Genomic Characterization of Multidrug-Resistant Myroides Odoratimimus from Pneumonic Piglets: A One Health Threat.

Myroides odoratimimus is an emerging opportunistic pathogen increasingly implicated in infections across human and animal populations. We previously reported the first outbreak of pneumonia in post-weaning piglets associated with M. odoratimimus, marking a significant shift in its recognized host range. The affected swine herd exhibited co-infection with Porcine Circovirus types 2 and 3 (PCV2 and PCV3), likely contributing to proliferative dermatitis and nephropathy syndrome (PDNS), reproductive abnormalities and immunosuppression. This study presents the genomic characterization of a multidru

The role of syndromic testing in pneumonia diagnosis: a comparison with culture methods.

INTRODUCTION: Pneumonia is an infection in the pulmonary tissue that is caused by bacteria, viruses, fungi, or parasites. Based on Indonesian Health Survey data for 2023, the prevalence of pneumonia was 10.8%. The rapid detection of pathogens accompanied by their antibiotic sensitivity pattern is crucial to obtain relevant outcomes. The gold standard test by bacterial culture needs approximately 72 hours. In order to speed this up, the PCR-based test was developed, such as syndromic testing. METHODOLOGY: The present study was a retrospective study conducted from May 2021 to July 2024, using t

Streamlining antibiotic use in community acquired pneumonia: A quality improvement initiative.

BACKGROUND: Evidence suggests a 3-day total duration and early transition to oral therapy is safe in hospitalized patients with community acquired pneumonia (CAP)-though such care is not standard in the United States. To implement these evidence-based practices, a multidisciplinary group led by a hospitalist and antimicrobial stewardship updated an order set targeted at non-intensive care unit (ICU) patients with CAP. OBJECTIVE: Here, we assess the impact of our intervention on diagnostic and antibiotic treatment measures and associated clinical outcomes. METHODS: This is a retrospective, pr

Overexpression of the crp gene promotes biofilm formation and increases antibiotic resistance in bovine-derived Klebsiella pneumoniae.

BACKGROUND: The multidrug resistance of bovine-derived Klebsiella pneumoniae is a significant concern, with biofilm formation serving as a major factor in the escalation of antibiotic resistance. The function of cAMP receptor protein (CRP), which is encoded by the crp gene and acts as a central regulator of environmental sensing and virulence, remains unclear in pathogenic strains derived from livestock. PURPOSE: This study aims to investigate the influence of CRP overexpression on biofilm formation and antibiotic resistancein bovine-derived Klebsiella pneumoniae, with a particular focus on i

METTL14/IGF2BP1 m6A axis promotes pyroptosis in Streptococcus pneumoniae-induced pneumonia by regulating NEK7 mRNA stability.

Streptococcus pneumoniae (S. pneumoniae) infection induces pyroptosis in human pulmonary artery epithelial cells (HPAEpiCs), which contributes to pneumonia pathogenesis. We aimed to investigate the regulatory role of N6-methyladenosine (m6A) modification mediated by methyltransferase-like (METTL) 14 in this process and elucidate the underlying molecular mechanisms. HPAEpiCs were infected with S. pneumoniae. Cell viability was assessed using the cell counting kit-8 assay, while cytokine concentrations were measured by enzyme-linked immunosorbent assay. Pyroptosis levels were analyzed through fl

Mortality among Children Aged 1-59 Months in India: Evidence from a Systematic Review and Meta-Analysis (2009-2024).

BACKGROUND: Mortality among Indian children aged 1–59 months remains a concern, yet evidence arises from hospital and community settings that reflect different populations and metrics. A setting-specific synthesis is needed to clarify mortality trends, causes, and associated factors. METHODS: We reviewed observational studies (2009–2024) on deaths, causes, or risk factors among children aged 1–59 months. Two reviewers independently extracted data. Hospital and community studies were analyzed separately, using setting-specific mortality definitions. Random-effects GLM models pooled mortality an

Molecular characterization of virulence and antibiotic resistance genes in Klebsiella pneumoniae isolated from sputum samples at a tertiary hospital in Ethiopia.

Klebsiella pneumoniae, a Gram-negative, encapsulated rod and prominent member of the ESKAPE pathogen group, ranks among the leading causes of bacterial pneumonia worldwide. It has become increasingly problematic due to multidrug resistance (MDR) and the emergence of hypervirulent strains. In Ethiopia, there are growing reports of the increasing prevalence of MDR K. pneumoniae strains. However, molecular data on the resistance mechanisms and virulence determinants among clinical isolates remain limited. This study molecularly identified K. pneumoniae in sputum samples from patients clinically s

Exploration of the correlation between clinical indicators and prognosis in hospitalized children with pneumonia and construction of a risk prediction model based on machine learning algorithms.

BACKGROUND: Childhood pneumonia is a leading cause of hospitalization and death in children under 5 years globally. Its prognosis varies individually and is affected by multiple clinical indicators, while traditional assessment lacks quantitative risk stratification tools. Machine learning (ML) enables comprehensive analysis of high-dimensional clinical data, making it valuable for identifying key prognostic factors and building robust prediction models to optimize clinical decision-making. METHODS: A total of 582 hospitalized children (1 month-5 years) with community-acquired pneumonia were

Comparative study of plasma microbial cell-free DNA sequencing to culture and polymerase chain reaction in pediatric community-acquired pneumonia with parapneumonic effusion or empyema.

Plasma microbial cell-free DNA (mcfDNA) sequencing is a novel diagnostic tool for pediatric complicated community-acquired pneumonia (cCAP). However, rigorous evaluation of real-world mcfDNA performance is lacking. We compared mcfDNA sequencing to a composite reference standard consisting of blood cultures, pleural fluid (PF) cultures, and targeted PF PCRs in children with cCAP requiring pleural effusion or empyema drainage at Children's Hospital Colorado from 2022 to 2024. We calculated positive/negative percent agreement (PPA/NPA), Jaccard similarity index, and theoretical time to pathogen d

Tau is necessary for Pseudomonas aeruginosa-induced blood-brain barrier dysfunction.

Many patients suffer from incident dementia after lung infections. Previous studies demonstrated that cytotoxic tau is released from the lungs in response to bacterial pneumonia, causing cognitive deficits and tau seeding. We aimed to determine the impact pneumonia has on blood-brain barrier (BBB) permeability, glial activation, and tau phosphorylation in the brain following infection and the involvement of tau. We found that lung infection with Pseudomonas aeruginosa (P. aeruginosa) increased BBB permeability, astrocyte activation, and phosphorylated tau (ptau) levels in the brain 24-hours (h

Reactive Infectious Mucocutaneous Eruption (RIME): An Australian Case Series.

Whilst Reactive Infectious Mucocutaneous Eruption (RIME) displays a low mortality rate, the condition results in significant morbidity and resource allocation, including prolonged hospital admission. As such, early diagnosis and optimisation of treatment is imperative. A retrospective review of paediatric patients in South Australia suffering with RIME across 2023 and 2024 was conducted, analysing trends in aetiology, diagnosis, management and long-term outcomes. Our findings emphasise the need for early input from a specialist paediatric dermatologist and highlight the need for vigilance duri
