Clinical characteristics of hypertrophic olivary degeneration following brainstem or cerebellar hemorrhage.

Patients who have experienced bleeding in the posterior circulation of the brain often develop Hypertrophic Olivary Degeneration (HOD). This condition can lead to new neurological problems several months after the initial hemorrhage, potentially worsening the overall outcome for these patients. However, its pathogenesis and prognosis remain inconclusive. The research included 214 patients diagnosed with brainstem or cerebellar hemorrhage, of which 36 developed secondary HOD. The study aimed to analyze the clinical data of these patients, investigate the risk factors associated with HOD develop

Salvage medullary perforating artery revascularization using the muscular branch of the occipital artery following occipital artery-posterior inferior cerebellar artery bypass for trapping a ruptured vertebral artery dissecting aneurysm.

Vertebral artery dissecting aneurysms (VADAs) presenting with subarachnoid hemorrhage (SAH) are relatively rare. The re-rupture and mortality rates during the acute phase of SAH remain significantly high. Endovascular coil trapping is one of the definitive treatments to prevent re-rupture; however, it carries a potential risk of medullary infarction derived from occlusion of the medullary perforating arteries (Mperf). Here we present a case with poor-grade SAH resulting from a ruptured left VADA affecting the posterior inferior cerebellar artery (PICA). After obtaining informed consent, open s

Trends in Incidence of Intracerebral Hemorrhage and Its Bleeding Sites from 1985 to 2017 in Japanese Rural Communities.

INTRODUCTION: Evaluating long-term trends for the incidence of intracerebral hemorrhage is a priority for primary prevention. It is also important to assess the trends in the proportions of bleeding sites because the pathogenesis, prognosis, and operative procedures differ among them. METHODS: A prospective community-wide stroke registry in two rural Japanese communities (Ikawa Town and Kyowa Town, with populations of approximately 5,000 and 15,000, respectively) was conducted. The age-adjusted incidence of intracerebral hemorrhage from 1985 to 2017 was calculated by the direct method using t

Symptomatic Porencephalic Cyst in the Chronic Postoperative Phase Following Evacuation of Cerebellar Hemorrhage: A Case Report.

Porencephalic cysts are typically observed in fetal or pediatric patients and are rarely reported in adults. While some cases have been reported in the acute or subacute phase following intracerebral hemorrhage, delayed formation during the chronic phase is exceedingly uncommon, particularly in the posterior fossa. We report the case of a 53-year-old woman who developed progressive ataxia and dysarthria eight months after surgical evacuation of a cerebellar hemorrhage. Imaging revealed a 55-mm CSF-filled cyst in the left cerebellum. She underwent cyst wall fenestration and opening of the ciste

Perinatal Neuroprotection in Preterm Birth.

Preterm birth is one of the main causes of perinatal morbidity and mortality. The rate of grade III and IV cerebral hemorrhages in infants with a birth weight of less than 1500 g in Germany in 2022 was 2.97% and the periventricular leukomalacia rate was 1.07%. In addition to these severe forms of brain damage which are visible on sonography, recent MRI studies carried out at the calculated due date of affected children also showed diffuse white and grey matter injuries, especially of the basal ganglia and the cerebellum, indicating impaired brain development and function. To offer these childr

Dual Left-Sided Posterior Inferior Cerebellar Artery Aneurysms Presenting with Subarachnoid Hemorrhage, Intraventricular Hemorrhage and Hydrocephalus: A Rare Case Report and Systematic Review.

BACKGROUND: Aneurysms of the posterior inferior cerebellar artery (PICA) are among the least common intracranial aneurysms, accounting for approximately 0.5% to 3% of cases. Those arising distally are even rarer and are known to rupture at smaller sizes. Their rupture frequently results in subarachnoid hemorrhage, intraventricular hemorrhage (IVH), and hydrocephalus, posing considerable treatment challenges due to the surrounding vital brain structures. CASE PRESENTATION: A 62-year-old woman presented to the emergency department with a sudden severe headache, nausea, vomiting, and unconscious

Analysis of Location-Specific Volumetric Cutoffs in Intraparenchymal Hemorrhage: A Case Series of 94 Patients.

BACKGROUND: Accurate prognostic assessment is essential for guiding management in intracerebral hemorrhage (ICH). While hematoma volume and location are key determinants, recent evidence highlights the need for a more precise topographic evaluation. We investigated whether newly proposed location-specific volumetric cutoffs improve the prediction of 6-month functional outcome, assessed with the modified Rankin Scale, and 30-day mortality. METHODS: We retrospectively analyzed 94 patients with spontaneous ICH, performing semi-automated hematoma volume segmentation and categorizing its topograph

A Pediatric Case Report of Acute Torticollis Secondary to Atraumatic Cerebellar Hemorrhage.

INTRODUCTION: There exists a wide differential of etiologies for pediatric torticollis that extends beyond musculoskeletal factors. CASE REPORT: We present a novel case of a pediatric patient with an acute atraumatic hemorrhage of the left cerebellum presenting with gradual worsening torticollis. Upon further diagnostic workup, he was found to have an intracerebral hemorrhage due to a cerebellar cavernous malformation. Although the hemorrhage boundaries were extensive, the patient had only exhibited transient dysmetria and facial weakness, with ultimate resolution of torticollis and these neu

Double origin of the superior cerebellar artery from the Basilar artery and the P1 segment of the posterior cerebral artery.

A 50-year-old patient admitted for a subarachnoid hemorrhage underwent a successful surgical clipping of two aneurysms involving the anterior communicating artery and the supraclinoïd and ophthalmic segments of the right internal carotid artery. The analysis of the vertebrobasilar system through the brain computed tomography angiography and 3D modeling revealed a double origin of the right superior cerebellar artery from the basilar artery and the posterior cerebral artery. Two channels arose separately from the basilar artery and the P1 segment of the right posterior cerebral artery then merg

Combination therapy with 3-Hydroxybenzaldehyde and Albendazole modulates mitochondrial protein expression in astrocytes after Angiostrongylus cantonensis infection.

BACKGROUND: Angiostrongylus cantonensis is a zoonotic nematode that causes eosinophilic meningitis and central nervous system injury in humans; 3-hydroxybenzaldehyde (3-HBA) is a benzaldehyde compound that exhibits antioxidant and anti-inflammatory activities. Brain injury promotes Ca²⁺ influx and mitochondrial Ca²⁺ loading via voltage-dependent anion channel 1 (VDAC1) and the mitochondrial Ca²⁺ uniporter (MCU), leading to mitochondrial dysfunction and cytochrome c-mediated apoptosis. METHODOLOGY/PRINCIPAL FINDINGS: This study aimed to evaluate the therapeutic effects of 3-HBA combined with a

The Mechanism of Astragaloside IV in NOD-like Receptor Family Pyrin Domain Containing 3 Inflammasome-mediated Pyroptosis after Intracerebral Hemorrhage.

BACKGROUND: Intracerebral hemorrhage (ICH) is one of the most common subtypes of stroke. OBJECTIVES: This study aimed to investigate the mechanism of Astragaloside IV (AS-IV) on inflammatory injury after ICH. METHODS: The ICH model was established by the injection of collagenase and treated with ASIV (20 mg/kg or 40 mg/kg). The neurological function, water content of the bilateral cerebral hemisphere and cerebellum, and pathological changes in brain tissue were assessed. The levels of interleukin-1 beta (IL-1β), IL-18, tumor necrosis factor-alpha, interferon-gamma, and IL-10 were detected by

Assessment of brain structure and volume reveals neurodevelopmental abnormalities in preterm infants with low-grade intraventricular hemorrhage.

There is increasing evidence of abnormal neurodevelopmental outcomes in preterm infants with low-grade intraventricular hemorrhage (IVH). The purpose of the study was to explore whether brain microstructure and volume are associated with neuro-behavioral outcomes at 40 weeks corrected gestational age in preterm infants with low-grade IVH. MR imaging at term-equivalent age (TEA) was performed in 25 preterm infants with mild IVH (Papile grading I/II) and 40 control subjects without IVH. These subjects all had neonatal behavioral neurological assessment (NBNA) at 40 weeks' corrected age. Microstr

A Case of Idiopathic Intracranial Hypertension Complicated with both Infratentorial and Supratentorial Cortical Superficial Siderosis: Novel Imaging Findings on Intravoxel Incoherent Motion Magnetic Resonance Imaging Offering Clues to Pathophysiology.

The pathology of idiopathic intracranial hypertension (IIH), a disease characterized by papillary edema and increased intracranial pressure (IICP), is not yet understood; this disease significantly affects quality of life due to symptoms including vision loss, headache, and pulsatile tinnitus. By contrast, superficial siderosis (SS), a disorder in which hemosiderin is deposited on the surface of the cerebral cortex and cerebellum, potentially causes cerebellar ataxia or hearing loss. So far, no cases of IIH with infratentorial and supratentorial cortical SS have been reported. Herein, we repor

Associations of Microbleeds and Their Topography With Imaging and CSF Biomarkers of Alzheimer Pathology in Individuals With Down Syndrome.

BACKGROUND AND OBJECTIVES: Cerebral hemorrhages are an exclusion criterion and potential adverse effect of antiamyloid agents. It is, therefore, critical to characterize the natural history of cerebral microbleeds in populations genetically predisposed to Alzheimer disease (AD), such as Down syndrome (DS). We aimed to assess microbleed emergence in adults with DS across the AD spectrum, defining their topography and associations with clinical variables, cognitive outcomes, and fluid and neuroimaging biomarkers. METHODS: This cross-sectional study included participants aged 18 years or older f

Prenatal Diagnosis of Poretti-Boltshauser Syndrome - a Case Report of a Molar Tooth Sign Mimic.

We report the prenatal diagnosis of Poretti-Boltshauser Syndrome (PBS) in a 36-year-old primigravida woman. At 22 weeks and 6 days of gestation, fetal ultrasound revealed a normally shaped but hyperechogenic cerebellum with all supratentorial structures appearing normal. Differential diagnosis included cavernous hemangioma, capillary telangiectasia, and cerebellar hemorrhage. Subsequent fetal cerebral MRI showed diffuse bilateral cerebellar modifications, reduced cranio-caudal diameter of the vermis, and pathological elongation and thickening of the superior cerebellar peduncles indicative of

Utility of Diffusion Tensor Imaging in Preterm Infants with Germinal Matrix Hemorrhage and Intraventricular Hemorrhage: A Systematic Review.

OBJECTIVE: Intraventricular hemorrhage (IVH) and germinal matrix hemorrhage (GMH) are the most common brain injuries in preterm infants. Neonates with these injuries are at greater risk of impaired neurodevelopmental outcome. Current guidelines recommend screening infants with cranial ultrasound (CUS); however, this is prone to missing subtle injury patterns, particularly within the posterior fossa. The present report sought to discuss the utility of diffusion tensor imaging (DTI) in preterm infants. METHODS: A systematic review of PubMed, Ovid MEDLINE, and Ovid EMBASE was conducted following
