BACKGROUND: Intestine transplants are highly sensitive to ischemia-reperfusion injury (IRI). Static cold storage (SCS) is the standard for multivisceral (MVT) allograft preservation but carries known risks. While several devices have been developed for liver, lung, and heart graft preservation, there are no systems validated specifically for MVT grafts. We report the first use of the Paragonix LIVERguard® Donor Liver Transport System for hypothermic static preservation of a donation after brain death (DBD) MVT allograft, transported approximately 1363 miles.

BACKGROUND: Tropheryma whipplei (TW), a Gram-positive bacterium, is the etiologic agent of Whipple's disease, a chronic and systemic condition that primarily involves the gastrointestinal tract. Chlamydia psittaci (C. psittaci) is a zoonotic pathogen that primarily leads to psittacosis after bird exposure. BACKGROUND: Worldwide, there has been a steady increase in the number of cases of pulmonary cryptococcosis diagnosed in immunocompetent patients, where symptoms can range from mild to severe. Clinical and radiological distinction of disease may be made when compared with immunodeficient cases and in those presenting with primary lung carcinoma. In the latter case confusion can lead to initial misdiagnosis and delayed treatment. A 31-year-old farmer is being treated for suspected pneumonia. As the symptoms persist despite antibiotic treatment, the suspicion of hypersensitivity pneumonitis type Farmer´s lung arises after taking into account the patients occupational history. Information from various examination modalities and the clinical course confirm the suspected diagnosis.
