Spending and Utilization for Chronic Rhinosinusitis With Nasal Polyps Therapies Among Commercially Insured Beneficiaries.

The Food and Drug Administration (FDA) recently approved several novel therapies to treat chronic rhinosinusitis with nasal polyps (CRSwNP), though high prices may burden patients and payers. CRSwNP-specific spending varied widely by prescription drug among commercially insured patients, ranging from $7050 (EDS-FLU) to $28,324 (dupilumab). Condition-specific costs owed out-of-pocket ranged from $1036 (other drugs) to $3549 (mepolizumab).

Clinical efficacy and mechanisms of biologics for chronic rhinosinusitis with nasal polyps.

The management of chronic rhinosinusitis with nasal polyps (CRSwNP) can be challenging, particularly when standard treatments including intranasal corticosteroids and endoscopic sinus surgery do not result in adequate symptom control. CRSwNP is frequently characterized by a type 2 immune signature, and many patients have other comorbid type 2 conditions, including asthma. There are currently 3 biologic therapies approved for the treatment of CRSwNP-omalizumab, mepolizumab, and dupilumab-and there are promising therapies in development. Biologic therapies allow for improved patient quality of l

Traditional Chinese medicine in chronic rhinosinusitis: Mechanisms and postoperative recovery.

BACKGROUND: Chronic rhinosinusitis (CRS) is inflammation of the sinuses and nasal passages that lasts for >3 months. Its pathogenesis is complex, treatment is difficult, and it has multiple effects on patients. Although surgical treatment can effectively relieve the symptoms, the recurrence rate is high, and there are postoperative complications such as infection. At present, nasal spray hormone, antibiotics and other western drugs are used in clinical treatment, but there are drug dependence and toxic side effects. However, traditional Chinese medicine (TCM) has made remarkable progress in th

Comparing Medical and Surgical Management of Chronic Rhinosinusitis: A Systematic Review of Dupilumab and Endoscopic Sinus Surgery.

Chronic rhinosinusitis (CRS) is a multifactorial condition that significantly impacts patients' quality of life through persistent nasal obstruction, loss of smell, and facial pain. Endoscopic sinus surgery (ESS) has historically been the definitive option for patients who fail first-line therapies, yet emerging evidence suggests that Dupixent (dupilumab), an IL-4/IL-13 inhibitor, may provide robust medical management by directly targeting the underlying type 2 inflammatory pathways. This systematic review adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISM

Radiological score, asthma and NSAID-exacerbated respiratory disease predict relapsing chronic rhinosinusitis.

OBJECTIVES: The aim was to evaluate the predictive potential of Sinonasal Radiological (SR) and the Lund-Mackay (LM) score of sinus computed tomography (CT) scans on postoperative relapses of chronic rhinosinusitis (CRS). MATERIALS AND METHODS: CRS patients (n = 483, 12-80 years) underwent routine sinus CT scans. The SR score was defined by obstructed frontal recess (0 = no, 1 = yes) and visualization of middle and inferior turbinate (0 = anatomy can be easily visualized, 1 = anatomy cannot be easily visualized) on each side (a total of 0-6 points). Associations were analyzed by nonparametric

Long-term outcomes of functional endoscopic sinus surgery with selective vidian neurectomy for chronic rhinosinusitis with nasal polyps combined with allergic rhinitis and asthma.

BACKGROUND: Patients with allergic rhinitis (AR), chronic rhinosinusitis (CRS) with nasal polyps (ARwCRSwNP), and asthma often exhibit high recurrence rates after functional endoscopic sinus surgery (FESS), sometimes requiring revision surgery. This study evaluated the long-term outcomes of combining FESS with selective vidian neurectomy in patients with ARwCRSwNP and asthma. METHODS: Fifty-six ARwCRSwNP patients, including 27 with asthma, were divided into two groups: one received FESS with selective vidian neurectomy (group A), and the other underwent FESS alone (group B or control). Diseas

Impact of Topical Corticosteroid Irrigations on Sinonasal Wound Healing.

BackgroundChronic rhinosinusitis with nasal polyps (CRSwNP) is a chronic inflammatory disease frequently requiring surgical intervention and the long-term use of topical corticosteroids to control patient symptoms. However, corticosteroids may delay postoperative recovery following sinus surgery by disrupting tissue inflammation, a key mediator of early wound healing. This study therefore seeks to assess the impact of topical corticosteroids on sinonasal epithelial healing following local injury.MethodsPrimary human nasal epithelial cells (HNECs) were collected from two patients with CRSwNP an

Allergic Fungal Rhinosinusitis and Eosinophilic Chronic Rhinosinusitis Have Different Phenotypes in Japan.

BACKGROUND: Allergic fungal rhinosinusitis (AFRS) is a subtype of chronic rhinosinusitis driven by Types 1 and 3 allergies to fungi. In Japan, it is relatively rare and characterized by prominent eosinophilic infiltration of the sinonasal mucosa, together with eosinophilic mucin containing scattered fungi in the sinus cavity. Eosinophilic chronic rhinosinusitis (eCRS) involves similar eosinophilic infiltration and shares some clinical features with AFRS. However, the clinical differences between eCRS and AFRS remain to be fully elucidated. The aim of this study was to clarify the phenotypes of

Allergic Fungal Rhinosinusitis Diagnosis, Management, Associated Conditions, Pathophysiology, and Future Directions: Summary of a Multidisciplinary Workshop.

Allergic fungal rhinosinusitis (AFRS) is a unique endotype of chronic rhinosinusitis with nasal polyps (CRSwNP). Despite high recurrence rates and often more severe presenting signs compared with other subtypes of CRSwNP, research dedicated to AFRS has been lacking. Diagnostic criteria are outdated, the mechanistic relationship of AFRS to other associated diseases is unclear, and the pathophysiology of disease and risk factors for recurrence have not been well studied. In December 2023, a multidisciplinary group of rhinologists, otolaryngologists, pulmonologists, allergists, immunologists, sci

Reappraisal of Biologic Efficacy from Phase 3 Trials in Refractory Chronic Rhinosinusitis and Nasal Polyps.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is commonly associated with type 2 inflammation and may be punctuated by exacerbations requiring systemic corticosteroids and eventually a need for surgery. The use of biologics to target type 2 inflammation has greatly ameliorated the management of refractory CRSwNP. Currently available biologics act upstream by blocking the epithelial cytokine thymic stromal lymphopoietin (TSLP) or downstream blocking type 2 cytokines including Interleukin (IL4), IL5, and IL13, or Immunoglogulin E (IgE). Synthesizing the data from phase 3 randomized control t

An Insidious Source of Chronic Sinusitis and Recurrent Epistaxis: A Rare Case of Skull Base Glomangiopericytoma.

Glomangiopericytoma (GLC) is a rare malignant tumor of the sinonasal cavity that can lead to a variety of non-specific symptoms and consequently cause inaccurate diagnosis and delayed management. This can ultimately cause irreversible damage to the quality of life and result in a poor prognosis. We present the case of a 55-year-old male patient who suffered from the symptoms of chronic rhinosinusitis, anosmia, and multiple episodes of epistaxis with an endoscopic finding of a right-sided polyp-like mass within the sinonasal cavity. Further imaging studies revealed a mass originating from the s

Prevalence and Clinical Characteristics of Allergic Fungal Rhinosinusitis: A Retrospective Analysis From Two Health Centers in Kuwait.

Introduction Allergic fungal sinusitis (AFS) is a noninvasive form of chronic rhinosinusitis (CRS) driven by an allergic response to fungal organisms, primarily affecting atopic individuals. It represents a clinically significant yet frequently underrecognized condition. AFS commonly presents with nasal polyps, and their co-occurrence can aid early diagnosis. Understanding this relationship is essential, as delayed recognition may lead to recurrent symptoms and the need for more aggressive intervention. Methods A retrospective cohort study was conducted on 110 adult patients with nasal polypos
