Ruptured pulmonary AV malformation presenting as spontaneous haemothorax in pregnancy.

We report a rare and potentially life-threatening case that emerged when a young postpartum woman presented with a ruptured pulmonary arteriovenous malformation (PAVM) resulting in spontaneous haemothorax. The patient, 3 days postpartum following a stillbirth at 36 weeks of gestation, arrived at the emergency department with worsening respiratory symptoms and right-sided chest pain for 5 days. The patient was haemodynamically stable with mild respiratory distress. The chest radiograph showed a moderate to massive right pleural effusion. An intercostal chest drain was inserted, and haemorrhagic

Spontaneous haemothorax due to a non-invasive mediastinal thymoma.

A male patient in his late 60s presented with sudden-onset chest pain, dry cough and exertional dyspnoea. He had a history of renal cell carcinoma treated with nephrectomy 10 years ago, and a biopsy-proven benign anterior mediastinal mass, thymoma, diagnosed 7 years earlier, for which he denied surgical resection and was lost to follow-up. Imaging revealed a large anterior mediastinal mass with associated left-sided moderate pleural effusion. Image-guided biopsy of the mass confirmed it to be type A thymoma. Thoracoscopy showed normal pleura with haemothorax. Positron emission tomography-CT sc

Microwave technology for detecting traumatic chest injuries in a porcine model.

The electromagnetic properties of microwaves are altered by the dielectric properties of biological tissues, allowing microwave technology (MWT) to be used to detect free air and blood in the body. This animal study investigated whether MWT could be used as a point-of-care approach for the diagnosis of pneumothorax (PTX), haemothorax (HTX) and haemopneumothorax (HPTX). A porcine model of PTX (50–1500 mL), HTX (50–750 mL) and HPTX (750 mL blood and 50–1500 mL air) was established, by injecting air and blood in the pleural cavity. A MWT device with eight antennas was positioned around the chest,

When suicide mimics homicide: a forensic analysis of three cases involving multiple sharp-force injuries.

Suicide by multiple sharp-force injuries represents an uncommon and diagnostically complex occurrence in forensic pathology. The presence of numerous stab or incised wounds can mimic homicidal patterns, especially when typical self-inflicted indicators are absent or unclear. Differentiating between suicide and homicide in such cases requires a multidisciplinary assessment involving autopsy findings, scene investigation, and psychological history. We report three rare cases of suicide involving multiple sharp-force injuries. Case 1: A 44-year-old man with a history of depression and alcohol abu

What is the relationship between the Number of Significant rib fractures and Haemothorax in adults with blunt chest trauma?

BACKGROUND: The study aimed to determine the correlation between the number of significant rib fractures in patients with blunt chest trauma (BCT) and the amount of haemothorax as recorded by Closed Thoracostomy Tube Drainage (CTTD). METHODOLOGY: This was a cross-sectional study of all patients with significant rib fractures following BCT over a period of two years. "Significant rib fractures" was defined as the fracture of 3 or more ribs with 50% or more displacement of each fractured rib edge. Patients with massive haemothorax, patients with severe neurological, or patients who required eme

Initial experience of intrathoracic rib fixation using RibFix Advantage™ at a major trauma centre: operative technique and case series.

Multiple rib fractures are the most frequently encountered traumatic chest injury. Randomised control trials have shown superiority of surgical stabilisation of rib fractures (SSRF) for reduction of displaced fractures in comparison to non-operative management. As SSRF techniques continue to evolve, new technologies have enabled less invasive approaches, including intrathoracic rib fixation, which can achieve chest wall stability while allowing for a less invasive approach. This article reports our initial experience with intrathoracic SSRF (I-SSRF), our operative technique, case series descri

A Massive Haemothorax Due to Pleural Extramedullary Haematopoiesis in a Patient With Myelodysplastic Syndrome/Myeloproliferative Neoplasm.

Extramedullary haematopoiesis (EMH) refers to blood cell production outside the bone marrow, typically in reticuloendothelial organs such as spleen, liver, and lymph nodes, as a compensatory response to haematopoietic stress. We describe a rare case of recurrent haemothorax caused by pleural EMH in a female patient in her 60s with myelodysplastic/myeloproliferative neoplasm (MDS/MPN) not otherwise specified (NOS). She had recurrent presentations with transfusion-dependent anaemia, fevers, splenomegaly, and bone marrow morphology consistent with MDS/MPN. Over the course of her disease, she deve

Manual versus Markerless (Image-guided System) Toric Intraocular Lens Implantation Outcomes for Astigmatic Correction in Cataract Surgeries.

The cerebellum is a potent anti‑epileptic target for deep brain stimulation in patients with drug‑resistant epilepsy. The effects of such stimulation, however, may also favor seizure activity. Our goal was to investigate the effect of cerebellar electrical stimulation (ES) alone and in combination with the anti‑epileptic drug diazepam (DIA) on seizure outcome. We used a rat model of pentylenetetrazol kindling, which is characterized by seizures followed by deteriorations in central benzodiazepine‑GABAA (BDZ‑GABAA) receptors. We tested the effects of ES alone and in combination with DIA (0.1 an

Noninvasive 3D-CT simulation versus glue injection to localize small pulmonary nodules prior to anatomical segmentectomy: a randomized controlled trial.

OBJECTIVES: This study aimed to investigate whether adding glue injection to three-dimensional computed tomography bronchography and angiography (3D-CTBA) has extra benefits to facilitate anatomical segmentectomy for pulmonary nodules. METHODS: We conducted a randomized controlled trial. The patients undergoing thoracoscopic segmentectomy assisted with 3D-CTBA simulation were enrolled. Then, they were divided into the 3D-CTBA group and the glue-labelling group who received additional computed tomography-guided percutaneous glue (2-octyl cyanoacrylate) injection to label the nodules. The prima

Management of hepatic hydropericardium with open drainage, maximal medical therapy and terlipressin.

We present the case of a woman in her 60s with Child-Pugh C cirrhosis who developed pericardial tamponade during an admission for a haemothorax secondary to a mechanical fall. The patient developed haemodynamic compromise with a rapid decline in renal function. During an open subxiphoid drain tube insertion, a pre-existing peritoneopericardial communication was noted, with ascites in the peritoneal cavity on view. The serum ascites albumin gradient was 14 g/L. Maximal medical therapy was commenced including diuresis and albumin, with adjunctive terlipressin infusion which restored her baseline

Positive Outcomes of Physiotherapy Intervention in a Wedge Compression Fracture of the L1 Vertebra: A Case Report.

The thoracolumbar spine is prone to vertebral compression fractures (VCFs). An injury mechanism known as flexion compression is responsible for thoracolumbar spine compression fractures. Usually, this mechanism affects the longitudinal ligament at the front and the front part of the vertebral body as the first components. Pain is the first and foremost symptom; here we present a case report of a 34-year-old male, who came to the hospital with complaints of back pain, and difficulty in breathing followed by a road traffic accident (RTA). MRI and X-ray investigations were done. The patient was d

Right ventricular lead perforation with iatrogenic injury to an intercostal artery causing haemothorax after pacemaker implant.

A woman in her 80s experienced a life-threatening complication of pacemaker implant consisting of subacute right ventricular lead perforation causing iatrogenic injury to an intercostal artery, resulting in a large haemothorax. A CT scan confirmed active bleeding from the fourth intercostal artery. The patient underwent cardiothoracic surgery via a median sternotomy approach, during which the source of the bleeding was sealed, a new epicardial lead was positioned, and the original lead was extracted. This case emphasises the potentially severe consequences of pacemaker lead perforation and sec

Management of contained penetrating cardiac injury in a patient with prior cardiac surgery.

Penetrating cardiac injuries usually require emergent surgical intervention. Our patient presented to the trauma centre with multiple stab wounds to the neck, chest, epigastric region and abdomen. She arrived haemodynamically stable, and her initial Focused Assessment with Sonography for Trauma exam was negative. Her chest X-ray did not show any evident pneumothorax or haemothorax. Due to her injury pattern, she was taken to the operating room for exploratory laparotomy and neck exploration. Postoperatively, she was taken for CT and found to have a contained cardiac rupture. The injury was con

Performance of thoracic ultrasonography compared with chest radiography for the detection of rib fractures using computed tomography as a reference standard.

OBJECTIVE: Although there is growing evidence that ultrasonography is superior to X-ray for rib fractures' detection, X-ray is still indicated as the most appropriate method. This has partially been attributed to a lack of studies using an appropriate reference modality. We aimed to compare the diagnostic accuracy of ultrasonography and X-ray in the detection of rib fractures, considering CT as the reference standard. MATERIALS AND METHODS: Within a 2.5-year period, all consecutive patients with clinically suspected rib fracture(s) following blunt chest trauma and available posteroanterior/an

From diagnosis to therapy: the acute traumatic hemothorax - an orientation for young surgeons.

INTRODUCTION: This review aims to provide an overview of diagnosing and managing traumatic haemothorax for young surgeons. CONTENT: Of 27,333 polytrauma patients in Germany in 2021, 35 % were admitted with thoracic trauma. In polytrauma patients, chest injuries are an independent negative predictor of 30-day mortality. These patients should be treated in an evidence-based and standardized manner to reduce mortality and morbidity. There are several methods of immediate diagnosis that should be used depending on hemodynamic stability. In addition to physical examination and chest X-ray, more sp

Complications of ultrasound guided very small-bore chest drains for pleural effusions of different etiology.

BACKGROUND: Small-bore chest drains are now the most common drains for treating pleural effusion (PE), but knowledge on complications is limited especially in malignant PE and empyema. We aimed to evaluate rate of complications of ultrasound guided small bore chest drains [6-10 French (F)] by PE etiology. METHODS: Retrospective cohort study of 484 chest drains inserted in 330 adults in a Swedish department 2018-2020. Rate of complications (blockage, dislocation, infection, or misplacement) and repeat intervention (new drain within 2 weeks or surgery) was analyzed by effusion type (organ failu

Urban thoracic trauma: diagnosis and initial treatment of non-cardiac injuries in adults.

This comprehensive review aims to delineate the prevailing non-cardiac thoracic injuries occurring in urban environments following initial on-site treatment and subsequent admission to hospital emergency departments. Our study involved a rigorous search within the PubMed database, employing key phrases and their combinations, including "thoracic injury," "thoracic trauma," "haemothorax," "lung contusion," "traumatic pneumothorax," "rib fractures," and "flail chest." We focused on original research articles and reviews. Non-cardiac thoracic injuries exhibit a high prevalence, often affecting po

Role of low-pressure negative pleural suction in patients with thoracic trauma - a randomized controlled trial.

BACKGROUND: Thoracic trauma frequently includes a pneumothorax, hemothorax, or hemopneumothorax, which may necessitate an Intercostal drainage (ICD) for air and fluid evacuation to improve breathing and circulatory function. It is a simple and life-saving procedure; nevertheless, it carries morbidity, even after its removal. Efforts have been made continuously to shorten the duration of ICD, but mostly in non-trauma patients. In this study, we evaluated the impact of negative pleural suction over the duration of ICD. METHODS: This study was a prospective randomized controlled interventional t

Clinical application of CT-assisted body surface localization combined with intraoperative stereotactic anatomical localization in thoracoscopic lung nodule resection: a single-centre retrospective study.

BACKGROUND: Today, the detection rate of lung nodules is increasing. Some of these nodules may become malignant. Thus, timely resection of potentially malignant nodules is essential. However, Identifying the location of nonsurface or soft-textured nodules during surgery is challenging. Various localization techniques have been developed to accurately identify lung nodules. Common methods include preoperative CT-guided percutaneous placement of hook wires and microcoils. Nonetheless, these procedures may cause complications such as pneumothorax and haemothorax. Other methods regarding localizat
