Bicentric comparison of 3-year clinical outcomes between endoscopic ultrasound-guided embolization with coil and hemostatic adjunct versus balloon-occluded retrograde transvenous obliteration for treatment of bleeding gastric varices: A propensity-score matched study.

BACKGROUND AND AIMS: Endoscopic ultrasound-guided coil injection (EUS-COIL) and balloon-occluded retrograde transvenous obliteration (BRTO) performed by interventional radiology (IR) are both effective obliterative procedures to treat gastric varices. Comparative studies are lacking. Our aim was to assess clinical outcomes for EUS-COIL versus BRTO for treatment of acute gastric variceal bleeding. METHODS: This retrospective cohort study included all patients admitted to two tertiary care centers between 2009-2024 who underwent either EUS-COIL or BRTO for bleeding gastric varices. The primary

Venous Thromboembolism in Acute Necrotizing Pancreatitis Is Associated With Worse Outcomes, With Subtype-Specific Patterns: A Nationwide Analysis, 2016-2022.

OBJECTIVES: Venous thromboembolism (VTE) complicates acute necrotizing pancreatitis (ANP), but outcomes across extra-splanchnic deep vein thrombosis (DVT), portal vein thrombosis (PVT), and splenic vein thrombosis (SVT) remain poorly defined. We evaluated national trends and in-hospital outcomes in ANP with versus without VTE and across VTE subtypes. METHODS: Using the National Inpatient Sample (2016-2022), adults hospitalized with principal ANP were identified. Secondary diagnosis codes captured VTE subtypes. Primary analyses compared mortality and major complications in ANP with versus with

Portal venous system thrombosis after cyanoacrylate injection for gastric varices under endoscopic ultrasound versus conventional endoscopy: a propensity-matched study.

OBJECTIVE: To compare the incidence of portal venous system thrombosis (PVST) following endoscopic ultrasound-guided cyanoacrylate injection (EUS-CYA) versus direct endoscopic cyanoacrylate injection (DEI-CYA) for gastric varices, and to identify associated risk factors. METHODS: A total of 202 patients with cirrhosis who underwent either EUS-CYA or DEI-CYA for gastric varices were retrospectively enrolled. All patients were followed for at least 1 year after the procedure, and early PVST was assessed 3-5 months post-procedure. Propensity score matching was performed to balance baseline chara

Endoscopic ultrasound in hepatology: A European Society of Gastrointestinal Endoscopy (ESGE) Technical and Technology Review.

BACKGROUND: Endoscopic ultrasound (EUS)-guided techniques are increasingly applied in hepatology, including for tissue acquisition, vascular access, pressure measurement, and selected therapeutic interventions. However, adoption remains heterogeneous because of variability in technical approaches, devices, outcome definitions, operator expertise, and the quality of available evidence. METHODS: This European Society of Gastrointestinal Endoscopy (ESGE) Technical and Technology Review summarizes the current evidence on selected EUS-guided techniques in liver disease and portal hypertension, inc

A gastric dieulafoy lesion presenting with life-threatening upper gastrointestinal hemorrhage during recovery from hypertriglyceridemic acute pancreatitis: a case report.

BACKGROUND: Dieulafoy lesions are an uncommon but potentially life-threatening cause of non-variceal upper gastrointestinal bleeding, caused by rupture of a large-caliber submucosal artery through a minute mucosal defect; diagnosis is difficult because bleeding is intermittent and the surrounding mucosa appears nearly normal. Hemorrhage from a gastric Dieulafoy lesion during recovery from hypertriglyceridemic acute pancreatitis (HTG-AP) has rarely been reported. CASE PRESENTATION: A man in his twenties with severe obesity presented with acute epigastric pain after alcohol and a high-fat meal.

The efficacy and safety of endoscopic ultrasound selective variceal devascularization for the treatment of esophagogastric varices: a retrospective cohort study.

BACKGROUND: Gastroesophageal variceal bleeding is a life-threatening complication of portal hypertension. This study evaluated the efficacy and safety of endoscopic ultrasound-guided selective variceal devascularization (EUS-SVD) compared with conventional endoscopic cyanoacrylate injection. METHODS: This retrospective cohort study adhered to the STROBE guidelines. Between August 2022 and June 2024, 107 patients were enrolled and allocated to conventional endoscopy (n = 52) or EUS-SVD (n = 55). Propensity score matching was performed to reduce confounding. Primary outcomes included rebleeding

Genetic and functional characterization of a novel SERPIND1 variant in a family with isolated gastric varices and venous thrombosis.

BACKGROUND: Isolated gastric varices type I (IGV1) is a rare but severe manifestation of splenic vein thrombosis. While thrombophilia is a recognized cause, the underlying genetic determinants are often elusive. This study investigates the case of a proband presenting with IGV1, deep vein thrombosis (DVT), and Long QT syndrome Type 2 (LQT2), aiming to elucidate the genetic basis and functional impact of the identified SERPIND1 variants. METHODS: Whole-exome sequencing identified candidate variants in the proband, which were validated by Sanger sequencing. Functional assessment included quanti

Antibiotic Stewardship in Cirrhosis: Current Recommendations and Clinical Challenges.

Bacterial infections are a major cause of mortality in cirrhosis, with antibiotic prophylaxis and empirical treatment embedded in international guidelines. However, the prevalence of multidrug-resistant organisms and the epidemiology of infections have shifted considerably in recent decades. This review critically appraises the evidence underlying guideline recommendations on antibiotic prophylaxis for spontaneous bacterial peritonitis and acute variceal bleeding in cirrhosis, and antimicrobial management in acute liver failure. The evidence-underlying current recommendations is limited and pr

Early-Onset Gastric Varices After Fontan Completion Associated with a Pre-Existing Gastro-Renal Shunt.

Gastroesophageal varices are recognized complications of Fontan-associated liver disease, but clinically significant varices have generally been reported more than 10 years after Fontan completion. The mechanisms of early variceal formation remain unclear. We report a child with tricuspid atresia who developed isolated gastric varices approximately 7 years after extracardiac Fontan completion in the presence of a pre-existing gastro-renal shunt (GRS). The patient underwent bidirectional Glenn palliation at 9 months of age. Contrast-enhanced computed tomography (CT) performed before Fontan comp

Values of Blood Coagulation Parameters in Distinguishing Esophagogastric Variceal Bleeding from Peptic Ulcer Bleeding.

BACKGROUND: Esophagogastric variceal bleeding (EGVB), manifested with similar symptoms of hematemesis and/ or melaena, is often difficult to differentiate from peptic ulcer bleeding (PUB). It remains unclear whether blood coagulation parameters can be used to distinguish between EGVB and PUB. METHODS: A total of 37 EGVB patients (EGVB group) and 89 cases with PUB (PUB group), were enrolled for final analysis retrospectively. The levels of coagulation parameters, including prothrombin time (PT), activated partial thromboplastin time (APTT), Fibrinogen, and D-dimer, were tested in the emergency

Acute variceal bleeding in a patient with phaeochromocytoma: the critical choice of vasoactive therapy.

A woman in her late 70s with known left-sided phaeochromocytoma presented with acute variceal bleeding secondary to suspected cirrhosis. Terlipressin, a standard first-line vasoactive agent, was contraindicated due to the risk of precipitating hypertensive crisis. Octreotide was successfully used as the preferred alternative, combined with careful alpha-blockade optimisation and endoscopic variceal ligation, achieving complete haemorrhage control without cardiovascular complications. This case highlights the need to recognise contraindications in patients with rare comorbidities and offers gui

Timing of endoscopy and short-term outcomes in cirrhotic patients with acute variceal bleeding: a single-center retrospective study.

BACKGROUND: The optimal timing of endoscopy for acute variceal bleeding (AVB) remains uncertain in real-world clinical practice. This study aimed to evaluate the association between endoscopy timing and short-term outcomes in patients with AVB. METHODS: We retrospectively included cirrhotic patients with endoscopically confirmed AVB admitted between January 2020 and April 2025 who underwent endoscopic therapy during hospitalization. Patients were categorized according to time from admission to endoscopy: <24 h, 24-72 h, and ≥ 72 h. The primary outcomes were 6-week mortality and rebleeding. Se

A multicenter multistate modeling on the natural history of liver decompensation: Longitudinal analysis of 1777 patients.

BACKGROUND: Cirrhosis often remains silent until decompensation, after which survival deteriorates sharply. Contemporary, transition-aware estimates are limited, especially in Asian-predominant cohorts. We analyzed trajectories from compensated advanced chronic liver disease (cACLD) through sequential decompensations to death. METHOD: In this multicenter study, we analyzed retrospective data from an adult cohort with liver stiffness ≥15 kPa from vibration-controlled transient elastography across 19 institutions internationally. Decompensation included ascites, hepatic encephalopathy, spontane

Clip-first emergency hemostasis for acute esophagogastric variceal bleeding: a retrospective comparative study.

BACKGROUND: Active esophagogastric variceal bleeding can obscure the endoscopic field and make definitive therapy difficult during emergency gastroscopy. We evaluated a clip-first strategy designed to obtain immediate mechanical control before local cyanoacrylate reinforcement or staged secondary prophylaxis. METHODS: We retrospectively analyzed 115 acute bleeding episodes among 102 adults treated at a single tertiary center. Episodes were assigned to a clip-first group (n = 42) or a conventional emergency endoscopic therapy group without clip-first control (n = 73). Variceal anatomy was recl
