Traumatic basilar artery incarceration following longitudinal clival fracture: illustrative case.

BACKGROUND: Clival fractures are rare injuries that occur following high-energy blunt trauma. Longitudinal clival fractures can lead to incarceration of the basilar artery within the bone fragments, often leading to stenosis or occlusion of the basilar artery, subsequently causing vascular compromise, significant ischemic events, and neurological deficits. The authors present a case of basilar artery incarceration following craniofacial trauma and review the relevant literature. OBSERVATIONS: The reported case is of a 37-year-old male who presented following a 20-foot fall. CT angiography rev

Dynamic vertebral artery recanalization as a cause for recurrent posterior circulation thromboembolic stroke: illustrative case.

BACKGROUND: Posterior circulation strokes comprise one-fourth of ischemic strokes and can arise from many causes. One rare etiology is transient occlusion of the vertebral artery (VA) with head rotation due to an osteophyte, termed bow hunter's syndrome. The authors present the case of recurrent thromboembolic posterior strokes secondary to transient reopening of an osteophyte-occluded VA. OBSERVATIONS: A 71-year-old man presented with dizziness and visual changes with a history of prior cervical fusion and myocardial ischemia, controlled with Plavix and aspirin. CT angiography of the neck re

Intravenous thrombolysis versus endovascular thrombectomy in acute basilar artery occlusion-A multicenter cohort study.

BACKGROUND: Randomized controlled trials have demonstrated an improved outcome of basilar artery occlusion (BAO) with endovascular thrombectomy (EVT) compared to best medical treatment. However, a minority of the patients recruited up to 12-24 h from onset in the positive trials received intravenous thrombolysis (IVT), and a trial with a higher IVT rate did not show superiority of EVT. Thus, the efficacy and safety of EVT compared to IVT for BAO remain less clear. AIMS: We aimed to compare outcomes after IVT alone to EVT with or without IVT for acute BAO. METHODS: This international, observa

Endovascular recanalization of symptomatic long-segment nonacute vertebral artery occlusion: a case report.

BACKGROUND: Vertebral artery occlusive disease is a leading cause of posterior circulation ischemic stroke. Endovascular recanalization has been described as a viable treatment option for individuals with symptomatic vertebral artery occlusion who fail to respond to appropriate medicinal therapy. However, endovascular recanalization of symptomatic long-segment nonacute vertebral artery blockage is associated with significant surgical risk and the possibility of perioperative complications. CASE PRESENTATION: Despite rigorous pharmacologic therapy, a patient with a protracted segmental left ve

Impact of Etiology, Thrombectomy Techniques, and Lesion Localization on Endovascular Treatment Success in Acute Basilar Artery Occlusion: A Single-Center Retrospective Study.

BACKGROUND: Basilar artery occlusion (BAO) is a rare but severe form of large vessel occlusion associated with high rates of mortality and morbidity. Endovascular treatment (EVT) has become the primary therapeutic option; however, its efficacy and procedural risks in BAO remain controversial. This study aims to evaluate the clinical and procedural outcomes of EVT in BAO and to compare these results across different etiological subgroups. METHODS: We retrospectively analyzed 23 patients who underwent EVT for BAO between December 2019 and December 2024. Demographic data, procedural times, recan

Comparison of endoscopic third ventriculostomy with stereotactic prepontine stenting in patients with aqueductal stenosis.

OBJECTIVE: The standard treatment for noncommunicating hydrocephalus is endoscopic third ventriculostomy (ETV). An alternative procedure is the stereotactic implantation of a shunt catheter through the ventricles into the prepontine cistern, which serves as a stent (STS). This procedure may reduce the risk of stoma occlusion. The aim of this study was to compare the surgical and clinical results of both procedures. METHODS: Patients with aqueductal stenosis treated by either ETV or STS from January 2013 to July 2024 were included in this single-center retrospective study. Patient records were

Improving "No-Reflow" After Complete Reperfusion: The Role of Intravenous Thrombolysis in Vertebrobasilar Artery Occlusion Patients Undergoing Endovascular Treatment.

BACKGROUND: Previous studies have found that the use of intravenous thrombolysis (IVT) before endovascular treatment (EVT) could mitigate the "no-reflow" phenomenon in large-vessel occlusion of the anterior circulation. However, the effect of preoperative IVT on reducing the "no-reflow" phenomenon in vertebrobasilar artery occlusion (VBAO) is still uncertain. This study aimed to compare the outcomes of IVT before EVT versus EVT alone in patients with VBAO with complete reperfusion (mTICI [Modified Thrombolysis in Cerebral Infarction] 3).
