Sequential or Simultaneous Portal-Arterial Reperfusion During Liver Transplantation: A Retrospective Study.

BACKGROUND: Ischemia-reperfusion injury is a relevant complication in liver transplantation. Sequential (SeqR) and simultaneous reperfusion (SimR) techniques are the most commonly used strategies to reduce this injury; however, there is still no consensus regarding which technique provides better outcomes. This study compares these techniques in terms of graft recovery, complications, and patient survival. MATERIALS AND METHODS: This retrospective study included 872 patients who underwent liver transplantation, divided into 2 groups according to the reperfusion technique used. Patient charact

Overall Outcomes of Liver Transplantation Using Upfront Normothermic Machine Perfusion: A Real-World Single-Center Experience.

OBJECTIVE: To determine whether normothermic machine perfusion (NMP) mitigates the historical survival disadvantage of donation after circulatory death (DCD) liver transplantation compared with donation after brain death (DBD) livers. SUMMARY BACKGROUND DATA: NMP has improved outcomes in DCD LT compared with static cold storage (SCS), but whether it eliminates the historical disparity relative to DBD transplantation remains uncertain. METHODS: We performed a retrospective cohort study of adult LT at a high-volume US center between 2019 and 2024. Outcomes of DCD-NMP grafts were compared with

Refractory Ascites After Liver Transplantation due to Splenic Artery Steal Syndrome With Resolution After Splenectomy.

Postliver transplant ascites occurs in 5%-7% of recipients. This report describes splenic artery steal syndrome, causing refractory post-transplant ascites and ischemic biliopathy requiring splenectomy. A 65-year-old man with alcohol-related cirrhosis underwent orthotopic liver transplantation (Model for End-Stage Liver Disease 12) with pretransplant splenomegaly and splenic-artery dilatation. Postoperatively, computed tomography demonstrated persistent ascites, splenic-to-hepatic artery ratio of 2.5, and reduced hepatic arterial perfusion. Endoscopic retrograde cholangiopancreatography identi

Hypothenar hammer syndrome: An expanded Kaji angiographic classification based on 91 cases.

BACKGROUND: Hypothenar hammer syndrome (HHS) is characterized by ischemic symptoms of the hand related to a stenotic or occluded ulnar artery. The disease is considered rare, although there is evidence of underdiagnosis. Digital subtraction angiography (DSA) is the primary diagnostic tool, although staging is typically based on the Kaji classification, which captures most abnormalities, though some may be overlooked. This study proposes an all-encompassing angiographic classification system for HHS. METHODS: In this multicenter retrospective case series, electronic medical records were search

Robotic kidney autotransplantation with extracorporeal arterial and venous reconstruction for complex ureteral stricture: a technical note.

INTRODUCTION: Kidney autotransplantation is an organ-preserving option for selected patients with complex ureteral or renovascular pathology not amenable to in situ reconstruction. Conventional open autotransplantation, however, is associated with substantial morbidity. Robotic kidney autotransplantation (RKAT) has emerged as a minimally invasive alternative, but detailed technical reports of complex cases requiring vascular reconstruction remain limited. TECHNICAL DESCRIPTION: We report the technique of RKAT in a 32-year-old woman with a radiation-induced high-grade ureteral stricture refrac

Endoscopic Ultrasound-Guided Endotherapy for Visceral Artery Pseudoaneurysm: A Case Report and Systematic Review of the Literature.

Visceral artery pseudoaneurysms (VAP) develop following inflammatory pathology or trauma. Endovascular management remains the gold standard as per the Cardiovascular and Interventional Radiological Society of Europe (CIRSE) standards. However, when endovascular therapy is not feasible, other modalities like percutaneous or endoscopic ultrasound (EUS)-guided embolization have also been utilized. Lately, EUS-guided procedures have been reported increasingly in the literature. We report a case of post-laparoscopic cholecystectomy cystic artery pseudoaneurysm complicated by associated biliary inju

Retinal and Choroidal Vascular Metrics Predict Cerebral Atrophy and Cognitive Impairment in Noninfarcted Large Artery Stenosis.

PURPOSE: Large artery stenosis (LAS) can drive cognitive impairment and neurodegeneration even without overt infarction, yet scalable biomarkers for capturing this silent vascular brain injury are limited. We investigated whether retinal and choroidal microvascular and neurostructural measures reflect cerebral atrophy and cognitive impairment in noninfarcted LAS. METHODS: In a multicenter cohort of 1239 participants (1035 patients with noninfarcted LAS and 204 community-based controls), we integrated optical coherence tomography/angiography (OCT/OCTA), brain magnetic resonance imaging volumet

Outcomes of Pediatric ABO-Incompatible Living Donor Liver Transplantation: A Retrospective Single-Centre Experience from India.

BACKGROUND/AIMS: ABO incompatible liver transplant (ABOi-LT) is an essential alternative to blood-group-compatible liver transplant (LT) in children, and there is limited experience with this, especially from the developing world. The current analysis focuses on our experience of ABOi-LT in children. METHOD: A retrospective analysis of all children undergoing ABOi-LT at our centre from January 2011 to February 2024 was performed. Their clinical profile, desensitisation protocol, post-transplant complications, patient and graft survival were analysed. RESULTS: We performed 35 (5%) ABOi-LTs du

Computational Multieffect Analysis of Blood-Based Trihybrid Nanofluid Flow With Nanoparticles Through a Stenotic Artery.

This study is aimed at examining the computational modeling of the blood flow of a trihybrid nanofluid with nanoparticles such as Ag, Au, and Cu to simulate the hemodynamics parameters and heat transfer within a constricted channel geometry that directly parallels the flow conditions in a stenosed artery. The flow is defined as an unsteady, incompressible, laminar Newtonian fluid, with blood exhibiting Newtonian properties at elevated shear rates. An algorithm for mass, velocity, and energy, as well as a fine element-sized mesh, was developed using the computational fluid dynamics (CFD) techni

Unmasking of a missed right hepatic artery pseudoaneurysm by biliary decompression following Roux-en-Y hepaticojejunostomy for Strasberg type E3 bile duct injury.

Bile duct injuries (BDI) after cholecystectomy often coexist with vascular injuries, but hepatic artery pseudoaneurysms may be missed on initial imaging. We report a right hepatic artery (RHA) pseudoaneurysm that manifested as haemobilia only after biliary decompression by Roux-en-Y hepaticojejunostomy for a Strasberg type E3 injury. A 28-year-old man presented 4 weeks after open cholecystectomy with obstructive jaundice. Magnetic resonance imaging confirmed a Strasberg type E3 stricture and identified a missed 8-mm RHA pseudoaneurysm. Following hepaticojejunostomy, haematemesis occurred on po

Risk Factors of Arteriovenous Fistula Stenosis in Hemodialysis Patients Using Electronic Stethoscope Monitoring: Prospective Pilot Cohort Study.

BACKGROUND: Arteriovenous fistula (AVF) stenosis is an important complication of hemodialysis that may impair dialysis adequacy and patient outcomes. Conventional AVF monitoring relies largely on subjective physical assessment and may delay recognition of abnormal vascular access findings. OBJECTIVE: This prospective pilot study aimed to assess the feasibility of serial electronic stethoscope monitoring for detecting abnormal AVF sounds before routine nursing assessment, and explore factors associated with a high-risk AVF phenotype, defined as percutaneous transluminal angioplasty within the

Association of one-hour postload plasma glucose combined with metabolic dysfunction-associated steatotic liver disease and FIB-4 screening positivity with elevated LDL cholesterol and cerebrovascular stenosis.

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) is highly prevalent in type 2 diabetes mellitus (T2DM) and amplifies cardiovascular and metabolic burden. Elevated low-density lipoprotein cholesterol (LDL-C) is a key driver of atherosclerotic cerebrovascular stenosis, and MASLD-mediated hepatic lipid dysregulation is closely linked to LDL-C metabolism. One-hour postload plasma glucose (1h-PG) reflects early beta-cell dysfunction and may synergistically contribute to cerebrovascular injury alongside the MASLD/FIB-4 phenotype. METHODS: This single-center retrospectiv

Auxiliary Partial Orthotopic Liver Transplantation in Children Under 10 Years Old With Fulminant Hepatic Failure: A Multicenter Experience.

BACKGROUND AND AIMS: Auxiliary partial orthotopic liver transplantation (APOLT) for fulminant hepatic failure (FHF) allows potential withdrawal of lifelong immunosuppression (ISP) following native liver regeneration, which may be advantageous in young children with high regenerative capacity. Despite these potential benefits, APOLT has been rarely used, and long-term outcomes in children under 10 years remain unclear. This study aimed to evaluate perioperative safety, long-term outcomes, and native liver regeneration rates in children under 10 years undergoing APOLT for FHF at major centers in

Plaque Enhancement Grade on High-Resolution MRI and Recurrent Ischemic Events in Intracranial Stenosis: A Retrospective Cohort Study.

Recurrent ischemic events may occur in patients with mild-to-moderate intracranial atherosclerotic stenosis despite standardized medication; stenosis rate alone is insufficient for risk stratification, and plaque activity imaging markers are needed. Because plaque enhancement may reflect plaque activity beyond luminal narrowing, we hypothesized that higher enhancement grade would predict recurrent ischemic events of stenosis rate. This study evaluated the association between high-resolution magnetic resonance imaging (HR-MRI) plaque enhancement grading and recurrent ischemic events and examine

FLAIR vascular hyperintensity: association with infarction and vascular lesion burden in patients with middle cerebral artery stenosis.

OBJECTIVES: Fluid-attenuated inversion recovery (FLAIR) vascular hyperintensity (FVH) is a marker of hemodynamic impairment linked to poor outcomes. However, its underlying vascular pathology remains unclear. We aimed to evaluate the relationships between FVH, infarction, and vascular lesion burden, and explore whether infarction and vascular lesion burden can discriminate FVH presence and burden. MATERIALS AND METHODS: A total of 253 consecutive patients with middle cerebral artery lesions who underwent vessel wall imaging were retrospectively enrolled. Multivariate logistic regressions were

Implications of Cranial Arterial Stenosis and Dolichoectasia for Cerebral Small-Vessel Disease Etiopathogenesis: Findings From a Prospective Mild Stroke Cohort.

BACKGROUND: Stenosis and dolichoectasia of cranial arteries likely reflect distinct mechanisms. Their contributions to lacunar stroke and cerebral small-vessel disease (cSVD) remain contentious. We investigated the associations of large-artery stenosis (LAS) and arterial widening with stroke subtype, cSVD markers, incident infarcts, and clinical outcomes. METHODS: We prospectively recruited patients with lacunar or mild nonlacunar stroke, with demographic, stroke-related, cognitive, functional, and magnetic resonance imaging (index and incident infarcts, cSVD markers) assessments at baseline

Effect of Stenosis Severity on Outcomes After Rescue Stenting for Acute Middle Cerebral Artery Occlusions: a Real-world Multicenter Analysis.

PURPOSE: Rescue stenting (RS) can achieve durable recanalization in cases of acute large vessel occlusion due to underlying intracranial artery stenosis (ICAS), but its clinical effects may be influenced by procedural factors. This study aimed to evaluate whether the severity of stenosis affects the outcomes after RS. METHODS: In this multicenter retrospective study, patients with acute middle cerebral artery occlusion and underlying ICAS were divided into two groups based on the treatment they received: mechanical thrombectomy (MT) + RS (n = 172) or MT-only (n = 131).
