Assessment of cerebrovascular reserve using acetazolamide brain perfusion SPECT in Moyamoya disease.

OBJECTIVES: Moyamoya disease (MMD) is a rare, progressive steno-occlusive cerebrovascular disorder characterized by impaired cerebral perfusion and an elevated risk of ischemic events. Accurate cerebrovascular reserve (CVR) assessment is crucial for guiding surgical decision-making. This study evaluated the clinical utility of acetazolamide (ACZ)-challenged brain perfusion single-photon emission computed tomography (SPECT) in assessing CVR in patients with MMD. METHODS: We retrospectively analyzed 10 patients (6 males, four females; aged 5-65 years) with angiographically confirmed MMD who und

Preliminary analysis of long-term prognosis outcomes of modified extracranial-intracranial bypass reconstruction for adult ischemic moyamoya disease.

This retrospective cohort study compared long-term outcomes between modified extracranial-intracranial bypass (mECIC) and encephalo-duro-arterio-myo-synangiosis (EDAMS) in adults with ischemic moyamoya disease. Among 50 patients analyzed (mECIC: n = 18; EDAMS: n = 32), the mECIC group demonstrated better neurological recovery, with 88.9% achieving functional independence (mRS ≤ 2) versus 62.5% in the EDAMS group (p = 0.018) at 60-month follow-up. Cognitive performance, assessed by the Montreal Cognitive Assessment (MoCA), showed greater improvement in the mECIC cohort (ΔMoCA: +8.2 ± 1.5 vs. +

Single-cell transcriptomic profiling of peripheral blood mononuclear cells reveals monocyte heterogeneity in patients with Moyamoya disease.

OBJECTIVE: Moyamoya disease (MMD) is a rare cerebrovascular disorder characterized by progressive stenosis or occlusion of the internal carotid artery, with an abnormal vascular network forming as compensation. The etiology of MMD remains largely unknown, though genetic and immune factors have been implicated. This study aimed to investigate the landscape of peripheral immune cells in MMD patients using single-cell RNA sequencing (scRNA-seq) to identify potential biomarkers and mechanisms involved in the disease. METHODS: Peripheral blood mononuclear cells (PBMCs) were collected from six MMD

Cortical venous redness as a semi-quantitative marker for predicting transient neurological events in adult moyamoya disease.

Transient neurological events (TNEs) frequently occur after direct revascularization in adult moyamoya disease, often due to acute alterations in cerebral hemodynamics. Cortical venous redness has been linked to perioperative hemodynamic alterations; therefore, we investigated whether changes in cortical venous redness, quantified as red signal intensity (R intensity) around the anastomosis site, could predict TNE occurrence. In this retrospective study, we analyzed 52 hemispheres from 42 patients who underwent combined revascularization surgery for moyamoya disease. Cortical venous R intensit

Angiographic outcomes after indirect revascularization in pediatric Moyamoya disease: a systematic review and Meta-analysis.

Moyamoya disease is a rare progressive cerebral vasculopathy characterized by narrowing or occlusion of the internal carotid arteries, leading to the formation of fragile collateral vessels and a high risk of ischemic events, especially in pediatric patients. Although direct revascularization techniques are commonly used, they are technically challenging in children due to vascular fragility and anatomy. Indirect revascularization techniques favor the formation of new blood vessels through alternative flow routes. Their efficacy is usually assessed using the Matsushima scale, which measures th

Artificial intelligence for microembolic signal detection by transcranial Doppler in ischemic stroke: a mini-review.

Microembolic signals (MES) detected by transcranial Doppler (TCD) provide real-time information on ongoing embolic activity in patients with ischemic stroke and transient ischemic attack. MES have been associated with stroke recurrence and high-risk conditions including large-artery atherosclerosis, atrial fibrillation, moyamoya disease, cancer-related stroke, and complex aortic arch plaques. Despite its clinical value, conventional TCD is limited by operator dependency, suboptimal acoustic windows, and limited ability to discriminate embolus characteristics. Recent advances in artificial inte

Research on the application of cerebral blood flow reconstruction technology in the surgical treatment of moyamoya disease.

PURPOSE: Moyamoya disease (MMD) is characterized by stenosis and occlusion of the cerebral arteries, leading to chronic progressive narrowing at the termini of the bilateral internal carotid arteries (ICA). It represents one of the primary etiologies for both ischemic and hemorrhagic strokes ( 1, 2).Currently, the primary treatment strategy for moyamoya disease (MMD) remains blood flow reconstruction surgery utilizing a bypass from the superficial temporal artery (STA) to the middle cerebral artery (MCA). This approach encompasses direct blood flow reconstruction, indirect blood flow reconstru

PASAformer: Cerebrovascular Disease Classification With Medical Prior-Guided Adapter and Pathology-Aware Sparse Attention.

Cerebrovascular diseases (CVDs) such as aneurysms, arteriovenous malformations, stenosis, and Moyamoya disease are major public health concerns. Accurate classification of these conditions is essential for timely intervention, yet current computer-aided methods often exhibit limited representational capacity, feature redundancy, and insufficient interpretability, restricting clinical applicability. We propose PASAformer, a Swin-Transformer-based framework for cerebrovascular disease classification on DSA. PASAformer incorporates a Pathology-Aware Sparse Attention (PASA) module that emphasizes

High flow, at a higher cost? A pilot study comparing direct vs. indirect bypass for Moyamoya disease using time-driven activity-based costing.

BACKGROUND AND OBJECTIVE: In the management of adult moyamoya disease (MMD), there is clinical equipoise regarding direct bypass (DB) vs indirect bypass (IB). One factor that may influence the choice of procedure is cost. Therefore, we sought to compare the true intraoperative costs (not charges or reimbursement) of DB vs. IB using a novel costing methodology, time-driven activity-based costing (TDABC). METHODS: A retrospective cohort study (2017-22) was conducted of all patients undergoing revascularization for symptomatic MMD. TDABC was used to calculate intraoperative costs. Supply cost wa

Delineating the Genetic Basis of RNF213-related vasculopathies: The association of PKHD1 variants with bilateral cerebral vasculopathy.

Moyamoya disease (MMD) is an idiopathic cerebrovascular disorder characterized by progressive stenosis of the internal carotid artery termini and the formation of an abnormal network of fragile perforators. Although the RNF213 gene has been implicated as a susceptibility factor for MMD, the precise genetic basis of the disease remains elusive. This study aimed to investigate other genetic factors contributing to differences in disease manifestation and vascular phenotypes. We conducted whole-exome sequencing of patients with RNF213-related vasculopathy and healthy controls. In total, 122 patie
