Comparative Evaluation of Supraclavicular Versus Infraclavicular Approach With Regard to Right Subclavian Vein Catheterisation by Blind Approach.

Background In settings where real-time ultrasonography (USG) is unavailable or delayed, landmark-guided central venous access remains essential. Anatomically, the right supraclavicular (SC) route offers a more direct course to the superior vena cava than the infraclavicular (IC) approach, which could shorten procedural time without increasing risk. Methods We conducted a prospective, single-centre, parallel-group randomised controlled trial in adult elective cardiac surgery patients (n=70; 35 per arm) comparing landmark-guided right subclavian venous catheterisation via SC versus IC approaches

Life-threatening haemorrhage due to rupture of pulmonary sequestration arterial supply.

We present a case of a young adult patient with haemoptysis and back pain following 2 days of worsening cough. CT imaging revealed an intralobar pulmonary sequestration of the left lower lobe with an anomalous arterial supply from the thoracic aorta and a large associated haemothorax. The patient developed haemodynamic instability and was taken emergently to the operating room for left thoracotomy. Intraoperatively, active haemorrhage from the aberrant systemic vessel was identified, controlled and ligated. A left lower lobectomy was performed, and the patient made an excellent recovery.Pulmon

Pleural malpositioning of a haemodialysis catheter causing haemothorax despite the use of ultrasound guided placement and chest radiograph confirmation.

We report an unusual case of haemodialysis catheter malposition into the pleural space causing haemothorax, despite ultrasound-guided insertion and X-ray confirmation. A patient with chronic kidney disease presented in respiratory failure due to pulmonary oedema and was scheduled for urgent haemodialysis. A haemodialysis catheter was inserted under ultrasound guidance, with apparent blood backflow, and postinsertion chest radiograph suggested proper placement. However, dialysis was interrupted multiple times due to persistently inadequate extracorporeal blood flow and visibly diluted blood in

Ruptured pulmonary AV malformation presenting as spontaneous haemothorax in pregnancy.

We report a rare and potentially life-threatening case that emerged when a young postpartum woman presented with a ruptured pulmonary arteriovenous malformation (PAVM) resulting in spontaneous haemothorax. The patient, 3 days postpartum following a stillbirth at 36 weeks of gestation, arrived at the emergency department with worsening respiratory symptoms and right-sided chest pain for 5 days. The patient was haemodynamically stable with mild respiratory distress. The chest radiograph showed a moderate to massive right pleural effusion. An intercostal chest drain was inserted, and haemorrhagic

Spontaneous haemothorax due to a non-invasive mediastinal thymoma.

A male patient in his late 60s presented with sudden-onset chest pain, dry cough and exertional dyspnoea. He had a history of renal cell carcinoma treated with nephrectomy 10 years ago, and a biopsy-proven benign anterior mediastinal mass, thymoma, diagnosed 7 years earlier, for which he denied surgical resection and was lost to follow-up. Imaging revealed a large anterior mediastinal mass with associated left-sided moderate pleural effusion. Image-guided biopsy of the mass confirmed it to be type A thymoma. Thoracoscopy showed normal pleura with haemothorax. Positron emission tomography-CT sc

Microwave technology for detecting traumatic chest injuries in a porcine model.

The electromagnetic properties of microwaves are altered by the dielectric properties of biological tissues, allowing microwave technology (MWT) to be used to detect free air and blood in the body. This animal study investigated whether MWT could be used as a point-of-care approach for the diagnosis of pneumothorax (PTX), haemothorax (HTX) and haemopneumothorax (HPTX). A porcine model of PTX (50–1500 mL), HTX (50–750 mL) and HPTX (750 mL blood and 50–1500 mL air) was established, by injecting air and blood in the pleural cavity. A MWT device with eight antennas was positioned around the chest,

When suicide mimics homicide: a forensic analysis of three cases involving multiple sharp-force injuries.

Suicide by multiple sharp-force injuries represents an uncommon and diagnostically complex occurrence in forensic pathology. The presence of numerous stab or incised wounds can mimic homicidal patterns, especially when typical self-inflicted indicators are absent or unclear. Differentiating between suicide and homicide in such cases requires a multidisciplinary assessment involving autopsy findings, scene investigation, and psychological history. We report three rare cases of suicide involving multiple sharp-force injuries. Case 1: A 44-year-old man with a history of depression and alcohol abu

Initial experience of intrathoracic rib fixation using RibFix Advantage™ at a major trauma centre: operative technique and case series.

Multiple rib fractures are the most frequently encountered traumatic chest injury. Randomised control trials have shown superiority of surgical stabilisation of rib fractures (SSRF) for reduction of displaced fractures in comparison to non-operative management. As SSRF techniques continue to evolve, new technologies have enabled less invasive approaches, including intrathoracic rib fixation, which can achieve chest wall stability while allowing for a less invasive approach. This article reports our initial experience with intrathoracic SSRF (I-SSRF), our operative technique, case series descri

Trends in Mesothelioma Mortality in the United States Between 1999 and 2020.

INTRODUCTION: Mesothelioma is a rare but aggressive cancer primarily caused by asbestos exposure. In March 2024, the Environmental Protection Agency banned asbestos in the United States, but its use will take years to phase out. Therefore, asbestos remains a threat, and incidence may remain stable or slowly decrease due to the long latency between exposure and diagnosis. This study investigates mesothelioma mortality trends in the United States from 1999 to 2020, focusing on demographic and geographic variations. METHODS: Data on mesothelioma-related deaths from 1999 to 2020 were extracted fr

Chylothorax After Thoracic Surgery: How We Manage It.

Chylothorax is a rare but insidious condition, characterized by the accumulation of chyle in the pleural space, which is particularly common after cardiothoracic surgeries. It presents significant challenges in both diagnosis and treatment. In this technical report, we present our experience in managing four cases of postsurgical chylothorax, each one treated with a different approach. The first and second cases were successfully managed with Lipiodol lymphangiography, which allowed for the visualization and occlusion of the injured lymphatic duct, leading to the resolution of the chylothorax.

A review of how colors clue us into gross diagnosis in domestic animals.

Different tissues have a normal color spectrum that reflects their cellular composition and/or metabolic features. Similarly, distinct color variations may occur in tissues that have undergone pathologic or nonpathologic changes. Common examples of color changes in domestic animal tissues include red (associated with erythrocytes, hemoglobin, and myoglobin), brown (ferric hemoglobin or myoglobin, suppurative inflammation, lipid oxidation, postmortem autolysis, formalin fixation, neoplasms arising from cytochrome-rich tissues), yellow (hemoglobin and iron degradation, biliary pigment and by-pro

Beyond the Milky Pleural Fluid: A 14-Year Study on Non-Traumatic Chylothorax From a Tertiary Care Hospital in India.

Chylothorax is an uncommon type of pleural effusion caused by the accumulation of chyle in the pleural space. It represents 2% to 3% of pleural effusions and can be classified into two categories: traumatic and non-traumatic. Chylothorax may carry high morbidity and mortality, especially when associated with trauma or malignancies. We present five cases of non-traumatic chylothorax, confirmed by pleural fluid analysis, with different etiologies seen at the Metro Center for Respiratory Diseases, Noida, from 2010 to 2024. These cases represent the wide spectrum of conditions associated with this

Real-Time Identification of Lymph Vessels Using Indocyanine Green in a Patient With Chylothorax Associated With Lymphangioleiomyomatosis.

INTRODUCTION: Lymphangioleiomyomatosis (LAM) is often complicated by chylothorax and may require surgical intervention; however, the treatment is complicated because of difficulties in identifying the location of the fistula intraoperatively. This is the first report to identify the site of a chyle fistula associated with LAM in real time during surgery by using indocyanine green (ICG) lymphangiography. MATERIALS AND SURGICAL TECHNIQUE: A 56-year-old woman received a diagnosis of a treatment-resistant left chylothorax associated with LAM. To identify the chyle fistula during surgery, 1 mL of

A systematic review of lymphovenous anastomosis to treat nontraumatic chylous anasarca.

BACKGROUND: Anomalies in chyle flow, obstructive, functional or malformative, are responsible for leakage of chyle into the peritoneal, pericardial, or pleural spaces. Although rare, chronic chylous effusions can have disastrous consequences. Current treatments do not provide permanent and satisfactory solutions. Lymphovenous anastomosis can be a suitable treatment in this context; however, it needs to be fully evaluated regarding its efficacy, indications, and means of achievements. The objective of the study is to assess the efficacy of lymphovenous anastomosis to treat nontraumatic chylous

Chylothorax.

The diagnosis and management of chylothorax present significant challenges due to the diverse clinical entities that can result in a chylous effusion. A thorough understanding of the underlying disease processes is essential for effective treatment. Although various treatment algorithms have been proposed, the limited availability of high-quality trials precludes a definitive recommendation of any single approach. This article aims to review the current literature on the evaluation and management of chylothorax.

Primary Idiopathic Chylopericardium in an Adult Female.

Chylopericardium (CP), defined as the accumulation of chyle in the pericardial cavity, is a rare condition, especially in the absence of an identifiable secondary cause. Primary idiopathic chylopericardium (PIC) is even more uncommon, with limited cases reported in the literature. We report the case of a 43-year-old South Asian woman who presented with palpitations and fatigue. Echocardiography revealed a large pericardial effusion with signs of cardiac tamponade, necessitating emergent pericardiocentesis. The pericardial fluid aspirated was pinkish and turbid, which turned to milky white afte
