[Hybrid surgery for acute iliofemoral artery thrombosis (case report)].

One of the main causes of acute arterial thrombosis of the lower extremity is an occlusive stenotic atherosclerotic vascular lesion. Thrombosis in this situation can develop on the background of the existing chronic limb ischemia, as well as in the absence thereof, manifesting as intense pain in the leg. The article presents a clinical case report regarding successful application of hybrid surgery for subacute thrombosis of the external iliac and common femoral arteries against the background of their atherosclerotic asymptomatic lesion. This method was chosen due to the exclusion of the embol

Relationship of the Novel Scoring System for Lower Extremity Venous Thrombosis with Pulmonary Embolism.

RATIONALE AND OBJECTIVE: To develop a new scoring system, the "Lower extremity venous Doppler ultrasound scoring system" (LEVDUS), to predict the diagnosis of pulmonary embolism (PE) localization in patients with deep vein thrombosis (DVT). METHODS: This single-center retrospective study included 182 patients aged ≥ 18 years. We used scoring according to thrombosis localization and stage in Doppler US. Patients with PE were divided into three categories based on the pulmonary artery (PA) location on CT pulmonary angiography. LEVDUS values were compared according to the PE classification. The

Can Routine Investigation for Occult Pulmonary Embolism Be Justified in Patients with Deep Vein Thrombosis?

PURPOSE: This study aims to investigate whether routine screening for silent pulmonary embolism (PE) can be justified in patients with deep vein thrombosis (DVT). MATERIALS AND METHODS: We retrospectively analyzed the medical records of 201 patients with lower-extremity DVT admitted to the vascular surgery department of a single tertiary university center between 2019 and 2023. All patients underwent clinical evaluation, basic laboratory exams, a whole-leg colored duplex ultrasound, and a computed tomography pulmonary angiography (CTPA), to screen for an occult, underlying PE. RESULTS: The o

Causal role of circulating inflammatory cytokines in cardiac diseases, structure and function.

BACKGROUND: Inflammation is implicated in cardiovascular disease (CVD) pathogenesis, but causal roles of specific circulating inflammatory cytokines remain unclear. Mendelian randomization (MR) studies are well-poised to provide etiological insights beyond constraints of conventional research. METHODS: We conducted a large-scale MR study to investigate potential causal relationships of 91 inflammatory proteins with CVD outcomes and cardiac remodeling using summary-level genetic data. Outcomes included coronary artery disease, myocardial infarction, stroke, atrial fibrillation, heart failure,

Mid-term Outcomes of Image-Guided Surgical Thromboembolectomy and Routine Intraoperative Angiography for Native Vessel Acute Lower-Limb-Threatening Ischemia.

BACKGROUND: Standard balloon-catheter thromboembolectomy (TE) is an established effective treatment for acute lower-limb ischemia (ALI) with recognized limitations when there is an underlying arterial lesion or thromboembolism of the infrapopliteal arteries. The aim of this study was to evaluate the efficacy and safety of image-guided surgical TE combined with routine intraoperative completion angiography in the treatment of ALI patients. METHODS: Between September 2020 and August 2022, this prospective study included all consecutive adult patients presenting to a tertiary center with unilate

Efficacy and safety of uterine artery embolization via the distal radial approach for uterine fibroids or adenomyosis: a single-center retrospective cohort study.

BACKGROUND: The standard approach for transarterial embolization of uterine fibroids or adenomas is via the femoral artery, but this approach limits the patient's quality of life and increases the risk of deep vein thrombosis in the lower extremities. We applied the distal radial approach technique for the treatment of uterine artery embolization, and aimed to explore the feasibility and safety of uterine artery chemoembolization through the distal radial approach. METHODS: We conducted a retrospective study at The First Hospital of Jilin University from January 1, 2021 to November 30, 2023.

Epidemiology, Etiology, and Pathophysiology of Pulmonary Embolism.

Over the last 20 years, there has been a progressive increase in the incidence of pulmonary embolism (PE) diagnosis in the United States, Europe, and Australia. Increased use of computed tomography pulmonary angiography has likely contributed in part to this rising incidence. However, it is pertinent to note that the burden of comorbidities associated with PE, such as malignancy, obesity, and advanced age, has also increased over the past 20 years. Time-trend analysis in North American, European, and Asian populations suggests that mortality rates associated with PE have been declining. The re

Recurrent Deep Vein Thrombosis (DVT) and Inferior Vena Cava (IVC) Filter: Should a Computed Tomography (CT) Venogram and Inferior Vena Cavagram Be the Standard of Care?

A pulmonary embolism (PE) is a life-threatening complication of deep vein thrombosis (DVT). Although timely anticoagulation is the first-line treatment for DVT, an inferior vena cava (IVC) filter can be considered when anticoagulation is contraindicated. Unfortunately, IVC filters come with complications of their own, including thrombus formation in or around the filter. An 89-year-old man with a past medical history of coronary artery disease, congestive heart failure, chronic obstructive pulmonary disease, and prior DVT status post IVC filter implantation five years ago in 2018 presented wit

Unveiling May-Thurner Syndrome in a Case of Recurrent Deep Venous Thrombosis With Bilateral Pulmonary Embolism.

May-Thurner syndrome (MTS) is a rare cause of deep venous thrombosis (DVT), characterized by the external compression of the left common iliac vein by the right common iliac artery against bony structures. Risk factors for MTS include female sex (postpartum, multiparous, and using oral contraceptive pills), spinal abnormalities like scoliosis, prior aortoiliac vascular stent placement, dehydration, and hypercoagulability. MTS patients with partial obstruction can be asymptomatic, but progression to extensive symptomatic DVT and/or chronic venous insufficiency can occur. MTS can be diagnosed by

Molecular Insight into Acute Limb Ischemia.

Acute limb ischemia (ALI) is defined as a sudden reduction in blood flow to a limb, resulting in cessation of blood flow and, therefore, cessation of the delivery of nutrients and oxygen to the tissues of the lower limb. Despite optimal treatment to restore blood flow to ischemic tissues, some patients may suffer from ischemia/reperfusion (I/R) syndrome, the most severe complication after a revascularization procedure used to restore blood flow. There are multiple molecular and cellular factors that are involved in each phase of ALI. This review focuses firstly on molecular and cellular factor

Editor's Choice - Comparison of Clinical Outcomes in Patients with Acute Lower Limb Ischaemia Undergoing Endovascular Therapy and Open Surgical Revascularisation: A Large Scale Analysis in Japan.

OBJECTIVE: The recommended revascularisation methods for acute limb ischaemia (ALI), which is caused by embolism and atherosclerotic thrombosis, include endovascular therapy (EVT) and open surgical revascularisation (OSR); however, treatment choices based on patient characteristics remain controversial. This retrospective analysis from the Japanese Registry of All Cardiac and Vascular Diseases - Diagnosis Procedure Combination database (April 2012 to March 2020) evaluated differences in clinical outcomes and identified prognostic predictors in patients with ALI. METHODS: This study analysed 1

Cardiac Arrest Due to Pulmonary Embolism Without Clinical Features of Deep Vein Thrombosis in a Young Female: A Rare Presentation of May-Thurner Syndrome.

Deep vein thrombosis (DVT) is a serious condition with a high disease burden. Pulmonary embolism is one of the disastrous complications of DVT. The etiology of DVT includes factors responsible for hypercoagulation, venous injury, and factors causing stasis in the deep veins. May-Thurner syndrome (MTS) is one of the rarely thought of causes of DVT. MTS is an anatomical variant where the right common iliac artery compresses the left common iliac vein against the lumbar vertebrae. This leads to thrombus formation and stenosis in the left common iliac vein at the site of cross-over, resulting in a

Pulmonary artery in situ thrombosis due to patent ductus arteriosus: a case report.

BACKGROUND: Pulmonary Artery in situ Thrombosis (PAIST) refers to a thrombus forming within the pulmonary arterial system, distinct from an embolus originating from elsewhere in the body (e.g., the deep veins of the lower extremities) and traveling to the lungs where it lodges and forms. CASE PRESENTATION: We present a case of PAIST caused by the arterial ductus arteriosus. The patient primarily presented with dyspnea, and the chest pain dichotomy Computed Tomography Angiography(CTA) suggested that a nodular low-density filling defect was seen in the lumen of the left pulmonary artery trunk.

Vascular Ultrasound.

Vascular point-of-care ultrasound is a useful tool for emergency department (ED) clinicians to evaluate lower extremity pain and swelling. It can quickly and safely detect deep vein thrombosis, a serious condition that can lead to pulmonary embolism and post-thrombotic syndrome as well as diagnose pseudoaneurysm, hematoma, and acute arterial occlusion, a vascular emergency that requires urgent surgical intervention. Point-of-Care Ultrasound can be a useful tool for the evaluation of ED patients with vascular emergencies.

Isolated Trochlear Fracture in an Elderly Lady : A Rare and Interesting Case Treated in a Rural Coal Mines Hospital.

INTRODUCTION: Major orthopedic procedures place patients at risk for Deep venous thrombosis (DVT) and pulmonary embolism (PE). DVT has a 10-40% incidence after isolated fractures of the tibia and distal bones of the lower extremity. Diagnostic techniques are venous compression ultrasonography, venography, and pulmonary angiography. Prevention methods for venous thromboembolism (VTE) include mechanical prophylaxis and pharmacological prophylaxis. Inferior vena cava filter (IVCF) intercepts thrombus in inferior vena cava and prevents it from reaching the pulmonary artery. CASE REPORT: A 39-year

Post-operative Saddle Pulmonary Embolism: A Case Report.

Pulmonary embolisms are serious complications that can arise from surgical procedures involving the extremities due to the risk of deep vein thrombosis (DVT) and subsequent embolism. This life-threatening condition occurs when an embolus lodges at the bifurcation of the main pulmonary arteries, compromising blood flow to the lungs. Treatment options for pulmonary embolism primarily include anticoagulation therapy, thrombolysis, thrombectomy, and inferior vena cava (IVC) filter placement. Given that cases of saddle pulmonary embolism are rare but potentially fatal, healthcare providers must mai

A Review of the Effectiveness and Safety of Catheter-Directed Thrombolysis for Venous Thromboembolism.

Venous thromboembolism (VTE), including deep venous thrombosis (DVT) and pulmonary embolism (PE), is a major cause of morbidity, mortality, and elevated healthcare costs. Between 20% and 50% of patients with lower extremity deep vein thrombosis will develop an advanced complication called post-thrombotic syndrome (PTS) within 2 years. Early mortality rates for DVT and acute high-risk pulmonary embolism are 3.8% and 38.9%, respectively. Additionally, PE is the most common and preventable cause of in-hospital death. Early removal of thrombus and improvement of vascular function can reduce the in

Assessing the Impact of Peripheral Artery Disease on Total Knee Arthroplasty Outcomes.

BACKGROUND: Total knee arthroplasty (TKA) is a crucial orthopaedic procedure used to relieve pain from knee joint degeneration. Concurrently, peripheral artery disease (PAD) presents a major challenge, complicating orthopaedic interventions, particularly TKA, due to its impact on vascular health. Despite advances in surgical techniques and care, patients who have PAD undergoing TKA face heightened risks. Our retrospective study aimed to assess the impact of PAD on TKA outcomes. METHODS: This retrospective cohort study drew data from a healthcare database platform from January 1, 2003, to Janu
