Vaccine adjuvants as stand-alone immunoprophylaxis in strategies for 100-day rapid responses to future pandemics.

The COVID-19 pandemic accelerated vaccinology progress, driving rapid vaccine development for infectious and non-infectious diseases. However, challenges persist: malaria, HIV, and dengue lack fully effective vaccines, whereas influenza and tuberculosis face waning efficacy. Emerging pathogens and drug-resistant strains further highlight the need for improved vaccines, particularly those offering rapid deployment, broad immunogenicity, and durable protection against variants. Adjuvants can play a dual role in this context: as new stand-alone tools for an early response to a pandemic-aiming at

The Indispensable Value of Small-Molecule Antivirals in Epidemic and Pandemic Preparedness.

Harbingers of infectious viral pandemics, such as the H1N1 influenza, SARS, Zika, Ebola, MERS, and SARS-CoV-2, caused major outbreaks in the first two  decades of the 21st century. Despite warnings, therapeutic tools that could be rapidly and sustainably scaled at a global level when SARS-CoV-2 emerged were lacking. Small-molecule antivirals can play a crucial role in both individual patient care and broader public health strategies for controlling and mitigating the impact of viral diseases. Despite their utility, the lack of R&D investment in this class of intervention has prevented the worl

Disparate Kinetics of Viable Virus Shedding in Immunocompromised Patients: SARS-CoV-2 Versus Influenza Virus-A Prospective Real-World Cohort Study.

There are limited data comparing viral shedding between influenza and SARS-CoV-2 in immunocompromised patients. We prospectively enrolled 70 SARS-CoV-2 and 16 influenza patients, with 48 SARS-CoV-2 patients matched to 16 influenza patients by propensity score. Weekly collected respiratory samples were subjected to viral culture and antigen testing. In matched cohorts, the time to culture-confirmed viral clearance was shorter for influenza than SARS-CoV-2 (median, 4.5 vs. 29 days; adjusted hazard ratio [aHR] 27.5; P < .001), and antigen negativity occurred faster (3 vs. 23 days; aHR 19.2; P < .

Temporal association and seasonal variation of respiratory viruses and invasive group A streptococcal infections in western Sweden: A 15-year analysis across pre- and post-pandemic periods.

OBJECTIVES: To examine the seasonal variation of invasive group A streptococcal infections (iGAS) in relation to respiratory viral circulation, with special reference to pandemic restrictions, and to assess whether increases in viral activity may explain recent increases in iGAS. METHODS: We performed a time-series analysis using a multivariate negative binomial regression model, with weekly counts of respiratory virus detections as the independent variable and weekly counts of iGAS cases as the dependent variable, covering the period from January 2010 to December 2024. RESULTS: We found a s

Comparative Analysis of Flight Volume Effects on COVID-19 and Influenza Transmission Across Variable Control Intensities, 2019-2024.

BACKGROUND: Air travel has played a critical role in the global spread of infectious diseases, facilitating rapid movement of pathogens across continents. This study quantifies the effects of intercontinental flight volumes on both influenza and COVID-19 transmission patterns across countries with varying intensities of public health interventions. METHODS: We analyzed monthly global passenger numbers from January 2019 to July 2024, coupled with comprehensive surveillance data on both pathogens. Using a hierarchical Bayesian linear mixture modeling framework, we examined the relationship betw

Enabling global access to potent subunit vaccines with a simple and scalable injectable hydrogel platform.

Vaccines have been crucial to dramatic improvements in global health in recent decades, yet next-generation vaccine technologies remain out of reach for much of the world. In particular, there are two overarching global needs: (i) develop vaccines eliciting more potent and durable immune responses, especially to reduce incidence of highly communicable diseases, and (ii) enable simple and cost-efficient formulation to maximize global access. Here, we develop an injectable hydrogel depot technology prepared through physical mixing of commercially available, generally recognized as safe (GRAS) po

Recommendations for estimating and reporting vaccine effectiveness by time since vaccination: a COVID-19 case study.

Estimating COVID-19 vaccine effectiveness (VE) by time since vaccination (TSV) is essential for understanding how protection may change over time and enables meaningful comparisons across studies. This is important for accurate comparisons of VE against different SARS-CoV-2 variants/sublineages, across age groups, during different periods post vaccination campaign, or by vaccine type/brand. We provide recommendations for case-control VE studies on estimating and reporting VE analyses by TSV, with the aim of improving quality of these estimates. Our recommendations cover study design and pre-an

Interdiscursivity and its historical shifts in China's public health risk communication letters: from SARS to the post-COVID-19 era.

Interdiscursivity refers to the blending of multiple discourses, genres, or styles within a single communicative event. This study investigates the interdiscursivity of Chinese public health risk communication letters issued during major health crises over the past two decades, including SARS, H1N1, COVID-19 and seasonal influenza. It examines how multiple discourse types-advice-giving, scientific, literary, authoritative, affective and informal-are interwoven and strategically adapted across different crises to guide public behaviour, mobilise communities and foster trust. Using qualitative d

Coverage and inequalities in COVID-19 and influenza vaccination in Brazil: a nationwide cross-sectional survey.

This nationwide cross-sectional survey in Brazil investigated COVID-19 and influenza vaccination coverage and inequalities. Data from 33 250 interviews conducted between March and June 2024 in 133 sentinel cities revealed high overall coverage: 90.2% for at least one dose of COVID-19 vaccine and 82.6% for influenza. However, significant disparities emerged. COVID-19 vaccination coverage increased with age, was higher among women and Whites, and strongly associated with higher education levels and wealth. Influenza vaccine coverage showed a U-shaped pattern by age and was lower among evangelica

Learning from pandemics: Feature selection and clustering for robust digital disease nowcasting.

Disease now-casting increasingly relies on non-traditional data, such as online activity, which offer fast, inexpensive insights but are often noisy, unstructured, and influenced by behavioral confounders. Models using search engine data have frequently suffered from overfitting and unstable performance. However, pandemics may help distinguish true disease signals from behavioral noise, as both tend to surge during outbreaks. We propose a hybrid approach to improve prediction that combines model evaluation with behavioral insight. We apply simple supervised and unsupervised methods to differen

Perceived changes in health care personnel attitudes toward influenza vaccination since the COVID-19 pandemic: A Los Angeles County hospital survey.

Influenza vaccination among health care personnel has declined nationwide since the COVID-19 pandemic began in 2020. We surveyed acute care hospitals within Los Angeles County to examine worker attitudes regarding influenza vaccination and the impact of COVID-19. Comparing before and during the pandemic, we found increased safety and efficacy concerns, and decreased perception of disease severity and risk of influenza, suggesting that redundancy of established COVID-19 measures and pandemic fatigue may have contributed to vaccine refusal.

The Role of Artificial Intelligence and Machine Learning in Predictive Virology: Forecasting, Tracking, and Combating Viral Threats.

The escalating threat of viral pandemics, dramatically illustrated by the COVID-19 crisis, has exposed the critical shortcomings of conventional reactive virology in addressing rapidly evolving pathogens. This review introduces predictive virology (PV) as an artificial intelligence (AI)-driven discipline within broader epidemic intelligence and public health surveillance that uses advanced computational tools to forecast viral threats and accelerate countermeasure design. The current review systematically examines how AI-driven approaches (e.g., machine learning and deep learning) are reshapin

Pre-pandemic preventive healthcare engagement and all-cause excess mortality during COVID-19: a county-level ecological study in the United States.

OBJECTIVE: Community resilience during COVID-19 has been linked to social conditions, public-health capacity, and acute-care resources, but the contribution of pre-pandemic realized preventive-care engagement remains incompletely characterized. We examined whether county-level preventive-care engagement was associated with all-cause excess mortality during the COVID-19 period in the United States. METHODS: In this ecological study, county-specific linear trends fitted to the age-adjusted all-cause premature-death/YPLL-75 indicator for 2012-2018 were used to predict expected mortality in 2020-

Varicella-zoster virus encephalitis in an elderly patient with influenza A infection.

Varicella-zoster virus (VZV) encephalitis is a rare but severe condition associated with significant morbidity and mortality. Its clinical presentation is often nonspecific and may overlap with other neurological conditions, including complications of influenza infection. We report a case of a 76-year-old man with confirmed influenza A infection who developed progressive neurological deterioration. Notably, no cutaneous manifestations suggestive of herpes zoster were observed. Cerebrospinal fluid analysis revealed lymphocytic pleocytosis, and polymerase chain reaction confirmed the presence of

Reactive Infectious Mucocutaneous Eruption Following a Respiratory Infection in an Adolescent: A Case Report.

Reactive infectious mucocutaneous eruption (RIME) is a recently defined mucocutaneous manifestation associated with respiratory infections, most commonly reported secondary to Mycoplasma pneumoniae. The core diagnostic criteria of RIME involve greater than or equal to two mucosal areas with erosive mucositis, minimal cutaneous lesions, a recent infectious prodrome, and exclusion of drug-induced etiologies. RIME presents with acute mucositis involving the oral, ocular, and genital mucosa, with presentations similar to other mucocutaneous disorders such as Stevens-Johnson syndrome (SJS), erythem

Case Report: Coinfection of influenza A and Chlamydia psittaci pneumonia.

The coinfection of influenza A and Chlamydia psittaci pneumoniae is rarely described in China. However, the clinical features of these diseases pose a significant diagnostic challenge. Therefore, it is necessary to report and review such a potentially fatal case. We report an 89-year-old male patient requiring admission to the intensive care unit with acute hypoxemic respiratory failure, shock and MODS. He presented with fever, fatigue, anorexia and dyspnea. The patient exhibited severe hypoxemia, shock, hepatic dysfunction (elevated liver enzymes and serum bilirubin), acute kidney injury and

A case report series of influenza A virus-induced pulmonary disease with diverse clinical manifestations.

BACKGROUND: Influenza A virus (IAV) typically causes influenza-like symptoms; however, in patients with underlying chronic respiratory diseases, it may progress to severe pneumonia and acute respiratory distress syndrome (ARDS). In addition to parenchymal lung injury, some patients may develop severe airway hyperresponsiveness (AHR), including bronchospasm, mucus plug formation, and refractory carbon dioxide retention, often resulting in poor responses to conventional respiratory support. METHODS: This study reports three cases of IAV-associated pulmonary disease complicated by respiratory fa

Impact of Digital Contact Tracing and Other Nonpharmaceutical Interventions on Pandemic Control: Microlevel, Behavior-Driven Agent-Based Model Analysis.

BACKGROUND: Nonpharmaceutical interventions (NPIs), including digital contact tracing (DCT), are central to control the spread of airborne pathogens. Nevertheless, the effectiveness of individual interventions and the role of personal behavior remain insufficiently understood. OBJECTIVE: This study aimed to understand how NPI combination and the individual's behavior contribute to reducing airborne pathogen transmission. METHODS: We disentangle the efficacy of individual NPIs, including DCT, with a novel microlevel, behavior-driven agent-based model (ABM) that simulates individual human beha
