VP1 molecular evolution of type 2 immunodeficiency-related vaccine-derived polioviruses (iVDPV2) in a patient with primary immunodeficiency disease (PID).

Vaccine-derived polioviruses (VDPVs) arising after use of oral poliovirus vaccine (OPV) are an important impediment to the polio endgame, particularly in settings with low population immunity or prolonged infections in immunodeficient individuals. If immunocompromised individuals such as patients with primary immunodeficiency (PID) receive OPV, the live attenuated vaccine virus will replicate in their intestinal tract and chronically shed in the stool. Long-term proliferation of the vaccine virus in the enteric cells of the PID patients causes several mutations, which eventually culminates in t

Chronobiology of neurotropic viruses: rhythmic viral entry and arrhythmic host clocks.

Neurotropic viruses invade neural tissues, resulting in severe diseases such as poliomyelitis, rabies, herpesviral encephalitis, and viral meningitis. Given this neurotropism, we investigated whether the infection of the host by these viruses is under circadian control. In this study, we found that the expression of most neurotropic virus receptors exhibits rhythmicity across cells, cerebral organoids, and animal models, with host cell susceptibility modulated by the circadian clock. We identified E2F8 as a clock-controlled gene that mediates the indirect regulation of the circadian clock on n

Assessing Seroprevalence and Infection Dynamics of Oncogenic Gammaherpesviruses in South African Paediatric Patients Presenting with Inflammatory Conditions.

Kaposi's Sarcoma-associated herpesvirus (KSHV) and Epstein-Barr virus (EBV) are oncogenic gammaherpesviruses with high prevalence in sub-Saharan Africa. Both viruses are typically acquired during childhood, establishing lifelong latency. While viral reactivation into the lytic cycle has been mainly studied in adult HIV-infected populations-and more recently in the context of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) co-infection-the dynamics of KSHV and EBV infection in children remain poorly understood. Here, we characterize pediatric patients (n = 175; median age 4.6 years

Immunogenicity, Safety and Immune Persistence of Sabin Strain Inactivated Poliovirus Vaccine: A Phase III Clinical Trial.

The phased transition from oral poliovirus vaccine (OPV) to full-course immunisation with inactivated poliovirus vaccine (IPV) is a critical strategy for the global eradication of poliomyelitis. A randomised, double-blind, multi-centre, active-controlled clinical trial was conducted among healthy infants in China to evaluate the immunogenicity and safety of the Sabin strain IPV (sIPV). A total of 1260 healthy 2-month-old infants who had not previously received poliovirus vaccination were randomly assigned to receive three doses of either the investigational vaccine (sIPV) or the control vaccin

Evaluation of hyper-attenuated S19 poliovirus strains for use in poliovirus neutralization assays.

Poliovirus eradication efforts have relied on effective vaccination campaigns and stringent laboratory containment of poliovirus strains. Serological testing is the primary means to address population immunity, vaccine efficacy, and individual poliovirus exposures. However, strict containment requirements for Sabin type 2 and wild poliovirus types 2 & 3 limit access to serological testing. To address these limitations, we evaluated the performance of hyper-attenuated, temperature-sensitive S19 poliovirus strains (S19-PV), which retain the antigenic characteristics of Sabin and Salk strains but

Associations between activity limitations and participation restrictions in people with late effects of polio.

BACKGROUND: Late effects of polio (LEoP) are defined as new symptoms, for example, muscle weakness, fatigue, and pain, occurring many years after an acute polio infection. These impairments can lead to difficulties performing everyday activities (ie, activity limitations) that can reduce a person's ability to take part in society (ie, perceived participation restrictions). Yet, no study has explored how activity limitations are associated with participation restrictions in people with LEoP. OBJECTIVE: To explore the association between activity limitations and participation restrictions among

Genetic Diversity and Excretion Kinetics of Enteroviruses Excreted by Patients with Primary Immunodeficiency in Tunisia over a Five-Year Period (2020-2024).

Enteroviruses (EVs) are small, non-enveloped RNA viruses that can cause diverse clinical outcomes, particularly severe in patients with primary immunodeficiency (PID) due to their impaired ability to clear infections. This study aimed to characterize EV excretion among 138 Tunisian PID patients over a five-year period, to identify circulating EV serotypes and assess their genetic diversity. A total of 558 stool samples were collected and analyzed by virus isolation and intratypic differentiation using RT-qPCR. Molecular typing was performed through Sanger sequencing of the VP1 region and whole

Language, trust, and the polio endgame: words matter in vaccine communication.

SCOPE: Vaccine hesitancy remains a major public health challenge, particularly in the post-COVID-19 era, and is exacerbated by misinformation and social distrust. Terminology used in scientific and public health communication may influence perceptions of vaccine safety and credibility. This Position Paper examines the use of the term 'vaccine-derived poliovirus' (VDPV) and its potential impact on vaccine confidence during the final phase of polio eradication. METHODS: We reviewed published epidemiological, virological, and social science literature on circulating VDPV, vaccine hesitancy, and

Leveraging community insights and navigating logistical challenges: a case study of the 2024 polio vaccination campaign in Gaza, State of Palestine.

This case study describes how high vaccination coverage was achieved during the 2024 polio vaccination campaign in Gaza amid an ongoing humanitarian crisis marked by damaged critical infrastructure, an obstruction of the entry of health supplies, mass displacement and security concerns for health workers. Despite the immense challenges, 559 161 children were vaccinated in the first round (94% of the revised target of 591 714) and 556 774 in the second round, exceeding expectations. While this represents an impressive achievement, some areas remained inaccessible, which prevented an estimated 7

Machine learning framework for early detection of polio outbreaks from acute flaccid paralysis surveillance data.

Poliomyelitis remains a global public health concern despite remarkable progress toward eradication. The continued circulation of vaccine-derived polioviruses underscores the need for enhanced surveillance systems capable of early detection and rapid response. Acute Flaccid Paralysis (AFP) surveillance, a key component of the polio eradication, plays a vital role in detecting poliovirus transmission but is often limited by delays in data collection, analytical capacity, and fragmented reporting structures. Recent advances in artificial intelligence (AI) offer opportunities to address these cha

Coverage and associated factors of inactivated polio vaccine uptake among children aged 12-23 months in Sub-Saharan Africa.

The Inactivated Polio Vaccine (IPV) protects against all strains of the polio virus and poses no risk of vaccine-associated paralytic poliomyelitis. However, its coverage remains below an optimal level in Sub-Saharan Africa (SSA), and instances of vaccine-derived poliovirus have been reported. While several studies explored vaccine coverage, research specifically focused on IPV in SSA remains limited. Hence, this study aimed to assess the coverage and determinants of IPV uptake among children 12–23 months of age. A secondary data analysis was conducted using data from the recent demographic an

Poliomyelitis, the years before vaccination. A reading through art.

INTRODUCTION: Writing the history of polio - or of any disease - requires more than a clinical account of its causes, symptoms, and progression. It also demands an exploration of how illness is experienced by individuals and interpreted by society. Beyond defining disease as a biological dysfunction, it is essential to consider the patient’s lived experience, shaped not only by personal suffering but also by cultural frameworks that influence how that suffering is understood and expressed. Equally important is the societal lens through which illness is viewed and contextualized. OBJECTIVE: Thi

Total Knee Arthroplasty in Postpolio Residual Paralysis: A Systematic Review and Pooled Analysis of Its Outcomes and Complications.

BACKGROUND: Total knee arthroplasty (TKA) in patients who have postpolio residual paralysis (PPRP) is technically challenging due to bone deformities, ligamentous laxity, and variable quadriceps muscle strength. Despite these challenges, TKA is increasingly being performed in this population, yet outcomes remain variably reported. This review aimed to systematically assess postoperative complications and revision rates following TKA in patients who had PPRP through a pooled analysis of available studies. METHODS: A systematic review was conducted in accordance with the Preferred Reporting Ite

Application of a humeral intramedullary nail for subtrochanteric fracture in a patient with poliomyelitis sequelae: a case report.

OBJECTIVE: To investigate the clinical feasibility and efficacy of using a humeral intramedullary nail for the treatment of subtrochanteric fracture in a patient with ipsilateral poliomyelitis sequelae. METHODS: A case of a 54-year-old female patient with a left subtrochanteric fracture caused by a traffic accident was reported. The patient had a 50-year history of ipsilateral poliomyelitis, leading to developmental deformity and severe stenosis of the femoral medullary canal in the affected limb, which could not accommodate a conventional femoral intramedullary nail. Therefore, we innovative

Sub-national modelling of surveillance sensitivity to inform declaration of disease elimination: A retrospective validation against the elimination of wild poliovirus in Nigeria.

A fundamental question in the global commitment to polio eradication is how long a period of absence would be consistent with regional elimination, and the safe withdrawal of the oral polio vaccine is contingent on the answer. We present a statistical framework to model the time-varying sensitivity of two key components of polio surveillance - environmental sampling and clinical cases of acute flaccid paralysis - for detecting infection on a monthly basis at the local government authority level. We use this to estimate the probability of freedom from infection (FFI) at a critical prevalence le
