Plasmacytoid dendritic cells modulate the production of mucosal IgA in IgA nephropathy.

BACKGROUND AND HYPOTHESIS: Aberrantly glycosylated immunoglobulin A (IgA) plays a central role in the pathogenesis of IgA nephropathy (IgAN). The activation of Toll-like receptor 9 (TLR9) has been shown to induce aberrant glycosylation of IgA via the a proliferation-inducing ligand (APRIL)-mediated pathway. TLR9 is known to be highly expressed in B cells and plasmacytoid dendritic cells (pDCs). Although the stimulation of TLR9 in B cells has been reported to promote the production of aberrantly glycosylated IgA, the effects of TLR9 stimulation on pDCs remain unclear. Therefore, in this study,

Recurrent post-tonsillectomy bleeding in pediatric patients.

OBJECTIVE: Post-tonsillectomy hemorrhage is a common risk after tonsillectomy. The goal of this study is to analyze the rate of recurrent PTH (R-PTH), and to determine potential risk factors for recurrent bleeding. METHODS: Retrospective cohort study of patients <18 years of age evaluated at a tertiary care pediatric hospital for PTH. Risk factors for patients with R-PTH episodes were evaluated, when compared to patients with a single bleeding episode. An assessment of healthcare utilization in those with R-PTH was performed. RESULTS: Among 18,819 tonsillectomies performed in the study perio

Resolution of central sleep apnea following upper airway surgery: A case report.

INTRODUCTION: Central sleep apnea (CSA) and obstructive sleep apnea (OSA) are characterized by recurrent pauses in breathing secondary to decreased central respiratory drive and upper airway obstruction, respectively. Oropharyngeal surgery is not a treatment option for CSA but may be considered in select patients with OSA. CASE PRESENTATION: A 37-year-old female presented with an Apnea Hypopnea Index (AHI) of 122/hour that included a Central Apnea Index (CAI) of 74/hour, and chronic tonsillitis with tonsillar hypertrophy and a baseline BMI of 36 kg/m2. Complete resolution of CSA and marked im

Preoperative Assessment of the Oral Environment in Patients Undergoing Tonsillectomy.

OBJECTIVE: Although maintaining oral hygiene may help prevent tonsillitis, the association between the two remains unclear. Tonsillectomy is a commonly performed procedure in otorhinolaryngology and has been studied from various perspectives; however, no studies have evaluated its relationship with the oral environment. We evaluated the influence of the oral environment on the pathogenesis of tonsillitis by comparing the preoperative oral environment in patients undergoing tonsillectomy according to surgical indication in a retrospective study. METHODS: We included 123 patients (64 male patie

Intracapsular vs Extracapsular Tonsillectomy Recovery Time: The FINITE Randomized Clinical Trial.

IMPORTANCE: Morbidity after traditional extracapsular tonsillectomy (ECTE) is high in adolescents and adults. Alternatively, intracapsular tonsillectomy (ICTE) may have a similar efficacy and reduce postoperative morbidity. OBJECTIVE: To investigate the recovery time and morbidity after ICTE compared to ECTE in 16- to 65-year-olds with recurrent or chronic tonsillitis. DESIGN, SETTING, AND PARTICIPANTS: The FINITE randomized, patient-blinded, 3-arm clinical trial compared ECTE, intracapsular coblation tonsillectomy (ICTEc), and intracapsular microdebrider tonsillectomy (ICTEm) in the treatme

Comparison Between Bipolar Diathermy and Coblation Assisted Tonsillectomy in Pediatric Population: A Randomized Experimental Study.

BACKGROUND AND AIM: Tonsillectomy is one of the most commonly performed surgical procedures on children. Indications for tonsillectomy include recurrent tonsillitis, obstructive sleep apnea, and peritonsillar abscess, among others. Bipolar diathermy and coblation surgeries are the most common used surgeries. Bipolar Diathermy efficiently manages postoperative hemorrhage but often results in increased postoperative pain due to higher thermal tissue destruction. In contrast, coblation is a procedure that offers a lower output of heat and results in lesser pain and shorter healing times. The pres

Chronic tonsillitis and its potential alternative treatment: Exploration of the antibacterial activity of ethanolic Mentha piperita leaf extract against bacterial isolates from patients.

Chronic tonsillitis is commonly associated with persistent bacterial infection, predominantly involving Gram-positive organisms, and increasing antimicrobial resistance has encouraged exploration of alternative therapeutic agents derived from medicinal plants. Mint leaves (Mentha piperita) are known to contain bioactive compounds with potential antibacterial activity; however, their antibacterial efficacy against isolates specifically obtained from patients with chronic tonsillitis has not been clearly established. The aim of this study was to evaluate the antibacterial activity of ethanolic M

Incidence and risk factors of pediatric post-tonsillectomy hemorrhage: a systematic review and meta-analysis.

BACKGROUND: Tonsillectomy ranks among the most frequently performed surgical procedures in the pediatric population. Post-tonsillectomy hemorrhage (PTH) represents one of its most severe complications, potentially resulting in anemia, hemodynamic instability, reoperation, and even mortality. Nevertheless, the incidence of and risk factors for PTH are not well defined. Consequently, we performed a systematic review and meta-analysis to examine and identify risk factors for this postoperative complication. METHODS: An extensive literature search was executed across six electronic databases (Pub

Morphometric and immunohistochemical reorganisation of the palatine tonsil in chronic tonsillitis and its response to combined ozone-ultrasound therapy.

BACKGROUND: Chronic tonsillitis is a common upper respiratory tract disease for which there is no objective tissue-level basis for judging conservative treatment. We characterised the morphometric and immunohistochemical reorganisation of the palatine tonsil in chronic tonsillitis and its response to a combined ozone-ultrasound protocol. METHODS: In a single-centre observational study with a pre-post design and no randomised comparator group, we examined 480 palatine tonsil specimens: histologically normal controls (n = 160), chronic tonsillitis (n = 180) and post-treatment specimens (n = 140

Systemic Immunoglobulin Changes Following Adenoidectomy, Tonsillectomy, and Adenotonsillectomy in Patients With Chronic Tonsillitis and Adenoiditis: A Prospective Study.

BACKGROUND: Chronic tonsillitis and adenoiditis are common inflammatory disorders of Waldeyer's ring that may lead to persistent systemic immune stimulation due to recurrent infection of tonsillar lymphoid tissue. Because the palatine tonsils and adenoids participate in mucosal immune defense and antibody production, concerns have occasionally been raised regarding the potential immunologic consequences of surgical removal of these structures. METHODS: A prospective clinical investigation was conducted involving 100 participants, including 75 patients with chronic tonsillitis or adenoiditis a

Cardiovascular outcomes with tirzepatide versus semaglutide in type 2 diabetes.

Preterm birth is associated with the majority of all death and chronic disability related to pregnancy, birth and the neonatal period. The costs to families and to the health care system are enormous. Current approaches to prevent or arrest preterm labour have been unsuccessful. This failure is largely based on our poor understanding of the regulation of the timing and maintenance of parturition. Oxytocin (OT) is the most potent known uterine stimulant. It is produced in the hypothalamus and secreted into the maternal bloodstream. However, OT also is produced within the uterine decidua in late
