Postoperative pulmonary complications after esophagectomy: risk factors and prediction model.

BACKGROUND: Postoperative pulmonary complications (PPCs) affect up to one-third of patients undergoing esophagectomy and remain a major contributor to postoperative morbidity. This study aimed to identify pre- and perioperative risk factors for PPCs and to develop a predictive model. METHODS: This retrospective cohort study included patients who underwent esophagectomy for esophageal or gastroesophageal junction cancer at Amsterdam UMC between 2013 and 2023. PPCs included pneumonia, pleural effusion, pneumothorax, atelectasis, respiratory failure, aspiration, acute respiratory distress syndro

Pleural Infection as the First Presentation of a Large Retroperitoneal Abscess.

Empyema originating from a retroperitoneal source is rare due to the anatomical separation between the retroperitoneal and pleural spaces. We report the case of an 81-year-old man who presented with pleuritic chest pain following empirical antibiotic treatment for a urinary tract infection. Imaging revealed a large retroperitoneal gas-containing fluid collection and a left-sided loculated pleural effusion with a small pneumothorax. Pleural and retroperitoneal collection fluid both grew Escherichia coli suggestive of trans-diaphragmatic spread. A large hiatal hernia, structural defects of the h

Beyond the Usual Course: A Rare Case of Persistent Traumatic Pulmonary Pneumatoceles With 8-Year Favorable Evolution.

BACKGROUND Traumatic pulmonary pneumatoceles (TPP) are rare sequelae of chest trauma, most commonly observed in children due to their compliant chest wall. While typically self-limiting and resolving within weeks to months, large or long-term persistent lesions can pose diagnostic and therapeutic challenges. CASE REPORT We present the case of a 1-year-old boy who sustained severe head and blunt thoracic trauma following a high-impact motor vehicle accident. Chest computed tomography revealed 2 large right-sided pulmonary pneumatoceles, measuring up to 52 mm, associated with pulmonary contusion

Novel Colorimetric Capnography for Confirmation of Thoracostomy Placement in a Porcine Model of Pneumothorax.

INTRODUCTION: Tension pneumothorax is a rapidly fatal but pre ventable cause of death. Needle thoracostomy remains the first-line intervention, yet misplaced catheters fail to relieve pressure and may prolong hypoxia, resulting in cardiovascular compromise and potentially causing injury. This study evaluated the utility of a novel colorimetric capnography device (Capnospot™) designed to provide visual confirmation of pleural entry during decompression. METHODS: Two swine models of tension pneumothorax were created using transdiaphragmatic CO2 insufflation. After each induction, either needle

Subclavian approach without ultrasound guidance is safe and effective for pediatric central venous access.

BACKGROUND: The American College of Surgeons (ACS) recommends US guided internal jugular (IJ) approach for central venous access devices (CVADs); however, this is suboptimal in children. We use the subclavian without US guidance for access in children and adolescents and young adults (AYAs) at a high-volume oncology center. STUDY DESIGN: We undertook a prospective observational study of patients ≤ 26 years undergoing CVAD placement between 1/1/15-9/30/23. US guidance was used in IJ but not in subclavian CVADs, and all procedures were performed by or under direct attending supervision followin

Extensive Spontaneous Pneumomediastinum With Suspected Recurrence in a Young Woman: A Case of Hamman Syndrome.

Hamman syndrome, also known as spontaneous pneumomediastinum (SPM), is a rare and often under-recognized condition characterized by the presence of free air within the mediastinum in the absence of trauma or iatrogenic causes, most commonly affecting young adults after events that increase intrathoracic pressure. We report a case of SPM in a previously healthy 19‑year‑old woman presenting with neck and chest pain after throat discomfort and repeated vomiting. Imaging confirmed extensive pneumomediastinum, and perforation was excluded. She was managed conservatively with complete recovery. This

Rare pulmonary lymphangioleiomyomatosis complicated with tuberculosis: a Case Report and literature review.

BACKGROUND: Pulmonary lymphangioleiomyomatosis (PLAM) is a rare progressive interstitial lung disease characterized by diffuse cystic lesions predominantly affecting women of childbearing age. Its clinical manifestations are diverse and nonspecific. Cases of PLAM complicated with pulmonary tuberculosis (TB) are extremely rare. Here, we retrospectively report one case of PLAM with concurrent TB and review the relevant literature to summarize its clinical, imaging, pathological, diagnostic, therapeutic, and prognostic features, with the aim of providing references for the pathological diagnosis

Severe pneumonia in children with influenza A-Staphylococcus aureus coinfection: case report.

Severe community-acquired pneumonia caused by coinfection with influenza A virus and Staphylococcus aureus (S. aureus) is rare but may lead to rapid clinical deterioration and high mortality. We report two pediatric cases of severe pneumonia associated with influenza A virus-S. aureus coinfection. Both patients initially presented with nonspecific influenza-like symptoms, followed by rapid progression to acute respiratory failure. One case was characterized by necrotizing pneumonia complicated by pneumothorax and empyema, with S. aureus isolated from respiratory specimens and pleural fluid. Th

Extrapleural Saline Hydrodissection Before CT-Guided Lung Biopsy to Reduce Pneumothorax: A Prospective Comparative Study in Pleura-Adjacent Lesions.

BACKGROUND: To evaluate whether extrapleural saline hydrodissection (EPSH) performed before CT-guided percutaneous transthoracic needle biopsy (PTNB) reduces pneumothorax in pleura-adjacent lung lesions. METHODS: In this prospective study, 90 patients with pulmonary lesions ≤1.5 cm from the pleura underwent CT-guided PTNB and were assigned to standard PTNB (Group 1, n = 45) or PTNB with EPSH (Group 2, n = 45). Demographic, lesion-related, procedural variables and complications were compared. Multivariable logistic regression evaluated the effect of EPSH on pneumothorax. In Group 2, lesions we

Severe Progression of Scoliosis Beyond Skeletal Maturity in Partial Trisomy 13: A Case Report Expanding the Phenotypic Spectrum.

Partial trisomy 13 (Patau syndrome) is a rare chromosomal disorder with variable survival and a wide spectrum of congenital anomalies. While skeletal abnormalities have been described, scoliosis remains infrequently reported, particularly in adolescents with partial trisomy 13 who survive beyond early childhood. We report the case of an 18-year-old male with partial trisomy 13 due to an unbalanced 13;21 translocation who developed progressive thoracolumbar scoliosis during adolescence. The patient was diagnosed with scoliosis at age 13, measuring 33°, and was initially managed with Boston brac

Feasibility of Wedge Resection for Subpleural Hilar Lung Nodules Using CT-guided Hookwire Localization and Ultrasonic Dissection.

OBJECTIVE: To evaluate the safety and efficiency of a novel computed tomography (CT)-guided hookwire localization technique, aiming to enable parenchyma-sparing wedge resection using video-assisted thoracoscopic surgery (VATS) for small, hilar-proximal, ground-glass opacity (GGO)-dominant nodules. SUMMARY BACKGROUND DATA: Localizing hilar-proximal GGOs remains challenging. For small, hilar-proximal, GGO-dominant lung nodules, there is no consensus regarding the optimal surgical approach. METHODS: This was a multicenter retrospective study from January 2023 to July 2025 including 79 patients

Two-Year Outcomes in Children Born With a Congenital Pulmonary Malformation.

BACKGROUND: Families and clinicians need more information about complications associated with congenital pulmonary malformations (CPMs) to engage in shared decision-making about surgery for asymptomatic children. The study's aim was to prospectively describe symptoms in children with a CPM and to identify markers associated with surgical removal because of symptoms. METHODS: MALFPULM is a prospective population-based nationally representative cohort including pregnant women whose fetus had a CPM. Events occurring between birth and 2 years of age were compared between children who were not ope

Safe and Simple Costal Cartilage Harvest.

Most rhinoplasty patients have insufficient reconstructive cartilage, which is often required for structural considerations like tip support, reconstructing internal or external nasal valves, or for dorsal augmentation or camouflage. When there is insufficient septal cartilage, the surgeon must turn to other cartilaginous sources including ear cartilage, autologous rib cartilage, or cadaveric rib cartilage. While adding rib cartilage harvest, surgeons often hesitate with autologous rib cartilage as the case may be prolonged or possible increased morbidity due to the risk of a pneumothorax. How

Safety of flexible bronchoscopy in elderly patients.

BACKGROUND: Flexible bronchoscopy is an indispensable tool in respiratory medicine. In the context of an aging society and increasing life expectancy, the number of elderly, often multimorbid patients is growing. This raises important questions regarding the safety of flexible bronchoscopy in this population. METHODS: In this retrospective study 1841 flexible bronchoscopies performed at two sites of Charité Universitätsmedizin Berlin in the years 2022 and 2023 were assessed and classified into two age groups: patients ≥ 70 years (elderly group) and patients < 70 years (non-elderly group). The effect of suction on chest tube progression in children with parapneumonic effusions.

BACKGROUND: Suction is frequently used on chest tubes draining parapneumonic effusions, but its impact on clinical progression remains uncertain. While suction may expedite drainage, it might also delay healing by perpetuating leaks or pneumothorax. This study used Markov modeling to assess the effect of suction on transitions between clinically meaningful states. METHODS: With REB approval, we reviewed the charts of 81 children who had 95 chest tubes placed for parapneumonic effusions between 2014 and 2024. Necrotizing pneumonia patients were excluded. Six clinical states were categorized as

Metastatic High-Grade Endometrial Stromal Sarcoma Presenting With Recurrent Bilateral Pneumothoraces.

Endometrial stromal sarcoma is a rare malignancy that can be difficult to diagnose due to its nonspecific presentation. Unfortunately, the presenting symptoms of abnormal uterine bleeding (AUB), anemia, and pelvic pain overlap with symptoms of benign uterine fibroids, which can delay diagnosis. Additionally, initial evaluations with imaging or even a negative endometrial biopsy may provide false reassurance. Due to novelty, mimicry of benign uterine processes, and high false-negative endometrial biopsy rates, this malignancy is often diagnosed at advanced stages. In this case vignette, we desc
