Outcomes of Video-Assisted Thoracoscopic Lobectomy in Octogenarians Following Enhanced Recovery After Surgery Protocols.

OBJECTIVES: The improvements from minimally invasive surgery and Enhanced Recovery After Surgery (ERAS) protocols have enabled more octogenarians to be considered for curative treatment of early-stage lung cancer. However, evidence on short-term outcomes after ERAS-guided Video-Assisted Thoracoscopic Surgery (VATS) lobectomy in this age group remains limited. The aim was to evaluate perioperative outcomes and assess specific challenges in octogenarians undergoing VATS lobectomy following a well-established ERAS-protocol. METHODS: We conducted a retrospective observational study of consecutive

Effect of Donor Lung Resection Technique on Bronchopleural Fistula in Transplantation: Pulmonary Tailoring Versus Hilar Dissection.

PURPOSE: Donor shortages in many countries necessitate the use of marginal donor lungs despite challenges such as size mismatch, and donor lung pathologies requiring resection, such as consolidation or anatomical abnormalities. One way to address these challenges is through major pulmonary resection of the donor lung, which can be in the form of hilar dissection (HD), an anatomical resection, or pulmonary tailoring (PT), a non-anatomical resection. No studies have compared these two techniques; hence, we aimed to compare their intraoperative and postoperative outcomes. MATERIALS AND METHODS:

Artificial Intelligence-Assisted Ultrasound in Austere Trauma Environments: Implications for Role 1 Military Medicine.

Pararescue jumpers are United States Air Force medical tactical operators who provide advanced trauma and prolonged casualty care in austere, high-risk environments. Trauma represents the predominant clinical scenario within their operational mission set, and care is frequently delivered at or near the point of injury when evacuation to definitive treatment is delayed or unavailable. Despite extensive medical training, care at the most forward level, referred to as Role 1, remains fundamentally limited by the absence of diagnostic imaging. Clinical assessment relies primarily on physical exami

Identification and Management of Intraoperative Pneumothorax During Laparoscopic Surgery: A Rare Complication.

Pneumothorax is a rare but potentially life-threatening intraoperative complication during general anesthesia, with early recognition often challenging due to nonspecific hemodynamic and respiratory changes that may mimic other anesthetic or surgical issues. The risk is increased in laparoscopic procedures due to pneumoperitoneum and possible diaphragmatic or pleural injury, as well as in the presence of preexisting pulmonary disease, elevated airway pressures, central venous catheterization, and airway manipulation during intubation. Although it is an uncommon complication in laparoscopic sur

Simple X-ray Metric Predicts Postoperative Course After Spontaneous Pneumothorax Surgery.

BACKGROUND: Predicting postoperative outcomes in patients with spontaneous pneumothorax (SP) remains clinically valuable. This study investigates whether a radiological thoracic (RT) index, calculated as the chest height-to-width ratio on posteroanterior (PA) chest radiographs (CXR), can serve as a predictor after SP surgery. METHODS: In this retrospective study, 195 patients who underwent surgery for SP were included. Patients were grouped according to surgery reasons as recurrence (n=101), preoperative persistent air leak (PAL) (n=58), and bilateral pneumothorax history (n=36). The RT index

Real-world diagnostic yield and safety of electromagnetic navigation bronchoscopy for pulmonary nodules: Experience of a single centre.

Electromagnetic navigation bronchoscopy (ENB) is a minimally invasive technology enabling the navigation of bronchoscopic instruments to reach peripheral pulmonary lesions (PPLs) that are otherwise challenging to reach. To date, ENB has primarily been assessed in high-volume centres or within the framework of multicentre trials. Consequently, the actual diagnostic yield in a real-world setting, particularly among patients with varying levels of risk, remains largely unknown. This prospective single-centre cohort study enrolled consecutive patients with PPLs between February 2023 and April 2024.

Research priorities in pleural disease: study protocol for a joint patient-provider Delphi consensus statement.

INTRODUCTION: There is a wide global variation in research priorities and current clinical practise among pleural medicine practitioners. Research performed today will inform future clinical practice, but the patient's voices are often not heard in setting the research agenda. The International Multicentre Pleural Research Collaborative (IMPACT) European Respiratory Society (ERS) Clinical Research Collaboration (CRC) aims to present a consensus document from both pleural disease experts and patients, which will guide pleural disease research in the immediate future. The objective is to ensure

Mortality and diagnostic practice variation in interstitial lung disease admissions: insights from a multicentre UK cohort study.

BACKGROUND: Interstitial lung diseases (ILDs) are a heterogeneous group of often progressive, unpredictable diseases. They frequently result in hospitalisations secondary to respiratory decompensation, termed ILD-related admissions. A proportion are due to an acute exacerbation of ILD (AEILD). All are associated with high mortality but are poorly characterised in real-world populations. AIM: To evaluate mortality outcomes and associated risk factors following ILD-related hospital admissions, including AEILD. METHODS: We conducted a multicentre retrospective cohort study of primary Internatio

Life-Threatening Pneumomediastinum Following Tracheal Perforation as a Complication of Total Thyroidectomy: A Case Report.

Total thyroidectomy, a common surgical procedure, is generally safe with a low morbidity and mortality rate. While complications such as hematoma and recurrent laryngeal nerve injury are well recognized, tracheal perforation is a rare but life-threatening event. We discuss the risk factors associated with tracheal perforation during thyroidectomy and review current best practices for its management. This case report describes a 55-year-old woman who experienced a rare but critical complication following total thyroidectomy: tracheal perforation leading to bilateral pneumothorax and mediastinal

From Qi Flow to Air Leak: Bilateral Apical Pneumothoraces After Acupuncture.

Acupuncture is a popular form of traditional Chinese medicine used for management of pain, stress, and other chronic conditions. In recent years, it has been increasingly incorporated as an adjunctive therapy in Western medicine for acute and chronic pain control. While it is usually safe when performed by trained practitioners, complications including bleeding, infection, and injury to surrounding structures near acupoints can occur. We herein report a case of a patient who developed bilateral small apical pneumothoraces following acupuncture at thoracic acupoints, which was managed conservat

Spontaneous Pneumothorax Precipitated by a Forceful Sneeze in a Young Male: A Case Report.

Primary spontaneous pneumothorax (PSP) typically affects young, tall, slender males and is often associated with subpleural bleb rupture. While many cases occur at rest, sudden changes in intrathoracic pressure can serve as triggers. A 22-year-old male smoker (BMI: 21.2 kg/m²) presented with acute pleuritic chest pain and dyspnea immediately following a forceful sneeze. Clinical examination and radiography confirmed a massive left-sided pneumothorax. An emergent tube thoracostomy was performed, resulting in immediate lung re-expansion and symptomatic relief. This case highlights the importance
