatening lesions with a high risk of rebleeding and brainstem infarction. This study describes our institutional experience with open surgical management of rVADAs, with a focus on perforator bypass strategies. We retrospectively analyzed 17 patients with rVADAs treated surgically between 2012 and 2025. Clinical data, imaging characteristics, surgical details, and outcomes were reviewed. Special attention was given to cases with perforator involvement and the use of bypass reconstruction. Clinical outcomes were assessed using

Acute ischemic stroke in a twig-like middle cerebral artery: A case report and review of etiology and thrombectomy risks.

BACKGROUND: Aplastic or twig-like middle cerebral artery (Ap/T-MCA) is a rare congenital vascular anomaly characterized by the absence or hypoplasia of the M1 segment, replaced by a network of small tortuous collateral arteries. Although Ap/T-MCA predominantly presents with intracerebral hemorrhage from rupture of fragile vessels, acute ischemic stroke can occur, presenting with diagnostic and therapeutic challenges. CASE DESCRIPTION: A 79-year-old woman presented with acute right hemiplegia and aphasia. Initial imaging suggested left M1 segment occlusion of the MCA, prompting emergency throm

Septic embolus resulting in M1 occlusion with separate, multiple distal mycotic aneurysms, complicated by surrounding concurrent cerebral abscess: illustrative case.

BACKGROUND: Intracranial mycotic aneurysms are rare, representing only 0.7%-5.4% of all intracranial aneurysms, and typically arise from septic emboli secondary to infective endocarditis. Large-vessel occlusion due to a septic embolus is exceedingly uncommon and carries high morbidity. OBSERVATIONS: A 29-year-old man with methicillin-resistant Staphylococcus aureus endocarditis and intravenous drug use presented with intracerebral and subarachnoid hemorrhage. CT angiography revealed a left parieto-occipital hematoma with midline shift. An emergency craniotomy for hematoma evacuation exposed a

Endovascular treatment for symptomatic vasospasm following traumatic brain injury: illustrative case.

BACKGROUND: The optimal treatment for cerebral vasospasm (CV) following traumatic brain injury (TBI) has not been established, and management generally follows strategies developed for aneurysmal subarachnoid hemorrhage. OBSERVATIONS: The authors report a case of a patient with traumatic subarachnoid hemorrhage and cerebral contusion who presented with worsening consciousness and disorientation and subsequently developed symptomatic CV on day 6 of hospitalization. MRI and 3D CT demonstrated narrowing of the left internal carotid artery and middle cerebral artery (MCA) with reduced cerebral pe

Direct aspiration vs. stent retriever for middle cerebral artery M2 occlusion: a systematic review and meta-analysis.

Thrombectomy with direct aspiration (DA) or stent retriever (SR) are equally effective for endovascular treatment of proximal occlusions. However, their efficacy and safety in distal occlusions, such as those in the M2 segment of the middle cerebral artery, remain uncertain. Therefore, we aim to compare the efficacy and safety of these two approaches in M2 occlusions. This systematic review was registered in PROSPERO (CRD42025649001). We searched PubMed, EMBASE, Web of Science, and Cochrane Library for studies comparing DA to SR in patients with middle artery M2 occlusion until June 2025. The

First-in-Human Prospective, Observational, and Comparative Clinical Study of Simultaneous Invasive and Non-Invasive Intracranial Pressure Pulse Wave Monitoring.

Monitoring intracranial pressure (ICP) dynamics is critical for the management of traumatic brain injury, stroke, other neurosurgical conditions, and cerebral blood flow autoregulation; however, invasive ICP monitoring carries risks such as infection, hemorrhage, and sensor zero drift. Increasing evidence suggests that ICP waveform morphology provides clinically relevant information beyond mean ICP value alone. In this first-in-human prospective comparative clinical study, we evaluated the feasibility and accuracy of a novel, fully passive, non-invasive ICP pulse waveform monitoring system (Ar

Intraoperative ultrasound for localization of a ruptured M2-M3 middle cerebral artery aneurysm with temporal hematoma: A case report.

We present a patient who had a ruptured distal middle cerebral artery (MCA) aneurysm at the M2-M3 junction with a temporal lobe hematoma, in which intraoperative ultrasound (iUS) proved critical for real-time aneurysm localization when neuronavigation was compromised by brain shift. An elderly patient with subarachnoid hemorrhage and a large temporal lobe intracerebral hematoma underwent craniotomy, hematoma evacuation, and aneurysm clipping. Preoperative computed tomography angiography (CTA) identified a 5-mm aneurysm at the left M2-M3 junction. During surgery, neuronavigation was initially p

Management of Distal Anterior Cerebral Artery Aneurysms in a Tertiary Center in Pakistan: A Retrospective Analysis of Treatment Modalities and Angiographic and Short-Term Functional Outcomes.

Background Distal anterior cerebral artery (dACA) aneurysms are an uncommon subset of intracranial aneurysms and pose unique anatomical and technical challenges due to their deep midline location and complex vascular anatomy. While management outcomes are well described in high-resource settings, data from low- and middle-income countries (LMICs) remain limited, particularly in the context of treatment delays and resource-driven decision-making. Objective The objective of the study is to describe the treatment pathways, immediate angiographic occlusion rates, periprocedural complications, and

Convexity Subarachnoid Hemorrhage Due to Large Artery Atherosclerotic Stenosis or Occlusion: A Report of 3 Cases.

INTRODUCTION: Convexity subarachnoid hemorrhage (cSAH) is a rare subtype of nontraumatic subarachnoid hemorrhage. Large artery atherosclerotic stenosis or occlusion has emerged as a prevalent etiology in China. The diagnosis of cSAH is primarily based on imaging examinations. However, there are no established guidelines or consensus for its treatment. CASE REPORT: This report presented 3 cases of cSAH associated with atherosclerotic stenosis or occlusion. All 3 patients had significant large-artery stenosis or occlusion. In 2 cases (cases 1 and 3), computed tomography (CT) angiography reveale

Pulsatility Index Measurement via the Submandibular Internal Carotid Artery: An Alternative to Transcranial Assessment.

BACKGROUND/OBJECTIVE: Pulsatility index (PI) measurements using transcranial color flow imaging via the temporal bone are one of the clinical substitutes for invasive intracranial pressure (ICP) monitoring. Although the PI measurements obtained through this window are commonly used, they cannot be evaluated in up to 20% of the patients. Given that PI in the internal carotid artery (ICA) can be easily measured through the submandibular window in all cases, this study aimed to investigate whether PI measurements through the submandibular window could be used as a substitute for PI measurements i

Microsurgical clipping of an unusual progressive enlarging venous aneurysm associated with cerebral proliferative angiopathy.

BACKGROUND: Cerebral venous aneurysms are usually caused by abnormalities in the venous drainage system or alterations in the venous pressure, occurring in arteriovenous fistulas, developmental venous anomalies, arteriovenous malformations, and proliferative angiopathy. A rupture may represent a tragic event, since it may be associated with subarachnoid or intraparenchymal hemorrhage, edema and hydrocephalus, and hence, worsening of the quality of life. CASE DESCRIPTION: A previously healthy 31-year-old female presenting with a lowering of consciousness level, with investigation examinations

The predictive value of cerebral blood flow perfusion for cSAH following flow-restricting head and neck artery angioplasty.

OBJECTIVE: The pathogenesis of convexal subarachnoid haemorrhage (cSAH) remains unclear. Investigating the role of cerebral blood flow (CBF) perfusion in cSAH holds clinical value. METHODS: We retrospectively reviewed the clinical, laboratory, and preoperative computed tomography perfusion (CTP) or arterial spin labelling data of patients who underwent angioplasty for flow-restricting head and neck artery lesions at four centres (2021–2024). A standardised protocol was used for multi-slice region of interest analysis in watershed regions to calculate the affected/unaffected perfusion ratio. Fi

Intraprocedural Rupture During Coil Embolization of a Ruptured, Rapidly Thrombosing Anterior Cerebral Artery Aneurysm: A Case Report.

Acute thrombosis is often observed following the rupture of an intracranial aneurysm, and optimal coil selection during endovascular coiling is challenging. A 75-year-old woman presented with a subarachnoid hemorrhage. Computed tomography angiography revealed aneurysms at the right anterior cerebral artery A2/3 junction and the right middle cerebral artery. Based on vessel wall imaging, the anterior cerebral artery aneurysm was diagnosed as the rupture site. Initial digital subtraction angiography demonstrated a 4.4 × 3.4 × 3.4 mm aneurysm with a 1.6 mm neck. Coil embolization was performed 1

Vascular Ehlers-Danlos Syndrome Diagnosed after Cerebral Infarction with a Subarachnoid Hemorrhage.

Vascular Ehlers-Danlos syndrome (vEDS) is a rare genetic disorder characterized by either an arterial rupture or organ perforation, which is challenging to diagnose prior to the onset of major complications. A 46-year-old male presented with sudden severe headache and left hemiplegia. Magnetic resonance imaging (MRI) revealed simultaneous cerebral infarction and subarachnoid hemorrhage (SAH) due to middle cerebral artery (MCA) dissection. Genetic testing confirmed vEDS through identification of a pathogenic COL3A1 variant (c.2438G>A). Clinicians should consider vEDS in young patients demonstra
