Patient and Disease Factors Linked to Intraoperative Findings in Lymphaticovenous Anastomosis Surgery for Lymphedema: An International Multicenter Study.

BACKGROUND: Lymphaticovenous anastomosis (LVA) is an established microsurgical treatment for lymphedema, but predictors of intraoperative vessel characteristics and immediate anastomosis patency remain unclear. This study investigated associations between preoperative patient and disease factors and intraoperative findings during LVA surgery. METHODS: In this prospective international multicenter study, 183 patients undergoing 426 LVAs were included. Preoperative patient and disease characteristics were recorded. Surgeons documented intraoperative findings using a standardized semiquantitativ

Cystic pulmonary amyloidosis with lymphoid interstitial pneumonia in Sjögren's disease: a case report.

BACKGROUND: Interstitial lung diseases are commonly seen in association with Sjögren's disease, especially lymphoproliferative disorders, such as lymphoid interstitial pneumonia. In occasional and rare cases, pulmonary amyloidosis with cysts, nodules, and/or ground-glass opacites as radiological features, can also become a complication of Sjögren's disease. On the other hand, diffuse cystic lung disease, without nodules, concomitant with chylous fluid accumulation in female leads to initial suspicion of lymphangioleiomyomatosis. Interestingly, it's challenging to differentiate if diffuse cysti

Diagnostic Accuracy of Ultrasound for Localizing Lymphovenous Anastomosis Sites in Lymphedema Surgery.

BACKGROUND: Preoperative vessel mapping is critical for successful lymphovenous anastomosis (LVA) in supermicrosurgical lymphedema treatment. Indocyanine green lymphography is common but limited by poor depth penetration and reduced visibility in advanced disease. This study aimed to evaluate the diagnostic accuracy, measurement agreement with intraoperative findings, and procedural efficiency of preoperative ultrasound in identifying surgically compatible LVA sites. METHODS: This study retrospectively analyzed prospectively collected data between April 2019 and January 2024 at a secondary ca

[Lipedema: update on diagnosis and treatment].

Lipedema is a chronic adipose tissue disorder that primarily affects women, characterized by symmetrical and disproportionate fat accumulation in the lower and/ or upper extremities, sparing hands and feet. This review aims to update knowledge on diagnosis, staging classification, and available therapeutic options. Diagnosis is fundamentally clinical, based on characteristic adipose tissue distribution, presence of pain, easy bruising, after excluding other similar entities. Treatment includes conservative measures (complete decongestive therapy, exercise, compression) and surgical procedures

Evaluating the impact of triple-drug mass drug administration using human and mosquito-based indicators of lymphatic filariasis in Samoa, 2019 - 2024.

BACKGROUND: For lymphatic filariasis (LF) elimination, the World Health Organization recommends multiple rounds of mass drug administration (MDA) to interrupt transmission. While LF antigen (Ag) is routinely used to monitor progress, recent evidence suggests more time-sensitive indicators are needed during the immediate post-MDA period. In Samoa, triple-drug MDA was distributed in 2018 and 2023. This study aimed to evaluate the impact of Samoa's second round of triple-drug MDA on human and mosquito-based indicators of transmission. METHODOLOGY: Surveys were conducted in eight primary sampling

Effectiveness of upper extremity lymphaticovenular anastomosis for breast cancer-related trunk lymphedema: Evaluating with a novel subjective scoring system.

BACKGROUND: Secondary trunk lymphedema (TL) commonly develops after breast cancer treatment (BCT) and frequently exists with upper extremity lymphedema (UEL). Although several diagnostic methods have been proposed, treatment options are mainly limited to manual lymphatic drainage (MLD). In this study, the efficacy of upper extremity lymphaticovenular anastomosis (LVA) for TL was evaluated. METHODS: Patients with secondary TL after BCT who underwent complex decongestive therapy for over 6 months, followed by upper extremity LVA, were included in this study. Subjective trunk symptoms were asses

Targeting Piezo1/2 ion channels for modulation of the lymphatic system: mechanisms, applications, and therapeutic prospects.

In recent years, the role of mechanosensation in lymphatic system development, functional homeostasis, and disease progression has received increasing attention. As key mechanosensitive receptors in lymphatic endothelial cells, Piezo1/2 ion channels respond to biomechanical stimuli such as fluid shear stress and interstitial pressure, thereby regulating calcium influx and downstream signaling pathways. These channels play pivotal roles in physiological processes, including lymphatic network formation, valve development, and the maintenance of meningeal lymphatic function. This review systemati

Short- and Long-term Patency of Prophylactic and Therapeutic Lymphovenous Anastomosis for Extremity Lymphedema: A Systematic Review.

BACKGROUND: The clinical effectiveness of lymphovenous anastomosis (LVA) for extremity lymphedema often decreases after 6-12 months. This waning effect is believed to be associated with loss of anastomotic patency, but patency rates remain poorly defined in the current literature. Consequently, the goal of this systematic review is to summarize the available evidence on short- (≤12 mo) and long-term (>12 mo) patency following supermicrosurgical LVA. METHODS: A preferred reporting items for systematic reviews and meta-analyses-compliant systematic literature search for lymphedema, LVA, and pat

Lymphatic Wire-Guided Identification of Proximal Collecting Lymphatic Vessels in Advanced Lower Extremity Lymphedema.

In advanced lower-limb lymphedema, identifying functional lymphatic vessels suitable for lymphaticovenular anastomosis (LVA) has become increasingly complex because of vessel degeneration and depth. This study introduces the use of a novel intraluminal guidewire system (LymTracer) to identify functional collecting lymphatic vessels in patients with advanced lymphedema. Between December 2022 and December 2024, 21 patients with advanced secondary lower limb lymphedema underwent LVA. All patients had limited ICG-detectable lymphatic vessels in the distal lower legs. The LymTracer was inserted int

Biopsy of Secondary Angiosarcoma Arising in Native Mastectomy Skin of a Reconstructed Breast Without Prior Radiotherapy.

Secondary breast angiosarcoma usually develops after radiotherapy or in the setting of chronic lymphedema. We report a rare case of non-radiation-associated secondary cutaneous angiosarcoma arising in the native mastectomy skin overlying a reconstructed breast after immediate tissue expander placement and delayed deep inferior epigastric perforator flap reconstruction for breast cancer. The patient's clinical course, imaging, histopathological findings, and treatment were retrospectively reviewed. The clinicopathological features were compared with previously reported cases of angiosarcoma ari

Lymphedema therapy and wound healing: a retrospective analysis of pretreatment and posttreatment wound volume.

BACKGROUND: Venous leg ulcers (VLUs) frequently exhibit prolonged healing times, particularly in the presence of lower extremity lymphedema. One approach to managing VLU is to combine traditional wound care with lymphedema therapy (LT). Despite recognition of compression as an effective agent for management of chronic wounds, use of lymphedema services remains insufficient among health care providers. OBJECTIVE: To investigate the effectiveness of LT for tissue repair and examine the effect of various factors on wound healing. MATERIALS AND METHODS: Electronic medical records of patients wit

Excision of a giant cervical-thoracic aggressive fibromatosis: a case report and review of literature.

BACKGROUND: Aggressive fibromatosis is a rare, locally invasive tumor with a marked tendency to recur, and it is often diagnosed only after the lesion has already reached a considerable size. Although histologically benign, these tumors can behave aggressively at the local level. CASE DESCRIPTION: We report the case of a male patient with a large mass occupying the entire left hemithorax and extending into the left side of the neck. The tumor compressed the surrounding organs and produced corresponding symptoms. It encircled the left subclavian artery and vein. The patient underwent a complet

Cross-cultural adaptation and comparative psychometric evaluation of four patient-reported outcome measures for breast cancer-related lymphedema: measurement properties and clinical discrimination.

BACKGROUND: Breast cancer-related lymphedema (BCRL) can substantially impair health-related quality of life. Patient-reported outcome measures (PROMs) may complement objective limb measurements by capturing symptoms, functional limitations, and psychosocial consequences; however, comparative evidence on BCRL-specific PROMs and validated Chinese versions remains limited. This study aimed to cross-culturally adapt four BCRL-specific PROMs and compare their classical and Rasch measurement properties, known-groups validity, and exploratory discrimination relative to a circumference-based reference

Ultra-High-Frequency Ultrasound-Detected Lymphatic Structural Transition Points Are Located Central to Early Dermal Backflow Transition Points on Indocyanine Green Lymphography.

BACKGROUND: In lymphaticovenular anastomosis (LVA), discrepancies between functional assessment of lymphatic flow using indocyanine green (ICG) lymphography and structural evaluation using ultrasound remain a challenge. This study aimed to clarify the spatial relationship between the early dermal backflow transition point (eDBF-TP) identified by ICG and lymphatic structural transition points (LSTPs) detected by ultra-high-frequency ultrasound (UHFUS). METHODS: This prospective observational study included 60 sites in patients with secondary limb lymphedema. After identifying eDBF-TP using ICG

A Systematic Review and Meta-Analysis of Inguinal Node Relapse Risk in Anal Squamous Cell Carcinoma Following Chemoradiotherapy Omitting Prophylactic Inguinal Node Irradiation.

OBJECTIVES: Prophylactic inguinal node irradiation (PINI) is standard in chemoradiotherapy for anal squamous cell carcinoma. However, PINI increases toxicity, including bowel and sexual dysfunction, bone marrow suppression, and lymphoedema. Evidence on inguinal node relapse risk without PINI remains limited, warranting systematic review. METHODS: A systematic literature review was performed in accordance with the PRISMA standards using using Scopus, Embase, Cochrane Library and Pubmed for relevant studies between 1980 and 2025. Studies of > 10 patients treated with chemoradiotherapy without P

Effect of BMI-stratified management based on the meridian-lymph axis theory on lower extremity lymphedema after gynecological cancer surgery: a randomized controlled trial.

BACKGROUND: Lower-extremity lymphedema (LEL) is a chronic complication after gynecological cancer surgery. Obesity may aggravate lymphatic dysfunction and reduce the effectiveness of standard physical management. We evaluated a BMI-stratified integrated intervention informed by a meridian-lymph conceptual framework. METHODS: A total of 60 patients with gynecological cancer-related LEL were randomly assigned in a 1:1 ratio to an intervention group (n=30) or a control group (n=30). The intervention group received BMI-stratified meridian massage and dietary and health guidance for 6 months, wher

Psychometric Properties of the Turkish Version of the LYMPH-Q Upper Extremity Module with Breast Cancer-Related Lymphedema Patients: Validity, Reliability, and Responsiveness.

BACKGROUND: Although several patient-reported outcome measures (PROMs) exist to assess health-related quality of life (HRQoL) in patients with breast cancer-related lymphedema (BCRL), most lack sufficient patient input. The LYMPH-Q Upper Extremity (UE) module was developed to address this gap, providing a comprehensive assessment of BCRL from the patient's perspective. The aim of this study was to adapt LYMPH-Q Upper UE module into Turkish and to evaluate its psychometric properties in patients with BCRL. METHODS AND RESULTS: A total of 91 women with unilateral BCRL were included in the prese
