Cerebral microbleed patterns and the risk of incident dementia in elderly adults: The ARIC study.

OBJECTIVE: The association between cerebral microbleeds (CMBs), an indicator of microvascular damage on MRI, and incident dementia (ID) remains inconclusive. We aim to investigate the interplay of CMB presence, number, and location with ID in a large community-based elderly cohort. METHODS: The study included 1532 dementia-free participants (aged ≥ 65 years) derived from baseline examinations (2012-2013) of the Atherosclerosis Risk in Communities Neurocognitive Study (ARIC-NCS). The ID cases were diagnosed at visits 6 (2016-2017), 7 (2018-2019), and 8 (2019-2020). Cox proportional-hazards mod

Occupational lung disease: a case of pulmonary siderosis in a welder worker.

BACKGROUND: Pulmonary siderosis (PS), or welder's lung, is an occupational lung disease caused by chronic inhalation of iron dust or fumes. Unlike other pneumoconioses, it typically follows a benign course without significant fibrosis, though prolonged exposure may lead to respiratory complications. CASE PRESENTATION: A 44-year-old male welder presented with progressive dyspnea (mMRC grade 3), reduced exercise tolerance, and a history of recurrent chest infections. Diagnostic evaluations, including imaging and pulmonary function tests, were conducted to assess his condition. Findings suggeste

Cerebral Amyloid Angiopathy Imaging Markers, Radiological Boston Criteria, and Thrombolysis Outcomes in Ischemic Stroke.

BACKGROUND: Cerebral amyloid angiopathy (CAA) is thought to increase the risk of postthrombolytic intracranial bleeding, yet CAA neuroimaging markers and magnetic resonance imaging criteria have not been systematically evaluated in large acute stroke trials. We, therefore, examined the association of radiological Boston CAA criteria and their constituent markers with hemorrhagic risks and functional outcomes after intravenous thrombolysis in the AcT trial (Alteplase Compared to Tenecteplase). METHODS: Blinded raters recorded lobar cerebral microbleeds, cortical superficial siderosis, white ma

[Infratentorial superficial siderosis : a diagnostic challenge from the general practice to the hospital].

Superficial siderosis is a rare neurological evolutive condition that encompasses two distinct entities classified according to their cortical or infratentorial localisation. The disease is caused by the neuronal destruction of the central nervous system linked with hemosiderin deposits. The aetiology is a sustained low-intensity bleeding located at the supratentorial subarachnoid space for the cortical superficial siderosis or from the subarachnoid space for the infratentorial superficial siderosis. The triad sensorineural hearing loss, cerebellar ataxia and pyramidal signs should suggest inf

Lifelong trajectories of modifiable vascular risk factors and relation to cerebral small vessel disease in the Framingham Heart Study.

BACKGROUND: Cerebral small vessel disease (CSVD) is strongly linked to stroke and dementia risk, frequently predating clinical events for years to decades. Preventive efforts focus on treatment of modifiable vascular risk factors (VRF), but the complex changes in VRF over long periods in relation to CSVD burden remain unclear. Thus, we aimed to characterize life-long VRF trajectories in community-dwelling individuals and relate them to CSVD burden. METHODS: Framingham Heart Study participants from the Original and Offspring cohorts with six or more repeated VRF assessments over their lifetime

The Rusty Eye: Ocular Siderosis Masquerading as Chronic Anterior Uveitis.

Ocular siderosis (OS) is a progressive, vision-threatening condition caused by a retained, iron-containing intraocular foreign body (IOFB). It can be easily mistaken for chronic anterior uveitis, especially in the absence of a clear history of prior ocular trauma. This diagnostic pitfall can delay appropriate management, worsening the visual prognosis. A healthy 43-year-old man presented with recurrent redness, photosensitivity, and progressive blurred vision in the left eye, of one-year duration, worsening over the past three months. He was initially diagnosed with chronic anterior uveitis wi

Probable cerebral amyloid angiopathy - related inflammation in a 32-year-old woman with down syndrome.

Inflammatory cerebral amyloid angiopathy (CAA-RI) is a variant of CAA, rarely seen in young individuals with Down syndrome (DS). We describe a 32-year-old female with DS who developed a systemic inflammatory state followed by acute neurological symptoms, including seizures and hemiparesis. Brain MRI disclosed asymmetric T2/FLAIR hyperintensities with leptomeningeal enhancement and focal superficial siderosis on susceptibility-weighted imaging, without microbleeds; cerebrospinal fluid showed marked inflammation. After partial improvement with corticosteroids, rituximab was started due to persis

Ocular siderosis caused by a subretinal iron foreign body masquerading as a blood clot: a case report.

PURPOSE: We report a case of ocular siderosis caused by a subretinal iron foreign body. OBSERVATIONS: A 30-year-old male construction worker injured by a small piece of shrapnel while working visited an eye clinic for decreased vision in left eye on day X, and was subsequently referred to our hospital with a diagnosis of vitreous hemorrhage on day X+1. Although a penetrating foreign body was suspected, no obvious wound or intraocular foreign body was found even after the hemorrhage subsided. On X+3 months, a rhegmatogenous retinal detachment was found and a lens-sparing vitrectomy was perform

Low-intensity rim on T2-weighted brainstem imaging: a universally observed structure exhibiting a negative magnetic susceptibility effect.

PURPOSE: In healthy participants, T2 physiological rim (T2-PR) similar to superficial siderosis has been observed in the brainstem. This study aimed to determine the incidence and causes of T2-PR. MATERIALS AND METHODS: To determine the incidence, T2-weighted imaging (T2WI) was performed on 106 patients (65 males; mean age: 62.5 years) using a 3.0-T scanner and visually assessed by two radiologists. T2-PR presence in 22 brain areas was evaluated using the following system: 0 for absence, 1 for < 50% surface, 2 for ≥ 50% but not the entirety, and 3 for the entirety. After assessing inter-rater

Topography and cognitive impact of brain iron deposition in cerebral amyloid angiopathy: Evidence from 7T quantitative susceptibility mapping.

INTRODUCTION: While brain iron accumulation has been linked to cognitive impairment in aging and small vessel diseases, its spatial pattern and clinical significance in cerebral amyloid angiopathy (CAA) remain poorly defined. METHODS: Quantitative susceptibility mapping (QSM) was used to assess region-specific iron deposition across cortical, subcortical, and limbic areas. Associations with conventional cerebral small vessel disease (CSVD) markers-white matter hyperintensities (WMHs), perivascular spaces (PVS), cerebral microbleeds (CMBs), and cortical superficial siderosis (cSS)-were analyze

Perivascular Spaces Are Associated With CSF Aβ in Cerebral Amyloid Angiopathy but Not in Deep Perforator Arteriopathy.

BACKGROUND: Centrum semiovale perivascular spaces (CSO PVS) are common to different cerebral small vessel disease (CSVD) subtypes, yet their enlargement to the point of visibility on magnetic resonance imaging is thought to occur through distinct mechanisms. In cerebral amyloid angiopathy (CAA), CSO PVS are thought to reflect impaired perivascular Aβ (amyloid-β) drainage, whereas in deep perforator arteriopathy (DPA) they may result from microangiopathy-related arterial stiffening and altered fluid dynamics. The extent to which this can be confirmed in vivo remains unclear. METHODS: We retros

Two autopsy cases of superficial siderosis: ultrastructural similarity across traumatic and non-traumatic contexts and forensic uncertainties.

Superficial siderosis of the central nervous system (SSCNS) is increasingly recognized radiologically but remains rarely documented in medicolegal autopsies, where its role in the cause and manner of death is often uncertain. We aimed to characterize the neuropathological features and medicolegal implications of SSCNS in two autopsy cases. We examined two men with SSCNS, one with a remote traumatic subarachnoid hemorrhage and one diagnosed with idiopathic SSCNS. In both cases, the brain and upper cervical cord showed symmetrical brown discoloration of the cerebellum and brainstem, with widespr

[Amyloid β-related angiitis (ABRA) with dural contrast enhancement].

An 82-year-old woman presented with a rapidly progressive decline in cognitive function, which had previously been worsening over several months. Brain MRI revealed a lesion predominantly involving the leptomeninges of the right parietal lobe. Based on the presence of leptomeningeal-predominant lesions, cerebral microbleeds, and superficial siderosis, amyloid β-related angiitis (ABRA) was suspected. However, due to atypical findings such as dural enhancement, we performed a brain biopsy, leading to a definitive diagnosis of ABRA. Steroid therapy resulted in improvements in both clinical sympto
