Comparison of the long-term effects of digestive endoscopy variceal ligation and propranolol in preventing variceal rebleeding in patients with liver cirrhosis.

This study aimed to assess the long-term effects of endoscopic variceal ligation (EVL) and propranolol in preventing variceal rebleeding in patients with liver cirrhosis and esophageal varices. A retrospective analysis was conducted on patients with hepatitis B-related liver cirrhosis and class A Child-Pugh scores who received either EVL (n = 51) or propranolol (n = 53) for variceal rebleeding prevention. Baseline characteristics, hemodynamic parameters, variceal rebleeding frequency, complications, quality-of-life scores, and mortality rates were compared between the 2 treatment groups using

Leucine Aminopeptidase to Spleen Length Ratio: A Novel Noninvasive Model for Predicting Esophageal Gastric Variceal Bleeding in Patients With Liver Cirrhosis.

BACKGROUND: Esophageal gastric variceal bleeding (EGVB) is a common complication of liver cirrhosis. However, esophagogastroduodenoscopy (EGD) for detecting venous pressure is invasive, limiting its clinical application. To reduce unnecessary EGD screening, here we screened multiple commonly used noninvasive examination indicators and evaluated their potential for predicting the risk of EGVB in patients with cirrhosis. METHODS: Patients with liver cirrhosis who were diagnosed with esophageal gastric varices (EGVs) for the first time were divided into groups with and without bleeding. All pati

Percutaneous Transhepatic Variceal Embolization as a Therapeutic Bridge for Variceal Bleeding: An Initial Alternative in the Absence of Endoscopy or Transjugular Intrahepatic Portosystemic Shunt (TIPS).

Variceal gastrointestinal bleeding in patients with liver cirrhosis is a potentially fatal complication that requires prompt hemostatic control. Standard management includes vasoactive therapy, antibiotic prophylaxis, and urgent endoscopic intervention, with transjugular intrahepatic portosystemic shunt reserved for selected high-risk or refractory cases. However, in resource-limited settings, timely access to endoscopy or definitive portal decompression may be delayed or unavailable, increasing the risk of hemodynamic deterioration and early mortality. We report the case of a 64-year-old woma

Preventing First and Further Decompensation in Advanced Chronic Liver Disease.

Advanced chronic liver disease (ACLD) remains a major cause of global morbidity and mortality. Preventing hepatic decompensation-both the first event and subsequent recurrences-has become a central therapeutic goal to prolong survival. The transition from the compensated phase (cACLD) to the decompensated phase (dACLD) is driven by clinically significant portal hypertension (CSPH) and continuous exposure to etiologic factors, and is often precipitated by systemic triggers such as infections, portal vein thrombosis, and hepatocellular carcinoma. Thus, effective prevention requires a multidiscip

EUS-guided esophagogastric variceal obliteration: real-world cohort of three generation glue embolization (with video).

BACKGROUND AND AIMS: Esophagogastric variceal bleeding (EGVB) remains life-threatening in portal hypertension. This real-world cohort documents stepwise EUS-guided glue embolization evolution for secondary prophylaxis of GOV-type EGVB, comparing sequential approaches: conventional ESVD (Endoscopic selective variceal devascularization), EUS-PCSS-D/P (EUS-guided variceal puncture with cyanoacrylate selective seal targeting the distal segment of the source vessel or perforating branch vessels), and EUS-PCSS-T (targeting the source vascular trunk). We aimed to evaluate the impact of these technica

Upper Gastrointestinal Bleeding Due to Ruptured Gastric Varices: A Diagnostic Challenge.

Gastric variceal (GV) bleeding is a life-threatening complication of portal hypertension, most commonly associated with cirrhosis. However, non-cirrhotic portal hypertension (NCPH) is an important and under-recognized cause, with portosinusoidal vascular disorder (PSVD) being a key entity, particularly in patients exposed to chemotherapy. A 39-year-old man with a history of childhood neuroblastoma treated with dacarbazine, cyclophosphamide, doxorubicin, and vincristine presented with hematemesis and syncope. On admission, he was hypotensive and tachycardic. Laboratory tests revealed anemia (he

The evolving role of endoscopic ultrasound from diagnosis to therapy in hepatic vascular interventions.

Endohepatology, the integration of advanced endoscopy into hepatology, is rapidly evolving beyond standard diagnostics. This review focuses on its burgeoning vascular applications, particularly through endoscopic ultrasound (EUS). EUS-guided portal pressure gradient measurement (EUS-PPG) is establishing a new paradigm for direct portal hypertension assessment. Therapeutically, EUS enables precise vascular interventions such as coil and glue embolization for gastric varices and spontaneous portosystemic shunts, moving beyond blind techniques. Pioneering, bench-to-bedside procedures like the cre

A proposal for a scoring system for hemoglobinuria following ethanolamine oleate sclerotherapy in children with venous malformations.

BACKGROUND: Ethanolamine oleate (EO) is a sclerosing agent commonly used to treat slow-flow vascular malformations (SFVMs). Macroscopic hemoglobinuria is the most frequent complication of EO sclerotherapy and may occasionally progress to acute renal failure. Because EO was originally developed for adult esophageal and gastric varices, data on its pediatric use remain limited. This study aimed to identify risk factors for macroscopic hemoglobinuria and to develop a practical scoring system to support the safe use of EO in children. METHODS: This retrospective study included patients aged ≤15 y

Transient elastography for accurate staging of liver fibrosis and predicting complications in children with autoimmune hepatitis.

OBJECTIVES: Autoimmune hepatitis (AIH) presents as hepatitis, chronic or acute liver failure. Liver fibrosis may progress to liver cirrhosis. Pharmacological treatment aims to preserve liver function and induce remission. Transient elastography (FibroScan®, TE) has already been applied in many chronic liver diseases for non-invasive liver stiffness/fibrosis assessment. We aimed to evaluate the usefulness of liver stiffness measurement (LSM) in relation to liver fibrosis on biopsy, selected clinical features and laboratory markers of liver function in the largest paediatric AIH cohort studied.

Acute-on-chronic liver failure triggered by postpartum variceal bleeding in a patient with alcohol-associated liver disease.

Pregnancy in women with cirrhosis carries substantial maternal risk, with variceal bleeding being one of the leading causes of mortality. Acute-on-chronic liver failure (ACLF) is a distinct clinical entity, yet cases triggered by postpartum variceal bleeding are uncommon. We describe a 41-year-old woman with alcohol-associated cirrhosis who conceived through in vitro fertilization and had no varices on endoscopy performed one year before delivery. On postpartum day 4 after cesarean section, she developed massive hematemesis from newly developed esophageal varices, culminating in ACLF with grad

Diagnostic accuracy of liver and spleen stiffness measured by two-dimensional shear wave elastography and transient elastography for high-risk varices: A prospective cross-sectional study.

BACKGROUND: Early detection of high-risk varices (HRV) in patients with compensated advanced chronic liver disease (cACLD) is crucial for improving prognosis. We aimed to compare the diagnostic accuracy of liver stiffness (LS) and spleen stiffness (SS) measured by two-dimensional shear wave elastography (2D-SWE) and transient elastography (TE) in the diagnosis of HRV, and to develop an HRV diagnostic model based on 2D-SWE for the patients with cACLD. METHODS: From October 2023 to March 2025, 104 consecutive adult patients with cACLD (defined as TE measured LS ≥ 10 kPa) without previous decomp

Reassessing the Baveno based strategy in China: a cost-effectiveness analysis of screening for high-risk varices in cirrhosis.

BACKGROUND & AIMS: The Baveno consensus recommends using liver stiffness measurement (LSM) and platelet count to avoid endoscopy in low-risk patients with compensated advanced chronic liver disease (cACLD). This study aimed to compare the cost-effectiveness of the Baveno-based selective screening vs. universal screening strategy for high-risk varices (HRV) in a Chinese cACLD cohort. METHODS: A state-transition Markov model was constructed from the Chinese healthcare system perspective, simulating a cohort of 1,000 patients with cACLD over a five-year horizon. Model inputs were derived from Ch
