Analysis of Risk Factors for Olfactory Dysfunction in CRSsNP Patients: A Prospective Case-Control Study.

OBJECTIVE: This study aimed to identify the risk factors associated with olfactory dysfunction (OD) in patients with chronic rhinosinusitis without nasal polyps (CRSsNP), a prevalent condition where impaired smell significantly reduces quality of life. The goal was to provide insights for improving clinical management strategies. METHODS: A prospective study was conducted on patients undergoing endoscopic surgery. CRSsNP patients were categorized into an OD group (n = 36) and a normosmic group (n = 36) based on olfactory tests, with 21 septoplasty patients as healthy controls. Demographic and

Isolation and characterization of mesenchymal stem cells from nasal polyps and olfactory mucosa: a comparative analysis of proliferation and multi-lineage differentiation capacity.

BACKGROUND: Nasal polyps (NPs) and olfactory mucosa (OM) are emerging as potential sources of mesenchymal stem cells (MSCs), which hold promise for regenerative medicine due to their multipotent differentiation capabilities. This study aimed to isolate MSCs from nasal polyps (NP-MSCs) and evaluate their differentiation potentials in comparison with healthy olfactory mucosa (OM-MSCs). METHODS: 16 human samples from NP and OM tissues were studied and MSCs were isolated enzymatically. Flow-cytometry analysis was done to characterize the isolated cells; then, tri-lineage differentiation capacity o

Successful Treatment with Stapokibart in a Severe Uncontrolled CRSwNP Patient: A Case of Rapid Symptom Resolution After Biologic and Surgical Failures.

Chronic rhinosinusitis with nasal polyps (CRSwNP), particularly the type 2 endotype driven by IL-4/IL-13, remains challenging in a subset of patients refractory to surgery, allergen immunotherapy (AIT), and existing biologics. This case report describes a 29-year-old male with severe, multirefractory CRSwNP who achieved rapid and sustained remission with stapokibart (CM310), a novel anti-IL-4Rα monoclonal antibody, after failing 2 endoscopic sinus surgeries, omalizumab, and AIT. Following a regimen of 300 mg every 2 weeks, the patient exhibited significant olfactory improvement within 1 week a

Importance of Smell Loss to Patients With Chronic Rhinosinusitis With Nasal Polyps: Options for Management and Recovery.

Primary diffuse type 2-dominant chronic rhinosinusitis with nasal polyps (CRSwNP) is an inflammatory disease of the nasal cavity and paranasal sinuses associated with significant morbidity. Impaired sense of smell is a cardinal symptom of CRSwNP and one of the most burdensome for patients, impacting quality of life, mental health, and even safety. Mechanisms of smell loss in CRSwNP may be related to conductive losses due to significant burden of nasal polyps, as well as the impact of type 2 inflammatory mediators on olfactory sensory neurons. Initial medical management frequently involves intr

Epithelial histone deacetylase 7 (HDAC7) constitutes a negative feedback loop restraining interleukin (IL)-13-driven eosinophilic inflammation in chronic rhinosinusitis.

BACKGROUND: Epithelial dysfunction is a central driver of type 2 inflammation in chronic rhinosinusitis with nasal polyps (CRSwNP). Although epigenetic mechanisms are known to regulate mucosal immunity, the specific role of histone deacetylase 7 (HDAC7) in modulating the magnitude of epithelial type 2 responses remains unclear. We hypothesized that HDAC7 functions as an inducible checkpoint to restrain excessive inflammation. OBJECTIVE: To explore the role of epithelial HDAC7 in restraining excessive inflammation in CRSwNP. METHODS: Primary human nasal epithelial cells were exposed to inflam

Case Report: Triple autoimmune overlap: rheumatoid arthritis, systemic lupus erythematosus, and hypereosinophilic asthma with systemic manifestations.

BACKGROUND: Overlap between rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE) ("rhupus") is recognized, but coexistence with a severe eosinophilic asthma syndrome is exceptionally rare. We describe a triple autoimmune overlap of RA, SLE, and hypereosinophilic asthma with systemic manifestations (HASM), initially managed as ANCA-negative eosinophilic granulomatosis with polyangiitis (EGPA) and subsequently re-classified in light of evolving concepts. CASE PRESENTATION: A 44-year-old woman with a 10-year history of seropositive RA developed alopecia, Coombs-positive hemolytic ane

Can microbial profiles influence type II inflammation in chronic rhinosinusitis with nasal polyps?

OBJECTIVE: This study aimed to assess how different treatments - dupilumab vs surgery - influence the nasal microbiota, type 2 inflammation, and clinical outcomes in CRSwNP patients. METHODS: This was a prospective observational study of 44 CRSwNP patients assigned to 6 months of biweekly dupilumab injections or functional endoscopic sinus surgery (FESS). Nasal microbiotas were analyzed at baseline and 6 months using culture techniques. Inflammatory biomarkers (IgE, eosinophils) and clinical endpoints (polyp score, SNOT-22, smell test) were measured. Patients were also stratified into groups

Peripheral blood eosinophils increase the risk on the occurrence of acute rhinosinusitis and transition to chronic rhinosinusitis: A Mendelian randomization study.

Our previous published research proved that patients with a higher level of peripheral blood eosinophils would suffer a higher risk of nasal polyps. Nevertheless, previous studies did not determine the causality between peripheral blood eosinophils and the risk of acute/chronic rhinosinusitis. This study aims to investigate the causal effect of peripheral blood eosinophils on acute/chronic rhinosinusitis using the Mendelian randomization (MR) method. The genome-wide association studies (GWAS) catalog was extracted from the integrative epidemiology unit open GWAS project. Based on the same alle

[Different immunological types of CRSwNP in the context of the new European EAACI nomenclature : Part 3: Hypersensitivity reactions of type VI].

BACKGROUND: Chronic rhinosinusitis (CRS) has a prevalence of up to 11% in Europe and the USA, making it one of the most common chronic diseases. Classification based on immunological endotypes is increasingly being integrated into the disease definition, particularly for chronic rhinosinusitis with nasal polyps (CRSwNP). Depending on the specific mechanisms underlying chronic tissue inflammation, different endotypes are characterized. Genetic and epigenetic changes in the mucosal immune system play a significant role in this context. Identifying endotypes can help to better understand disease

Efferocytosis deficiency exacerbates local inflammation and predicts recurrence in chronic rhinosinusitis with nasal polyps.

BACKGROUND: The role of efferocytosis in chronic rhinosinusitis (CRS), particularly CRS with nasal polyps (CRSwNP), remains poorly understood. OBJECTIVE: We comprehensively characterized efferocytosis in CRS and determined its association with inflammatory endotypes and clinical outcomes in CRSwNP. METHODS: Efferocytosis-related marker expression between nasal polyps and healthy nasal mucosa was detected by quantitative real-time PCR and immunohistochemistry. Public single-cell RNA sequencing profiles of CRS were reanalyzed to dissect efferocytosis at single-cell resolution. Associations bet

Development and Validation of a Nomogram Integrative of Clinical Characteristics and Serum Inflammatory Biomarkers for Predicting Postoperative Recurrence in Patients with Chronic Rhinosinusitis with Nasal Polyps.

INTRODUCTION: Postoperative recurrence remains a significant challenge in the management of chronic rhinosinusitis with nasal polyps (CRSwNP), imposing a heavy burden on patients and healthcare systems. Existing prediction models often rely on invasive tissue sampling or limited clinical phenotype. This study aimed to construct a broad-spectrum, noninvasive nomogram by integrating clinical characteristics with a panel of serum inflammatory biomarkers to predict the risk of recurrence after endoscopic sinus surgery (ESS). METHODS: A total of 312 patients with CRSwNP who underwent ESS at our ce

Clinical Remission in Chronic Rhinosinusitis With Nasal Polyps: a Delphi Consensus from the Rhinosinusitis Italian Network (RINET).

BACKGROUND: The concept of clinical remission in chronic rhinosinusitis with nasal polyps (CRSwNP) is gaining growing relevance in the era of biologic therapies. However, current definitions remain heterogeneous and lack standardized, operational criteria. This variability limits comparability across studies and hinders the implementation of remission as a therapeutic target in routine practice. OBJECTIVE: To develop a multidisciplinary, evidence-based, and clinically applicable definition of clinical remission in CRSwNP, integrating symptom-based, endoscopic, therapeutic, and timing-related
