Ipsilateral hemiparesis caused by subarachnoid hemorrhage in a patient with a ruptured middle cerebral artery aneurysm: a case report.

The authors report a patient with a ruptured middle cerebral artery (MCA) aneurysm who presented with ipsilateral hemiparesis. A computed tomographic (CT) scan and cerebral angiograms suggested that the ipsilateral hemiparesis was most likely due to compression by hematoma of the secondary motor area in the island of Reil. Two other cases that demonstrated ipsilateral motor weakness after rupture of MCA aneurysms have been reported. We emphasize not only the importance of considering dysfunction of the secondary motor area in patients with hemiparesis, but also the difficulty in differentiatin

[Posttraumatic cerebrovascular narrowing. A case report and review of the literature].

We, neurosurgeons have sometimes confronted with the case of posttraumatic cerebrovascular narrowing or spasm. However, there have been few reports on the pathophysiology of the posttraumatic narrowing or spasm in comparison with those the spasm following the subarachnoid hemorrhage caused by the rupture of the aneurysm. The authors report here a case of temporal lobe contusion who demonstrated cerebrovascular narrowing angiographically at the 21st days after head injury. This 53-years-old male was examined again by angiography one week after the above-described angiography and the narrowing d

[Studies on recurrence of cerebral aneurysm after operation].

It seems to be well established that the direct surgical treatment of intracranial aneurysm such as clipping, trapping and wrapping the aneurysm. These operations have brought about excellent results but are not perfect because recurrent aneurysm and rebleeding of aneurysm have been reported shortly after the operation. However, it is very rare that subarachnoid hemorrhage arising from recurrent aneurysm long time after the operation, authors have experienced 3 cases of recurrent aneurysm in the same region long after the surgery. Two cases were readmitted to our hospital following repeat epis

An experimental study of acute subarachnoid haemorrhage in baboons: changes in cerebral blood volume, blood flow, electrical activity and water content.

Subarachnoid haemorrhage following transection of the posterior artery was produced in 10 baboons. Cerebral blood volume (CBV) decreased transiently after subarachnoid haemorrhage. Two basic patterns of intracranial pressure (ICP) were observed; in one ICP returned to normal but in the other it remained elevated. In this latter group four out of five animals showed an increase in CBV above the original level. There were delays in sensory conduction (measured using somatosensory evoked potentials) bilaterally; those on the contralateral side to the bleed were correlated with ICP whereas other f

Development of contralateral symmetric (mirror) aneurysm after a trapping procedure. An 18-year observation.

An 18-year-old male medical student presented with subarachnoid hemorrhage. Other than stiffness of the neck, the neurological examination yielded normal findings. Bloody cerebrospinal fluid was obtained by lumbar puncture. Bilateral carotid arteriography revealed a right internal carotid artery aneurysm at the level of the posterior communicating artery. The angiogram of the left carotid artery was entirely normal. A right trapping procedure followed. The patient was asymptomatic for 4 years, when a repeat subarachnoid hemorrhage occurred. A new (mirror) aneurysm on the opposite (left) intern

Quantitative determination of plasma fibrinolytic activity in patients with ruptured intracranial aneurysms who are receiving epsilon-aminocaproic acid: relationship of possible complications of therapy to the degree of fibrinolytic inhibition.

Fifty-two patients were each given a constant infusion of 1.5 g of epsilon-aminocaproic acid (EACA) per hour after subarachnoid hemorrhage (SAH) from an intracranial aneurysm. Each patient's available plasminogen activity (APA), a measure of plasma fibrinolytic activity, was determined by fluorometric assay before and during EACA treatment. Five categories of potential EACA complications were identified: rebleeding, cerebral vasospasm, hydrocephalus, thrombosis, and miscellaneous (bleeding time prolongation, thrombocytopenia). The APA of the 37 patients with complications was significantly hig

Anomalies of the posterior communicating artery and their potential clinical significance.

This study of the posterior communicating artery (PCoA) in 126 cranial cavities of adult cadavers revealed some hitherto unreported anomalies. These included: the origin of one PCoA from the ophthalmic artery inside the optic canal; the presence of three PCoA's on one side; the occurrence of two posterior cerebral arteries of different origin on one side; and the existence of junctional dilatations at both ends of a PCoA. Junctional dilatation at the commencement of the artery was seen in 6.3% of cases. More common anomalies were a fetal type of posterior cerebral artery in 31.7% and macroaneu

Anterior choroidal artery arteriovenous malformation. Its clinical manifestations and surgical treatment.

Four cases of arteriovenous malformation of the anterior choroidal artery are reported. Three cases showed the typical triad of the anterior choroidal syndrome, but in one case there was a subarachnoid hemorrhage without other symptoms. There were no surgical deaths after removal of the arteriovenous malformation. We have emphasized in this report that these arteriovenous malformations should be dealt with as a distinct entity from other paraventricular arteriovenous malformations because of their operability and characteristic clinical manifestations.

[Transcranial Doppler sonography in neurosurgery].

Using a 2 MHz transcranial Doppler sonographic system, it has become possible to measure flow velocities in the large basal vessels of the circle of Willis through thin areas of the cranial bone. After having used the method for one year on patients with neurovascular diseases, we now know that it can be employed as a matter of routine, after one has become familiar with it, and that it is of great diagnostic value. Knowing the measuring depth, achived by range gating, and with the aid of compression tests and standardised recording angles, it is possible to reliably identify the investigated

Variations between individuals in cerebrovascular responsiveness.

1. Despite extensive research no explanation has been put forward to account for the fact that cerebral arterial spasm complicates the course of disease of many but not all patients suffering from rupture of an intracranial aneurysm. Although vasoactive material in hemorrhagic cerebrospinal fluid (CSF) most probably is of major importance in the pathophysiology of delayed cerebral vasospasm, recent studies have failed to demonstrate a correlation between CSF vasoconstrictor activity and the development of delayed cerebral vasospasm. 2. In the present study the reactivity of isolated human pial

Large and giant paraclinoid aneurysms: surgical techniques, complications, and results.

Twenty-five patients with giant (greater than 25 mm in diameter) and 9 patients with large (15 to 25 mm in diameter) aneurysms of the internal carotid artery in the ophthalmic or paraophthalmic region are reviewed. In 23 of these patients the aneurysm was clipped directly. There was 1 death in this group, and none of the survivors had disabling neurological complications outside the visual system. The other 11 patients were treated by a trapping procedure or by either common carotid ligation or internal carotid ligation in the neck. Of the 5 patients treated by internal carotid ligation preced

Aneurysms of the posterior inferior cerebellar artery. A clinical and anatomical analysis.

The clinical and anatomical features of 21 surgically treated saccular aneurysms of the posterior inferior cerebellar artery (PICA) are analyzed. Seventeen of these lesions originated from the PICA-vertebral junction, and four arose from distal PICA branching sites. Twelve lesions arose from the left PICA, nine were right-sided, and all were small (less than 12.5 mm). Most of these aneurysms occurred in females (16 of 21) and presented as classic subarachnoid hemorrhage. The lack of specific focal deficits prevented an accurate pre-angiographic determination of aneurysm location in most instan

Dissecting aneurysm of intracranial vertebral artery: case report and review of literature.

A spontaneous dissecting aneurysm of the intracranial portion of the dominant right vertebral artery presented as massive subarachnoid hemorrhage, excruciating headache, and respiratory arrest in a 57-year-old white man with a history of systemic hypertension. He died on the 3rd day. Postmortem examination revealed a dissecting hemorrhage extending for 2.1 cm along the artery; rupture of the intima, media, and adventitia could be demonstrated. The intramural accumulation of blood in the proximal segments appeared to be related to retrograde dissection within a media weakened by cystic degenera

Internal carotid velocity measurements in children with cerebrovascular disease.

Internal carotid artery flow velocity was evaluated by the Doppler ultrasound method in 32 children with cerebrovascular disease. The average maximal blood velocity (A/L) and end-diastolic blood velocity (d) were compared with each other as indices of blood flow velocity. The right and left mean A/L and d of moyamoya disease were significantly lower than those observed in normal children. The A/L and d of the affected side both in internal carotid artery occlusion and in middle cerebral artery occlusion were also significantly low. In the subarachnoid hemorrhage after rupture, these values wer

A small animal model for electrocardiographic abnormalities observed after an experimental subarachnoid hemorrhage.

The male Sprague-Dawley rat was successfully used to produce increases in arterial blood pressure and electrocardiographic abnormalities similar to those seen in patients after a subarachnoid hemorrhage. The model employs a less expensive, easily available laboratory animal than other existing models. Whole heparinized blood is introduced in the area of the circle of Willis to simulate a subarachnoid hemorrhage. This model ensures a constant body-blood volume and minimizes damage to brain tissue as no blood vessel has to be punctured. The volume of blood required to produce bradycardia and oth

Anomalous branch of the internal carotid artery supplying circulation of the anterior cerebral artery. Case report.

Angiographic and operative investigations revealed an anomalous branch of the internal carotid artery (ICA) in a patient with an anterior communicating artery (ACoA) aneurysm. The anomalous vessel originated from the right ICA at the level of the ophthalmic artery, and pursued an infraoptic and prechiasmatic path to supply both pericallosal arteries. The clinical features and possible genesis of this anomaly are discussed. This irregularity is frequently associated with intracranial aneurysms, especially those of the ACoA, and with other anomalies.

An experimental study of the acute stage of subarachnoid hemorrhage.

A baboon model of subarachnoid hemorrhage (SAH) has been developed to study the changes in cerebral blood flow (CBF), intracranial pressure (ICP), and cerebral edema associated with the acute stage of SAH. In this model, hemorrhage was caused by avulsion of the posterior communicating artery via a periorbital approach, with the orbit sealed and ICP restored to normal before SAH was produced. Local CBF was measured in six sites in the two hemispheres, and ICP monitored by an implanted extradural transducer. Following sacrifice of the animal, the effect of the induced SAH on ICP, CBF, autoregula
