Reclassification of NSAID hypersensitivity according to the WAO 2025 guidelines: insights from a cohort of 527 patients.

BACKGROUND: Nonsteroidal anti-inflammatory drugs (NSAIDs) are among the most common culprits of drug hypersensitivity. Previous classifications, including the European Academy of Allergy and Clinical Immunology (EAACI) and the European Network for Drug Allergy framework, left a subset of patients unclassified under the "blended" category, particularly those with anaphylaxis not fitting into standard groups. The World Allergy Organization (WAO) 2025 guidelines introduced new subcategories, such as mixed NSAID-exacerbated cutaneous disease (NECD) and mixed NSAID-exacerbated respiratory disease (

Relationship between staphylococcal enterotoxin superantigens and the development of chronic rhinosinusitis.

Chronic rhinosinusitis (CRS), a common upper respiratory disease, has been closely linked to Staphylococcus aureus and its enterotoxin superantigens. However, the specific mechanisms by which staphylococcal enterotoxin superantigens contribute to CRS pathophysiology remain unclear. To investigate the association between staphylococcal enterotoxin superantigen activity and CRS. A case-control epidemiological study was conducted from February 2022 to February 2024 at Northern Jiangsu People's Hospital. Seventy-eight patients with CRS constituted the case group, and 78 patients undergoing septum

Biologics in Chronic Rhinosinusitis with Nasal Polyps: The Otolaryngologist's Perspective.

PURPOSE OF REVIEW: Monoclonal antibodies ("biologics") have increasingly been used to manage chronic sinusitis with nasal polyps (CRSwNP). The aim of this manuscript was to compare biologic outcomes to traditional management of CRSwNP with endoscopic sinus surgery (ESS). RECENT FINDINGS: There are now four approved biologics for the treatment of CRSwNP in the United States: dupilumab, mepolizumab, omalizumab, and tezepelumab. Cost analysis and comparison between phase 3 clinical trial results of biologics and ESS show ESS is at least as effective as biologics in controlling symptoms with sign

DLX5 regulates nasal Epithelial Mesenchymal Transition and inflammation in EosCRSwNPs via the COL1A1-mediated Wnt/β-catenin Signaling.

Eosinophilic chronic rhinosinusitis with nasal polyps (EosCRSwNP) is a multifaceted and refractory inflammatory condition, characterized by a troubling propensity for recurrence and complex underlying pathophysiology. A critical aspect driving its progression is the epithelial-mesenchymal transition (EMT) combined with sustained chronic inflammation within the nasal mucosa. However, the upstream regulatory mechanisms orchestrated by epithelial cells remain insufficiently elucidated. In this study, we employed an RNA-sequencing-based transcriptomics approach to profile nasal mucosal tissues acr

Dupixent's Efficacy in Cystic Fibrosis Related Chronic Rhinosinusitis With Nasal Polyposis: A Pilot Study.

INTRODUCTION: Cystic fibrosis (CF) patients often contend with chronic rhinosinusitis with nasal polyposis (CRSwNP), with higher relapse rates after surgery compared to non-CF individuals. CRSwNP has a significant impact on their quality of life and increases pulmonary infections risk. Traditional medical and surgical interventions are frequently inadequate. Dupixent is endorsed for persistent CRSwNP following conventional treatment in non-CF subjects; its effectiveness in CF patients remains unexplored considering that those patients are frequently affected by mixed endotypes of CRS with some

Fractional exhaled nitric oxide in monitoring biological treatment for severe asthma in adults: Clinical implications and future perspectives.

Severe asthma (SA) is a heterogeneous disease that remains uncontrolled despite optimized, high-dose inhaled therapy. It is also associated with substantial morbidity, corticosteroid exposure, and a significant healthcare burden. The advent of targeted biologic therapies has transformed SA management, making accurate biomarker-guided phenotyping essential. Fractional exhaled nitric oxide (FeNO), a noninvasive biomarker of IL-4/IL-13-driven airway inflammation, is widely used in asthma; however, interpreting its levels in SA is complex. This narrative review provides a biology-driven analysis o

Predictive value of computed tomography for eosinophilic chronic rhinosinusitis with nasal polyps in different histopathologic criteria.

BACKGROUND: Computed tomography (CT) scan is a good and noninvasive prediction tool for eosinophilic chronic rhinosinusitis with nasal polyps (eCRSwNP), and how to choose the appropriate CT parameter is crucial-especially because there have not been unanimous histopathologic criteria to diagnose eCRSwNP. OBJECTIVE: This study sought to select the suitable CT parameter to predict eCRSwNP in different criteria. METHOD: This retrospective study included 147 CRSwNP patients who underwent a sinus CT scan and histopathological examination. Nine common CT parameters and 5 representative criteria of

A Systematic Review of the Specific Role of Biological Therapies in Aspirin-Exacerbated Respiratory Disease.

Aspirin-exacerbated respiratory disease (AERD) is characterized by chronic rhinosinusitis with nasal polyps (CRSwNP), asthma, and hypersensitivity to nonsteroidal anti-inflammatory drugs (NSAIDs). Although high-dose aspirin therapy after desensitization (ATAD) and surgery are effective, adverse effects and high dropout rates limit its use. The emergence of biologics for asthma and/or CRSwNP entails rethinking the management of AERD. There is no consensus on choosing between ATAD and biologics. This systematic review evaluates current evidence on the role of biologics in the management of AERD,
