Intracerebral Hemorrhage Expansion in Late Presenters: Proposed Target Expansion Criteria.

BACKGROUND AND OBJECTIVES: The benefit of therapies targeting hematoma expansion (HE) in acute intracerebral hemorrhage (ICH) is highly time dependent and greatest within the first 2 hours after onset. We aimed to develop imaging-based "target expansion criteria" to identify late presenters (2-12 hours from onset/last seen well) who remain at high risk of HE. To achieve this goal, a practical risk score (target expansion score) was developed. METHODS: We conducted a multicenter, international, retrospective cohort study. To mirror a clinical trial population, we excluded patients with brainst

External Lumbar Drainage for Refractory Intracranial Hypertension in Acute Brain Injury: A Narrative Review and Case Series.

Refractory intracranial hypertension in the setting of acute brain injury (ABI) is defined as sustained intracranial pressure (ICP) elevation above 22 mmHg despite escalating medical management. When unresponsive to these therapies, mortality reaches 84%-100%. External lumbar drainage (ELD) has emerged as a minimally invasive adjunct for cerebrospinal fluid (CSF) diversion, either as complementary to or alternative to external ventricular drainage (EVD), potentially avoiding the need for barbiturate coma or decompressive craniectomy. The objective of this paper is to review current evidence on

Association of Admission Serum Homocysteine with Discharge Neurocognitive Dysfunction after Acute Cerebrovascular Disease in a High-Altitude Setting.

Discharge-phase neurocognitive abnormalities may affect rehabilitation participation, self-management, and discharge safety after acute cerebrovascular disease, but early clinically accessible correlates remain incompletely defined. We examined the association between admission serum homocysteine and clinically adjudicated discharge neurocognitive dysfunction in a high-altitude clinical setting. This single-center retrospective cohort included 227 adults admitted for acute ischemic stroke, intracerebral hemorrhage, or transient ischemic attack. The first available serum homocysteine measuremen

[Systemic sclerosis with anti-human neutrophil cytoplasm antibody associated vasculitis: A case report].

Systemic sclerosis (SSc) is an autoimmune disease involving microvessels and a wide range of connective tissues. Anti-human neutrophil cytoplasm antibody-associated vasculitis (AAV) is a primary, systemic necrotizing small vasculitis. The coexistence of AAV and SSc is rare. We report a case of SSc combined with AAV complicated with alveolar hemorrhage, gastrointestinal hemorrhage, and cerebral hemorrhage. The patient was a 56-year-old woman with a 20-year history of Raynaud phenomenon, skin sclerosis on the fingertip and face. She developed dry cough for 1 year and shortness of breath after ac

Angiotensin II exacerbates intracerebral hemorrhage in mice via heparin-like antithrombin activation and coagulation dysfunction.

Hypertension has paradoxical prothrombotic-hemorrhagic predisposition toward cerebral infarction and intracerebral hemorrhage (ICH), but the mechanism remains poorly understood. We observed Angiotensin II (Ang II) inhibited coagulation by activating antithrombin through heparin-like acceleration of antithrombin-thrombin interaction. Ang II allosterically modulates the 386-397 loop of antithrombin to enhance thrombin inhibition. In vivo, single injection of Ang II inhibited coagulation, alleviating thrombosis and aggravating ICH. Conversely, anti-Ang II monoclonal antibody promoted coagulation,

Clinical Characteristics of Intracranial Hemorrhage in Pediatric Acute Leukemia.

BACKGROUND: Intracranial hemorrhage (ICH) is a life-threatening complication of pediatric acute leukemia. However, whether early-onset ICH and late-onset ICH are distinct clinical entities remains unclear. This study investigated the characteristics, neuroimaging findings, and outcomes of ICH. METHODS: Children with newly diagnosed acute leukemia were retrospectively reviewed. Clinical characteristics and OS were compared between patients with and without ICH, while neuroimaging findings and neurological outcomes were evaluated in patients with ICH. Early-onset ICH was defined as hemorrhage w

Minimally Invasive Hematoma Evacuation for Acute Spontaneous Supratentorial Intracerebral Hemorrhage: A Systematic Review and Meta-Analysis With Trial Sequential Analysis.

BACKGROUND: Intracerebral hemorrhage (ICH) is a severe form of stroke associated with high morbidity and mortality. While surgical evacuation aims to reduce hematoma burden and improve outcomes, its clinical benefit over medical management remains uncertain. Previous systematic reviews have evaluated a broad range of surgical interventions for ICH; however, evidence specific to MIS remains less clearly defined. Therefore, we performed a systematic review and meta-analysis to assess the efficacy and safety of minimally invasive hematoma evacuation in patients with acute spontaneous supratentori

Early hematoma expansion provides incremental prognostic information beyond admission characteristics for 6-month functional outcome after adult traumatic brain injury.

BACKGROUND: Early hematoma expansion (HE) reflects interval progression of traumatic intracranial hemorrhage, but its association with long-term outcome and its prognostic contribution beyond admission characteristics remain incompletely defined. We evaluated whether HE was independently associated with 6-month functional outcome and provided incremental prognostic information beyond admission characteristics. METHODS: This single-center retrospective study included 640 adults with traumatic brain injury, including 285 with HE. HE was determined from the first two in-hospital head computed to

Association of Location-Specific Perihematomal Edema with Clinical Outcomes in Deep Intracerebral Hemorrhage.

OBJECTIVE: Perihematomal edema (PHE) represents a major secondary injury mechanism following intracerebral hemorrhage (ICH). This study aims to evaluate the impact of early PHE expansion on functional outcomes in patients with deep ICH. METHODS: Patients with deep intracerebral hemorrhage who underwent baseline computed tomography (CT) within 24 h of symptom onset and a follow-up CT within 72 h were included. Absolute PHE increase was defined as the difference in PHE volume between the follow-up and baseline CT scans. Poor outcome was defined as a modified Rankin Scale score > 3 at 3 months.

Remote Ischemic Conditioning After Subarachnoid Hemorrhage: A Systematic Review and Meta-analysis.

BACKGROUND: Remote ischemic conditioning (RIC), an intervention in which brief episodes of ischemia are applied to an organ or tissue to induce remote organs' tolerance of subsequent ischemic injury, has shown potential in reducing the incidence of delayed cerebral ischemia (DCI) secondary an aneurysmal subarachnoid hemorrhage (aSAH). This review aimed to evaluate the effectiveness of RIC in patients with SAH. METHODS: A comprehensive search was conducted to identify peer-reviewed randomized controlled trials (RCT) and non-randomized studies comparing RIC with no conditioning or sham procedur

Use of Desmopressin in Spontaneous Nontraumatic Intracerebral Hemorrhage Among Patients Taking Antiplatelet Therapy: Systematic Review and Bayesian Meta-analysis.

Antiplatelet therapy-associated intracerebral hemorrhage (APT-ICH) remains a significant neurological concern due to high morbidity and mortality among these patients. Early trials of desmopressin (DDAVP) suggested clinical benefit; however, the number of studies available at the time was limited. This systematic review and Bayesian meta-analysis aim to answer whether administration of DDAVP among patients with APT-ICH improves clinical course. Five databases were searched for studies comparing DDAVP and placebo/control/usual care in patients with APT-ICH. Bayesian meta-analysis was performed

Preterm Birth Causes Decreased Postnatal Neurogenesis: A Novel Mechanism Underlying Adverse Neurodevelopmental Outcomes.

Although the survival rates of preterm infants have markedly improved, their neurodevelopmental outcomes remain poor. Historically, brain injury in preterm infants has been recognized as intraventricular hemorrhage or white matter injury, often resulting in cerebral palsy. More recently, increasing attention has focused on the elevated risk of neurodevelopmental disorders among preterm infants across a wide range of gestational ages. Furthermore, preterm birth is also associated with an increased risk of psychiatric disorders in adulthood, including depression and schizophrenia. However, the u

Neurologic complications in venoarterial versus venovenous extracorporeal membrane oxygenation: A propensity score-matched retrospective cohort study.

BACKGROUND: Neurologic complications are among the most devastating events associated with extracorporeal membrane oxygenation (ECMO), yet propensity-balanced comparisons of mode-specific neurologic risk between venoarterial (VA) and venovenous (VV) configurations remain limited. OBJECTIVES: To compare the 30-day incidence of five discrete neurologic outcomes: ischemic stroke, hemorrhagic stroke, seizures, hypoxic-ischemic encephalopathy (HIE), and brain death, between adult VA and VV ECMO patients. METHODS: In this retrospective cohort study using the TriNetX federated network, adults under

Incidental Finding of an Asymptomatic Perielectrode Cyst After Deep Brain Stimulation for Parkinson's Disease.

Implanted intracerebral devices can lead to infection, hemorrhage, or the formation of fluid-filled cysts. Formation of a perielectrode cyst after deep brain stimulation (DBS) has been reported in a limited number of patients. Additionally, diverse causes and clinical presentations have been associated with these cysts. We present a case of perielectrode cyst formation three months after implantation of a subthalamic nucleus DBS (STN-DBS) for Parkinson's disease (PD). A 59-year-old woman was diagnosed with PD in 2011. In November 2023, the patient underwent staged placement of an STN-DBS witho

Impact of FXIII Levels on Hematoma Expansion and Outcomes in Patients with Spontaneous Intracranial Hemorrhage: A Prospective Observational Cohort Study of 132 Patients.

BACKGROUND: Diminished levels of FXIII are significantly correlated with hematoma expansion (HE) and unfavorable clinical outcomes. Although the crucial function of FXIII in coagulation is well-recognized, the connection between FXIII deficiency and either HE or negative prognosis in patients with spontaneous intracerebral hemorrhage (sICH) is not well understood. METHODS: Patients with sICH were confirmed through baseline computed tomography scans conducted within 6 h of symptom onset. Plasma FXIII levels were assessed at the time of admission using a standardized enzyme-linked immunosorbent

Predominantly mild thrombocytopenia, haematoma expansion and clinical outcomes in intracerebral haemorrhage.

INTRODUCTION: Evidence regarding the prognostic significance of thrombocytopenia in spontaneous intracerebral haemorrhage (ICH) remains limited and inconsistent. We investigated its associations with haematoma expansion (HE), functional outcomes and mortality. PATIENTS AND METHODS: We retrospectively analysed prospectively collected data from consecutive patients with spontaneous ICH in a single-centre registry between August 2022 and March 2025. Thrombocytopenia was defined as an admission platelet count <150 × 109/L. HE was defined as a haematoma-volume increase >6 mL or >33% on follow-up n
