Comparison of clinician injury severity ratings with Abbreviated Injury Scale designations for thoracic injuries.

OBJECTIVES: The utility of the Abbreviated Injury Scale (AIS) for injury benchmarking and epidemiologic research depends on accurate injury severity designations. The objective of this study was to assess whether aggregated independent clinician assessments of injury severity differ from the official consensus-based AIS severity designations, focusing on common thoracic injuries. METHODS: Twenty-one thoracic injuries were selected, including rib fractures, sternal fractures, pneumothorax, and hemothorax. Radiographic images from patients with these injuries were obtained from the Crash Injury

Spontaneous Pneumomediastinum in a Patient With Asthma: A Case Report.

Pneumomediastinum is described as free air located within the mediastinum. In the absence of traumatic injury, iatrogenic injury, or a clear etiology, it is called spontaneous pneumomediastinum (SPM). SPM has been associated with many conditions and triggers, including bronchial asthma, forceful straining during exercise, inhalation of drugs, and activities associated with the Valsalva maneuver. This case details a 25-year-old female patient with a past medical history of asthma who presented to the emergency department with cough, shortness of breath, and chest pain. Initial chest X-ray demon

Congenital Pulmonary Airway Malformation Presenting as Pneumothorax: Two Case Reports.

BACKGROUND: Congenital pulmonary airway malformation (CPAM) is a rare developmental anomaly of the lower respiratory tract, with an estimated incidence ranging from 1 in 10,000 to 1 in 35,000 live births. CPAM may present during infancy with severe respiratory distress and complications such as pneumothorax or recurrent pulmonary infection. CASE PRESENTATION: We report two female infants aged 7 months and 2 months who presented with severe respiratory distress and radiological findings initially suggestive of pneumothorax. In Case 1, chest computed tomography (CT) demonstrated a large solitar

Long-Term Recurrence-Free Outcome After Doxycycline Pleurodesis With Pulmonary Rehabilitation and Nutritional Support in an Underweight Patient With COPD: A Case Report.

Secondary spontaneous pneumothorax (SSP), a complication of chronic obstructive pulmonary disease (COPD), recurs more frequently in underweight patients. This case report describes a 63-year-old malnourished man with COPD presenting with recurrent left-sided SSP shortly after hospital discharge for a previous episode. Following chest tube reinsertion and complete lung re-expansion, chemical pleurodesis was performed using 1000 mg doxycycline in saline. Transient pleuritic pain was controlled with intravenous ketorolac and intrapleural lidocaine. No other immediate or delayed adverse events occ

Esophageal Perforation During Dilation in a Patient With Hypopharyngeal Metastatic Squamous Cell Carcinoma.

We report the case of a 75-year-old female with a history of stage IVa hypopharyngeal squamous cell carcinoma who presented with dysphagia due to cervical esophageal stricture after prior laryngopharyngectomy and cervical esophagectomy. During rigid pharyngeal dilation, an acute and uncontained cervical esophageal perforation involving the superior mediastinum occurred, leading to pneumomediastinum. This was immediately complicated by her surgically altered anatomy creating bilateral pneumothoraces, tension pneumoperitoneum, and subsequent hemodynamic instability. Prompt recognition and emerge

Sequential Bilateral Pneumothorax Following Acupuncture Presenting as Left Tension Pneumothorax: A Case Report.

Acupuncture is commonly used for musculoskeletal pain and is generally regarded as safe; however, pneumothorax remains one of its most important mechanical complications, particularly when needling is performed over thoracic and upper-back sites. We report the case of a 30-year-old man who developed immediate pleuritic chest pain, chest tightness, and dyspnoea after acupuncture to the upper back, bilateral trapezius muscles, and infrascapular regions. He delayed presentation until the following morning, when symptoms worsened. Examination demonstrated tachypnoea, reduced left-sided air entry,

Uniportal VATS wedge pulmonary resection.

BACKGROUND/AIM: Video-assisted thoracic surgery (VATS) has emerged as a common approach in surgical procedures involving the lung, mediastinum, thoracic wall, and diaphragm. Recent studies have described the performance of diagnostic and therapeutic VATS procedures via a single port, and the technique has become the preferred diagnostic modality for various intrathoracic pathologies and for patients deemed unsuitable for fine-needle biopsy. MATERIALS AND METHODS: The records of 57 patients who underwent diagnostic and therapeutic uniportal VATS for wedge pulmonary resection between January 20

Study Protocol of the Endobronchial VAlves for Pneumothorax to Reduce Admission Time (EVAPoRATe) Pilot Multicentre Randomised Clinical Trial.

BACKGROUND: Pneumothorax with persistent air leak (PAL) is a debilitating condition. Patients with PAL often require prolonged chest drainage, with associated complications, such as, hospital-acquired infections, thromboembolism, and deconditioning. Endobronchial valves (EBVs) are an approved bronchoscopic treatment for PAL and may allow earlier chest tube removal and hospital discharge. Current evidence supporting the use of EBV is limited to case series and uncontrolled studies. EBVs are usually used as a last resort when other treatments failed. Whether early referral for EBV placement is f

Piecemeal submucosal tunneling endoscopic resection of a 4-cm esophageal leiomyoma.

BACKGROUND AND AIMS: Leiomyomas are the most common benign neoplasm of the esophagus. Although typically asymptomatic, they can cause retrosternal discomfort and dysphagia. Submucosal tunneling endoscopic resection (STER) has been increasingly used for resecting esophageal subepithelial lesions. We present this case of STER for resection of a 4-cm esophageal leiomyoma. METHODS: A 44-year-old man presenting with retrosternal discomfort, dysphagia, and heartburn was found on EGD to have a 4-cm subepithelial lesion in the mid esophagus. Subsequent endoscopic ultrasound and fine-needle biopsy con

Primary Chest Wall Malignant Lymphoma in a Patient Without a History of Tuberculosis or Chronic Pyothorax: A Case Report With a Review of the Literature.

Primary malignant lymphoma of the chest wall is a rare disease. Its pathogenesis has been reported to be associated with chronic pyothorax following tuberculous inflammation, artificial pneumothorax, and Epstein-Barr virus (EBV) infection. We herein report a surgically treated case of primary chest wall lymphoma without a history of tuberculosis or chronic pyothorax. A 65-year-old woman with a history of left rib fractures caused by domestic violence was found to have an abnormal shadow in the left chest on a chest radiograph obtained during a routine medical checkup. Contrast-enhanced compute

Thoracotomy for Urgent Repair of Lung Torsion Identified as an Incidental Finding.

We report a 68-year-old male with chronic obstructive pulmonary disease on home oxygen who presented with foot swelling and erythema, associated with a new right foot wound. He denied chest pain and overt shortness of breath at the time of his presentation. Initial chest radiography obtained during the evaluation raised concern for pneumothorax versus a large bulla. Computed tomography with intravenous contrast and computed tomography angiography revealed an expanding right lower lobe bulla extending into the upper lung zone, narrowing of the right bronchial tree, and abnormal right hilar vasc

Clinical, radiological, and histopathological features of checkpoint inhibitor-related pneumonitis: a retrospective cross-sectional analysis of 44 patients who underwent lung biopsy.

BACKGROUND: Checkpoint inhibitor-related pneumonitis (CIP) is a serious immune-related adverse event associated with relatively high mortality. Its clinical and radiological manifestations are non-specific and may overlap with infection, tumour progression, and other pulmonary diseases. Currently, the histopathological spectrum of CIP and its relationship with radiological patterns and clinical outcomes remain insufficiently defined. This study aimed to analyse the clinical, radiological, and histopathological features, as well as exploratory prognostic outcomes, of biopsy-evaluated patients w

A percutaneous sternal elevation technique using the Tian-Ping sternal lifter for Nuss repair of pectus excavatum.

The conventional Nuss procedure carries a risk of catastrophic injury during retrosternal tunnel creation. This study aimed to evaluate the feasibility, safety, and preliminary clinical outcomes of a novel percutaneous sternal elevation device, the Tian-Ping (Tian's) sternal lifter, in assisting the Nuss procedure for pectus excavatum. This study included patients who underwent chest wall correction via the Nuss procedure assisted by the Tian-Ping (Tian's) sternal lifter from July 2025 to October 2025. Patient demographics, operative time, intraoperative blood loss, complications, and postoper

Progressive Extension of a Post-intubation Tracheal Laceration Presenting As Massive Pneumomediastinum: A Case Report.

Post-intubation tracheal laceration is rare and is usually managed conservatively, provided the tear is protected from positive pressure, whether by spontaneous ventilation, by a cuff sited beyond the lesion, or by ventilation delivered without loss of tidal volume. We describe a woman in her early 70s intubated at a district hospital for progressive impairment of consciousness. Imaging before intubation already showed a hiatal hernia, right bronchial mucous plugging, lobar consolidation and bilateral effusions, and initial ventilation delivered 500 mL at a rate of 18 per minute, approximately

Paired Whole-Genome Sequencing of Scalp Angiosarcoma and Matched Lung Metastasis Reveals Common Clonal Origin and Lung-Specific Evolution.

Background and Clinical Significance: Cutaneous angiosarcoma frequently metastasizes to the lungs, where it may rarely present as diffuse cystic lung disease with recurrent pneumothorax, resulting in substantial diagnostic difficulty. We report a case of pulmonary metastatic cutaneous angiosarcoma in which paired whole-genome sequencing (WGS) of the primary and metastatic lesions was performed to clarify clonal origin and characterize metastatic evolution. Case Presentation: A 65-year-old man with recurrent right-sided pneumothorax and progressive bilateral cystic lung lesions underwent skin a

Predictors and Prevention of Recurrence After Secondary Spontaneous Pneumothorax: A Systematic Review.

Secondary spontaneous pneumothorax (SSP) carries a clinically important risk of recurrence, but the available evidence is dispersed across heterogeneous disease-specific and treatment cohorts. This systematic review aimed to identify clinical, radiological, physiological, disease-related, clinical-course, and treatment-related determinants of recurrence after SSP. PubMed/MEDLINE was searched on 19 July 2026 without a date restriction. Adult and adult-inclusive SSP cohorts were eligible, as were mixed spontaneous-pneumothorax cohorts containing patients with SSP when a recurrence determinant wa

A Retrospective Study Examining the Effectiveness of the Study of the Management of Blunt Chest Wall Trauma (STUMBL) Scoring Tool in Predicting Complications Following Rib Fractures.

Introduction Blunt thoracic trauma is a significant burden on the United Kingdom's National Health Service. A common scoring system utilised to predict complications and guide management is the STUMBL (Study of the Management of Blunt Chest Wall Trauma) tool. This study evaluates the utility of a STUMBL score of >11 in predicting complications. Methods A retrospective study of patients with rib fractures was conducted at the University Hospitals of North Midlands, a major trauma centre in England. The primary outcome was rib fracture-related complications, defined as the occurrence of ≥1 of th

Inserting With Care: Classification of Complications Related to Chest Drain Insertion.

Background Intercostal catheters (ICCs) are standard management for traumatic pneumothorax (PTx), haemothorax (HTx), and haemo/pneumothorax (H/PTx). Pigtail catheters inserted using the Seldinger technique have emerged as a recognised alternative. Insertion steps are well described through the 'triangle of safety' using the sterile technique. Complication rates for chest drains have been reported as a variable rate, but complication reporting varies considerably, and some studies may overstate the clinical significance of minor events that do not require intervention. The Clavien-Dindo (CD) cl

Point-of-Care Ultrasound in Blunt Chest Trauma: Diagnostic Performance and Clinical Relevance.

Chest trauma is a leading cause of death and illness among trauma patients, accounting for a significant proportion of emergency department admissions. Therefore, the early identification of intrathoracic injuries is crucial to ensure effective treatment and reduce the risk of complications, particularly those affecting the heart and lungs. Complementary diagnostic methods, such as ultrasound, due to their non-invasive nature and ability to provide real-time imaging, are therefore essential for making informed decisions based on correctly identifying these complications. In this context, this
