Severe retinal vasculopathy associated with myelin oligodendrocyte glycoprotein antibody optic neuritis in a toddler.

We report the case of a 22-month-old boy with retinal vascular disease caused by optic neuritis due to myelin oligodendrocyte glycoprotein antibody disease. He presented with acute disseminated encephalomyelitis without optic nerve involvement. Despite treatment, he developed optic neuritis and vision loss within days. In addition to the usual findings of optic neuritis, he had evidence of severe retinal venous stasis including intraretinal, subretinal, preretinal, and choroidal hemorrhages that were rapidly progressive. The retinal perfusion did not recover, leading to neovascularization. Thi

Retinal Vasculopathy With Cerebral Leukoencephalopathy Mimicking a Brain Tumor: A Case Report.

Retinal vasculopathy with cerebral leukodystrophy (RVCL) is an adult-onset, autosomal dominant disorder caused by heterozygous C-terminal frameshift mutations in TREX1, leading to mislocalization of its normally perinuclear exonuclease. Patients typically present with progressive visual impairment and neurological decline, while brain magnetic resonance imaging (MRI) commonly demonstrates punctate white matter lesions or tumor-like rim-enhancing masses. We report the case of a 53-year-old woman who initially presented with progressive headaches, followed by visual loss, cognitive impairment, a

First case of TREX1 mutation-driven retinal vasculopathy with cerebral leukoencephalopathy and systemic manifestations coexisting with lupus nephritis: a case report and mechanistic discussion.

BACKGROUND: The TREX1 gene is fundamental for the removal of cytosolic DNA and the preservation of immune tolerance. Mutations within this gene are implicated in a range of disorders, such as Retinal Vasculopathy with Cerebral Leukoencephalopathy and Systemic Manifestations (RVCL-S), Aicardi-Goutières syndrome (AGS), familial chilblain lupus, and systemic lupus erythematosus (SLE). To the best of our knowledge, this report documents the rare coexistence of TREX1-associated RVCL-S and biopsy-proven lupus nephritis in a single patient. CASE PRESENTATION: We report a case involving a middle-aged

Systemic irisin treatment modulates cognitive function through VEGF-associated hippocampal vascular signaling in chronic cerebral hypoperfusion.

Vascular dementia (VaD), primarily caused by chronic cerebral hypoperfusion (CCH), is characterized by progressive cognitive decline associated with neurovascular dysfunction. The present study aimed to investigate whether systemic administration of irisin, an exercise-induced myokine, modulates cognitive performance and VEGF-associated angiogenic signaling in the hippocampus under CCH conditions. Thirty-eight adult male Wistar albino rats were randomly assigned to five groups: control, sham, irisin, ischemia, and ischemia + irisin. CCH was induced via permanent bilateral common carotid artery

Megacapillaries on nailfold capillaroscopy are associated with white matter hyperintensities burden: a population study.

BACKGROUND AND AIMS: White matter hyperintensities (WMH) of presumed vascular origin are reliable neuroimaging markers of cerebral small vessel disease (cSVD). However, their underlying systemic correlates remain incompletely understood. Nailfold capillaroscopy (NFC) provides direct visualization of peripheral microvascular health, and megacapillaries represent a reproducible indicator of severe endothelial injury. Whether such peripheral microvascular abnormalities mirror structural brain changes in sporadic cSVD is unknown. We investigated the association between megacapillaries and WMH seve

Diagnostic Value of Inflammatory Biomarkers in Differentiating Vascular Dementia From Alzheimer's Disease: A Systematic Review and Meta-Analysis.

INTRODUCTION: Dementia encompasses distinct subtypes characterized by different underlying mechanisms-most notably, Alzheimer's disease (AD), driven by neurodegeneration, and vascular dementia (VaD), stemming from cerebrovascular pathology. Growing evidence emphasizes the critical role of neuroinflammatory processes in both conditions, highlighting inflammatory biomarkers as potential tools for differential diagnosis. This study assesses the diagnostic accuracy of inflammatory biomarkers in distinguishing AD from VaD. METHOD: A systematic search of PubMed, EMBASE, Web of Science, and Cochrane

NOTCH3 R545C mutation drives vascular-neuronal dysfunction and cognitive impairment in CADASIL pathogenesis.

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is the most common hereditary small vessel disease caused by mutations in the NOTCH3 gene. However, its pathogenic mechanisms remain incompletely understood. Given the high prevalence of the NOTCH3 p.Arg544Cys (R544C) mutation in East Asian populations, we developed a novel CRISPR/Cas9-mediated mouse model harboring the NOTCH3 p.R545C point mutation (orthologous to human R544C) to dissect the underlying CADASIL pathogenesis. Comprehensive phenotyping revealed age-dependent cognitive deficits, i

Beneficial Effects of Olive Oil and the Mediterranean Diet on Alzheimer's Disease and Vascular Dementia: A Review.

Background and Objectives: During the past 25 years, a significant body of research has been conducted reporting on the salutary effects of the Mediterranean diet and extra-virgin olive oil, one of its main components. The initial studies were epidemiological observations on populations with very low mortality rates due to significant reductions in myocardial infarction fatalities. Population-based studies demonstrated that the Mediterranean diet with olive oil consumption is associated with a lower prevalence of cardiovascular and cerebrovascular disease, obesity, arthritis, and cancer. CADASIL: a practical review for the neurologist.

CADASIL (cerebral autosomal dominant arteriopathy with subcortical infarcts and leucoencephalopathy) is a hereditary small vessel disease caused by pathogenic NOTCH3 variants, predominantly affecting cysteine residues in epidermal growth factor-like repeats. Clinically, patients present with migraine with aura, subcortical ischaemic strokes, mood disturbances, apathy and progressive cognitive decline, with disability accumulating over decades. Neuroimaging typically shows symmetrical white matter hyperintensities, lacunar infarcts and cerebral microbleeds. Its diagnosis relies on genetic testi

Microglial Plasticity in Vascular Dementia: Mechanisms and Therapeutic Reprogramming.

Vascular dementia (VaD) is a leading cause of cognitive decline and arises from heterogeneous cerebrovascular pathologies, most commonly cerebral small vessel disease and chronic cerebral hypoperfusion. Microglia, the brain's resident immune cells, exert a dual, stage-dependent influence during VaD progression, initially supporting neuroprotection through debris clearance and tissue repair, but later contributing to chronic neuroinflammation, synaptic loss, and white matter injury. Emerging evidence suggests that multiple molecular pathways, including purinergic receptors, Toll-like receptors

Exploring the Research Progress of Vascular Dementia and Key Regulatory Molecules: E2F1.

Vascular dementia (VaD) is a type of dementia caused by cerebrovascular factors, which can arise from both ischemic cerebrovascular disease and hemorrhagic cerebrovascular disease. The incidence rate of VaD is second only to Alzheimer's disease and dementia with Lewy bodies. Currently, there are no effective drugs specifically targeting VaD, making the discovery of new therapeutic targets of great significance. This article provides an overview of the research progress on VaD, with a focus on elucidating its pathogenesis, aiming to identify targets that play a regulatory role in the mechanism.

Neuroprotective effects of piracetam in vascular dementia: a review of toll-like receptor 4 -mediated neuroinflammatory pathways.

Vascular dementia (VaD) is a leading cause of cognitive impairment, typically occurring due to conditions that reduce blood flow to the brain. Increasing evidence indicates that neuroinflammation plays a key role in the progression of VaD, with Toll-like receptor 4 (TLR4) emerging as a key regulator of innate immune activation and inflammatory amplification. Therefore, therapeutic agents capable of targeting TLR4-driven neuroinflammation may represent a rational approach to mitigating disease progression in VaD. In this context, piracetam, a nootropic agent used for cognitive disorders, has sh

Integrated single-cell and Mendelian randomization analyses identify aging-induced brain endothelial SPARCL1 deficiency as a key driver of vascular dementia.

BACKGROUND: Vascular dementia (VaD) is a prevalent age-related neurocognitive disorder characterized by progressive cognitive impairment and a lack of disease-modifying therapies. Although age-related brain endothelial dysfunction has been implicated as a central contributor to the pathogenesis of VaD, the molecular mechanisms underlying this process remain incompletely elucidated. METHODS: We employed an integrative analytical framework combining Mendelian randomization (MR) and single-cell RNA sequencing (scRNA-seq). MR analysis was performed using plasma protein quantitative trait loci (pQ

Quantitative Relationship Between White Matter Hyperintensity Volume and Fazekas Score on Brain MRI.

BACKGROUND: The Fazekas score is widely used to grade white matter hyperintensities (WMHs) in cerebral small vessel disease, yet the equivalent volume of each grade is unclear. We quantified the correspondence between Fazekas scores and WMH volume, normalized ratios, and derived conversion equations across populations. METHODS: We analyzed 1220 participants from 4 mostly United Kingdom-based cohorts representing different cerebral small vessel disease severities: community-dwelling (LBC1936 [Lothian Birth Cohort 1936]), stroke (MSS2 [Mild Stroke Study 2] and MSS3 [Mild Stroke Study 3]), and a

Multimodal characterization of age-dependent neurovascular phenotypes in Notch3R170C CADASIL mice by ultra-high-field MRI and histological analyses.

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is a hereditary small vessel disease caused by mutations in NOTCH3 and characterized by progressive neurovascular alterations. Although magnetic resonance imaging (MRI) can detect vascular and white matter abnormalities in CADASIL, the relationship between imaging findings and underlying cellular and molecular changes remains incompletely understood. Here, we established an integrative, multi-scale framework combining ultra-high-field MRI, quantitative histology, and single-cell RNA sequencing

Unwinding the Neurovascular Unit: The Vascular Extracellular Matrix and Mechanochemical Collagen Functionalization in Vascular Dementia.

Vascular dementia (VaD) represents a spectrum of neurodegenerative disorders driven by chronic or acute cerebral hypoperfusion, converging on the structural collapse of the neurovascular unit (NVU). While vascular mechanics and neuroinflammation have been extensively characterized, the extracellular matrix (ECM) remains an under-appreciated driver of this pathology. Far from being a passive scaffold, the ECM serves as a dynamic regulator of brain homeostasis, governing the integrity of the blood-brain barrier (BBB), supporting synaptic plasticity via Perineuronal Nets (PNNs), and facilitating
