Varicella-zoster virus encephalitis in an elderly patient with influenza A infection.

Varicella-zoster virus (VZV) encephalitis is a rare but severe condition associated with significant morbidity and mortality. Its clinical presentation is often nonspecific and may overlap with other neurological conditions, including complications of influenza infection. We report a case of a 76-year-old man with confirmed influenza A infection who developed progressive neurological deterioration. Notably, no cutaneous manifestations suggestive of herpes zoster were observed. Cerebrospinal fluid analysis revealed lymphocytic pleocytosis, and polymerase chain reaction confirmed the presence of

Reactive Infectious Mucocutaneous Eruption Following a Respiratory Infection in an Adolescent: A Case Report.

Reactive infectious mucocutaneous eruption (RIME) is a recently defined mucocutaneous manifestation associated with respiratory infections, most commonly reported secondary to Mycoplasma pneumoniae. The core diagnostic criteria of RIME involve greater than or equal to two mucosal areas with erosive mucositis, minimal cutaneous lesions, a recent infectious prodrome, and exclusion of drug-induced etiologies. RIME presents with acute mucositis involving the oral, ocular, and genital mucosa, with presentations similar to other mucocutaneous disorders such as Stevens-Johnson syndrome (SJS), erythem

Safety and Efficacy of Therapeutic Agents in Viral Pneumonia in Immunosuppressed Patients.

INTRODUCTION/OBJECTIVE: Immunosuppressed Patients (ISP) - defined by congenital or acquired deficiencies in systemic defense mechanisms - are disproportionately susceptible to viral respiratory infections. Agents such as SARS-CoV-2, Respiratory Syncytial Virus (RSV), influenza A and B, and Cytomegalovirus (CMV) cause significant morbidity in both immunocompetent and immunosuppressed individuals. Clinical manifestations range from mild influenza-like illness to severe pneumonia, respiratory failure, multiorgan dysfunction, and concurrent bacterial infections. METHODS: A narrative synthesis of

Establishment and preliminary evaluation of an RT-ERA combined with CRISPR/Cas12a assay for detection of H5 and H7 influenza viruses.

BACKGROUND/OBJECTIVES: Novel influenza A viruses (NIAV) infection is a zoonotic acute respiratory disease. The H5N1 and H7N9 subtypes cause high mortality, posing major public health threats. This study aimed to establish rapid, field-deployable subtype-specific nucleic acid detection assays for H5 and H7 by integrating reverse-transcription enzymatic recombinase amplification (RT-ERA) with CRISPR/Cas12a. Two separate reactions are required per sample. METHODS: Conserved hemagglutinin (H) sequences were retrieved from the NCBI database, and primers, probes, and crRNAs were designed using Prim

Mental Health Needs of Families of Patients in Intensive Care Units and the Role of Mobile Health: Survey Study.

BACKGROUND: The experiences of patients with COVID-19 and their families manifested the most devastating effects of family separation since the 1918 Spanish influenza pandemic and, with it, a call for solutions to patient isolation and its effect on family mental health. OBJECTIVE: This study examined the recent experiences of families of critical care (intensive care unit; ICU) patients related to anxiety and depression (AD), satisfaction with clinician-family communication, and counseling from mental health and social services. This study explored correlations between these factors and fami

The temporal sequence of influenza H1N1 and Mycoplasma pneumoniae co-infection causes disease severity in Syrian hamster models.

INTRODUCTION: Influenza H1N1 virus is one of the most prevalent subtypes among influenza viruses, and co-infection with Mycoplasma pneumoniae (Mp) is frequently documented in clinical respiratory infections. However, the pathological mechanisms underlying the temporal sequence of H1N1-Mp co-infection remain poorly characterized, and relevant animal models are lacking. METHODS: In this study, we established a model of influenza H1N1 and Mycoplasma pneumoniae co-infection in Syrian hamsters and infected two pathogens in interval of 72 hours. Clinical manifestations, body temperature, body weigh

Increased detection rate of macrolide-resistant Mycoplasma pneumoniae during the Mycoplasma pneumoniae epidemic in children: A multi-campus hospital retrospective study in Guangzhou, 2023-2024.

BACKGROUND: Mycoplasma pneumoniae (MP) is a common cause of community-acquired respiratory infections in children, with manifestations ranging from mild upper respiratory tract illnesses to pneumonia. Mycoplasma pneumoniae pneumonia (MPP) represents a more severe form of infection that often necessitates medical intervention. In children diagnosed with MPP, antimicrobial agents active against MP are indicated, and macrolide antibiotics represent the first-line therapy. However, macrolide-resistant MP (MRMP) strains have emerged worldwide, with a notable upward trend in both MP infection incide

Respiratory syncytial virus infection in adults: Clinical manifestations, outcomes, and vaccines.

Respiratory syncytial virus (RSV) is an RNA virus transmitted via droplets and affects individuals across all age groups. Although RSV is increasingly being recognized as a key pathogen in adults, differentiating RSV infection from other acute viral infections solely on the basis of clinical features is challenging. Polymerase chain reaction is the gold standard for RSV detection. Notably, RSV accounts for 2% to 10% of all hospitalizations due to respiratory tract infections in adults. Among hospitalized adults with RSV infection, 18.9% require intensive care, experiencing a disease burden sim

Human Metapneumovirus: An Underestimated Cause of Severe Respiratory Infection.

Since its identification in 2001, human metapneumovirus (hMPV) has been recognized as an important cause of acute respiratory tract infection in all age groups. Although infection is often mild in healthy adults, hMPV can cause severe lower respiratory tract disease, including pneumonia, in young children, older adults, individuals with chronic diseases and immunocompromising conditions, and residents of long-term care facilities. Accumulating evidence demonstrates that hMPV contributes substantially to hospitalization, intensive care unit admission, and mortality in these high-risk population

Development of a rapid recombinase polymerase amplification assay for avian polyomaviruses.

Budgerigar fledgling disease, caused by avian polyomavirus (APV), is an immunosuppressive condition in budgerigars that typically manifests as abdominal swelling, absence of down feathers on the dorsal and ventral regions, subdermal bleeding in nestlings, and high mortality. A new APV detection relied on real-time quantitative recombinase polymerase amplification (real-time RPA) targeting the large T antigen gene using specific probe and primers. Detection can be accomplished within 25 minutes at 41°C, with this detection threshold of 2.56 × 102 copies/μL. The real-time RPA assay specific dete

Cattle and human organoids reveal 2.3.4.4b H5N1 cross-species transmission potential and neuraminidase-specific neutralizing antibodies in humans.

The unexpected circulation of clade 2.3.4.4b H5N1 influenza viruses in dairy cattle and the transmission to diverse mammalian species poses a pandemic risk. We sought to explore cattle and human respiratory susceptibility to the 2.3.4.4b H5N1 virus. We establish long-term expandable cattle airway and mammary organoids. The 2.3.4.4b H5N1 virus exhibits high replicative fitness in cattle mammary organoids, recapitulating its remarkable mammary tropism. The virus also replicates robustly in cattle airway organoids, suggesting an underrecognized respiratory component in ongoing outbreaks. Interest

Vaccine-Associated Enhanced Respiratory Disease: From Animal Models to Human Health.

Vaccines are a safe and highly effective tool for protecting both humans and animals against infectious diseases. Although vaccines currently in use in humans have robust safety records, historical examples show that some vaccines have resulted in adverse outcomes, including vaccine-associated enhanced respiratory disease (VAERD). VAERD occurs when preexisting vaccine-induced immunity worsens respiratory disease, rather than conferring protection during infection. Enhanced disease in the lower respiratory tract characteristic of VAERD includes worsened clinical manifestations such as pneumonia

Influenza A Co-infection With Atypical Bacteria: A Report of Two Cases.

Influenza A virus infection is associated with a wide range of clinical manifestations, from mild upper respiratory infections that resolve on their own to severe pneumonia and sepsis. Bacterial co-infections, particularly due to atypical pathogens such as Chlamydia pneumoniae and Mycoplasma pneumoniae, may alter disease severity and clinical presentation, especially in paediatric patients and adults with underlying comorbidities. We describe two cases of Influenza A virus infection complicated by atypical bacterial co-infections. The first case involved an elderly female with multiple chronic

Multifocal mucocutaneous herpes simplex virus type 1 infection temporally associated with seasonal influenza vaccination in a young adult without a known immunocompromising condition: A case report.

Seasonal influenza vaccination is widely implemented and generally safe; however, rare immunological adverse events have been reported. We describe a case of multifocal mucocutaneous Herpes simplex virus type 1 (HSV-1) infection temporally associated with seasonal influenza vaccination in a 28-year-old man without a known immunocompromising condition. Seven days after vaccination, he developed oral ulcerative lesions and multiple vesicular eruptions involving the anterior chest wall, pubic region, and penile shaft. A multiplex polymerase chain reaction assay performed on specimens obtained fro

Virus Evolution in Prolonged Infections of Immunocompromised Individuals.

BACKGROUND: Many viruses can cause persistent infection and/or viral shedding in immunocompromised hosts. This is a well-described occurrence not only with SARS-CoV-2 but for many other viruses as well. Understanding how viruses evolve and mutate in these patients and the global impact of this phenomenon is critical as the immunocompromised population expands. CONTENT: In this review, we provide an overview of populations at risk for prolonged viral shedding, clinical manifestations of persistent viral infection, and methods of assessing viral evolution. We then review the literature on viral

Risk Assessment of Spread of the Influenza A Virus in Cows in South Bulgaria.

In this article, we present an assessment of the risk of the potential introduction and spread of highly pathogenic avian influenza (HPAI) in cows in Bulgaria. In the spring of 2024, we witnessed an unprecedented spread of the virus in dairy herds in the USA. This crossing of interspecies barriers by the virus creates a real danger of pandemic manifestations in humans. The continued spread of H5N1 clade 2.3.4.4b in dairy populations and other mammalian species and efficient animal-to-animal transmission increases the risk of infection and subsequent spread of the virus in human populations. Clinical Manifestations and Outcomes of SARS-CoV-2 Infection in Children and Adolescents.

Compared to adults, children are more likely to experience asymptomatic infections or mild-to-moderate symptoms of SARS-CoV-2 infection that resemble other viral infections. However, a substantial proportion of children experience severe disease; more than 2000 US children have died of COVID-19, significantly exceeding the death toll from influenza. Risk factors for severe disease include age less than 6 months and 12 to 17 years, as well as the presence of underlying conditions, especially 2 or more conditions. Multisystem inflammatory syndrome in children is a life-threatening post-infectiou

Dairy cows develop protective immunity against reinfection with bovine H5N1 influenza virus.

Infection of highly pathogenic avian influenza (HPAI) H5N1 clade 2.3.4.4b in dairy cows causes severe mastitis and milk production losses. Whether cows can develop protective immunity is unclear. Here we infected three lactating cows with HPAI H5N1 genotype B3.13 via the hindquarters of the udder to mimic intra-mammary infection. Inoculated cows displayed clinical responses consistent with affected dairy herds in the United States including virus shedding almost exclusively in inoculated hindquarters that peaked between Days 2-4 post inoculation and gradually declined by Day 21. Histologically

Multiplex real-time reverse transcription recombinase-aided amplification assay for the detection of SARS-CoV-2, influenza A virus, and respiratory syncytial virus.

UNLABELLED: Influenza A virus (IAV), severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), and respiratory syncytial virus (RSV) are significant pathogens that induce respiratory symptoms in humans, significantly impacting healthcare systems, economic activities, and human life on a global scale. The clinical manifestations associated with SARS-CoV-2, IAV, and RSV exhibit similarities. Cases of clinical coinfection are prevalent, highlighting the necessity for rapid differential diagnosis to avoid treatment delays. Human metapneumovirus (hMPV) in 2025: emerging trends and insights from community and hospital-based respiratory panel analyses-a comprehensive review.

Human metapneumovirus (hMPV) is a significant respiratory pathogen, primarily impacting young, elderly, and immunocompromised populations. While the clinical presentations are similar to those of other respiratory viruses such as respiratory syncytial virus (RSV), influenza, and SARS-CoV-2, they can still lead to serious complications. The virus primarily transmits via respiratory droplets, with outbreaks peaking during winter and spring. In resource-limited settings, administration of multiplex PCR assays is essential for precise diagnosis, yet it presents significant challenges. Recent studi
