Fibrotic chronic eosinophilic pneumonia: from inflammation to fibrosis and therapeutic implications.

Chronic eosinophilic pneumonia is a rare inflammatory lung disease that typically responds to systemic glucocorticoids but is frequently complicated by relapses and treatment-related toxicity. In recent years, monoclonal antibodies targeting the interleukin-5 (IL-5) pathway have emerged as effective glucocorticoid-sparing therapies in relapsing or glucocorticoid-dependent chronic eosinophilic pneumonia. Alongside these advances, a substantial proportion of patients develops progressive fibrotic changes over time, challenging the traditional view of chronic eosinophilic pneumonia as a fully rev

Lower plasma resolvin D1 level and greater sputum eosinophilia characterize a cluster of severe asthma patients.

BACKGROUND: Severe asthma is a chronic disease of airway inflammation with substantial morbidity. Deficient specialized pro-resolving mediators (SPMs) are associated with persistent airway inflammation and impaired lung function in some patients with severe asthma. Resolvin D1 (RvD1) is an SPM agonist for inflammation resolution. METHODS: Plasma RvD1 was measured longitudinally over 5 years in 23 severe asthma patients in the Severe Asthma Research Program (SARP) to identify relationships between clinical parameters, type 2 inflammation, and sputum gene expression. RESULTS: The majority of s

Clinical phenotyping of asthma patients with elevated sputum eosinophils and low blood eosinophils: a post-hoc analysis of the multicentre ATLANTIS cohort.

BACKGROUND: Patients with eosinophilic asthma are responsive to treatment with corticosteroids and biologics. Eosinophilia is usually identified based on blood eosinophil counts, but there is discordance between blood and sputum in some cases. A subset of patients may have sputum eosinophilia despite low blood eosinophil levels. The clinical implications of this so-called isolated sputum eosinophilia are unknown. The aim of this study is to investigate the clinical expression of asthma in patients with isolated sputum eosinophilia. METHODS: In this post-hoc ATLANTIS analysis we included patie

Divergent Outcomes in Lung Adenocarcinoma with Paraneoplastic Hypereosinophilia in Patients Treated with Pembrolizumab: A Two-Case Report.

Paraneoplastic hypereosinophilia (PHE) is an underrecognized manifestation of solid tumors, particularly non-small cell lung cancer. We present two cases of metastatic lung adenocarcinoma with marked eosinophilia treated with PD-1 inhibitor immunotherapy, both of whom demonstrated changes in eosinophil counts but divergent clinical outcomes. While elevated eosinophil levels during immune checkpoint inhibitor therapy have been associated with improved treatment response in prior studies, the clinical significance of persistent or severe hypereosinophilia remains incompletely understood. These c

Exploring the mechanism of interleukin-24 in regulating eosinophil differentiation in chronic obstructive pulmonary disease.

Chronic obstructive pulmonary disease (COPD) exacerbation is a common respiratory condition, particularly when accompanied by eosinophilia, which mediates inflammatory responses that significantly impair the integrity of the airway mucosal barrier. This study aims to elucidate the roles of interleukin-24 (IL-24) and eosinophils (EOS) in COPD-associated tissue remodeling, focusing on their effects on the expression of pro-inflammatory mediators and extracellular matrix (ECM) components in pulmonary fibroblasts, as well as the underlying molecular mechanisms. Based on the differential expression

IL5 rs2069812 and IL13 rs1800925 Genetic variants as key determinants of clinically relevant asthma phenotypes.

Type 2-high airway inflammation in adults with asthma is heavily driven by the cytokines interleukin (IL)-4, IL-5, and IL-13. While genetic variations in these cytokines are known to influence asthma pathogenesis, their specific impacts on clinically relevant phenotypes remain to be fully elucidated. This study aimed to investigate the associations between inflammatory cytokine gene polymorphisms, clinical asthma phenotypes, inflammation cell subtypes, and lung function. A cross-sectional study was conducted involving 125 adults with asthma. Genotyping was performed for the following single-nu

Organ Involvement and Constitutional Symptoms in Idiopathic Hypereosinophilic Syndrome: A 72-Patient Single-Centre Cohort.

Background: Idiopathic hypereosinophilic syndrome (iHES) is a rare disorder of persistent hypereosinophilia with secondary organ damage, but the full spectrum of organ involvement and the constitutional-symptom burden under systematic ascertainment remain incompletely characterised. Objectives: To describe the clinical phenotype of iHES in a large single-centre cohort, with attention to symptoms typically under-reported in observational series. Methods: We followed 72 consecutive patients with iHES at a tertiary referral centre between January 2017 and May 2025. Defined HES variants were exclu

Chronic Eosinophilic Pneumonia: A Pneumopathy With Many Faces.

Chronic eosinophilic pneumonia (CEP), also known as Carrington's disease, is a rare idiopathic eosinophilic lung disorder characterized by eosinophilic infiltration of the pulmonary parenchyma and alveolar spaces. It typically presents with subacute respiratory and constitutional symptoms, including cough, dyspnea, fever, and weight loss, and can mimic infectious pneumonia or pulmonary tuberculosis, making diagnosis challenging. We report the case of a 40-year-old woman who presented with a 40-day history of progressively worsening dyspnea, persistent cough, subjective fever, and deterioration

Tezepelumab in severe asthma with pulmonary mucus plugs and eosinophilic chronic rhinosinusitis after dupilumab intolerance and suboptimal mepolizumab response: A case report.

BACKGROUND: Tezepelumab, a monoclonal antibody targeting thymic stromal lymphopoietin (TSLP), has demonstrated efficacy in the treatment of severe asthma and chronic rhinosinusitis with nasal polyps. However, real-world evidence remains limited, particularly for patients with eosinophilic chronic rhinosinusitis (ECRS) who exhibit inadequate or unfavorable responses to other biologics. CASE PRESENTATION: We report the case of a woman with severe asthma complicated by ECRS, who exhibited divergent clinical responses to multiple biologic agents. Treatment with dupilumab initially improved both a

Case Report: Benralizumab-induced remission in adolescent-onset ANCA-positive eosinophilic granulomatosis with polyangiitis.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis characterized by asthma, eosinophilia, and multi-organ involvement. Adolescent-onset disease is uncommon, and evidence guiding biologic therapy in this population remains limited. We report the case of an 18-year-old female with a history of chronic rhinosinusitis and eosinophilic asthma who presented with worsening asthma control, arthralgia, and peripheral eosinophilia (4, 600 cells/μL), with palpable purpura developing during hospitalization. Chest radiography was

Platelet-derived serotonin epigenetically programs macrophage alternative activation to orchestrate type 2 airway inflammation.

Type 2 inflammation (T2I) drives chronic airway diseases such as eosinophilic chronic rhinosinusitis (eCRS) and asthma, which are frequently accompanied by coagulation activation and platelet recruitment. However, whether and how these associated processes actively contribute to shaping inflammation remain largely unknown. Here, using single-cell transcriptomic profiling of human nasal polyp samples from eCRS patients with or without comorbid asthma, we identified a TGM2hi macrophage population enriched in inflamed tissues and strongly correlated with local eosinophilia and systemic disease bu

A novel adjunct for COPD: Could it be type II inflammation?

Chronic obstructive pulmonary disease (COPD) is a disease of the lungs characterized by irreversible airflow obstruction that has primarily been associated with neutrophilia; however, emerging evidence suggests an eosinophilic component in some patients. Management for COPD has focused on smoking cessation, pollutant avoidance, and inhaled therapy, but unfortunately, these carry therapeutic limitations for some patients. Biologic therapy has been an asthma treatment staple for years; however, it was not found to be beneficial for COPD until recently. In 2025, two biologic therapies were approv

Advances in the treatment of eosinophilic granulomatosis with polyangiitis.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a small-vessel vasculitis associated with anti-neutrophil cytoplasmic antibodies and characterized by blood and tissue eosinophilia, severe respiratory manifestations, and multiorgan involvement. The management of newly diagnosed EGPA still relies on therapeutic strategies that were initially validated for other forms of anti-neutrophil cytoplasmic antibody-associated vasculitis, including microscopic polyangiitis and granulomatosis with polyangiitis. Whereas the long-term prognosis of microscopic polyangiitis and granulomatosis with poly

Mechanisms and therapeutic strategies of asthma: from bench to bedside.

Asthma is a heterogeneous chronic airway disease arising from a complex interplay of genetic susceptibility and environmental exposures. Key pathobiological features include dysregulated immune responses (particularly type 2 inflammation), structural airway remodeling, and mucus hypersecretion. Recent advances have illuminated mechanisms from epithelial "alarmin" cytokine release (TSLP, IL-33, IL-25) to downstream cellular networks involving Th2/Th17 lymphocytes, group 2 innate lymphoid cells (ILC2s), eosinophils, and mast cells. These pathways converge on bronchial hyperresponsiveness and air

DRESS during chemotherapy with carboplatin, pemetrexed, and osimertinib for lung adenocarcinoma: A case report.

We report a case of a 40-year-old woman with lung adenocarcinoma who developed a drug reaction with eosinophilia and systemic symptoms (DRESS) during chemotherapy with carboplatin, pemetrexed, and osimertinib. She presented with fever, extensive erythematous rash, and hepatorenal dysfunction (RegiSCAR score = 7). Discontinuation of the offending drugs and supportive care alone led to the complete resolution of the symptoms. She was successfully transitioned to gefitinib without recurrence of symptoms. This case highlights the need for an accurate diagnosis and careful consideration of the indi

Clinical Insights into Strongyloides stercoralis Pulmonary Hyperinfection Syndrome.

BACKGROUND: Strongyloides stercoralis pulmonary hyperinfection syndrome (SPHS) is a rare, frequently fatal complication of strongyloidiasis that is difficult to recognize because of its nonspecific multisystem manifestations. Diagnosis requires a high index of suspicion. METHODS: This retrospective study identified 29 hospitalized patients with strongyloidiasis at Chaozhou Central Hospital between November 2018 and January 2026. We compared the clinical data of five patients with SPHS (SPHS group) and 21 patients with uncomplicated/chronic strongyloidiasis (non-SPHS group) and described the d
