Pulmonary hypertension in pediatric patients with cystic fibrosis during acute pulmonary exacerbations: prevalence and associated factors.

BACKGROUND: Cystic fibrosis (CF) is a multi-organ disorder in which respiratory complications account for the majority of its cause of mortality. This study aimed to investigate the factors associated with pulmonary hypertension (PH) in pediatric patients with CF and acute pulmonary exacerbations (PEx). METHODS: This is a prospective cross-sectional study that enrolled children with CF who were hospitalized with PEx in a university hospital between 2020 and 2022. All patients underwent echocardiography, and their pulmonary artery pressure (PAP) was measured. They were then divided into two gr

Likelihood of Usual Interstitial Pneumonia: A Novel Approach to Prognosis for Pulmonary Fibrosis.

BACKGROUND AND OBJECTIVES: In fibrotic lung disease, radiological features of usual interstitial pneumonia (UIP) are associated with adverse outcomes. The aim of this study was to evaluate the likelihood of definite UIP on HRCT as a determinant of outcome in patients with pulmonary fibrosis. METHODS: All available HRCT from the Australian IPF Registry (AIPFR) were retrospectively reviewed by two radiologists. The radiologists were asked to assign a likelihood (%) within each of the four ATS/ERS/JRS/ALAT 2018 UIP categories, summating to 100%, enabling them to not be constrained by a single UI

Identification of a Novel DNAAF3 Variant in a 54-Year-Old Patient With Newly Diagnosed Primary Ciliary Dyskinesia (PCD).

Primary ciliary dyskinesia (PCD) is a rare and heterogeneous inherited disease characterized by impaired mucociliary clearance. Patients with PCD typically present with recurrent respiratory infections resulting in the development of bronchiectasis. Even though awareness of the disease has increased over the years, PCD remains underdiagnosed. We here present a case of a newly diagnosed middle-aged female found to have a previously undescribed variant of the disease-associated DNAAF3 gene.

Haemophilus influenzae in the airways: canary in the coal mine or driver of disease?

INTRODUCTION: Overgrowth and colonization by non-typeable Haemophilus influenzae (NTHi) is a common feature of increasing disease severity, treatment resistance and increased susceptibility to disease exacerbations across chronic airways diseases (CADs). Whether NTHi is a driver of respiratory disease or reflects that the damaged airway has become a permissive environment for growth remains to be proven. AREAS COVERED: In this review, we discuss the potential roles of hypermutation, biofilm formation and intracellular living in allowing NTHi to adapt to living in the lungs of individuals with

Integrated traditional Chinese medicine and Western medicine strategies for the treatment of bronchiectasis: a comprehensive review.

Bronchiectasis is a complex and heterogeneous disease with various etiologies and clinical manifestations. While Western medicine (WM) primarily focuses on infection control, symptom management, and airway clearance techniques, traditional Chinese medicine (TCM) adopts a holistic strategy aimed at systemic regulation and immune modulation through herbal formulae and acupuncture. The integration of TCM and WM offers a comprehensive therapeutic framework that targets both clinical manifestations and the underlying pathophysiology. This review systematically outlines current WM treatment strategi

Myeloperoxidase-DNA complex: a marker and combined target for Pseudomonas aeruginosa-associated bronchiectasis.

Pseudomonas aeruginosa (P. aeruginosa)-associated bronchiectasis remains a clinical challenge due to lacking diagnostic biomarkers and targeted therapies. This multi-dimensional (clinical-cell-animal) study investigated the myeloperoxidase (MPO)-DNA complex, a marker of neutrophil extracellular traps (NETs), and the combination of AZD5904 (an MPO inhibitor) and DNase I (a NETs-DNA degrader). Key findings include: First, integrated evidence from bronchoalveolar lavage fluid analysis and immunofluorescence identified the MPO-DNA complex as a biomarker significantly associated with P. aeruginosa-

Alpha-1 antitrypsin deficiency in bronchiectasis: Evidence for an overlooked entity beyond COPD: A retrospective observational study.

Alpha-1 antitrypsin deficiency (AATD) is an autosomal co-dominant condition caused by mutations in the SERPINA1 gene. Chronic obstructive pulmonary disease/emphysema, asthma, and bronchiectasis are lung diseases associated with AATD. This study was designed to identify AATD in patients with bronchiectasis without emphysema and to demonstrate the frequency and distribution of AATD genotypes according to the type of bronchiectasis. The study was conducted as a single-center retrospective analysis between December 01, 2022 and December 31, 2024 in patients with bronchiectasis without emphysema. P

Pulmonary Nodule Prevalence at CT in Middle-Aged Participants from the Swedish CArdioPulmonary bioImage Study (SCAPIS).

Background Pulmonary nodules (PNs) are clinically challenging because differentiation between benign and malignant PNs is not possible at CT detection; recurring CT examinations and invasive procedures are often required in PN follow-up. Purpose To investigate PN prevalence in a middle-aged population and the risk factors associated with these nodules. Materials and Methods In this secondary analysis of the cross-sectional population-based Swedish CArdioPulmonary bioImage Study (known as SCAPIS, from November 2013 to November 2018), participants aged 50-64 years with chest CT data were analyze

Lipid profiling in nontuberculous mycobacterial pulmonary disease based on geospatial locations and disease severity.

BACKGROUND: The role of lipid profiles in the pathogenesis of nontuberculous mycobacterial pulmonary disease (NTM-PD) remains largely unexplored. This study aimed to identify lipid profile variations across geospatial lung lesions, their reflection in serum relative to disease severity and their diagnostic discriminative ability using lipidomic analysis. METHODS: Lipidomics was performed using hydrophilic interaction liquid chromatography-tandem mass spectrometry on lung tissues and serum samples. 960 lipid species were analysed across geospatial lung lesions (cavity wall, centre of cavity, g

Biofilm-mediated immune dysregulation in chronic pulmonary diseases: mechanisms and clinical implications.

Microbial biofilms are increasingly recognized as critical modulators of chronic airway inflammation and immune dysregulation in pulmonary diseases. This review summarizes current evidence on how biofilm formation and persistence alter host immune responses, contributing to the pathogenesis of chronic lung disorders. We first outline the characteristics of respiratory biofilms and the major pathogens involved. We then discuss how biofilms reshape innate and adaptive immunity-impairing pattern recognition receptor signaling, promoting neutrophil extracellular trap (NET) formation, altering macr
