Beyond Atherosclerosis: Takayasu Arteritis Presenting as Ischemic Stroke in a Young Patient With Significant Traditional Vascular Risk Factors.

A 46-year-old lady, with insulin-dependent type 2 diabetes, ischaemic heart disease, hypertension, high cholesterol, and being a current smoker, presented with acute left-sided weakness involving her upper and lower limbs. She was admitted with a clinical diagnosis of right lacunar ischemic stroke. Initial CT brain revealed an old right basal ganglia infarct, and CT carotid angiography revealed irregularities in the right middle cerebral artery with no occlusion, subtle mural thickening, and periarterial changes surrounding the origin of the great vessels arising from the aortic arch, features

High ischemic deficit in computed tomography perfusion is a risk factor for cerebral vascular ischemic events in Takayasu arteritis: a prospective observational study.

BACKGROUND: Patients suffering from Takayasu arteritis (TA) with stenosis or occlusion of supra-aortic trunks carry a high risk of cerebral vascular ischemic events (CVIEs). We explored the relationship between cerebral blood perfusion and CVIEs in TA. METHODS: Patients with involvement of supra-aortic trunks undergoing computed tomographic perfusion (CTP) were enrolled. The primary endpoint was CVIEs. The secondary endpoint was a poor outcome (modified Rankin scale, mRS > 3). The relationship between CTP parameters, ischemic deficit (time to maximum, Tmax >6 s) and ischemic core (cerebral blo

Severe Pediatric Takayasu Arteritis Presenting With Cerebral Ischemia and Supra-Aortic Trunk Occlusion.

Takayasu arteritis (TAK) is a rare, chronic, autoimmune, and granulomatous vasculitis that predominantly affects large vessels, especially the aorta and its main branches, often leading to stenosis, occlusion, or aneurysm formation. Although TAK primarily affects young adult females, its occurrence in pediatric patients is uncommon and frequently associated with a more aggressive clinical course. We report a severe neurovascular presentation of TAK in a 16-year-old Mexican female patient who developed sudden-onset right hemiparesis, global aphasia, and multiple syncopal episodes. Marked interl

Extended depth-of-field fluorescence microscope for stable imaging during random positioning machine running.

BACKGROUND: Takayasu arteritis (TA) is a chronic granulomatous large-vessel vasculitis that primarily affects the aorta and its major branches. It is rare in children and adolescents, often leading to diagnostic delays due to its nonspecific initial symptoms such as fever, fatigue, and arthralgia. CASE PRESENTATION: We report a 15-year-old male who presented with a several-month history of low-grade fever, easy fatigability, and progressive bilateral multiple joint pain. Notably, he had a history of BCG vaccination and no known tuberculosis exposure. Three weeks prior to diagnosis, he develop

[Giant cell arteritis revealed by bilateral internuclear ophthalmoplegia].

INTRODUCTION: We report an original observation of giant cell arteritis (GCA) revealed by bilateral internuclear ophthalmoplegia due to parenchymal inflammatory involvement of the central nervous system. CASE REPORT: An 84-year-old female patient presented to our emergency department because of diplopia due to bilateral internuclear ophthalmoplegia. Cerebral MRI found a T2 hypersignal with enhancement after gadolinium injection in T1 sequence of the two medial longitudinal fascicles. Lumbar puncture was negative, biology only found elevated acute phase response proteins, PET scan showed large

[Annuloaortic Ectasia Induced by Takayasu's Arteritis:Report of a Case].

A 50-year-old male was diagnosed with aortic annulus ectasia (AAE) and aortic valve regurgitation (AR), and was thus referred to our department for surgery. Computed tomography (CT) revealed a Valsalva aneurysm with a maximal diameter of 52 mm. Echocardiography revealed severe AR with left ventricular enlargement [left ventricular internal dimension in diastole (LVDd) 85 mm]. The creatinine (CRE) level was 0.97 mg/dl, and the C-reactive protein (CRP) level was 1.92 mg/dl. David's procedure was initiated as a therapeutic intervention; however, severe adhesion and enlargement of the ascending ao

Risk of Stroke Recurrence Following Isolated Retinal Artery Occlusion Versus Minor Stroke or Transient Ischemic Attack.

BACKGROUND: Retinal artery occlusion (RAO) is a form of ischemic stroke per the American Heart Association, yet high-level evidence guiding management and prognostication is limited. The risk of future cerebral infarction following nonarteritic isolated RAO (iRAO; without concomitant cerebral infarction) is unclear. This study compares the risk of stroke recurrence following iRAO versus nondisabling ischemic cerebrovascular events (NICEs), including transient ischemic attacks and minor ischemic strokes. METHODS: This was a retrospective cohort study using the 2016 to 2022 Nationwide Readmissi

Incomplete Susac Syndrome With Recurrent Retinal Occlusions and Vertigo: A Case of Early Immunosuppressive Success.

Susac syndrome (SS) is a rare autoimmune microangiopathy that targets the brain, retina, and inner ear. Its variable and often incomplete clinical presentation frequently leads to misdiagnosis, increasing the risk of morbidity. We report the case of a 53-year-old Caucasian woman with a history of pityriasis lichenoides et varioliformis acuta (PLEVA) and severe myopia. She experienced recurrent branch retinal artery occlusion (BRAO) and retinal vein occlusion, following an episode of intense vertigo. Notably, her initial vascular occlusion occurred prior to the onset of vertigo. A comprehensive

An Unusual Case of Cerebral Venous Sinus Thrombosis With Negative D-dimer Mimicking Giant Cell Arteritis.

Cerebral venous sinus thrombosis (CVST) is a rare, potentially life-threatening condition that presents with variable symptoms such as headache, visual disturbance, and seizures, often mimicking an arterial stroke or intracranial lesion. Diagnosis can be challenging, and delays may result in severe outcomes. Although an elevated D-dimer may suggest CVST, a negative result does not exclude it. We report a rare case of extensive CVST initially treated as giant cell arteritis (GCA) due to unilateral temporal headache, elevated CRP, and a negative D-dimer. A 62-year-old man presented with acute le

The Interplay Between Varicella-Zoster Virus and Giant Cell Arteritis: An In-Depth Narrative Review.

Giant cell arteritis (GCA) is a large-vessel vasculitis that is commonly associated with inflammation of the blood vessels within the head and neck. The condition primarily affects older and immunocompromised individuals and is diagnosed through a combination of physical examination, laboratory tests, and, in some cases, biopsy. Most patients present with headaches, visual disturbances, and jaw claudication accompanied by low-grade fever, fatigue, and weight loss. Varicella-zoster virus (VZV) may play a role in the pathogenesis of GCA. VZV DNA has been identified within the cerebral arteries o

Stroke in the Setting of Neurosyphilis: A Comprehensive Literature Review.

Background: syphilis is globally recognized as a great imitator due to its multiple manifestations and multi-organ involvement. This holds especially true in the context of neurosyphilis (NS), where stroke and other cerebrovascular manifestations are frequently overlooked. With the global reemergence of syphilis, meningovascular syphilis (MVS) and other vascular syphilitic affectations are now important, yet underdiagnosed causes of ischemic stroke. Purpose: this literature review focuses on syphilis in the context of stroke, examining the condition through this specific perspective. The patho

Nocardia farcinica Brain Abscess in a Glucocorticoid-Treated Patient with Giant-Cell Arteritis: A Case Report.

BACKGROUND Nocardia species are opportunistic pathogens that primarily cause infections in immunocompromised individuals. Giant-cell arteritis is a granulomatous vasculitis of large- and medium-sized arteries in individuals aged ≥50 years, typically requiring prolonged glucocorticoid therapy, which increases susceptibility to opportunistic infections. CASE REPORT We report the case of a 73-year-old man with giant-cell arteritis treated with prednisolone who developed disseminated nocardiosis. Four months after giant-cell arteritis diagnosis, a new pulmonary nodule was detected on follow-up ima

Recent advances in nanomedicine for the diagnosis and therapy of thyroid disorders.

INTRODUCTION: Inappropriate antibiotic prescribing drives antimicrobial resistance. Although the Swiss Society of Infectious Diseases has introduced national guidelines for common infectious diseases starting from 2019, it remains unclear whether family physicians and paediatricians adhere to them and what factors influence their prescriptions. The aims of this study were to assess whether Swiss family physicians and paediatricians make appropriate antibiotic choices in accordance with national guidelines and to identify physician- and patient-related factors associated with the prescribing of

Atypical Oral Presentation of Giant Cell Arteritis With Subsequent Middle Cerebral Artery Involvement.

Giant cell arteritis (GCA) is a large-vessel vasculitis classically affecting the temporal arteries and associated with visual loss and constitutional symptoms. We report a rare and diagnostically challenging case of GCA in an elderly patient presenting with progressive visual impairment, anterior circulation stroke, vasculitic mouth ulcer, and dysphagia requiring nasogastric feeding. Diagnosis was confirmed by ultrasound findings without histological confirmation. This case underlines the potential for GCA to present with multisystem ischemic manifestations and the importance of early recogni

Neuro-ophthalmic Manifestations of Multiple Sclerosis.

The human body is endowed with an immune system that protects the body from infections, microbes, diseases, and external or foreign elements that could be harmful or disease producing. The human immune system is a complex amalgamation of cells, proteins, organs, and organ systems that fight these harmful agents and defend the body against them. When the immune system turns against itself or the body, it results in the development of autoimmune conditions wherein there is destruction of specific organs or systems, which threaten their normal functioning. Consequently, there is either reduced fu

Imaging of Amyloid-beta-related Arteritis.

Cerebral amyloid angiopathy (CAA) is a cerebrovascular disorder marked by the accumulation of amyloid-beta peptide (Aβ) within the leptomeninges and smaller blood vessels of the brain. CAA can be both noninflammatory and inflammatory, and the inflammatory version includes Aβ-related angiitis (ABRA). ABRA is a vasculitis of the central nervous system related to an inflammatory response to Aβ in the vascular walls, which necessitates differentiating ABRA from noninflammatory CAA, as ABRA may require immunosuppressive treatment. MR imaging is typically the most effective imaging modality of choic
