Quantifying Chlamydia in the Mouse Gastrointestinal Tract.

Chlamydia trachomatis (CT) causes genital tract infection in humans, but the clinical significance of C. trachomatis detection in the gastrointestinal (GI) tract is an area of active research. Chlamydia muridarum (CM) is a mouse-adapted species used to investigate chlamydial biology and pathogenicity in both the genital and gastrointestinal tracts. Since chlamydial colonization of different segments of the GI tract may lead to distinct consequences, quantifying chlamydial yields across these regions will facilitate understanding of chlamydial biology and pathogenicity. This chapter describes a

Animal-Free Intestinal Epithelial 2D "Organoid-Sheet" Cultures Using Invasin.

The intestinal epithelium consists of a single layer of polarized cells that serves as a crucial barrier against pathogens while mediating food digestion and nutrient absorption. The differentiated villus cells, important for the intestine's function, originate from rapidly dividing Lgr5+ stem cells located in the intestinal crypts. These stem cells can be expanded in vitro and form three-dimensional (3D) "mini-organs" known as intestinal organoids, which closely phenocopy the intestinal epithelium. The development and maintenance of these 3D organoids depend on a defined growth factor cocktai

An Integrated Workflow for Culturing and AI-Based Profiling of Mouse Intestinal Epithelium.

Two-dimensional (2D) culture systems are powerful in vitro tools that have revolutionized the study of intestinal stem cell biology. These culture systems derived from mouse intestinal and colonic epithelia enable the modeling of tissue architecture, barrier function, and disease mechanisms under physiologically relevant conditions. This protocol details methods for isolating and maintaining mouse intestinal crypts as 2D monolayers. Monolayers are established via enzymatic dissociation and seeding onto ECM-functionalized hydrogels. This protocol also describes a deep learning pipeline that per

Updated Protocols for 3D and 2D Culture and Histological Analysis of Intestinal Organoids.

The intestinal epithelium maintains continuous self-renewal and multi-differentiation capacities through the integrated regulation of multiple signaling pathways, including Wnt, bone morphogenetic protein, epidermal growth factor (EGF), and Notch signaling pathways. Based on this principle, a defined combination of EGF, Noggin, and the Wnt agonist R-spondin has been shown to support long-term expansion of mouse intestinal stem cells and the formation of organoids with indefinite self-renewal and full differentiation capacity. In human intestinal organoid cultures, the addition of small molecul

Rapid and Efficient Genetic Manipulation of Intestinal Organoids Under Transient Two-Dimensional Culture Conditions.

Genetic manipulation of cells represents one of the fundamental techniques in current biological research, essential for elucidating the regulatory mechanisms underlying various biological phenomena, modeling many diseases, and investigating their therapeutic interventions. However, in the context of organoid biology, the manipulation of target genes and the selection of resultant genetically modified clones often encounter significant technical challenges due to the typically low gene introduction efficiency. This difficulty is largely attributed to the three-dimensional culture of organoids

Mass Spectrometry-Based Proteomics of Intestinal Organoids.

Proteomics, the large-scale study of proteins, enables the identification, quantification, and functional characterization of proteins, revealing post-translational modifications and protein interactions that are not apparent from transcriptomic data. Human organoids, which recapitulate the structural and functional complexity of native epithelial tissues, provide powerful tools to study disease mechanisms and personalize therapies. However, their culture poses challenges for efficient protein extraction and reproducible analysis. Here, we present a proteomics workflow optimized to maximize pr

Methods for Processing Stem Cell-Derived Organoids for Histological and Immunofluorescence Staining.

Intestinal stem cell (ISC)-derived organoids provide a physiologically relevant 3D culture system to model intestinal biology, regeneration, and disease mechanisms. Their complex architecture, recapitulating crypt structures, poses challenges for downstream histological and immunohistochemical analyses, necessitating optimized processing, and staining protocols. Here, we describe robust workflows for the fixation, embedding, sectioning, and staining of ISC-derived organoids. Each method is detailed stepwise, with emphasis on preserving morphology and antigenicity. We discuss reagent choices, c

Deep Insights Without Clearing: An Optimized Protocol for Patient-Derived Whole-Organoid Immunofluorescence Imaging.

Here, we present an optimized whole-organoid immunofluorescence staining protocol that preserves morphology, achieves uniform antibody penetration, and enables deep-tissue imaging without the need for tissue clearing. Organoids are three-dimensional, self-organizing structures that replicate many of the cellular and architectural features of their tissue of origin, making them powerful preclinical models for studying development, disease, and therapeutic responses. Most organoid research is performed on formalin-fixed, paraffin-embedded (FFPE) samples, in which organoids are sectioned across t

Diagnostic performance and testing costs of pooled testing for molecular detection of tuberculosis in seven countries: a prospective, observational, diagnostic accuracy study.

BACKGROUND: Globally in 2024, only 54% of people diagnosed with tuberculosis received an initial WHO-recommended rapid molecular diagnostic test. Pooled testing could expand access to molecular detection using existing laboratory capacity. We aimed to evaluate the performance of pooled tuberculosis testing in a multicentre diagnostic accuracy study. METHODS: In this prospective diagnostic accuracy study, we enrolled individuals with presumptive tuberculosis through facility-based case finding (district hospitals and primary health centres) and community-based case finding in Bangladesh, Brazi

Comprehensive Isolation Strategy for Intestinal Epithelial and Fibroblast Cells from Healthy and Tumor Human Tissues.

Organoids and fibroblasts derived from patient tissue serve as physiologically meaningful in vitro models to investigate tissue biology, diseases, and treatment responses. In this study, we present a robust protocol for the simultaneous isolation and long-term culture of patient-derived organoids (PDOs) and fibroblasts from both healthy and colorectal cancer samples. The workflow uses enzymatic dissociation followed by efficient separation into epithelial and stromal cell fractions. Organoids are embedded in Matrigel or Matrigel-Collagen I mixtures to support three-dimensional growth, whereas

A Straightforward and Reliable Mouse Model of Colorectal Cancer by Orthotopic Transplantation.

Colorectal cancer (CRC) is a major malignancy with significant global implications for human health. The development of a reliable and pathologically relevant orthotopic CRC model is essential for advancing our understanding of its molecular mechanisms and for developing more effective therapeutic interventions. However, the construction of such models is fraught with challenges primarily because of the technical complexities involved in the transplantation of CRC cells into the intestinal epithelium. In this chapter, we describe a recently developed method for generating an orthotopic CRC mod

An AI-driven evidence ecosystem for cardiometabolic nutrition: bridging the gap from multiomics to patient empowerment.

Cardiometabolic diseases remain a primary global health challenge, with nutritional interventions serving as a critical pillar of management. However, the rapidly expanding volume of nutritional research, spanning high-dimensional multiomics, real-world continuous monitoring, and highly heterogeneous clinical trials, has created fragmented knowledge silos, leading to a profound translation lag between scientific discovery and clinical practice. To overcome this methodological bottleneck, we propose a conceptual framework for an "AI-Driven Evidence Ecosystem." This perspective outlines a four-p

Acneiform eruption associated with Janus kinase inhibitors in pediatric inflammatory bowel disease: Case series and clinical management guidance.

Janus kinase inhibitors (JAKi) are increasingly used for refractory inflammatory bowel disease (IBD) in pediatric patients but are associated with significant dermatologic adverse effects. Acneiform eruptions have been reported in up to 30% of adults, with a greater incidence suspected in adolescents. We performed a retrospective review of 66 pediatric IBD patients treated with JAKi at UCSF Benioff Children's Hospitals (2022-2025), including 41 on upadacitinib and 25 on tofacitinib. Eight patients (12.1%) developed new-onset or worsening moderate to severe acne requiring dermatology referral f

Decoupling Neuroprotection from MAO Inhibition via Indane-to-Azaindane Bioisosteric Replacement of Rasagiline.

Rasagiline is a monoamine oxidase B (MAO-B) inhibitor with neuroprotective effects, whose dependence on MAO-B inhibition remains unclear. Here, indane-to-azaindane rasagiline isosteres (rac-2a and rac-2b) were developed to investigate whether neuroprotection can be decoupled from MAO-B inhibition. Direct regioselective oxidation of 4-azaindane catalyzed by Mn-(OAc)2/ t BuOOH afforded the key 4-azaindan-1-one intermediate (6a). In paraquat-insulted dopaminergic differentiated SH-SY5Y cells, both analogs showed neuroprotection at 10 and 100 μM, with rac-2b outperforming rasagiline. The azaindane

Employing AI-driven predictive analytics to enhance patient outcomes and streamline healthcare processes.

Artificial intelligence (AI) can advance healthcare analytics by generating predictive insights that extend beyond traditional retrospective reporting. This study examines how machine learning models applied to clinical, operational, and contextual data may identify patient risk factors, forecast disease progression, and support timely interventions. Rather than claiming definitive clinical impact, the manuscript presents an implementation-oriented framework showing how predictive models can inform readmission risk stratification, resource planning, and care quality improvement when supported

Sulbactam-Durlobactam for Carbapenem-Resistant Acinetobacter baumannii in a Child: A Novel Application of Adult Pharmacokinetics.

There are limited antimicrobial options for the treatment of carbapenem-resistant Acinetobacter baumannii (CRAB). Sulbactam-durlobactam was recently approved for the treatment of CRAB in patients 18 years and older. However, CRAB infections can have mortality rates up to 80% in the intensive care unit. We describe the successful novel use of sulbactam-durlobactam in a pediatric patient using available adult pharmacokinetic data and preliminary pediatric dosing suggestions from a phase 1b trial.
