Complications of Temporary Transvenous Cardiac Pacing by Access Site: A Systematic Review and Meta-Analysis.

INTRODUCTION: The femoral and internal jugular veins are the most commonly used access routes for TTVPM placement. Femoral access is associated with higher rates of infection and bleeding due to groin proximity, whereas jugular access may increase the risk of vascular injury or pneumothorax when performed without imaging guidance. Despite numerous observational studies comparing these access sites, the evidence remains inconclusive due to heterogeneous methodologies, small sample sizes, and varying definitions of complications. To address these limitations, we conducted a meta-analysis to comp

Reliability, validity, responsiveness, and minimal important difference of the St. George's Respiratory Questionnaire in patients with idiopathic pleuroparenchymal fibroelastosis.

BACKGROUND: Idiopathic pleuroparenchymal fibroelastosis (iPPFE) is a rare, progressive interstitial lung disease with a poor prognosis and limited treatment options. No validated instruments exist for assessing health status in patients with iPPFE. We aimed to evaluate the performance of the St. George's Respiratory Questionnaire (SGRQ) in this population. METHODS: We retrospectively analyzed data from 56 consecutive patients with iPPFE who were evaluated at Tosei General Hospital between January 2008 and May 2017. We examined internal consistency, test-retest reliability, construct validity,

Incidence, timing, and risk factors of severe complications after percutaneous ultrasound-guided tumor biopsy: a proposal for safe aftercare based on 30 years´ experience.

BACKGROUND: Percutaneous ultrasound (US)-guided core-needle biopsies (CNBs) is increasingly used to obtain tumor tissue across all anatomic regions to guide personalized cancer treatment; however, data with particular attention on serious adverse events (SAEs) and appropriate observation times to detect clinically-relevant SAEs are limited. We aimed to assess the incidence, influencing factors, and temporal manifestation of SAEs after US-guided CNB of tumors in the thorax and abdomen to identify patient safety measures with a focus on observation time that will allow safe post-biopsy patient d

Evaluation of the Emergency Care Preparedness of Frontline Junior Doctors: A Training Needs Assessment in Ghana.

Introduction In low- and middle-income countries (LMICs) like Ghana, junior doctors - house officers and medical officers - serve as the primary frontline providers of emergency care, often in resource-limited settings. Despite their critical role, emergency medicine (EM) training in undergraduate and housemanship curricula remains non-standardized. This study conducted a bottom-up training needs assessment to identify clinical and procedural gaps among junior doctors in Ghana. Methods A cross-sectional digital survey was conducted among 75 junior doctors (house officers and medical officers w

Cryobiopsy vs Forceps for Bronchoscopic Lung Biopsy: The FROSTBITE-2 Randomized Clinical Trial.

IMPORTANCE: Bronchoscopic biopsy is conventionally performed with forceps, which can result in small specimen sizes and poor specimen quality due to crush artifact. Cryoprobe use localizes freezing at the probe tip, enabling retrieval of larger, more intact biopsy specimens. OBJECTIVE: To evaluate the diagnostic yield of a 1.1-mm cryoprobe for transbronchial biopsy. DESIGN, SETTING, AND PARTICIPANTS: This open-label, outcome assessor-masked, multicenter randomized clinical trial included 500 patients aged 18 years or older scheduled to undergo transbronchial biopsy for lung nodules or masses

Left Ventricular and Lung Perforation Caused by a Pacemaker Lead Requiring Emergency Surgical Repair in a Nonagenarian.

Cardiac perforation is an uncommon but potentially life-threatening complication of permanent pacemaker implantation. Delayed perforation involving extracardiac structures is rare; left ventricular free-wall perforation with pulmonary extension is exceptionally uncommon and may be diagnostically challenging, especially when pericardial effusion is absent. A 91-year-old man residing on a Greek island developed delayed perforation of the presumed interventricular septal/paraseptal course with exit through the left ventricular free wall and extension toward the left lung after dual-chamber pacema

Unexpected Air on Trauma Computed Tomography: Iatrogenic Intravascular Air After Peripheral Venous Cannulation.

Small amounts of intravascular air are occasionally detected on computed tomography (CT), most commonly after intravenous contrast administration. However, intravascular air may also be introduced during routine peripheral venous access before contrast injection. In the trauma setting, this finding may be misinterpreted as traumatic mediastinal or thoracic pathology, particularly in elderly patients. We report the case of an 89-year-old man who presented after a ground-level fall with mild head trauma. A 20-gauge peripheral intravenous catheter was inserted into the left median cubital vein be

Cyst-predominant nonfibrotic hypersensitivity pneumonitis mimicking Birt-Hogg-Dubé syndrome.

Hypersensitivity pneumonitis (HP) is classified into fibrotic and nonfibrotic phenotypes. Rarely, nonfibrotic HP presents radiologically with multiple cysts. Histologically, nonfibrotic HP is characterized by cellular interstitial pneumonia, cellular bronchiolitis, and poorly formed non-necrotizing granulomas; however, the histopathological features and mechanisms underlying cyst formation remain poorly understood. We report a rare case of cyst-predominant nonfibrotic HP. A 42-year-old woman underwent pulmonary surgery for enigmatic lung cysts complicated by spontaneous pneumothorax. Histologi

Upper lung asymmetry identifies distinct clinical phenotypes in idiopathic pleuroparenchymal fibroelastosis.

BACKGROUND: Idiopathic pleuroparenchymal fibroelastosis (IPPFE) is a progressive interstitial lung disease characterized by upper lobe-predominant elastofibrosis. While disease asymmetry has been described in other interstitial lung diseases, its clinical significance in IPPFE remains unclear. METHODS: We evaluated 102 patients with IPPFE, 56 with idiopathic pulmonary fibrosis, and 50 healthy controls. Lung volumes were quantified using computed tomography to calculate the upper-lung asymmetry ratio (upper AR), defined as (combined volume of the right upper and middle lobe - left upper lobe v

Rapidly Progressive Subcutaneous Emphysema and Sequential Bilateral Pneumothoraces Following Pediatric Tracheocutaneous Fistula Closure: A Postoperative Air-Leak Syndrome Recognized in the Post-anesthesia Care Unit (PACU).

Tracheocutaneous fistula (TCF) closure is commonly performed in pediatric patients following successful decannulation from long-term tracheostomy dependence. Although the procedure is generally considered safe, postoperative air-leak complications, including subcutaneous emphysema, pneumomediastinum, and pneumothorax, have been reported. Early recognition is critical because progression may lead to significant respiratory compromise. We report the case of a three-year-old child who developed rapidly progressive subcutaneous emphysema in the post-anesthesia care unit (PACU) following TCF closur

Shape-sensing robotic-assisted versus electromagnetic navigation bronchoscopy for peripheral pulmonary lesions: a single-operator experience.

BACKGROUND: Electromagnetic navigation bronchoscopy (ENB) and shape sensing robotic assisted bronchoscopy (ssRAB) are used to sample peripheral pulmonary lesions (PPLs). Comparative data between ENB and ssRAB, particularly in the absence of advanced three-dimensional imaging, remain limited. We compared the performance of ENB and ssRAB performed by a single interventional pulmonologist at a large urban hospital. METHODS: We performed a single-center retrospective analysis of ENB procedures performed between December 2020 and June 2023 and ssRAB procedures performed between September 2023 and

Staple line coverage with suture fixation of polyglycolic acid patch for primary spontaneous pneumothorax.

BACKGROUND: Video-assisted thoracoscopic surgery is the standard surgical treatment for primary spontaneous pneumothorax (PSP). Various attempts, including absorbable mesh and fibrin glue, have been made to reduce surgical complications such as postoperative prolonged air leaks and recurrence. This retrospective study evaluates whether suturing a polyglycolic acid (PGA) patch to a surgical staple line can reduce postoperative air leakage. METHODS: This study included 135 patients under the age of 40 years who underwent thoracoscopic PSP surgery between September 2013 and February 2023. A PGA

Early hypoxemia detection with minimal FiO₂: Putting routine hyperoxygenation in anesthesia practice to the test.

BACKGROUND: High inspired oxygen fractions (FiO₂) are routinely administered during mechanical ventilation in anesthesia in order to prevent hypoxemia. However, liberal oxygen use often exceeds physiological requirements, increasing the risk of oxygen toxicity and absorption atelectasis. Moreover, by sustaining normal-appearing oxygen saturation, high FiO₂ may obscure the presence of pulmonary congestion, edema, acute respiratory distress syndrome (ARDS), inadvertent endobronchial intubation, pneumothorax, and other conditions associated with impaired gas exchange. This approach may postpone t

TSC gene-associated lesions of the lung.

Pathogenetic alterations of tumor suppressor genes TSC1 and TSC2 are responsible for the development of tuberous sclerosis complex (TSC) and various sporadic diseases, including sporadic lymphangioleiomyomatosis (LAM). TSC can be inherited and non-inherited and is characterized by indolent tumor formation in multiple organs. The most important pulmonary manifestation of TSC is LAM. It affects the majority of female and a small subset of male patients, heavily impacting their quality of life and survival. It is a cystic lung disease, which is now considered a PEComa-type neoplasm. In addition t

Does the use of costochondral graft in paediatric temporomandibular joint ankylosis result in satisfactory outcomes? A systematic review of the literature.

BACKGROUND AND PURPOSE: Paediatric temporomandibular joint ankylosis (TMJA) is a debilitating condition that impairs mandibular function and disrupts normal craniofacial growth. Costochondral graft (CCG) reconstruction has been widely used due to its biological compatibility and presumed growth potential; however, its long-term behaviour remains controversial. Purpose was to systematically evaluate the functional outcomes and growth potential of costochondral graft reconstruction in paediatric TMJA. METHODS: A systematic review was conducted in accordance with PRISMA guidelines. Electronic da

Recurrent Pneumothorax Complicated by Pleuroparenchymal Fibroelastosis Successfully Treated With Pleurodesis Using 50% Glucose: A Case Report.

Pleuroparenchymal fibroelastosis (PPFE), a rare interstitial lung disease primarily affecting the upper lung lobes, leads to pleural thickening and subpleural fibroelastosis. Patients with PPFE are often prone to recurrent pneumothorax because their lung tissues are fragile and do not expand properly. Here, we report a case of recurrent pneumothorax associated with PPFE that was successfully treated with pleurodesis using a 50% glucose solution. A 66-year-old non-smoking woman presented with a right pneumothorax and underwent thoracoscopic bullectomy after imaging revealed a pulmonary bulla an
