Recurrence after improvement of cerebral vasospasm following aneurysmal subarachnoid hemorrhage: illustrative case.

BACKGROUND: Cerebral vasospasm (CVS) following aneurysmal subarachnoid hemorrhage (SAH) typically occurs 3-5 days after the onset of SAH, reaching its peak at day 7, and persists for 2-3 weeks. Generally, once the period of CVS is over and radiological improvement has been observed, recurrence is rare. OBSERVATIONS: The authors present the case of a 92-year-old female with a complaint of unconsciousness. CT showed diffuse SAH and CT angiography revealed a 9-mm right internal carotid-ophthalmic artery aneurysm. On day 1, they performed balloon-assisted coil embolization. On day 8, MR angiograp

Microscopic, stereological, and biochemical investigation of the effects of vitamin B12 on vascular changes in an experimental subarachnoid hemorrhage model in rats.

Spontaneous subarachnoid hemorrhage (SAH) is a severe neurological condition with significant morbidity and mortality. Cerebral vasospasm, a common complication of SAH, is a leading cause of delayed ischemic injury. Oxidative stress and inflammation are central to the pathophysiology of vasospasm. Vitamin B12, a water-soluble vitamin with antioxidant and anti-inflammatory properties, has shown potential neuroprotective effects in experimental models. This study aimed to investigate the effects of vitamin B12 on vascular changes, oxidative stress, and inflammatory markers in an experimental rat

Spontaneous Intracranial Vertebral Artery Dissection: A Rare Cause of Ischemic Stroke.

The dissection of the V4 vertebral artery (VA) is the most prevalent form of intracranial dissection, which can manifest either as ischemia or as a subarachnoid hemorrhage (SAH). Patient outcomes are significantly affected by their initial presentation; ischemic symptoms often indicate that the dissection remains primarily subintimal or within the medial layer, though it can occasionally extend to the basilar artery. In contrast, patients with ruptured VA dissection (VAD) experience a considerably higher mortality rate, as the dissection can reach the adventitial layer, heightening the risk of

Successful endovascular occlusion of multiple fusiform aneurysms on the persistent primitive lateral basilovertebral anastomosis.

BACKGROUND: The primitive lateral basilovertebral anastomosis (PLBA) is a transient embryonic vessel in the vertebrobasilar system that typically regresses during cerebellar artery development. Persistent PLBA (PPLBA), which forms a vertebrobasilar duplication, is a rare vascular anomaly. This is the first reported case of multiple fusiform aneurysms associated with a PPLBA. CASE DESCRIPTION: A 14-year-old girl was diagnosed with intracranial aneurysms before undergoing coronary artery bypass grafting surgery for myocardial infarction. Digital subtraction angiography showed two fusiform aneur

Ruptured Basilar Artery Perforator Aneurysm: Nightmare of a Treating Neurosurgeon.

Objectives Subarachnoid hemorrhage (SAH) resulting from rupture of basilar artery perforator aneurysm (BAPA) is a neurological rarity. With increased awareness and advancements in imaging modalities, they are now more frequently detected. However, concerns regarding their suboptimal treatment and lack of proper analysis still exist. We are hereby reporting our experience of treating this entity in a small cohort. Materials and Methods  It is a retrospective study of all the cases of SAH resulting from rupture of BAPA, which were treated by the same surgical team. Demographic data, treatment ch

Stenting for a Ruptured Blood Blister-Like Aneurysm of the Basilar Artery: Case Report and Literature Review.

Blood blister-like aneurysms (BBAs) usually occur on the supraclinoid portion of the internal carotid artery. There are a few reports of BBAs in other locations. We provide the case details of a ruptured BBA of the basilar artery (BA) that was treated with stenting, with a good outcome. The patient was a 74-year-old Japanese woman with a World Federation of Neurosurgical Societies (WFNS) grade I subarachnoid hemorrhage. Digital subtraction angiography (DSA) performed on day 14 revealed a BBA with a maximum diameter of 1.5 mm and a wide neck on the posterior wall of the BA's tip. Single stentin

Stent-assisted coiling using the pEGASUS-HPC stent for acutely ruptured wide-necked intracranial aneurysms: A multicenter retrospective study.

BackgroundStent-assisted coiling (SAC) is increasingly used to treat ruptured wide-necked intracranial aneurysms. The pEGASUS-HPC stent, featuring a hydrophilic polymer coating (HPC) to reduce thrombogenicity, may offer a safe option in subarachnoid hemorrhage (SAH). This study evaluates its safety and efficacy in a multicenter retrospective cohort.MethodsBetween July 2021 and June 2024, 22 patients with ruptured wide-necked aneurysms were treated with pEGASUS-HPC SAC at four neurovascular centers. Intracranial aneurysm location: potential influencing factor in ruptures with internal carotid artery occlusion.

BACKGROUND AND PURPOSE: Intracranial aneurysms combined with spontaneous internal carotid artery occlusion (ICAO) are a rare and serious vascular disorder. Currently, there is only limited information available on the clinical characteristics of these patients and the risk factors for aneurysm rupture. Our objective is to describe the clinical features of these patients and predict the risk factors for the rupture of unruptured intracranial aneurysms (UIAs) combined with ICAO. METHODS: We retrospectively analyzed cases of intracranial aneurysms with concurrent spontaneous ICAO from the Chines

Usefulness of Direct Auricular Artery Injection as Refinement of the Well-Established Rabbit Blood Shunt Subarachnoid Hemorrhage Model.

INTRODUCTION: Given the impact of aneurysmal subarachnoid hemorrhage (aSAH) on patients' health, preclinical research is substantial to understand its pathophysiology and improve treatment strategies, which necessitates reliable and comprehensive animal models. Traditionally, aSAH models utilize iliac or subclavian access for angiography, requiring invasive procedures that are associated with significant risks and animal burden. This pilot study explores a less invasive method of digital subtraction angiography (DSA) by using the auricular artery (AA) as an alternative access point. Our aim wa

Ruptured vertebrobasilar junction aneurysm coexisting with isolated interrupted aortic arch and basilar artery hypoplasia in an adult female: illustrative case.

BACKGROUND: Vertebrobasilar junction (VBJ) aneurysms are frequently associated with basilar artery variations like fenestration or hypoplasia, altering hemodynamics. An interrupted aortic arch (IAA), a rare congenital malformation, may contribute to intracranial aneurysms via vascular wall defects, hemodynamic stress, and compensatory hypertension. Coexistence of IAA with cerebrovascular anomalies and VBJ aneurysms is exceptionally rare, with no prior documented cases. OBSERVATIONS: A 33-year-old female presented with a ruptured VBJ saccular aneurysm (2.0 × 4.1 × 2.9 mm). Emergency right tran

Neurogenic Stunned Myocardium as a Complication of Aneurysmal Subarachnoid Hemorrhage: A Case Report and Lessons for Clinical Practice.

Neurogenic stunned myocardium (NSM) is an entity resulting from two factors: left ventricular dysfunction coupled with a neurological condition. In our case, the state of our patient complicated things; she was fatally suffering from a Hunt and Hess grade 2 subarachnoid hemorrhage (SAH), which is not a common scenario. A 62-year-old female patient presented to the emergency department with an intense headache, dysarthria, and left hemiparesis that was attributed to CT findings of a Fisher grade 2 SAH, and the computed tomography angiography (CTA) also showed evidence of a ruptured aneurysm at
