Spontaneous Remission of Late Relapsing Membranous Nephropathy After Previous Immunosuppressant-Induced Remission: Case Report.

This is the first detailed report of spontaneous remission of late relapsing membranous nephropathy (MN) after previous immunosuppressant-induced remission. In March 2004, a 52-year-old Hispanic male presented with severe nephrotic syndrome with proteinuria of 13 g/24 h. The workup for infectious and autoimmune diseases was unrevealing. Renal biopsy showed MN (Stage II). A CT scan showed no malignancy but left pulmonary embolism and left renal vein thrombosis. Anticoagulation was started for asymptomatic thromboembolism. He achieved sustained complete remission of nephrotic syndrome after a pr

Unprovoked Pulmonary Embolism with Bilateral Deep Vein Thrombosis Revealing Occult Nephrotic Syndrome Due to Seronegative Membranous Nephropathy.

INTRODUCTION: Nephrotic syndrome is a well-recognised risk factor for venous thromboembolism (VTE) but may present without oedema, hyperlipidaemia or impaired kidney function, delaying diagnosis. CASE DESCRIPTION: A 73-year-old man presented with fatigue, exertional dyspnoea and weight loss over a period of three months. Computed tomography pulmonary angiography showed bilateral central pulmonary emboli. Cardiac biomarkers were elevated with preserved renal function. Therapeutic anticoagulation was initiated. To assess thrombus extent and guide management, lower limb compression ultrasonograp

Rising Trends and Safety of Outpatient Robotic Partial Nephrectomy: A Propensity-Matched National Analysis.

Introduction Minimally invasive partial nephrectomy (MIPN), laparoscopic or robotic partial nephrectomy (RPN), is the preferred treatment for T1 renal cell carcinoma (RCC). As outpatient surgery expands across urology, its safety and feasibility in oncologic procedures, like MIPN and especially RPN, remain underexplored. Methods Using American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) data (2022-2023), we identified 8,927 adult patients who underwent RPN. Trends in outpatient MIPN, both laparoscopic and robotic approaches, were observed from 2019 to 2023. Pa

Breaking the blockage by advancing treatment strategies for upper extremity deep vein thrombosis.

BACKGROUND: Catheter-directed thrombolysis (CDT) and percutaneous mechanical thrombectomy (PMT) are established endovascular strategies for the treatment of deep vein thrombosis (DVT). However, comparative outcomes data specific to upper extremity DVT (UEDVT) remain limited. Given that PMT is associated with improved outcomes in lower extremity DVT, we hypothesized that PMT would be associated with improved mortality and morbidity compared with CDT in treating UEDVT. This study evaluates outcomes associated with CDT vs PMT for UEDVT and Paget-Schroetter syndrome (PSS). METHODS: A retrospectiv

Outcomes of total hip arthroplasty in males undergoing testosterone replacement therapy: A propensity-matched analysis.

BACKGROUND: Testosterone replacement therapy (TRT) use has risen among older men, including those undergoing total hip arthroplasty (THA). While TRT may influence general musculoskeletal health and surgical outcomes, its relationship with postoperative outcomes following THA remains unclear. This study aimed to evaluate the potential association between preoperative TRT use and postoperative medical and surgical outcomes in male patients undergoing primary THA using a propensity-matched analysis. METHODS: The TriNetX database was queried for male patients undergoing primary THA. Patients with

Evaluation of six-month clinical outcomes of patients with venous thromboembolism treated with apixaban, rivaroxaban, edoxaban or dabigatran.

BACKGROUND: The advantages of direct oral anticoagulants (apixaban, rivaroxaban, edoxaban, dabigatran) over warfarin and low molecular weight heparin have been demonstrated in many studies in the treatment of patients with acute venous thromboembolism (VTE). However, a single-centre study comparing the effectiveness and safety of oral anticoagulants against each other has not yet been conducted. AIM: The study aimed to compare the results of the efficacy and safety profiles of direct oral anticoagulants (DOACs). The purpose of this study was to compare the effectiveness and safety profiles of

Tranexamic acid in high-risk shoulder arthroplasty patients: safety across thromboembolic, cardiac, renal, and neurologic risk profiles.

INTRODUCTION: Tranexamic acid (TXA) effectively reduces blood loss and transfusion requirements in shoulder arthroplasty. However, concerns regarding thromboembolic, neurologic, cardiac, and renal complications have limited its use in medically high-risk patients. This study evaluated temporal trends and the safety of TXA in high-risk patients undergoing total shoulder arthroplasty (TSA). METHODS: A retrospective cohort study was conducted using TriNetX, identifying patients who underwent shoulder arthroplasty (2012-2025). Patients were stratified by preexisting high-risk conditions, includin

A Rare Case of Multi-Organ Complications Due to Disseminated Citrobacter koseri Infection Originating From a Renal Abscess.

Citrobacter koseri is a gram-negative bacillus that can cause severe infections in immunocompromised individuals. While it is a rare urinary tract pathogen, its potential for dissemination and severe complications demands attention. We report the case of a 44-year-old male with poorly controlled type 2 diabetes mellitus who developed a disseminated C. koseri infection originating from a perirenal abscess. The infection extended locally, resulting in septic thrombosis of the left renal vein, and spread distantly, causing septic pulmonary embolism. The patient underwent multiple surgical interve

Intravenous versus oral tranexamic acid in shoulder arthroplasty.

PURPOSE: Tranexamic acid (TXA) is used in shoulder arthroplasty to reduce blood loss. While intravenous (IV) TXA is standard, oral TXA offers advantages in cost and administration. This study compares 90-day complications and postoperative laboratory values between IV and oral TXA in total shoulder arthroplasty (TSA). METHODS: A retrospective cohort study using TriNetX identified adults undergoing primary TSA (2005-2025) receiving TXA on the day of surgery, stratified by route (IV vs. oral). One-to-one propensity score matching was performed. Ninety-day outcomes included transfusion, thromboe

Diagnostic Management of Asymptomatic Pulmonary Embolism: A Predictive Model Integrating Inflammatory Markers and Clinical Characteristics in Patients With Deep Vein Thrombosis.

BackgroundPulmonary embolism (PE) may be asymptomatic due to the compensatory function of the lungs themselves, which easily leads to missed diagnosis and increased risks. Although computed tomography pulmonary angiography (CTPA) can improve the detection rate, routine CTPA screening for patients with concurrent lower extremity deep vein thrombosis (DVT) still faces challenges. These include limited medical resources, restricted use of contrast agents (e.g., allergies, renal function issues), and low specificity of D-dimer. Therefore, there is a need to develop optimized diagnostic strategies

Differentiation of venous thrombus consistency in patients with renal cell carcinoma using volumetric analysis of computed tomography first-order radiomic features.

BACKGROUND: Renal cell carcinoma (RCC) has a unique tendency to invade the inferior vena cava through the renal vein, forming a venous tumor thrombus (VTT). Precisely determining the solidity of the VTT before surgery is crucial for appropriate treatment planning and optimal outcomes, as the perioperative spread of friable thrombus can lead to life-threatening complications like pulmonary embolism. The aim of the study was to assess the applicability of CT volumetric analysis to differentiate VTT consistency in patients with RCC. METHODS: Twenty-eight patients with RCC and VTT were enrolled in

Complicated IVC Filter Placement in a Patient With Venous Abnormality: A Case Report.

As a serious, prevalent, and potentially fatal condition, deep venous thrombosis (DVT) results in a huge healthcare-related and socioeconomic burden. On the other hand, the placement of an inferior vena cava (IVC) filter is one of the last options to prevent the most dangerous complication of DVT, pulmonary embolism (PE). The patient was a 35-year-old man admitted to the hospital for dyspnea on exertion (NYHA class II) and lower extremity swelling. He had a history of pin implantation in his right leg and a recent history of DVT in his left calf and thigh, which was treated with warfarin. The

Long-term follow-up monitoring with multimodal ultrasound in a case of suspected renal angiomyolipoma with renal vein/inferior vena cava thrombus: a case report.

Surgical intervention is typically recommended for renal angiomyolipoma (AML) complicated by inferior vena cava (IVC) thrombus, given the potential risk of cardiopulmonary embolism. However, the natural history and corresponding imaging progression in such cases are not well understood. We describe a female patient with an imaging-based diagnosis of renal AML and an associated IVC tumor thrombus (Mayo Grade II), who presented to our center in December 2021. The initial suspicion raised by renal ultrasound (US) was subsequently confirmed via contrast-enhanced abdominal computed tomography (CT).

A rare case of malignant epithelioid angiomyolipoma: involvement of the lumbar vein pathway with downward extension to the iliac veins.

INTRODUCTION AND IMPORTANCE: Epithelioid angiomyolipoma (EAML) is a rare renal perivascular epithelioid cell tumor (PEComa) with malignant potential in up to one-third of cases. Venous invasion usually involves the renal vein or inferior vena cava, while extension through the lumbar venous pathway is exceedingly uncommon. CASE PRESENTATION: We describe a 51-year-old woman with malignant EAML presenting with a left renal mass and extensive tumor thrombus extending via the lumbar venous complex into the bilateral iliac veins. Preoperative CT angiography demonstrated collateral venous drainage,

A novel endovascular thermal ablation technique for pelvic venous disorders via basilic vein access: A prospective descriptive study.

BACKGROUND: Pelvic venous disorders causing pelvic congestion syndrome are a common, underdiagnosed source of chronic pelvic pain. Standard coil/sclerosant embolization may be limited by migration, incomplete occlusion, postembolization syndrome, symptom recurrence, and cost. Endovenous thermal ablation (EVLA) could provide precise, device-free closure with faster recovery and lower cost. OBJECTIVE: We evaluated the feasibility, safety, and early efficacy of basilic access 1470-nm EVLA for refluxing gonadal veins in women with pelvic venous disorders. METHODS: Prospective, single-center desc

Retained Guidewire Following Bedside Femoral Venous Catheterization: A Case Report.

Retained guidewire after central venous catheterization (CVC) placement is a rare but preventable complication. Despite advances in ultrasound-guided catheterization and procedural safety, guidewire retention continues to be reported and may result in vascular injury, thrombosis, embolization, arrhythmia, or infection. We report the case of a 62-year-old male who underwent ascending aorta and total arch replacement for acute type I aortic dissection. On postoperative Day 11, a hemodialysis catheter was inserted into the right femoral vein under ultrasound guidance for continuous renal replacem
