Pheochromocytoma crisis combined with median arcuate ligament syndrome: A rare case report.

RATIONALE: Left ventricular thrombus caused by acute catecholamine cardiomyopathy combined with pheochromocytoma crisis (PC) is extremely rare in clinical practice, and there are no reports of patients with median arcuate ligament syndrome (MALS) at the same time. This report describes the case of a 45-year-old woman admitted to the cardiology department with this rare condition. The patient presented with severe abdominal pain, vomiting, rapid blood pressure (BP) fluctuations, and recurrent hypotensive syncope. We discuss the patient's diagnosis and treatment and share our experience with thi

Effect of Percutaneous Transluminal Balloon Angioplasty on Inflammatory Biomarkers in Patients with Peripheral Arterial Disease.

BACKGROUND: Peripheral artery disease (PAD) is characterized by chronic inflammation and endothelial dysfunction. Percutaneous transluminal balloon angioplasty (PTA) restores limb perfusion, but its effects on systemic inflammatory burden are not fully defined. The objective of the study was to evaluate the effects of PTA on 30-day inflammatory biomarkers and hematological parameters in patients with lower extremity PAD. METHODS: In this prospective observational study, 47 adults with newly diagnosed PAD (lower extremity stenosis ≥50%, ankle-brachial index [ABI] <0.90) underwent PTA. Venous b

Computational analysis of blood flowing through an artery with post stenotic dilatation by implying Sisko model parametric approach.

This study looks into the intricate dynamics of blood flow within a blood vessel with both stenosis and dilatation. Unlike previous studies, blood is modeled as a Sisko non-Newtonian fluid, departing from the traditional Newtonian assumption, in order to accurately depict its non-Newtonian behavior. The Sisko model is a flexible tool for hemodynamic research since it is used for a broad variety of arterial circumstances, such as different dilation geometries and degrees of stenosis. To investigate the effects of changing the Sisko fluid properties on blood flow through the vessel, thorough par

Updated Systematic Review and Meta-Analysis of Duct-to-duct Versus Hepaticojejunostomy Reconstruction After Liver Transplantation for Primary Sclerosing Cholangitis.

Roux-en-Y (RY) and duct-to-duct (DD) anastomosis are 2 techniques for biliary reconstruction in orthotopic liver transplantation (OLT), but their effectiveness in patients with primary sclerosing cholangitis (PSC) remains debated. This meta-analysis compares outcomes of DD versus RY reconstruction in OLT for PSC. A systematic review identified studies comparing these surgical approaches. Primary outcomes were postoperative biliary complications, while secondary outcomes included graft and patient survival, cholangitis, cholangiocarcinoma, recurrent PSC, hepatic artery thrombosis, and need for

Arterial and biliary complications after transarterial chemoembolization for hepatocellular carcinoma.

Transarterial chemoembolization is a common treatment modality that significantly improves prognosis in patients with intermediate-advanced hepatocellular carcinoma. However, this procedure is associated with a spectrum of potential arterial and biliary complications, ranging from mild self-limiting ones to those severely affecting patient outcomes. This review systematically integrates recent studies to explore the epidemiological characteristics, risk factors, and management strategies of these two groups of complications. Arterial complications, primarily hepatic artery stenosis, pseudoaneu

Observation of the Angioplasty Effect of Stent-Retriever Thrombectomy in Acute Intracranial Atherosclerotic Artery Stenosis: A Case Series Analysis.

Intracranial atherosclerotic stenosis (ICAS) is one of the most common causes of ischemic stroke worldwide, and endovascular treatment is the primary therapeutic approach, but the clinical recanalization rate is low. Clinical practice has revealed the occurrence of an angioplasty effect following endovascular treatment, which may explain the low recanalization rate. Four patients with ICAS who underwent endovascular therapy were included in this retrospective case series. Demographic, medical histories, treatment, and outcome data were extracted. After undergoing stent-retriever thrombectomy,

Celiac Axis Stenosis in Pancreatoduodenectomy: Preoperative Predictors for Intervention Considering Stenosis Severity and Collateral Pathway Development.

BACKGROUND: Celiac axis stenosis (CAS) is a significant vascular condition in patients undergoing pancreatoduodenectomy due to division of the celiac circulation during the procedure. However, the reliable conditions for when to intervene remain undefined. METHODS: We retrospectively analyzed 1,042 consecutive patients who underwent pancreatoduodenectomy. The stenosis diameter and maximum diameter of the celiac axis were measured on preoperative multidetector CT to calculate the stenosis rate (SR). In the patients with CAS (SR ≥ 50%), two additional CT-based markers of arcade development were

A Statistical Shape Modeling Approach for the Derivation of a Data-Driven Geometry-Aware Lumped Arterial Stenosis Model.

Existing lumped arterial stenosis models struggle to accurately capture the pressure and flow relationship of complex lesion morphologies, thereby limiting their ability to accurately evaluate lesions. To overcome these limitations, we introduce a geometry-informed, data-driven lumped stenosis model that incorporates realistic lesion shapes using statistical shape modeling (SSM). By generating a large dataset of synthetic coronary stenoses, hence focusing on epicardial lesions, and evaluating them through high-fidelity 3D computational fluid dynamics (CFD), we derived reference pressure drops

Hepatic Artery Atherosclerosis Is a Risk Factor for Biliary Strictures After Liver Transplantation.

BACKGROUND AND AIMS: The aging of liver donors and transplant candidates, along with the growing prevalence of metabolic comorbidities, may lead to hepatic artery atherosclerosis. Since the biliary epithelium relies on arterial flow, hepatic artery atherosclerosis could increase the risk of biliary strictures after liver transplantation. METHODS: Cohort study including all adult patients undergoing liver transplantation in a single institution between 2012 and 2023 who had a blinded pathological evaluation of the donor and recipient hepatic artery to diagnose perianastomotic atherosclerosis.

Nomogram for predicting in-stent stenosis after pipeline embolization device treatment in patients with unruptured intracranial aneurysm: a multicenter model development and validation study.

BACKGROUND: Some intracranial aneurysms (IAs) still develop in-stent stenosis (ISS) even after successful pipeline embolization device (PED) implantation. ISS increases the risk of retreatment and ischemic complications, thereby affecting the long-term prognosis of IA patients. This study aims to identify predictors for ISS after PED treatment of IAs, and develop a nomogram for assessing individual risk. MATERIALS AND METHODS: This analysis included unruptured IA patients treated with PEDs between April 2016 and October 2023 at three institutions. The patients were grouped into the training c

Hemodynamics-driven magnetoelastic vascular grafts for stenosis diagnosis.

Conventional approaches for vascular graft stenosis diagnostics, including X-ray angiography, magnetic resonance imaging and Doppler ultrasound, although highly accurate, are cumbersome, used intermittently and often do not detect stenosis early enough, leading to diagnosis only after substantial narrowing. Here we report a magnetoelastic vascular graft (MVG) for post-implantation stenosis diagnosis that is hemodynamics-driven, biocompatible and waterproof. It enables wireless, real-time and continuous diagnosis of stenosis by converting arterial hemodynamics into high-fidelity electrical sign

Why do so many in academic health care tend to assume that subspecialty surgical training begets higher quality health care than general pediatric surgical training?

BACKGROUND: The author spent all his attending level practice in general hospitals, resulting in early exposure to developing technologies, allowing early adaptation. ILLUSTRATIVE EXAMPLES: The adaptation of balloon dilation of arterial stenoses to treat esophageal strictures in children, and the use of MRI in anorectal malformations. RESULTS: Balloon dilatation of esopageal strictures and MRI assessment of anorectal anomalies became routine care. CHANGES IN ETHICAL LANDSCAPE: Both of these innovations were discussed with the Ethics Committee. Neither were subjected to further ethical overs

Traumatic Urethral Pseudoaneurysm in a 12-Year-Old Boy Leading to Progressive Urethral Stricture and Complete Obstruction: A Case Report and Literature Review.

Urethral pseudoaneurysm is an extremely rare cause of urethrorrhagia in children. Most previously reported pediatric cases required transcatheter arterial embolization and generally resolved without long-term sequelae. We present a 12-year-old boy who sustained blunt perineal trauma and subsequently developed a bulbar urethral pseudoaneurysm requiring two sessions of arterial embolization. Unlike prior reports, the patient developed progressive circumferential urethral stricture leading to complete obstruction five months after initial injury. To our knowledge, this is the first reported pedia
