Elucidating the mechanism underlying incomplete immune recovery in people living with HIV through CD4⁺ and CD8⁺ T cell multi-omics.

BACKGROUND: A subset of people living with HIV (PLHIV), known as immunological non-responders (INR), fails to achieve adequate CD4⁺ T cell recovery despite viral suppression with antiretroviral therapy (ART). These individuals face an increased risk of adverse clinical outcomes. This study aims to explore the intracellular mechanisms and molecular signatures underlying this incomplete immune recovery using a multi-omics approach. METHODS: The study analysed CD4⁺ and CD8⁺ T cells from 100 ART-naïve PLHIV. Participants were classified into controls (baseline CD4⁺ > 200 cells/µL, n = 49) and cas

Lineage dynamics and risk factors underlying serotype 4 invasive pneumococcal disease in Spain.

BACKGROUND: The emergence of vaccine-covered serotypes causing invasive pneumococcal disease (IPD) is a serious concern worldwide. METHODS: We characterized all national serotype 4 IPD isolates from children and adults received at the Spanish Pneumococcal Reference Laboratory (2009-24). RESULTS: We investigated the unexpected rise of serotype 4 causing IPD in young adults after the COVID-19 pandemic, affecting elderly population in recent years. Epidemiological and genomic analysis confirmed that the rise started as an abrupt cluster of cases in Seville (Andalusia) in the year 2022 due to th

Targeting the Orai1 CRAC channel: translating molecular insights into therapeutic progress.

INTRODUCTION: The Ca2+ release-activated Ca2+ (CRAC) channel, triggered by the interaction between the Ca2+ sensor STIM1 and Orai1, is the primary mediator of store-operated calcium entry (SOCE). Dysregulation of this process drives numerous human pathologies, particularly immune disorders and cancer hallmarks. Due to its role, Orai1 has emerged as an attractive target for precision therapeutics. AREAS COVERED: This review outlines the pharmacological evolution of CRAC channel modulators, tracing their development from early chemical tools, such as SKF-96365, that exhibits several off-target

Genomic Surveillance and Antigenic Characterization of Respiratory Syncytial Virus (RSV) in Spain During the 2023-2024 Season of Nirsevimab Administration.

Respiratory Syncytial Virus (RSV) is a leading cause of respiratory infections in infants and older adults. In Spain, surveillance is supported by the SiVIRA system and the RELECOV genomic sequencing network. The 2023-2024 season marked the first nationwide administration of nirsevimab, a monoclonal antibody for preventing severe RSV in infants. This study analyzes the genomic evolution of RSV during this period, focusing on potential escape mutations in the F protein. RSV-A showed high genetic diversity with eleven circulating lineages, while RSV-B was dominated by lineage B.D.E.1. Phylogenet

Rocio virus sustained circulation in Brazil: first infection case in a horse highlights the need for enhanced arbovirus surveillance.

Rocio virus (ROCV), a neurotropic arbovirus of the Orthoflavivirus genus, caused Brazil’s largest encephalitis outbreak in the 1970s but has since been rarely detected. We report the first ROCV encephalitis clinical case after nearly 40 years and isolated and sequenced the virus from CNS tissue of a horse. Metatranscriptomics enabled full-genome sequencing, revealing divergence from the reference sequence at an evolutionary rate of 5.8–7.0 × 10⁻⁴, consistent with Orthoflavivirus. Thirty-one non-synonymous substitutions were identified, with the envelope protein most affected. Findings indicate

Prevalence of Active HCV Infection in Spain in 2022 Using Multiparameter Evidence Synthesis.

Hepatitis C virus (HCV) remains a major public health concern worldwide. Spain has implemented large-scale direct-acting antiviral (DAA) treatment programs, yet the prevalence of active HCV infection (aHCV) and its trends over time remain incompletely characterised. We applied Bayesian multiparameter evidence synthesis (MPES) to estimate the prevalence of aHCV in Spain in 2022 and its change from 2019 to 2022. The Spanish population aged 15-79 years was stratified into four non-overlapping risk groups: current people who inject drugs (PWID), ex-PWID, gay-bisexual and other men who have sex wit

Unveiling CMV Risk Signatures After Allogeneic Stem Cell Transplantation Through Machine Learning Approaches: A Multicenter Analysis From the GETH-TC.

Cytomegalovirus (CMV) infections remain a major complication following allogeneic hematopoietic stem cell transplantation (allo-HCT), with refractory infections associated with increased morbidity and mortality. Machine learning (ML) approaches may improve risk stratification and clinical management of CMV infections. This retrospective multicenter study analyzed 933 allo-HCT recipients from 21 Spanish centers (2014-2018) using data from the Spanish Hematopoietic Transplantation and Cell Therapy Group registry. Three ML models were developed to predict: (1) CMV DNAemia occurrence, (2) clinical

Risk of infections in patients with psoriasis treated with biologic agents and new oral small molecules: BIOBADADERM registry.

Infections are the most common adverse events associated with systemic treatment for psoriasis. Using data from the BIOBADADERM registry, we analyzed incidence rates of overall, serious, and special-interest infections associated with biologics and oral small molecules. Adjusted incidence rate ratios were calculated using propensity score analysis, with adalimumab as the comparator. The study included 4820 patients (8826 treatment cycles; 20,829 patient years of follow-up). The overall incidence of serious infections was low across all treatments (5.33%), with infliximab showing the highest in

Shifting vulnerabilities in suicide mortality from the COVID-19 crisis to the socioeconomic aftermath in Spain (2016-2024): A Bayesian triple-interaction analysis.

BACKGROUND: The transition from the acute Coronavirus Disease 2019 (COVID-19) crisis to the subsequent socioeconomic aftermath introduced complex stressors. We aimed to determine the differential impacts of pandemic onset (March 2020) and the socioeconomic aftermath (July 2021) on suicide mortality in Spain, examining heterogeneous effects by sex and age. METHODS: We analysed 108 months (2016-2024) of national registry data. Using a Bayesian Interrupted Time-Series (ITS) design with a Triple Interaction framework (Sex×Age×Event), we isolated immediate (level) and long-term (trend) risk trajec
