Recognizing Odontogenic Maxillary Sinusitis in the ENT Clinic: Four CT Findings That Should Prompt Dental Evaluation.

Objective: This retrospective cohort study evaluated the proportion of maxillary sinuses with radiographic findings consistent with an odontogenic contribution among patients with imaging-confirmed acute or chronic maxillary sinusitis presenting to a single academic ENT clinic over one year. Sixty-five CT scans (84 sinuses) underwent multidisciplinary review for prespecified radiographic features. By highlighting imaging findings indicative of dental origin this study aims to improve recognition of possible odontogenic disease and appropriate multidisciplinary evaluation. Methods: Sixty-five p

When Eosinophilic Granulomatosis With Polyangiitis (EGPA) Looks Like Granulomatosis With Polyangiitis (GPA): Navigating a Complex Case of Anti-neutrophil Cytoplasmic Antibodies (ANCA) Vasculitis.

Eosinophilic granulomatosis with polyangiitis (EGPA) is an anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis, with myeloperoxidase antibodies in 40% of cases. Proteinase 3 (PR3) positivity is rare and may overlap with granulomatosis with polyangiitis (GPA). We present a 43-year-old male with recurrent sinusitis, epistaxis, neuropathic pain, and acute kidney injury, with histopathology of kidney biopsy showing EGPA despite the absence of asthma, which is rare. His PR3 positivity and clinical presentation mimicked GPA, highlighting the diagnostic challenges of vasculitides. We re

Infected Frontal Sinus Mucocele Mimicking a Cutaneous Abscess.

Frontal sinus mucoceles are benign expansile lesions that may progressively erode the surrounding bone and extend toward the orbit or intracranial compartment. When expansion occurs anteriorly, however, they may present as an apparently superficial forehead swelling and be mistaken for a cutaneous abscess. A 77-year-old woman with chronic sinusitis developed recurrent left paramedian frontal swelling over several months, with only transient improvement following antibiotic treatment. The lesion was initially interpreted as a cutaneous abscess and incised in an outpatient setting. Clear, nonpur

[A case of paroxysmal sympathetic hyperactivity following hypoxic brain injury with ‍difficult treatment].

The patient was a woman in her 20s. In June of a certain year, she experienced cardiopulmonary arrest due to acute epiglottitis and was admitted to the ICU and required mechanical ventilation. From the first day of hospitalization, she exhibited generalized seizures and was diagnosed with acute symptomatic seizures following hypoxic brain injury. Treatment with diazepam and levetiracetam was initiated, leading to symptom resolution. However, around the seventh day of hospitalization, she began experiencing episodic limb muscle hypertonia and fever. Based on diagnostic criteria, she was diagnos

Postpandemic Trends in Pediatric Respiratory Virus Hospitalizations: A Multicenter Study, 2019-2025.

BACKGROUND: The COVID-19 pandemic profoundly altered the circulation of respiratory viruses in children, changing the epidemiology of acute lower respiratory tract infections. In the postpandemic period, respiratory pathogens showed marked temporal variability. The 2024-2025 season was the first respiratory syncytial virus (RSV) season in Italy during which RSV immunoprophylaxis with nirsevimab was implemented, allowing evaluation of recent trends in pediatric hospitalizations. METHODS: We conducted a retrospective multicenter study across 16 pediatric hospitals in Italy. Hospitalizations of

Group A Streptococcal Perimyocarditis Presenting With Acute Left Ventricular Dysfunction.

BACKGROUND: Nonrheumatic group A streptococcal (GAS) perimyocarditis is an uncommon but increasingly recognized clinical entity. CASE SUMMARY: We describe a 41-year-old man who presented with acute chest pain shortly after recent GAS pharyngitis. Electrocardiography demonstrated diffuse ST-segment elevation, inflammatory markers were elevated, and transthoracic echocardiography revealed new left ventricular systolic dysfunction despite nonobstructive coronary anatomy on coronary angiography. The patient was treated with antibiotics, ibuprofen, colchicine, and guideline-directed medical therap

Middle meningeal artery lidocaine infusion reduces headache after venous sinus stenting for idiopathic intracranial hypertension.

BACKGROUND: Headaches after venous sinus stenting (VSS) for idiopathic intracranial hypertension (IIH) are very common and challenging to manage. We evaluated the safety and efficacy of middle meningeal artery (MMA) lidocaine infusion to mitigate post-VSS cephalalgia. METHODS: We retrospectively reviewed 50 consecutive patients with medically refractory IIH who underwent VSS at a single institution (February 2024-January 2026). Seven patients received bilateral MMA infusion of 50 mg of 1% lidocaine immediately following stent deployment; 43 patients served as controls. The primary outcome was

Empirical dietary and lifestyle indices for hyperinsulinemia and insulin resistance in relation to metabolic health in overweight and obese children and adolescents.

OBJECTIVES: In this study, we investigated the clinical features, etiology, and also predictive factors of secondary erythema nodosum (EN) in patients with EN. METHODS: A total of 100 patients (mean age: 37 years) diagnosed with EN between 1993 and 2004 in our clinic were included in the study prospectively. A skin biopsy was performed in 46 of the patients. Patients were considered to have secondary EN when an underlying condition was found, and to have primary EN when no such condition was found. For the diagnosis of the underlying diseases, the pertinent diagnostic criteria and/or diagnost

Safety and Effectiveness of a Fixed-Dose Combination of Cephalexin Extended Release and Clavulanate Potassium in Bacterial Sore Throat and Uncomplicated Skin and Soft-Tissue Infections: A Multicentre, Open-Label, Phase IV Study in India.

BACKGROUND AND OBJECTIVES: Respiratory tract infections and skin and soft-tissue infections are some of the most common causes of outpatient visits. Upper respiratory tract infections (URTIs) are the most common type of infection globally, as well as in India. In 2019, its global incidence was 17.2 billion. Additionally, the Infectious Diseases Society of America 2012 guidelines for the management of Group A streptococcal pharyngitis strongly recommend penicillin or amoxicillin for individuals without penicillin allergy. Therefore, we conducted this study to assess the real-world safety and ef

Overview of 2025 Clinical Practice Guideline Update by the Infectious Diseases Society of America on Group A Streptococcal (GAS) Pharyngitis: Risk Assessment Using Clinical Scoring Systems in Children and Adults.

This publication represents the first part of an update to the clinical practice guideline on the diagnosis and management of group A streptococcal (GAS) pharyngitis, developed by the Infectious Diseases Society of America (IDSA). Diagnosis of GAS pharyngitis by clinician judgment alone is unreliable, and universal testing incurs cost and inconvenience for individuals at low risk of having GAS infection. Clinical scoring systems have been used to quantify the probability of a positive GAS throat culture based on standardized criteria such as the presence of fever, tonsillar enlargement or exud

Probiotic intervention not beneficial to prevent antibiotic-associated diarrhea in absence of antibiotic-induced microbiome disruption.

Probiotics are live microorganisms that, when administered in adequate amounts, confer a health benefit on the host. One of the most common uses of probiotics is for prevention of antibiotic-associated diarrhea (AAD). Here we report on the PLAY-ON study (Probiotics: Live and Active Yogurt Cultures for Healthy Children ON Antibiotics) which was a randomized, placebo-controlled, double-blinded study in healthy children who were prescribed antibiotics for acute outpatient upper respiratory illnesses, such as bacterial sinusitis or streptococcal pharyngitis. The treating clinician determined the a

Guttate Psoriasis Triggered by Streptococcal Pharyngitis in Older Patients: A Case Report.

BACKGROUND Guttate psoriasis is a form of psoriasis that often occurs following infections and is most commonly triggered by group A Streptococcus. The link between streptococcal pharyngitis and the development of guttate psoriasis is well documented in younger populations; however, this presentation in older adults is less common. Additionally, older adult populations can have multiple comorbidities that could influence the development and clinical course of guttate psoriasis. CASE REPORT We report the case of a 66-year-old woman with multiple comorbidities, including type 2 diabetes mellitus

Insights into Arcanobacterium haemolyticum: A Narrative Review of an Emerging Pathogen Revisited.

Arcanobacterium haemolyticum is a facultative anaerobic, Gram-positive bacillus that has garnered attention due to its role in human infections, particularly among adolescents and young adults. Traditionally associated with pharyngitis, this organism is increasingly recognized for its involvement in systemic infections, including bacteremia, central nervous system abscesses, and Lemierre's syndrome. The pathogenicity of A. haemolyticum is attributed to its production of hemolysins and neuraminidase, facilitating tissue invasion and immune evasion. Clinically, infections often present with sore

Mesenteric Lymphadenitis on Point-of-Care Ultrasound in Pediatric Patients With Group A Streptococcal Pharyngitis: A Case Series.

Abdominal pain is a common complaint in pediatric patients with Group A streptococcal (GAS) pharyngitis. Etiology of abdominal pain from GAS infections are unknown. Point-of-care ultrasound (POCUS) is often used by pediatric emergency medicine physicians to evaluate children with abdominal pain. We describe 12 pediatric patients who presented to our Pediatric Emergency Department with symptoms suggestive of acute pharyngitis and abdominal pain, tested positive for GAS pharyngitis, and were found to have mesenteric lymphadenitis (mLAD) on POCUS. Findings on POCUS suggest that abdominal pain in

A case report of nonrheumatic myocarditis following group G streptococcus pharyngitis.

Infectious myocarditis commonly presents following viral illness. Patients with myocarditis are at increased risk of cardiomyopathy, cardiogenic shock, and ventricular tachyarrhythmias. However, due to the rarity of post-bacterial myocarditis, less is known about the presentation, treatment, and prognosis of patients in this sub-population. In particular, myocarditis attributed to group G streptococcus is exceedingly rare. This report describes a 22-year-old man who presented to the hospital with 2 days of chest pain following group G streptococcal pharyngitis. Cardiac magnetic resonance imagi
