Bilateral medial frontal infarction in a case of azygous anterior cerebral artery stenosis.

We describe the unusual case of a 63-year-old woman with a history of arterial hypertension who presented a sudden weakness of the lower limbs followed by mutism, akinesia and dyspraxia. Magnetic resonance images showed a bilateral medial frontal infarction. Digital subtraction angiography documented a right azygous anterior cerebral artery with severe stenosis in its sub-callosal tract; the left anterior cerebral artery showed mild hypoplasia with only sub-frontal and fronto-polar branches. No embolic source was documented. Afterwards the patient presented a gradual and partial recovery of bo

Natural history of stenosis from intracranial atherosclerosis by serial angiography.

BACKGROUND AND PURPOSE: Knowledge of the natural history of stenoses due to intracranial atherosclerosis may be useful for evaluating possible treatments such as angioplasty. METHODS: We retrospectively reviewed records over a 7-year period to identify patients with intracranial atherosclerotic stenoses and serial angiograms. Quantitative measurements of stenoses were made in a blinded manner, and clinical outcomes were reviewed. RESULTS: We identified 21 patients with 45 intracranial stenoses who underwent repeat angiography at an average interval of 26.7 months. The average stenosis for al

Relationship between short-term outcome and occurrence of cerebral artery stenosis in survivors of bacterial meningitis.

To evaluate the influence of cerebral artery stenosis on the outcome of patients with bacterial meningitis we examined prospectively 47 consecutive patients [33 men, 14 women, mean (SD) age, 53 (17) years, range 18-81] with bacterial meningitis caused by various bacterial pathogens. The patients were examined with the use of the Glasgow Coma Scale (GCS) on days 1, 3, 5, 8, 14 and with the use of the Glasgow Outcome Scale (GOS) on day 21 after admission. In addition, focal cerebral signs were recorded separately. At each clinical examination, the patients underwent transcranial Doppler sonograp

Symptomatic haemodynamically significant carotid occlusion treated by posterior circulation revascularization.

Haemodynamic cerebral ischaemic symptoms secondary to occlusive atheromatous disease of the internal carotid artery may resolve with revascularization. We describe a case of symptomatic internal carotid artery occlusion and ipsilateral subclavian stenosis with duplex and angiographically demonstrated vertebrobasilar steal and a patent ipsilateral posterior communicating artery with flow towards the steal. The patient suffered frequent ipsilateral middle cerebral artery territory transient ischaemic attacks and one minor stroke. Transcranial Doppler demonstrated markedly impaired ipsilateral ca

Hemodynamic effects of middle cerebral artery stenosis and occlusion.

BACKGROUND AND PURPOSE: Middle cerebral artery (MCA) stenosis and occlusion may cause ischemic symptoms through both hemodynamic and embolic mechanisms. The purpose of this investigation was to determine the hemodynamic effects of these lesions. METHODS: Ten patients with angiographically confirmed symptomatic occlusion (n = 5) or stenosis (n = 5) of the M1 segment of the MCA were studied by clinical examination, arteriography, and positron emission tomography (PET). Arterial supply to the distal MCA territory was classified from a review of the angiogram as being through the stenosis or from

Cerebral revascularization using muscle free flap for ischemic cerebrovascular disease in adult patients.

The efficacy of encephalo-myo-synangiosis (EMS) using muscle free flap was evaluated for the treatment of ischemic cerebrovascular disease in adult patients. Three patients with adult moyamoya disease and three patients with atherosclerotic ischemic cerebrovascular disease were treated. EMS used four latissimus dorsi muscles and two serratus anterior muscles. Postoperative selective angiography showed collateral circulation from the transferred muscle to the brain in four of the six patients. The other two patients showed patent nutrient artery of the transferred muscle flap. Cerebral blood fl

Stroke risk factors and development of collateral flow in carotid occlusive disease.

BACKGROUND AND PURPOSE: Blood flow through collateral vessels compensates for reduced blood flow through stenotic or occluded extracranial carotid arteries. Previous studies have shown that extent of collateral flow influences likelihood of stroke and its outcome. Here we analyzed the relationship between stroke risk factors (hypertension, coronary artery disease, diabetes mellitus, tobacco smoking and hypercholesterolemia) and number of patent intracranial collaterals detected by transcranial Doppler ultrasonography. SUBJECTS AND METHODS: We studied 182 patients with various degrees of angio

Carotid disease in acute stroke.

BACKGROUND: the Oxfordshire Community Stroke Project (OCSP) devised a simple clinical classification for acute stroke which predicted mortality, functional recovery and patterns of recurrent stroke. We aimed to determine whether this could predict the presence of carotid disease and be used to select which patients with acute stroke should be referred for carotid imaging with a view to subsequent carotid endarterectomy. METHODS: we assessed patients with acute stroke admitted to seven hospitals over a 10-month period. Patients were classified according to the OCSP system and their carotid art

Ischemic optic neuropathy associated with internal carotid artery dissection.

BACKGROUND: Ischemic optic neuropathy (ION) is an infarction of the anterior or, less frequently, posterior part of the optic nerve, usually due to a disease of small arteries supplying the optic nerve. Carotid stenosis or occlusions are rare causes, and among them, carotid dissections have been so far reported in only 5 cases. METHODS: We describe 4 patients with ION (2 anterior and 2 posterior) due to internal carotid artery dissection of a consecutive series of 110 patients with internal carotid artery dissection (3.6%). RESULTS: None of the patients had signs of central retinal artery oc

Percutaneous transluminal angioplasty of intracranial artery stenosis: clinical results in 24 patients.

Circumscribed stenotic lesions of the intracranial arteries can cause cerebral ischemia by hemodynamic and/or thromboembolic mechanisms. Anticoagulation therapy, antiplatelet therapy, and bypass surgery are treatment strategies that have no direct impact on the underlying lesion. This study summarizes the experience of a single institution at which percutaneous transluminal angioplasty (PTA) of intracranial atherosclerotic stenoses was performed. The authors performed a retrospective analysis of 24 consecutive patients. Their medical histories (cardiovascular risk factors, current clinical sig

Transcranial Doppler ultrasound battery reliably identifies severe internal carotid artery stenosis.

BACKGROUND AND PURPOSE: There is a clinical imperative for noninvasive tests for carotid disease that have high sensitivity. Previous studies have shown that transcranial Doppler ultrasound (TCD) can identify intracranial collateral flow patterns and other hemodynamic consequences of carotid occlusion. We hypothesized that a battery of such TCD findings would have a greater sensitivity than any one TCD finding alone and would have clinical utility in identifying carotid disease. METHODS: We determined the prevalence of seven TCD findings in patients with various degrees of carotid stenosis as

Prevalence and time course of microembolic signals in patients with acute stroke. A prospective study.

BACKGROUND AND PURPOSE: Cerebral emboli can be identified by the presence of typical microembolic signals (MES) in transcranial Doppler (TCD) spectral curves. The usefulness of this technique was studied by evaluating the prevalence and time course of MES in patients with acute stroke. In addition, we examined the influence of anticoagulation therapy on the occurrence of MES. Another study objective was to identify the value of MES in elucidation of the underlying pathology of cerebral ischemia in patients with acute stroke. METHODS: We used bilateral TCD monitoring of the middle cerebral art

Pilot study of carotid surgery for acute stroke.

BACKGROUND: Carotid endarterectomy is usually delayed for 2 months following an acute stroke, but the stroke may progress or a further stroke may occur. A randomized pilot study of urgent carotid surgery for acute stroke was undertaken to assess the feasibility of a definitive multicentre trial. METHODS: Seven Manchester hospitals referred patients with acute stroke (less than 7 days previously), defined according to the Oxfordshire Community Stroke Project classification The carotid arteries were examined by portable continuous-wave Doppler ultrasonography. Revascularization of the anterior cerebral artery using a free superficial temporal artery graft: a case report.

BACKGROUND: Various bypass procedures have been used to treat occlusive cerebrovascular diseases. Most procedures were performed to prevent further ischemia in the territory of the middle cerebral artery, while only a few of the papers discuss the anterior cerebral artery (ACA) territory. METHOD AND RESULTS: A patient who initially presented with several episodes of transient monoparesis in the left leg was found to have severe stenosis of the right A2-A3 junction. A free superficial temporal artery (STA) graft was anastomosed between the right A2 and A3 portion intracranially. Transient isch

[A case of spontaneous middle cerebral artery occlusion associated with a cerebral aneurysm angiographically disappearing after STA-MCA anastomosis].

We report a case of a rapidly growing cerebral aneurysm in the basal abnormal vascular network associated with spontaneous middle cerebral artery (MCA) occlusion. The aneurysm disappeared spontaneously shortly after performing STA-MCA anastomosis. A 54-year-old female was admitted to our hospital because of repeated attacks of right hemisensory disturbance and dysarthria. CT scan and MRI images showed the infarcted focus in the left parieto-occipital lobe. Bilateral MCAs were undetectable on MRI images. Cerebral angiography revealed that the bilateral MCAs were occluded in their proximal origi

[A case of multiple cerebral arterial thrombosis due to congenital protein C deficiency].

We report a 49-year-old man who had right hemiparesis and motor aphasia. A computed tomography revealed hypodense areas in the left frontal subcortex. A cerebral angiography demonstrated occlusion of the left distal internal carotid artery and both anterior cerebral arteries, as well as stenosis of the left internal carotid artery at the cervical portion. The second angiogram obtained a month later showed no changes. The diagnosis of atherothrombotic cerebral infarction was established on the basis of clinical profile and angiographic findings. Cerebral hemispheric low-flow infarcts in arterial occlusive disease. Lesion patterns and angiomorphological conditions.

BACKGROUND AND PURPOSE: Among the pathogenetic causes of subcortical hemispheric infarcts are small-vessel disease, thromboembolic occlusions of small arteries, and hemodynamic compromise in low-flow conditions. A topographic classification of these infarcts based on CT and MRI can be misleading. METHODS: We evaluated 30 consecutive patients with presumed supratentorial low-flow infarcts. CT was available in all cases, with additional MRI in 14 patients. In all cases the occlusion pattern of the extracranial and intracranial arterial system was studied in detail with angiography. RESULTS: Th
