Exiting Surveillance From Abdominal Aortic Aneurysm Screening: The Views of Clinicians, and Men in Surveillance and Their Family Members.

BACKGROUND: The NHS Abdominal Aortic Aneurysm (AAA) Screening Programme in England screens men aged 65. Men with small aneurysms enter annual surveillance. The current 'exit strategy' is to leave surveillance after 15 years if the aneurysm remains small. OBJECTIVES: The aim was to explore the views of clinicians, men in surveillance and their family members about exiting surveillance. DESIGN: A sequential study involving a Clinical Stakeholder Workshop to explore clinicians' views about factors that should be considered in any exit strategy, followed by a qualitative interview study to explo

Protocol for dorsal ovariectomy and elastase-induced abdominal aortic aneurysm in rats.

Women have increased abdominal aortic aneurysm (AAA) growth rates and a higher rupture risk, yet hormonal effects remain unclear. We present a protocol combining dorsal ovariectomy with elastase-induced AAA formation in female rats. We describe bilateral dorsal ovariectomy, a 42-day hormone depletion period, and ventral elastase infusion for AAA induction. This approach prevents adhesions encountered with traditional ventral techniques, increasing experimental feasibility and success. For complete details on the use and execution of this protocol, please refer to Sastriques-Dunlop et al.1.

Sirtuin 6 mitigates thoracic aortic aneurysm progression via maintenance of mitochondria homeostasis in vascular smooth muscle cells.

Progressive loss of vascular smooth muscle cells (VSMCs) is the pathophysiological basis for aortic aneurysm and dissection (AAD), a life-threatening disease, but the underlying mechanisms are largely unknown. Sirtuin 6 (SIRT6), a class III histone deacetylase, is critical for maintenance of VSMC homeostasis and prevention of vascular remodeling-related diseases. In this study, we investigated the role of VSMC SIRT6 in AAD and the molecular mechanism. We showed that the expression levels of SIRT6 were significantly reduced in VSMCs of the thoracic aorta in AAD patients. We constructed a VSMC-s

Automated quantification of abdominal aortic calcification using 3D nnU-Net: a novel approach to assess AAA rupture risk.

BACKGROUND: Abdominal aortic aneurysms (AAA) pose a serious rupture risk, heightened by aortic calcification. Traditional calcification scoring methods are slow and require expertise. This study aims to construct a convolutional neural network (nnU-Net) model for automatic quantification and segmentation of abdominal aortic calcification from a single CTA scan. METHODS: This retrospective study included 100 patients who underwent abdominal aortic CTA between January 2018 and October 2023, meeting specific inclusion criteria. Vessel and calcification segmentation were manually scored by two ph

Hemodynamic predictors of rupture in abdominal aortic aneurysms: a case series using computational fluid dynamics.

BACKGROUND: Abdominal aortic aneurysm (AAA) rupture is a life-threatening event traditionally predicted by aneurysm diameter. However, many clinical observations have revealed that rupture can occur even in small aneurysms, suggesting the influence of additional biomechanical factors such as hemodynamics. The aim of this case series was to perform computational fluid dynamics (CFD) analyses based on CT scans of patients with confirmed abdominal aortic aneurysm rupture and to evaluate correlations between rupture sites and hemodynamic factors derived from simulations. METHODS: This study analy

Delay in Surgery for Ruptured Aortic Aneurysm in Hemodynamically Stable Patients: A Multicenter Retrospective Analysis.

INTRODUCTION: Thoracic aortic aneurysm or abdominal aortic aneurysm (TAA/AAA) is a fatal surgical emergency, and time to surgery can be a key factor in improving survival outcomes in patients. In this study, we examined the association between systolic blood pressure on arrival and door-to-surgery time in patients with ruptured TAA/AAA, hypothesizing that patients with ruptured thoracic or abdominal aortic aneurysms without hypotension may have longer door-to-surgery times than those with hypotension. METHODS: This retrospective study was conducted at two community hospitals, Shonan Kamakura

EcoHIV Infection Promotes Atherosclerosis Progression in LDLR-Deficient Mice.

BACKGROUND: People with HIV are at higher risk of atherosclerotic cardiovascular disease than uninfected individuals; however, the molecular mechanisms behind this association remain elusive due to the lack of suitable animal models. METHODS: To study the impact of HIV on atherosclerotic cardiovascular disease, we infected the atheroprone Ldlr-/- mice with the chimeric virus EcoHIV. RESULTS: In comparison to uninfected controls, EcoHIV infection increased the ratio of circulating inflammatory monocytes, monocyte recruitment, and CD68+ content in the atherosclerotic lesion. These changes occu

Endoscopic Closure Combined With the Endoscopic Submucosal Dissection Technique and Over-the-scope Clip for Chronic Aortoesophageal Fistula: A Case Report.

Aortoesophageal fistula (AEF) is a rare but life-threatening condition. Initial management typically includes thoracic endovascular aortic repair (TEVAR) or aortic graft replacement to achieve hemostasis, followed by esophagectomy with aortic graft replacement and greater omentum wrapping to eliminate the source of infection. We report a case of successful endoscopic closure of a chronic esophageal fistula secondary to AEF. A 63-year-old man presented with hematemesis. He had a history of two ascending aortic replacements and one descending aortic replacement for aortic dissection. Computed to

Case Report: preoperative prediction of aneurysm rupture site using aortic morphological and biomechanical analysis validated by intraoperative imaging.

Prediction of aneurysm rupture has been a great challenge for decades. We report a successful rupture site prediction on a 97 mm abdominal aortic aneurysm (AAA). A 73-year-old man with an 11-year history of AAA presented to our outpatient clinic with a one-week history of hemoptysis. After undergoing multidimensional dynamic CTA imaging, the high rupture risk region was predicted through comprehensively inspecting the image-derived characteristics of biomechanics, morphology, and intraluminal thrombus distribution. Owing to financial difficulties, the patient declined treatment. The patient re

A Narrative Review of Preemptive Aortic Side Branch Embolization in Preventing Type II Endoleak after Endovascular Abdominal Aneurysm Repair.

Preemptive side branch embolization may help prevent type II endoleak, reduce reintervention rates, and promote early aneurysm sac shrinkage after endovascular aneurysm repair. However, evidence of its effectiveness in preventing aneurysm rupture, reducing aneurysm-related mortality, ensuring safety, and maintaining cost-effectiveness is limited. The 2024 European Society for Vascular Surgery guidelines do not recommend routine preemptive embolization due to a lack of high-quality evidence. Concerns about radiation exposure and financial costs remain unresolved. Further research is needed to i

A Review of Treatment for Type II Endoleak after Endovascular Abdominal Aortic Aneurysm Repair.

Type II endoleak is the most common complication after endovascular abdominal aortic aneurysm repair. Type II endoleak with aneurysm sac growth is not benign for long-term outcomes of endovascular abdominal aortic aneurysm repair and should be treated to prevent secondary stent graft-related complications and aneurysm rupture. The current consensus is to consider treatments for persistent type II endoleak with significant aneurysm sac growth. For complete embolization of type II endoleak to obliterate the endoleak cavity with the elimination of all supplying arteries, it is necessary to select

Anaesthesia management of endovascular aortic arch repair using the NEXUS TRE™ stent graft: A case report.

Endovascular aortic arch surgery is an evolving field that offers less invasive alternatives to open surgery. The emergence of new devices and their implantation imply new considerations in the management by the anaesthesiologist during the procedure. An example of this is the NEXUS TRE™ stent, designed to preserve the patency of the supra-aortic trunks without the need for extra-anatomic bypass. We present the case of a 75-year-old man diagnosed with an aortic arch aneurysm with accelerated growth and risk of rupture, who underwent endovascular repair of the aortic arch and its three branches

Use of Phantoms to Enhance Mechanical Knowledge About Aortic Aneurysms: A Systematic Review.

Aortic aneurysms represent a major public health issue, whose growth and rupture are driven by complex interactions between hemodynamics, wall mechanics, and biochemical factors. While the biological understanding of the arterial network has advanced, its mechanical behavior remains less explored. Experimental simulations, particularly those using arterial phantoms, are essential for approximating physiological conditions and improving biomechanical understanding, which may ultimately support a shift from statistical to individualized risk assessment. This systematic review aims to identify ke

Treatment of type IIIa endoleak and 7-year follow-up: case report.

Type III endoleaks are characterized by a problem with the endograft structure, such as fracture of the metallic structure, separation, or rupture. They constitute a rare complication, occurring in 2.1% of patients after treatment of abdominal aortic aneurysm by endovascular repair, with higher incidence in first and second generation endografts, and can occur early (after 30 days) or later. This type III classification is subdivided into IIIa -modular separation of components - and IIIb - mesh fracture or rupture involving the endograft. This case report describes an asymptomatic patient who

Assessing calcification effects in TEVAR procedures: a computational analysis.

Thoracic endovascular aortic repair (TEVAR) procedure is sometimes discouraged from clinical guidelines in the presence of calcifications and thrombus along the sealing zones. This computational study aims to understand which is the effect of calcification on stent graft displacement after TEVAR procedure, simulated in a patient-specific anatomy with a penetrating aortic ulcer (PAU).A patient-specific anatomy without calcification is taken as reference, and four models with idealized calcifications positioned in different regions and with different material properties are analyzed. Opening are
