Pseudomonas aeruginosa phenotypes associated with radiological severity in non-cystic fibrosis bronchiectasis.

BACKGROUND: Pseudomonas aeruginosa (PA), the most significant respiratory pathogen in patients with non-cystic fibrosis (CF) bronchiectasis, exhibits various phenotypes in vitro. We hypothesized that PA phenotypes are associated with radiographic severity in adults with non-CF bronchiectasis. METHODS: We conducted a retrospective cross-sectional study to determine the association between PA phenotypes and radiographic severity. We analyzed 140 adults with non-CF bronchiectasis who underwent high-resolution computed tomography (HRCT) between July 2022 and December 2023. We characterized 12 bact

A deep Siamese network framework for precision phage selection in pulmonary infections.

Pulmonary infections pose a significant global health challenge to human life and health. In patients with chronic pulmonary diseases such as cystic fibrosis and bronchiectasis, structural abnormalities of the airways and impaired mucociliary clearance contribute to recurrent and challenging pulmonary infections. These infections are frequently complicated by antimicrobial resistance, making them difficult to treat with conventional antibiotics. As a result, phage therapy has emerged as a promising alternative for treating resistant pulmonary infections. Recently, the integration of artificial

Treatable traits in children with bronchiectasis.

Bronchiectasis in children is a heterogeneous, chronic respiratory condition for which standardised, evidence-based management is essential. While international guidelines have improved diagnostic and therapeutic consistency, the treatable traits (TT) approach offers a complementary precision-medicine framework aimed at identifying all clinically relevant, measurable, and modifiable factors in individual patients. Through a comprehensive literature review, we identified 40 potential TT in paediatric bronchiectasis, spanning aetiological, pulmonary, extrapulmonary, and behavioural/environmental

Short and long-term effects of a play-based therapeutic exercise programme in children with non-cystic fibrosis bronchiectasis: A randomised controlled trial.

OBJECTIVES: To determine whether an 8-week, play-based therapeutic exercise program increases the proportion of children with bronchiectasis who remain exacerbation-free over 12-months. Secondary objectives were to assess effects on time of exacerbations, fundamental movement skill (FMS) proficiency, moderate-to-vigorous physical activity (MVPA), aerobic fitness, perceived movement competence, and health-related quality of life (HRQoL). DESIGN: Multi-site, observer-blinded, parallel-group randomised controlled trial. METHODS: Children aged 4 to 13-years with bronchiectasis, were randomised 1

Assessing anaerobe detection in routine sputum analyses from cystic fibrosis patients.

Pulmonary involvement in cystic fibrosis (CF) includes bronchiectasis and chronic airway infection, with nearly half of the airway bacteria being anaerobes. Some anaerobic species, such as Porphyromonas catoniae, have been identified as predictive biomarkers for Pseudomonas aeruginosa colonization, with P. catoniae decreasing as the disease progresses. Although 16S rRNA metagenomics offers a comprehensive view of airway anaerobes, it is not routinely performed in clinical microbiology laboratories. This study aimed to evaluate the ability of routine sputum culture to identify strict anaerobes

Prophylactic antibiotics in chronic pediatric lung disease: balancing benefit, resistance, and future care models.

INTRODUCTION: Children with chronic suppurative or inflammatory lung diseases, including cystic fibrosis (CF), non-CF bronchiectasis (NCFB), chronic suppurative lung disease (CSLD), and primary ciliary dyskinesia (PCD), experience recurrent infection and persistent neutrophilic airway inflammation that together drive progressive lung injury. Prophylactic antibiotic strategies have therefore been explored to interrupt this infection - inflammation cycle, encompassing both pathogen-specific approaches and longer-term anti-exacerbation or immunomodulatory therapies. AREAS COVERED: We review clin

Clinical implications of a novel SERPINA1 variant c.236 T > A: Challenges in characterizing new rare alpha-1 antitrypsin mutations.

Severe alpha-1 antitrypsin deficiency (AATD) is a rare genetic condition characterized by low levels of alpha-1 antitrypsin (AAT), leading to progressive lung and/or liver disease. Most severe cases are linked to the Z allele (c.1096G > A (p.Glu366Lys)) of the SERPINA1 gene but characterizing patients with rare mutations remains challenging. This case report discusses the clinical significance of a novel SERPINA1 variant, c.236 T > A (p.Val79Glu; ClinVar accession SCV007334878), and the challenges in profiling such cases. Two male siblings carried both the common Z allele mutation and the nove

Association of Chronic Rhinosinusitis and Pseudomonas Aeruginosa in Sputum of Patients With Non-Cystic Fibrosis Bronchiectasis.

BACKGROUND: Pseudomonas aeruginosa (P aeruginosa), Haemophilus influenzae (H influenzae), and Staphylococcus aureus (S aureus) may chronically infect bronchiectatic airways. Chronic rhinosinusitis (CRS) is common in people with bronchiectasis. Bacterial airway infection and CRS are associated with greater bronchiectasis disease severity. However, the relationship between these pathogens and CRS in people with bronchiectasis is unclear. RESEARCH QUESTION: Is history of CRS associated with sputum positivity for P aeruginosa, S aureus, and/or H influenzae in people with bronchiectasis? STUDY DE

Inhaled Antibiotic and Biologic Formulations Targeting Pseudomonas aeruginosa.

Lower respiratory tract infections caused by Pseudomonas aeruginosa are a global concern. Patients with chronic lung diseases such as cystic fibrosis and non-cystic fibrosis bronchiectasis often do not receive adequate antibiotic delivery through conventional routes. P. aeruginosa employs several mechanisms, including biofilm formation and efflux pumps to limit the accumulation of bactericidal drug concentrations. Direct drug delivery to the lung epithelial lining fluid can increase antibiotic concentration and reduce treatment failure rates. This review discusses current research and developm

Genetic strategies for negative or variant of uncertain significance findings in exome sequencing in hereditary bronchiectasis: a case series.

Hereditary bronchiectasis comprises a group of rare monogenic disorders, with cystic fibrosis (CF) and primary ciliary dyskinesia (PCD) representing the major subtypes. Exome sequencing (ES) remains a central modality for molecular diagnosis; however, it leaves more than half of clinically suspected cases unresolved, largely because it cannot reliably detect copy number variations, deep intronic variants, pseudogene-associated variants, and frequent identification of variants of uncertain significance (VUS). This case series describes five hereditary bronchiectasis cases with initial ES-negati

Emphysema or Bronchiectasis with Pulmonary Nodules: Impact on The Risk of Malignancy.

OBJECTIVE: The increasing use of computed tomography (CT) has led to a significant rise in the detection of pulmonary nodules. This has resulted in more studies aimed at identifying risk factors associated with increased detection of early-stage lung cancer. The aim of this study is to investigate the effect of coexisting emphysema or bronchiectasis on the incidence of malignancy in pulmonary nodules. MATERIAL AND METHODS: The study included 212 patients with pulmonary nodules detected on CT images. They were divided into three groups based on the presence of emphysema or bronchiectasis. The

[Autoantibody positivity with bilateral pulmonary ground-glass opacities and consolidations].

A 52-year-old male patient presented with a 6-month history of cough and exertional dyspnea that had worsened over 2 days. Six months earlier, he had developed exertional dyspnea accompanied by a paroxysmal dry cough. At another hospital, anti-RNP/Sm and anti-Jo-1 antibodies were weakly positive, anti-Ku antibodies were positive, and the rheumatoid factor level was elevated. High-resolution computed tomography (HRCT) of the chest demonstrated pulmonary emphysema, honeycomb pattern in the lower lobes, scattered patchy ground-glass opacities and consolidations, as well as traction bronchiectasis

LILRB1 Modulates Neutrophil Migration, NETosis, and Inflammation in Drug-Resistant Pseudomonas aeruginosa-Associated Bronchiectasis.

BACKGROUND: Patients with drug-resistant Pseudomonas aeruginosa (P. aeruginosa)-associated bronchiectasis often exhibit persistent neutrophilic airway inflammation. The immunomodulatory receptor leukocyte immunoglobulin-like receptor B1 (LILRB1) is known to function as an inhibitory checkpoint in immune responses, yet its specific role in regulating neutrophil function in drug-resistant P. aeruginosa-associated bronchiectasis remains incompletely understood. This study aimed to characterize the function of LILRB1 in this clinical context. METHODS: Clinical samples were obtained from bronchiec
