This paper reports a rare case of congenital hairy polyp arising from the posterior edge of the nasal septum in a child. The patient presented with poor ventilation in the right nasal cavity since birth, which gradually progressed to bilateral nasal obstruction with purulent rhinorrhea and nasal malodor. Over the years, the patient underwent multiple MRI examinations at several hospitals, which consistently suggested"posterior nasal mass: meningoencephalocele teratoma ". In June 2024, the patient underwent "endoscopic transoral-nasal combined plasma resection of the nasal septal mass" under ge

A 3-year-old Connemara X filly was referred to a UK equine hospital for further investigation of unilateral nasal discharge and facial swelling. Diagnostic imaging documented the presence of a heterogenous, soft tissue opacity with areas of focal mineralisation within the right paranasal sinuses, completely obliterating the nasal cavity, in addition to endodontic disease and significant periapical pathology associated with the right permanent fourth premolar (Triadan 108). Subsequent histopathologic examination confirmed the mass as a fibro-inflammatory polyp.

Current guidelines recommend the presence of type 2 inflammation as one of the indications for biologic therapy in chronic rhinosinusitis with nasal polyps (CRSwNP). Although blood eosinophil (Eos) levels are commonly used as a biomarker for type 2 inflammation, their value in predicting differential responses to biologic treatment has not been verified by high-quality evidence. We searched the PubMed, Embase, Web of Science, and Cochrane Central Register of Controlled Trials (Central) databases, and hand-searched Clinicaltrials.gov for all RCTs that used biologic therapies for CRSwNP. This study aims to propose a recurrence risk assessment framework that incorporates anatomical severity, inflammatory status, comorbidity profile, and treatment adherence, and assist in the early identification of high-risk patients and guide personalized postoperative interventions. A total of 152 patients with chronic rhinosinusitis with nasal polyps who underwent functional endoscopic sinus surgery at our hospital between June 2021 and June 2024 were retrospectively included. All patients completed a standardized postoperative follow-up of 6 to 36 months (median duration: 18.4 months). Base

PURPOSE OF REVIEW: This review synthesizes current evidence and expert consensus on the long-term management of severe chronic rhinosinusitis with nasal polyps (CRSwNP) treated with biologics, as established by the Italian CHRONOS project.
RECENT FINDINGS: Accumulating real-world and clinical trial data confirm the sustained efficacy and safety of biologics targeting type 2 inflammation, enabling durable control and remission in a significant proportion of patients. Personalized dosing regimens, including dose-spacing strategies, appear feasible. The CHRONOS project provides practical guidanc

Mepolizumab is a monoclonal antibody targeting interleukin-5 that has been approved for the treatment of severe eosinophilic asthma (SEA) and chronic rhinosinusitis with nasal polyps (CRSwNP). This study aims to evaluate the sinonasal outcomes and safety of mepolizumab in patients with CRSwNP and SEA. This retrospective, real-life study included 19 patients aged 18 years and older who received 100 mg of subcutaneous mepolizumab every 4 weeks for at least 12 months. Sinonasal symptom severity, rhinorrhea, nasal congestion/obstruction, postnasal drip, and loss of smell were assessed using the vi

BACKGROUND: Nonsteroidal anti-inflammatory drugs (NSAIDs) are among the most common culprits of drug hypersensitivity. Previous classifications, including the European Academy of Allergy and Clinical Immunology (EAACI) and the European Network for Drug Allergy framework, left a subset of patients unclassified under the "blended" category, particularly those with anaphylaxis not fitting into standard groups. The World Allergy Organization (WAO) 2025 guidelines introduced new subcategories, such as mixed NSAID-exacerbated cutaneous disease (NECD) and mixed NSAID-exacerbated respiratory disease (

Chronic rhinosinusitis (CRS), a common upper respiratory disease, has been closely linked to Staphylococcus aureus and its enterotoxin superantigens. However, the specific mechanisms by which staphylococcal enterotoxin superantigens contribute to CRS pathophysiology remain unclear. To investigate the association between staphylococcal enterotoxin superantigen activity and CRS. A case-control epidemiological study was conducted from February 2022 to February 2024 at Northern Jiangsu People's Hospital. Seventy-eight patients with CRS constituted the case group, and 78 patients undergoing septum

PURPOSE OF REVIEW: Monoclonal antibodies ("biologics") have increasingly been used to manage chronic sinusitis with nasal polyps (CRSwNP). The aim of this manuscript was to compare biologic outcomes to traditional management of CRSwNP with endoscopic sinus surgery (ESS).
RECENT FINDINGS: There are now four approved biologics for the treatment of CRSwNP in the United States: dupilumab, mepolizumab, omalizumab, and tezepelumab. Cost analysis and comparison between phase 3 clinical trial results of biologics and ESS show ESS is at least as effective as biologics in controlling symptoms with sign

Eosinophilic chronic rhinosinusitis with nasal polyps (EosCRSwNP) is a multifaceted and refractory inflammatory condition, characterized by a troubling propensity for recurrence and complex underlying pathophysiology. A critical aspect driving its progression is the epithelial-mesenchymal transition (EMT) combined with sustained chronic inflammation within the nasal mucosa. However, the upstream regulatory mechanisms orchestrated by epithelial cells remain insufficiently elucidated. In this study, we employed an RNA-sequencing-based transcriptomics approach to profile nasal mucosal tissues acr

INTRODUCTION: Cystic fibrosis (CF) patients often contend with chronic rhinosinusitis with nasal polyposis (CRSwNP), with higher relapse rates after surgery compared to non-CF individuals. CRSwNP has a significant impact on their quality of life and increases pulmonary infections risk. Traditional medical and surgical interventions are frequently inadequate. Dupixent is endorsed for persistent CRSwNP following conventional treatment in non-CF subjects; its effectiveness in CF patients remains unexplored considering that those patients are frequently affected by mixed endotypes of CRS with some

BACKGROUND: Systemic type 2 inflammation, driven by T helper 2 (Th2)-type immune responses, is central to allergic inflammatory diseases, including atopic dermatitis, allergic rhinitis, asthma, nasal polyposis, and food allergy. These conditions frequently coexist and share common immunological mechanisms. The objective of this study is to describe the design and development of T2-Reg, a regional registry for Th2-mediated diseases, aimed at collecting real-life data to improve patient management and research.
METHODS: This multicentric retrospective and prospective observational study has bee

Severe asthma (SA) is a heterogeneous disease that remains uncontrolled despite optimized, high-dose inhaled therapy. It is also associated with substantial morbidity, corticosteroid exposure, and a significant healthcare burden. The advent of targeted biologic therapies has transformed SA management, making accurate biomarker-guided phenotyping essential. Fractional exhaled nitric oxide (FeNO), a noninvasive biomarker of IL-4/IL-13-driven airway inflammation, is widely used in asthma; however, interpreting its levels in SA is complex. This narrative review provides a biology-driven analysis o
