Surgical management of feline idiopathic chylothorax remains unpredictable, with reported success rates varying widely despite combined procedures. We report a case of refractory idiopathic chylothorax in a cat that was successfully managed through a strategic, sequential surgical approach guided by indocyanine green (ICG) fluorescence. Initially, the cat underwent thoracoscopic thoracic duct occlusion (TDO) via clipping, but chylous effusion persisted. A 2nd surgery was conducted to achieve definitive thoracic duct ligation using sutures combined with additional clipping, yet the effusion rem

BACKGROUND: Although a largely safe procedure, a variety of complications may arise from pacemaker implantation. One rare complication involves chylous collections forming at the pacemaker pocket.
CASE SUMMARY: A 58-year-old woman with myotonic dystrophy and a dual-chamber pacemaker presented with chronic swelling over her pacemaker site. Imaging revealed a large fluid collection. Needle aspiration yielded 600 mL of milky, odorless fluid with a triglyceride level of 403 mg/dL, consistent with a chylous effusion. Lymphangiography and lymphoscintigraphy suggested impaired thoracic duct drainage

Postoperative chylothorax after lung resection and mediastinal lymph node dissection is a rare complication that may be difficult to manage when conservative treatment is ineffective, and the thoracic duct or leak point cannot be identified intraoperatively. Chylothorax is thought to result from thoracic duct injury associated with mediastinal lymph node dissection. Although surgical thoracic duct ligation is recommended in refractory cases, identification of the leakage site or thoracic duct itself is often challenging, particularly in patients with intrathoracic adhesions or in facilities wh

BACKGROUND: Chylothorax, a common complication post-lung cancer surgery, lacks standardized laboratory diagnostic criteria. Misdiagnosis remains prevalent due to variable pleural fluid appearance.
METHODS: We analyzed pleural fluid via biochemical tests, lipid profiling, and Sudan III staining. Ratios of effusion-to-serum triglycerides (P/S TG > 1) and cholesterol (P/S CHOL < 1) were applied to confirm chylous effusion.
RESULTS: The patient's pleural fluid exhibited milky yellow appearance, elevated TG (5.46 mmol/L), P/S TG ratio (3.62), and positive Sudan III staining. P/S CHOL ratio (0.43)

OBJECTIVES: In this study, we investigated the clinical features, etiology, and also predictive factors of secondary erythema nodosum (EN) in patients with EN.
METHODS: A total of 100 patients (mean age: 37 years) diagnosed with EN between 1993 and 2004 in our clinic were included in the study prospectively. A skin biopsy was performed in 46 of the patients. Patients were considered to have secondary EN when an underlying condition was found, and to have primary EN when no such condition was found. For the diagnosis of the underlying diseases, the pertinent diagnostic criteria and/or diagnost

BACKGROUND: When systemic lupus erythematosus (SLE) affects the lymphatic system, it disrupts the normal physiological function of the lymphatic circulation, leading to clinical chylous effusion and lymphedema. Currently, the computed tomography lymphangiography (CTL) or technetium-99m dextran (99Tcm-DX) lymphoscintigraphy features of patients with SLE complicated by chylous effusion and lymphedema have not been published, and studies on the imaging features of different subtypes are lacking. This study aimed to analyze the imaging characteristics of CTL and 99Tcm-DX lymphoscintigraphy in 23 p

Primary effusion lymphoma (PE) is a rare form of non-Hodgkin lymphoma characterized by malignant lymphatic effusion in the pleural cavity. Non-traumatic chylothorax is usually caused by lymphomas with a discrete lymph node mass. Our report presents a case where chylothorax and chylous ascites were clinical presentation of primary effusion lymphoma (PEL), a rare entity that does not show the typical presence of an identifiable malignant mass. Managing PEL complicated by chylothorax is generally more challenging and has a poor prognosis, as illustrated by a patient in this case who died weeks af

Kaposi sarcoma (KS) is an HHV-8-mediated vascular malignancy encompassing four distinct clinical variants: classic, endemic, iatrogenic, and AIDS-associated. While classically indolent, KS can pursue an aggressive course in the context of HIV infection. Pleural involvement, particularly chylous effusion, represents a rare but grave manifestation of the disease. In cases of chylous effusion, pleural fluid analysis typically reveals a triglyceride concentration >110 mg/dL or the presence of chylomicrons, findings that signify true chylous involvement. We report the case of a 33-year-old man with

Chylopericardium is an extremely rare disorder in dogs, and its underlying mechanisms and optimal management strategies remain poorly defined. This report describes a dog with chylopericardium complicated by concurrent chylothorax, bilateral brachiocephalic vein thrombosis, and mesothelioma. A 6-year-old neutered male Shiba Inu presented with respiratory distress and was diagnosed with chylothorax and chylopericardium based on fluid analysis. Computed tomography revealed multiple thoracic ducts and bilateral brachiocephalic vein thrombosis but no definitive site of lymphatic leakage. Surgical

Lymphangioleiomyomatosis (LAM) is a rare cystic lung disease that primarily affects women of reproductive age. The disease may involve extrapulmonary sites, including the lymphatic system, and can present with a lymphocyte-predominant pleural effusion mimicking tuberculosis. We report a 35-year-old woman who presented with a three-year history of progressive dyspnea and recurrent lymphocyte-predominant exudative pleural effusions. She was empirically started on anti-TB therapy at a local hospital but showed no clinical improvement. Upon evaluation at a tertiary center, there was decreased air

Chylothorax is an uncommon but clinically relevant complication following thoracic aortic surgery. Although conservative management is usually the initial approach, persistent high-output chylous effusion may require surgical intervention, for which accurate identification of the leakage site is important. We describe two cases of refractory postoperative chylothorax following aortic surgery, in which surgical management was guided by intranodal lipiodol lymphangiography combined with computed tomography (CT). The first patient developed chylothorax after descending aortic replacement via left

The objective of this study, which is part of a retrospective case series, was to report on single-session, minimally invasive thoracic duct ligation (TDL), cisterna chyli ablation (CCA), and partial pericardiectomy (PPE) for dogs with chylothorax. A total of 9 client-owned dogs with chylothorax participated in the study. A right-sided or bilateral thoracoscopic and left-flank laparoscopic approach was carried out in sternal recumbency for TDL and CCA, respectively. Subsequent partial pericardiectomy was done thoracoscopically via a paraxiphoidal approach. Combined (TDL, CCA, and PPE), minimal

We report a case of chylopericardium with cardiac tamponade after lung cancer surgery. A 47-yearold female underwent right lower lobectomy with mediastinal lymph node dissection and postoperative adjuvant therapy for lung cancer with mediastinal lymph node metastases. Three months postoperatively, chylothorax developed, and pleurodesis was performed. One month after receiving treatment for chylothorax, the patient returned to the hospital with complaints of chest tightness and dyspnea on exertion. The patient developed hypotension, and echocardiography revealed a massive pericardial effusion c

Chylothorax is a rare but insidious condition, characterized by the accumulation of chyle in the pleural space, which is particularly common after cardiothoracic surgeries. It presents significant challenges in both diagnosis and treatment. In this technical report, we present our experience in managing four cases of postsurgical chylothorax, each one treated with a different approach. The first and second cases were successfully managed with Lipiodol lymphangiography, which allowed for the visualization and occlusion of the injured lymphatic duct, leading to the resolution of the chylothorax.

Different tissues have a normal color spectrum that reflects their cellular composition and/or metabolic features. Similarly, distinct color variations may occur in tissues that have undergone pathologic or nonpathologic changes. Common examples of color changes in domestic animal tissues include red (associated with erythrocytes, hemoglobin, and myoglobin), brown (ferric hemoglobin or myoglobin, suppurative inflammation, lipid oxidation, postmortem autolysis, formalin fixation, neoplasms arising from cytochrome-rich tissues), yellow (hemoglobin and iron degradation, biliary pigment and by-pro

Chylothorax is an uncommon type of pleural effusion caused by the accumulation of chyle in the pleural space. It represents 2% to 3% of pleural effusions and can be classified into two categories: traumatic and non-traumatic. Chylothorax may carry high morbidity and mortality, especially when associated with trauma or malignancies. We present five cases of non-traumatic chylothorax, confirmed by pleural fluid analysis, with different etiologies seen at the Metro Center for Respiratory Diseases, Noida, from 2010 to 2024. These cases represent the wide spectrum of conditions associated with this

INTRODUCTION: Lymphangioleiomyomatosis (LAM) is often complicated by chylothorax and may require surgical intervention; however, the treatment is complicated because of difficulties in identifying the location of the fistula intraoperatively. This is the first report to identify the site of a chyle fistula associated with LAM in real time during surgery by using indocyanine green (ICG) lymphangiography.
MATERIALS AND SURGICAL TECHNIQUE: A 56-year-old woman received a diagnosis of a treatment-resistant left chylothorax associated with LAM. To identify the chyle fistula during surgery, 1 mL of

BACKGROUND: Lymphangioleiomyomatosis (LAM) is a rare neoplastic disorder characterized predominantly by dyspnea, recurrent pneumothorax, chylous effusion and diffuse pulmonary cystic changes in women. Vascular endothelial growth factor-D (VEGF-D) is an important biomarker for LAM.
RESULTS: This study cohort comprised 631 LAM patients and investigated the correlations between serum VEGF-D levels and clinical manifestations of LAM. The median serum level of VEGF-D was 1452pg/ml (820.0-2659pg/ml) among the study population. Patients with highly-elevated VEGF-D levels exhibited younger age, lower
