Rupture of an experimentally induced aneurysm in a primate.

Cerebral aneurysms are extremely rare in all animals except man. We report the occurrence of a carotid-ophthalmic aneurysm in a cynomolgous monkey two months following accidental occlusion of the ipsilateral cervical internal carotid artery. This was documented angiographically. The aneurysm was deliberately ruptured by the insertion and withdrawal of fine needles. This produced a large subarachnoid hemorrhage visualized on CT scan. There was a progressive severe fall in cerebral blood flow and moderate vasospasm. The neurological status deteriorated on the tenth day and the animal was sacrifi

Cerebrovascular Moyamoya disease.

Clinical features of ten Japanese children with cerebrovascular Moyamoya disease are reported and non-Japanese paediatric cases of the disease are reviewed from the world literature. The most common initial manifestations were headache in four cases (40%), motor deficit and convulsion in three cases (30%). As their recurrent and/or residual symptoms, eight children (80%) developed motor deficit consisting of hemiplegia in five cases and paresis or weakness of the extremities in three cases, and four (40%) had headaches. The mode of presentation in our cases was similar to that of non-Japanese

[Multiple cerebrovascular occlusive disease associated with neurofibromatosis].

Multiple cerebrovascular occlusive disease is rarely seen in patients with neurofibromatosis. Two cases of such lesions are presented and literatures dealing with the clinical and angiographical aspects of this occlusive disease are reviewed. Case 1; A 38-year-old normotensive man had sudden onset of vomiting, left hemiparesis and disturbance of consciousness, one day before the admission. He had family history of neurofibromatosis, and examination showed café au lait spots over the body. CT scans revealed a subcortical hematoma in the right temporal lobe. Angiogram revealed multiple occlusive

Short vein grafts for cerebral revascularization.

Experience with the use of a short saphenous vein bypass graft for cerebral revascularization is reviewed. Twenty patients with symptomatic atherosclerotic occlusive disease underwent a total of 21 bypass procedures. Cerebral revascularization was performed using a short (5 to 10 cm) saphenous vein graft (SVG) extending from the superficial temporal artery (STA) trunk anterior to the ear in 19 bypasses, or from the occipital artery (OA) immediately behind the mastoid process to the posterior temporal or angular branch of the middle cerebral artery (MCA) in two bypasses. The early patency rate

[Limits of preventive extra-intracranial anastomoses. A propos of a meningioma of the clinoid process with stenosis of the internal carotid artery].

In the case reported here a voluminous meningioma attached to the anterior clinoid process had invaded the cavernous sinus, involving and constricting the internal carotid artery and its terminal branches. To prevent occlusion of the carotid-sylvian system during surgery, an extra-intracranial anastomosis was carried out and its permeability was controlled and confirmed. However, excision of the meningioma, which entailed resection of the carotid segment, resulted in hemiplegia. The authors emphasize the importance of haemodynamic and anatomical factors in such cases.

[Aneurysm of anterior communicating artery associated with aortitis syndrome--a treated case by aneurysmal neck clipping (author's transl)].

A rare case of the cerebral aneurysm associated with aortitis syndrome was reported. It seems to be the first case in which neck clipping was successfully performed for aneurysm of the anterior communicating artery. The patient was a 48-year-old female afflicted with pulseless disease. She was admitted to Iwate Central Hospital with severe headache, vomiting and unconsciousness on November 16, 1980. Her radial pulse was faint on the right side. The left carotid angiography revealed an aneurysm in the anterior communicating artery, and neck clipping of the aneurysm was performed 17 days after t

[Angiographic assessment of collateral circulation pathways in the stenosis and occlusion of the extracranial cerebral arteries].

Visualization in angiographic investigations of the common carotid artery of certain collateral circulation routes can sometimes facilitate evaluation of the site of occlusion and its extent. Collateral circulation via the arterial network of the nape of the neck indicates stenosis or occlusion of the subclavian artery. Collateral circulation through the muscular branches of the occipital artery to the vertebral and basal arteries suggests lesions in the vertebral artery. Collateral circulation via the anterior communicating artery suggests stenosis or occlusion of the contralateral internal c

Cerebral infarction secondary to sickle cell disease: arteriographic findings.

Cerebral angiograms were performed in 14 children with sickle cell disease and clinical findings of cerebral infarction. The angiogram was normal in four patients. Ten patients had abnormalities in the carotid artery or its branches. The internal carotid artery was stenosed or occluded usually with bilateral disease. The anterior and middle cerebral arteries were involved in nine patients and branch occlusion was frequent. The posterior fossa arteries were not involved in these patients. The risk of angiography is low if appropriate precautions are taken. In these patients the indications for

Occlusive cerebrovascular disease in young adults.

13 patients under 40 years of age had cerebral infarction and angiographic evidence of arterial stenosis or occlusion. None of them had cardiac disorders prone to cerebral embolism. Five patients had occlusion at the origin of the internal carotid artery, one had occlusion of the common carotid artery and one each had stenosis at the origin of the internal carotid and common carotid arteries, respectively. Two patients had unilateral occlusion of the supraclinoid portion of the carotid artery with basal collaterals that had some resemblance to the Moyamoya disease. Three patients demonstrated

External carotid-vertebral artery anastomosis for vertebrobasilar insufficiency.

Two patients with previous brain stem infarction and current symptoms of vascular insufficiency in the basilar circulation had evidence of vertebral artery occlusive disease. Angiography demonstrated ostial stenosis of the dominant right vertebral artery and retrograde flow down the left vertebral artery to the level of the transverse process of C2 in the first case and to the level of a severely stenosed origin in the second case. Carotid circulation was patent in both cases. The blood flow in the posterior circulation was patent in both cases. The blood flow in the posterior circulation was

[Takeuki and Nishimoto's syndrome: an angiographic and nefrographic study of one case (author's transl)].

Takeuki and Nishimoto's syndrome has been described as more common though not exclusive in the Japanese people; it occurs in the pediatric or young adult age. The clinical syndrome is most variable (transitory ischemic attacks or circulatory insufficiencies with permanent neurological deficit, as well as subarachnoid hemorrhage). Conversely, the angiographic syndrome is quite typical: I) stenosis of both internal carotid arteries and the anterior part of Willis circle; II) presence of an angiomatous net at the level of the basal ganglia; III) presence of several anastomosis between internal an
