Pathology of emphysema in chronic obstructive pulmonary disease.

Emphysema is an almost constant finding in the lungs of chronic obstructive pulmonary disease patients. Several types of emphysema are recognized by pathologists, but only the centrilobular (CLE) and panlobular (PLE) emphysemas are found in association with smoking. In this review, the morphological and functional differences between CLE and PLE are described, and it is suggested that they could arise as different abnormalities as a result of the same insult, cigarette smoke. In CLE: 1) the destruction of the lung is uneven and originates around the airways; 2) the membranous bronchioles are t

Distribution of lung density and mass in patients with emphysema as assessed by quantitative analysis of CT.

STUDY OBJECTIVE: To assess the effects of emphysema on the apex-to-base gradient of lung density (D) and lung mass (M) and to explore the relationship between M and lung function. METHODS: CT scans of whole lungs were performed in 12 healthy subjects and 29 patients who were breathing at functional residual capacity, after which lung function tests were performed. Whole D and M and regional D (RLD) and M (RLM) were calculated. The degree of emphysema was scored. RESULTS: The RLM for each height did not differ significantly between patients with disease and healthy subjects, while RLD was sig

[Role of emphysema in the etiology of functional impairment in patients with severe chronic obstructive pulmonary disease. Study with high resolution computerized tomography].

PURPOSE: To investigate whether high-resolution CT (HRCT) can detect the subjects with massive emphysematous destruction in a group of patients with severe chronic obstructive pulmonary disease (COPD) and therefore be of help in selecting the candidates to surgical lung volume reduction. MATERIAL AND METHODS: We examined 40 former smokers with severe COPD (FEV1, < or = 40% of the predicted value, with no major improvement after inhalation of bronchodilators). Clinico-functional assessment included: a flow/volume loop (mean FEV1 = 28.6% of predicted), arterial blood gas analysis at rest breath

The usefulness of 99mTc-Technegas scintigraphy for diagnosing pulmonary impairment caused by pulmonary emphysema.

X-ray computed tomography (CT) has been used for diagnosis of pulmonary emphysema because it can reveal the morphology of low attenuation areas. Recently, 99mTc-Technegas imaging, one of several types of scintigraphic techniques, has been used for ventilation scintigraphy. Technegas scintigraphy was performed on 15 patients with pulmonary emphysema, and we compared the extent and degree of abnormal findings on Technegas scintigraphy with the extent of low attenuation areas shown by CT. We classified the findings of Technegas imaging into three grades, from mild to severe, according to the exte

Which type of diffuse emphysema is adequately contracted by the Nd:YAG laser. An ex-vivo experiment.

Diffusely emphysematous lungs are not always effectively contracted by laser therapy; however, which type of diffuse emphysema that responds to laser therapy remains unclear. We macroscopically and histopathologically examined human lung tissue, which was resected from patients with carcinoma, after irradiation with an Nd:YAG laser. Forty-six lung lobes were irradiated with a non-contact mode Nd:YAG laser at a power setting 15 watts. Macroscopically, twenty samples of normal lungs revealed moderate contraction, fourteen samples of predominantly centrilobular diffuse emphysema showed significan

Relations between cardiopulmonary exercise testing and quantitative high-resolution computed tomography associated in patients with alpha-1-antitrypsin deficiency.

High-resolution computed tomography (HRCT) can be used to diagnose and quantify emphysema noninvasively, as significant correlations have been found between the histological grade on resected lung specimens and quantified (q) computed tomography (CT). In this study, we performed thin section qHRCT in patients with severe hereditary alpha-1-antitrypsin (AAT) deficiency. AAT deficiency is the most common genetic cause of emphysema in adults, and exercise intolerance is the most disabling, distressing consequence of emphysema for the majority of patients. qHRCT was used to quantify precisely the

[The recording of parenchymal lung changes in smokers by high-resolution computed tomography].

PURPOSE: It has been the aim of the study to assess parenchymal changes in the lung with high-resolution CT in healthy heavy, moderate, and non-smokers. MATERIAL AND METHODS: We prospectively evaluated CT changes in 42 healthy heavy smokers (gr. (group) 2, > or = 30 pack-years), 40 moderate smokers (gr. 1, < 30 pack-years) and 38 non-smokers (gr. 0). For CT-analysis, we used a visual grading and computer-based analysis system (Mipron, Kontron GmbH, Munich, Germany). RESULTS: Productive cough, dyspnoea and chronic bronchitis were more common in smokers than in non-smokers (p < 0.05). Patholog

A scanning electron microscopic investigation of genetic emphysema in tight-skin, pallid, and beige mice, three different C57 BL/6J mutants.

Three mutants of the C57 BL/6J strain, i.e., the tight-skin (Tsk), pallid (pa), and beige (bg) mice have been reported to develop spontaneous emphysema. However, the pathogenic mechanisms of this lesion may be different in the three mutants. Differences and similarities of these models were investigated by means of scanning electron microscopy. A light microscopic investigation provided the background for the SEM study. C57 BL/6J (control), pa, Tsk, and bg mice were killed when they were 1, 12, and 24 months old. At light microscopic investigation the lungs of the controls appeared normal at a

[Comparison between high-resolution computed tomography and 99mTc-technegas SPECT pulmonary emphysema].

Scintigraphy with 99mTc-technegas was recently introduced for clinical imaging of lung ventilation. This method has been found to be useful in emergencies, to be more suitable for single photon emission computed tomography (SPECT) than other agents used in ventilation scintigraphy, and could reveal abnormalities in ventilation more easily than high resolusion computed tomography (HRCT) in pulmonary emphysema. We compared 99mTc-technegas SPECT with HRCT in six regions: the right upper, middle, and lower lobes, the left upper lobe, the lingula, and the left lower lobe, in 15 patients with pulmon

[Loss of alveolar attachments in smoker's lungs and its relation to small airway abnormalities].

The alveolar attachments and small airway abnormalities in the lungs of smokers were measured by computerized imaging to study the relationship between loss of alveolar attachments and small airway abnormalities, the smoker's lungs were divided into three groups: no emphysema (NE), centrilobular emphysema (CLE) and panlobular emphysema (PLE). The distance between normal alveolar attachments (Distance) and percent abnormal alveolar attachments (% Abnormal) in CLE were significantly higher than in NE (P < 0.05 and P < 0.01), and no significant difference in the number of normal alveolar attachme

Bronchiectasis and homozygous (P1ZZ) alpha 1-antitrypsin deficiency in a young man.

Pulmonary complications of alpha 1-antitrypsin deficiency are most commonly manifested by panlobular emphysema. This report describes histologically proven bronchiectasis in a 21 year old man with massive haemoptysis and homozygous deficiency of alpha 1-antitrypsin. Neither panlobular emphysema nor cirrhosis of the liver were present. Bronchiectasis must be considered part of the spectrum of the pulmonary pathology that may be encountered in individuals with alpha 1-antitrypsin deficiency.

Can computed tomography quantify pulmonary emphysema?

In the last decade, several studies have suggested the possible role of computed tomography (CT) in the detection and the quantification of pulmonary emphysema. In order to verify whether this new method is adequately validated, this article gives an overview of these studies. The review shows that most of the studies used conventional CT and were based on visual scoring. Only a few were based on high resolution CT (HRCT) or concerned objective measurements of computed density. In addition, only a few studies included normal subjects and distinguished centrilobular from panlobular emphysema. T

[Lung transplantation in COPD].

Pulmonary transplantation can be performed as a treatment for panlobular emphysema. Young patients can benefit from pulmonary transplantation: double lung transplantation for patients aged less than 55 years and single lung transplantation for patients aged less than 60 years. Only patients with major functional impairment should be considered for transplantation. It is assessed by pulmonary function tests, but also by evaluating the intensity of dyspnea and the decreased quality of life. In emphysema, transplantation is the treatment of the respiratory handicap without decreased life expectan

Morphometry of small airways in smokers and its relationship to emphysema type and hyperresponsiveness.

Based on our previous finding, of increased small airways disease in centrilobular emphysema (CLE) when compared with panlobular emphysema (PLE), we hypothesized that smokers who develop CLE would have increased airway responsiveness associated with airway inflammation and exaggerated airway narrowing, but not smokers with PLE. We compared preoperative methacholine challenge with the morphologic and cellular inflammatory characteristics of the airways in the lungs of six nonsmokers, 10 smokers with CLE, and five smokers with PLE. The airways of the CLE group were narrower than those of the non

Loop-sheet polymerization: the structural basis of Z alpha 1-antitrypsin accumulation in the liver.

1. The Z deficiency variant of alpha 1-antitrypsin predisposes the homozygote to early-onset panlobular emphysema and results in the accumulation of antitrypsin within the hepatocyte, which leads to hepatocellular damage and cirrhosis. The mechanism of this accumulation has been shown to be due to the Z mutation (Glu-342-->Lys) perturbing the structure of the protein, allowing a unique interaction between the reactive-centre loop of one molecule and the A sheet of a second. This loop-sheet polymerization occurs spontaneously at 37 degrees C in purified plasma Z but not M antitrypsin. The rate

Morphological and cellular basis for airflow limitation in smokers.

Airflow limitation has two well-defined components, increased resistance, which is found predominantly in the small airways, and loss of elastic recoil. Small airways contribute to the increased resistance to flow by the narrowing of the airway lumen. Morphometric studies have shown that smokers have increased epithelial abnormalities, cellular inflammatory infiltrates in the airway wall, increased muscle and fibrosis, when compared with nonsmokers. Along with these anatomical changes, an increased percentage of airways < 400 microns in diameter is found. In addition to the measured changes, o

[A case of Swyer-James syndrome--a morphological study with inflated fixed lung].

Morphological features of Swyer-James syndrome were studied in a specimen obtained during left pneumonectomy. A 67-year-old woman was admitted because her left lung was hyperlucent and contained a solitary nodule. Primary lung adenocarcinoma complicated with Swyer-James syndrome was diagnosed, and left pneumonectomy done. The left upper lobe was inflated and fixed for morphological study. The volume of the specimen was obviously lower than normal but the bronchial branching was normal. Macroscopically, the specimen had irregular bronchial walls, bronchiectatic changes, and dilation of distal a
