Lymphedema therapy and wound healing: a retrospective analysis of pretreatment and posttreatment wound volume.

BACKGROUND: Venous leg ulcers (VLUs) frequently exhibit prolonged healing times, particularly in the presence of lower extremity lymphedema. One approach to managing VLU is to combine traditional wound care with lymphedema therapy (LT). Despite recognition of compression as an effective agent for management of chronic wounds, use of lymphedema services remains insufficient among health care providers. OBJECTIVE: To investigate the effectiveness of LT for tissue repair and examine the effect of various factors on wound healing. MATERIALS AND METHODS: Electronic medical records of patients wit

Intermittent pneumatic thigh compression for the treatment of lower limb wounds (IPCOTT): a randomised control trial.

OBJECTIVE: Hard-to-heal (chronic) venous leg ulcers (VLUs) are challenging to heal with standard of care (SoC) alone. This study evaluated whether adding a thigh-administered intermittent pneumatic compression (IPC) device (WoundExpress, Huntleigh Healthcare Ltd., UK) to SoC improved outcomes in patients with hard-to-heal venous or mixed leg ulcers (venous and arterial), was suitable for home use and was safe. METHOD: A multicentre, pragmatic, open-label, randomised controlled trial (RCT) was conducted across the UK, US, Germany and France. Adults with hard-to-heal lower limb VLUs and mixed u

Heel-Free Compression Bandaging for Venous Leg Ulcers: A Non-Inferiority Trial of Interface Pressure.

To determine whether heel-free compression bandaging is non-inferior to heel-wrapped bandaging for maintaining interface pressure in adults with venous leg ulcers. Randomised, controlled, parallel-group, non-inferiority trial. Adults with open venous leg ulcers (CEAP C6) treated at a French university hospital were randomised to heel-free or heel-wrapped (control) bandaging for 4-6 weeks. The primary outcome was interface pressure 48 h after the final bandage application, analysed in the per-protocol population. Secondary outcomes were ankle oedema, change in interface pressure and patient pre

The effects of polidocanol endovenous microfoam 1% on wound healing of hard-to-heal venous leg ulcers.

AIM: Venous leg ulcers (VLUs) are the most common type of hard-to-heal lower extremity ulcers, significantly impacting quality of life and causing financial burden. The aim of the present study was to investigate the healing time and recurrence rate following polidocanol endovenous microfoam 1% injection in patients with hard-to-heal VLUs. METHODS: Data from consecutive patients undergoing polidocanol endovenous microfoam injection between August 2019 and February 2021 at a university-affiliated referral wound care centre in Edison, NJ, US were retrospectively analysed. Their healing time and

Review and analysis of healing durability in lower extremity wounds treated with intermittent cyclically pressurised topical wound oxygen.

OBJECTIVE: Diabetic foot ulcers and venous leg ulcers (VLUs) frequently recur after closure, and because the underlying disease persists, a more meaningful endpoint to consider is durable healing: sustained closure with low recurrence and fewer complications. Mechanistically, topical wound oxygen supports antimicrobial defence, angiogenesis and collagen cross-linking, while cyclical compression reduces oedema and enables timely resolution of inflammation, improving the structural quality of repaired tissue rather than merely closing the wound. Combining both modalities in one device offers a p

The Effectiveness, Safety and Cost-Effectiveness of Platelet-Rich Plasma and Platelet-Rich Fibrin in the Treatment of Venous Leg Ulcers: A Systematic Review and Meta-Analysis.

Venous leg ulcers occur due to chronic venous insufficiency in the lower extremities and are often difficult to heal. Platelet-rich plasma and platelet-rich fibrin are products that contain high concentrations of platelets. This systematic review evaluated the effectiveness, safety and cost-effectiveness of these products for the treatment of venous leg ulcers. Guided by the 2020 Joanna Briggs Institute systematic review of effectiveness guideline, this review included original studies that investigated platelet-rich plasma and platelet-rich fibrin in the treatment of venous leg ulcers from da

Innovative wound management of refractory venous ulcers with topical oxygen therapy.

OBJECTIVE: Topical oxygen therapy (tOT) is a novel treatment method capable of expediting granulation tissue formation in patients with non-healing lower extremity venous leg ulcers (VLUs). tOT provides cyclic oxygen with compression and is able to be administered at home, unlike chamber-based oxygen therapy. Although previous randomized prospective trials have demonstrated effectiveness of tOT in treating ulcers of diabetic etiology, its ability to promote healing in refractory venous ulcers requires additional exploration. Thus, we investigated preliminary outcomes of tOT administration in t

Clinical Evaluation of a Novel Synthetic Nanofiber Wound Matrix for the Treatment of Chronic Wounds.

Chronic wounds, including Diabetic Foot Ulcers (DFUs), Venous Leg Ulcers (VLUs) and Pressure Ulcers (PUs), present significant challenges for the patients, clinicians and healthcare systems. There remains a strong need for novel and effective technologies to accelerate the healing of these wounds. The objective of this prospective, single-arm pilot study was to evaluate the clinical performance of a novel nanofiber wound matrix for the treatment of chronic lower extremity wounds refractory to standard-of-care treatment at a single centre. A total of 15 patients with 15 chronic wounds (5 DFUs,

Innovative femoral vein valve repair combined with deep femoral vein ligation for treatment of chronic venous insufficiency: a case report.

Chronic deep venous insufficiency (CVI) of the lower extremities results from structural or functional abnormalities of the venous valves, leading to blood reflux and increased venous pressure. Common clinical manifestations include edema, varicose veins, skin hyperpigmentation, and ulcers. The primary etiologies of CVI include congenital valve defects and secondary damage from venous thrombosis, both of which significantly impair quality of life. Surgical treatments for CVI include valve repair, valve transposition, and valve transplantation. Valve repair is effective when the valve structure

Assessing the Outcomes and Complications of Skin Allografts in Healing Diabetic Foot and Venous Leg Ulcers: A Systematic Review of Randomised Controlled Trials.

Chronic diabetic foot ulcers and venous leg ulcers remain difficult to treat due to delayed healing and high complication rates. Biologic skin allografts derived from living or amniotic tissue provide extracellular matrix components, cytokines, growth factors and viable cells that facilitate tissue repair. This investigation systematically reviewed randomised controlled trials comparing Apligraf, Theraskin, Amnioband, Amnioexcel, EpiCord and Epifix for chronic lower extremity wounds. Searches were conducted in PubMed, Embase and Cochrane databases. Primary outcomes included complete wound clos

Platelet-Rich Plasma in a Refractory Venous Leg Ulcer Following Multiple Failed Skin Grafts.

Chronic lower-extremity ulcers remain a major therapeutic challenge. Conventional treatments-including compression therapy, debridement, dressings, antibiotics, pharmacologic agents, hyperbaric oxygen, and skin grafting-frequently fail to provide durable closure, largely due to limited modulation of the wound-healing cascade. Autologous platelet-rich plasma (PRP) is gaining interest as a regenerative adjunct capable of delivering concentrated endogenous growth factors. We report a 58-year-old woman with more than 20 previous skin grafting procedures for recurrent lower-leg ulcers, arising from

Chinese expert consensus on wound repair of venous leg ulcers (2025 edition).

Venous leg ulcers (VLUs) result from a multifactorial interplay of etiological factors, posing significant challenges in clinical management. Effective treatment requires a comprehensive assessment to identify the underlying pathophysiological mechanisms, followed by the implementation of an individualized, multidisciplinary approach that integrates the expertise of vascular surgeons, wound care specialists, and other pertinent healthcare professionals. Although both domestic and international vascular surgery societies, as well as other relevant specialties, have issued consensus statements o

Association between vitamin D deficiency and incident varicose veins: a propensity score-matched cohort study.

BACKGROUND: Evidence linking vitamin D deficiency (VDD) to varicose veins remains limited and largely derived from small or cross-sectional studies. We examined whether VDD is associated with incident varicose veins in a large real-world cohort. METHODS: Using the TriNetX Global Collaborative Network, we conducted a retrospective propensity score-matched cohort study of adults aged ≥40 years with at least two serum 25-hydroxyvitamin D [25(OH)D] measurements between 2010 and 2023. VDD was defined as two measurements ≤19.9 ng/mL, and controls had two measurements ≥30.0 ng/mL. A 1-year landmark

Cost-effectiveness of a prosthetic venous valve versus compression therapies and wound care for treatment of infra-inguinal deep vein reflux.

BACKGROUND: Patients with chronic venous insufficiency secondary to infra-inguinal deep vein reflux have few treatment options available to them, given the focus-to-date has been directed toward obstruction rather than reflux. The SAVVE trial was a prospective, non-blinded, single arm, multicenter study designed to assess the safety and efficacy of VenoValve, a bioprosthetic replacement venous valve that is surgically implanted into the deep venous system for treatment of patients with severe infra-inguinal deep venous reflux (Clinical, Etiological, Anatomical, and Pathophysiological classific

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. A qualitative exploration of the barriers and facilitators to early lower limb assessment and onward referral for specialist treatment for patients with venous ulceration.

Guidance for venous leg ulceration (VLU) recommends compression therapy and early referral for specialist vascular assessment within two weeks. Few patients receive timely assessment and referral. Reasons for this are unclear. The aim of this work was to explore nurses' perceptions of the barriers and facilitators to early assessment of VLU and referral for specialist treatment. One-to-one semi-structured interviews explored experiences caring for and referring patients with VLU to a vascular specialist. Maximum variation sampling and inductive thematic analysis were used. Eighteen nurses part
