A rare case report of Salmonella infection: severe necrotizing pneumonia with empyema in an immunocompetent child.

Salmonella is a gram-negative bacillus that typically causes gastrointestinal disease and rarely affects the pulmonary system, particularly in immunocompetent children. We report a 4-year-old immunocompetent Saudi boy who presented with severe respiratory distress requiring intubation and was found to have left-sided pleural effusion. Pleural fluid and tracheal aspirate cultures grew Salmonella species, while blood, stool, and CSF cultures were negative. Chest computed tomography revealed complicated empyema with necrotizing pneumonia. Despite appropriate antimicrobial therapy and supportive c

Chylothorax as an Unusual Initial Manifestation of Diffuse Large B-Cell Lymphoma in an 86-Year-Old Woman: A Reflection of Diagnostic Reasoning and Age in Therapeutic Decisions.

Chylothorax is a rare presentation of lymphoproliferative disorders, responsible for only a small proportion of non-traumatic pleural effusions in large series. In older adults, its recognition can be challenging, often delaying diagnosis and treatment. We report an 86-year-old woman admitted with progressive dyspnea and a large right pleural effusion. Thoracentesis yielded a milky fluid with high triglycerides, confirming chylothorax. Cytology was negative for malignant cells, and a CT scan revealed an extensive retroperitoneal mass encasing the aorta and inferior vena cava, highly suggestive

Losartan, toceranib, and carboplatin combination therapy for progressive pulmonary metastasis in canine osteosarcoma: a case report.

Osteosarcoma (OSA) is the most common and aggressive primary bone tumor in dogs. OSA frequently metastasizes to the lungs and has a poor prognosis despite current standard treatment. This case report described the clinical course of a 3-year-old castrated male Jindo dog diagnosed with right tibial OSA and a pre-existing pulmonary metastasis. Initially, the patient received adjuvant chemotherapy and maintained a stable disease state for approximately five months (21 weeks from initial diagnosis) until radiographic and computed tomography (CT) examinations revealed progressive pulmonary metastas

The diagnostic value of CDC20 for malignant pleural effusion of lung adenocarcinoma.

Lung adenocarcinoma (LUAD) is characterized by difficult early detection, rapid tumor progression and low surgical resection rate in the late stage. The common complication of patients with lung adenocarcinoma is malignant pleural effusion (MPE). Compared with lung biopsy, cytological examination of exfoliated pleural effusion is a less invasive operation. Therefore, there is an urgent need to discover new and effective cytological biomarkers for the exfoliation of LUAD pleural effusion. This work took the differentially expressed genes of LUAD tumors and adjacent tissues in the Gepia database

Managing pleural infections: a nationwide survey of respiratory physicians in Singapore.

BACKGROUND: Pleural infection remains a major cause of morbidity and mortality, with a significant healthcare burden. Despite growing research and guideline recommendations, real-world management continues to vary widely. METHODS: An anonymous cross-sectional survey evaluating current practice was conducted among respiratory physicians across all public hospitals in Singapore. The survey examined key aspects of management, including pleural fluid diagnostics, chest drain management, surgical referral and intrapleural fibrinolytic therapy (IPFT). RESULTS: 95 respondents (58% response rate) co

Characteristics and Clinical Outcomes of Postoperative Pleural Space Infections Among Lung Transplant Recipients.

BACKGROUND: Pleural space infections (PSI) are a common complication after lung transplantation (LT) and have historically been linked to impaired allograft function and decreased survival, though contemporary outcome data are limited. METHODS: This retrospective cohort included adult LT recipients (LTRs) who underwent pleural effusion drainage ≤ 90 days posttransplant from June 2013 to December 2023. Pulmonary function tests (PFTs; FEV1 and FVC percent predicted) and 1-year survival were compared between LTRs with and without culture-proven PSI using linear mixed-effects models. Survival was

Dasatinib Associated Pleural Complications- A Case Series.

Dasatinib, a second-generation tyrosine kinase inhibitor, is highly effective in chronic myeloid leukaemia (CML) but is associated with pleural complications in approximately 28%-37% of patients. We report four patients with CML who developed dasatinib-induced pleural effusion after prolonged treatment (2-10 years), including one case of chylothorax. All presented with dyspnea, and imaging with diagnostic thoracentesis confirmed exudative pleural effusions. One patient demonstrated triglyceride-rich fluid consistent with chylothorax. Dose reduction of dasatinib to 50 mg/day led to complete cli

Bilateral Chylothorax After 13 Years of Dasatinib Therapy in a Patient With Chronic Myeloid Leukaemia: A Case Report.

A 52-year-old man with chronic myeloid leukaemia (CML) who had been receiving dasatinib 100 mg/day for 13 years presented with progressive dyspnea. Chest radiography revealed bilateral pleural effusions, and further evaluation confirmed the diagnosis of chylothorax. Dasatinib-induced chylothorax was suspected, and discontinuation of the drug led to clinical improvement. Dasatinib is a tyrosine kinase inhibitor (TKI) used for the treatment of CML and Philadelphia chromosome-positive acute lymphoblastic leukaemia (Ph + ALL). Although pleural effusion is a relatively common adverse effect, chylot

Integrating pleural PD-1+CD8+ T cell as a complement variable into LENT score to assess patients with lung adenocarcinoma complicated with MPE.

INTRODUCTION: Malignant pleural effusion (MPE) is a common complication of advanced non-small cell lung cancer (NSCLC), particularly in lung adenocarcinoma, and is associated with poor prognosis. A better understanding of the role of PD-1+CD8+ T cells in the pleural environment and their relevance to patient survival could facilitate better clinical decision-making. METHODS: We performed a cohort study involving NSCLC patients with MPE. The abundance of pleural PD-1+CD8+ T cells was measured using flow cytometry. We also assessed the presence of epidermal growth factor receptor (EGFR) mutatio

The STAT3-ZEB1 axis contributes to CCL2-mediated resistance to osimertinib in lung cancer.

OBJECTIVE: Osimertinib, a third-generation EGFR tyrosine kinase inhibitor (TKI), is effective in NSCLC patients with EGFR-activating or T790M mutations, but acquired resistance remains a major challenge. Although CCL2 has been implicated in EGFR-TKI resistance via AKT activation, the precise downstream mechanisms are not fully understood. METHODS: We analyzed malignant pleural effusion samples from patients with resistant NSCLC and conducted functional assays in lung cancer cell lines with ectopic CCL2 expression or knockdown, combined with in vivo xenograft models. Key downstream signaling p

B-cell lymphoma initially presenting as isolated pleural effusion: a challenging diagnostic case report.

BACKGROUND: B-cell lymphoma is a highly heterogeneous group of diseases with complex clinical manifestations. While lymphoma-associated pleural effusion occurs in approximately 20-30% of cases, lymphoma presenting initially with pleural effusion as the primary manifestation is relatively uncommon. This case report demonstrates a rare presentation where massive pleural effusion was the primary clinical manifestation in the absence of typical lymphoma symptoms, leading to potential misdiagnosis and delayed treatment. The unique contribution to existing literature lies in highlighting the diagnos

Using actigraphy to evaluate physical activity in patients with pleural effusion: a pilot study.

BACKGROUND: Malignant pleural effusions (MPEs) are common in cancer patients and significantly impair physical activity. Although symptom relief is the primary treatment goal, the extent of activity limitation and the effectiveness of interventions remain unclear. We used actigraphy to assess the impact of thoracentesis on activity level. The primary objective was to evaluate the association between physical activity (via ActiGraph), quality of life [Short Form 6-Dimension index (SF-6D) utility score], and dyspnea (Borg scale) at weeks 1 and 2 following thoracentesis. METHODS: We conducted a

Consistency analysis of programmed cell death ligand-1/thyroid transcription factor-1 dual staining in malignant pleural effusion and lung adenocarcinoma histology specimens.

BACKGROUND: Lung cancer remains the leading cause of cancer-related deaths worldwide, with non-small cell lung cancer (NSCLC) constituting the majority of cases. Immune checkpoint inhibitors have significantly improved outcomes in advanced NSCLC, particularly for patients with programmed cell death ligand-1 (PD-L1) expression. However, obtaining tissue biopsies for PD-L1 assessment in advanced disease is often challenging. Malignant pleural effusion (MPE), frequently associated with advanced lung adenocarcinoma, offers an accessible alternative source for PD-L1 testing. Nevertheless, the relia

Mast cell-derived MIF Polarizes Th17 cells to drive lung adenocarcinoma progression.

BACKGROUND: The tumor microenvironment in lung adenocarcinoma (LUAD) exhibits complex cellular interactions that drive disease progression. While mast cells (MCs) are known to infiltrate tumors, their specific immunomodulatory functions remain incompletely characterized. METHODS: We conducted a retrospective analysis of hematological parameters, including plasma factors such as interleukin-17 (IL-17). Multiplex immunohistochemical (mIHC) staining was performed on LUAD tissue to investigate the spatial distance and distribution of MCs and Th17 cells. It was integrated with single-cell RNA sequ
