Cardiac surgery with extracorporeal circulation in 23 infants weighing 2500 g or less: short and intermediate term outcome.

OBJECTIVE AND METHODS: From September 1990 to February 1997, 23 consecutive critically ill infants (12 males, 11 females) weighing 2500 g or less underwent cardiac surgery necessitating extracorporeal circulation (ECC). A retrospective study was carried out to evaluate short- and intermediate-term outcome. Mean weight at operation was 2265 g (range 1750-2500 g). Mean age at operation was 24 days. The indications for surgery were transposition of the great arteries (TGA; 7), ventricular septal defect (VSD; 4), aortic stenosis (AS; 3), univentricular heart (UVH; 2), tetralogy of Fallot (TOF; 2),

[Functional results and long-term outcome after bilateral lung transplantation for pulmonary hypertension].

Pulmonary hypertension (PH) signifies elevated blood pressure in the pulmonary circulation either due to clearly defined causes (cardiac, pulmonary-parenchymatous, systemic) or of idiopathic origin (primary PH, PPH). While conservative treatment is beneficial only for a small number of patients, lung transplantation represents a curative measure. The optimal form of transplantation [i.e. single lung (SLTX), bilateral lung (BLTX) or combined heart-lung transplantation (HLTX)] is still under discussion. This study is a retrospective analysis of 16 patients with different forms of PH who underwen

[Acute respiratory insufficiency after inhaling concrete dust--a case report].

UNLABELLED: Inhalation of inorganic, inert dusts, like concrete dust, has generally not been considered dangerous. Very rarely alterations following chronic exposures can be observed, such as airflow obstruction and increased mucous secretion. Acute reactions in terms of acute respiratory failure have not been described so far. CASE REPORT: The present case report introduces a 54-year old male patient who developed acute respiratory failure after sawing a concrete block for several hours without wearing a face mask. Save for a chronic obstructive pulmonary disease he was unremarkable for his

[Re-expansion pulmonary edema after excision of an intrathoracic tumor].

We report an acute respiratory insufficiency following the removal of a large intrathoracic fibroma (3.1 kg) in a 6 year-old child, caused by a re-expansion pulmonary oedema (unilateral oedema occurring within one hour after expansion). This oedema improved rapidly and was followed by a well-tolerated pleural effusion. This complication is due to discrepancy between a small lung and a large thoracic cavity, due to the prolonged time course of the tumor growth. These oedemas are caused by rapid lung re-expansion, the volume of the removed tumor and the depth of postoperative pleural suction. Th

Clinical presentation and outcome of tuberculosis in kidney, liver, and heart transplant recipients in Spain. Spanish Transplantation Infection Study Group, GESITRA.

BACKGROUND: Tuberculosis is unusual in transplant recipients. The incidence, clinical manifestations, and optimal treatment of this disease in this population has not been adequately defined. The present study was undertaken to assess the incidence, clinical features, and response to therapy of Mycobacterium tuberculosis infection in solid-organ transplant recipients. METHODS: We evaluated retrospectively the incidence, clinical characteristics, diagnostic procedures, antituberculous treatment, clinical course, and factors influencing mortality in 51 solid-organ transplant recipients who deve

[Acute eosinophilic pneumonia in a pregnant woman].

Acute eosinophilic pneumonia is a recently described pulmonary phenomenon involving rapidly progressing respiratory insufficiency. Although it can appear at any age, it has never been reported during pregnancy and its impact on gestation is therefore unknown. We describe the clinical signs and course of disease in this first report of acute eosinophilic pneumonia in a pregnant woman. We emphasize the diagnostic utility of bronchoalveolar lavage, the resolution of symptoms without corticoid treatment and, mainly, the absence of adverse repercussions of the disease on pregnancy.

[Pulmonary transplantations. Current assessment].

After fifteen years of experience, 3-year survival rates after lung transplantation have reached 60% for heart-lung, two-lung and single-lung procedures. At 7 years, the rate is currently 41%. Lung transplantation has thus become the ultimate treatment for end-stage respiratory failure, and, as we were able to establish in 1988, single lung transplantation is now the indication of choice. Most of the early complications after transplantation, including edema and post-operative shock, though relatively frequent, can generally be controlled. There is however the problem of acute rejection during

Minimally invasive direct coronary artery bypass grafting: two-year clinical experience.

BACKGROUND: Interest in minimally invasive coronary artery bypass grafting has been increasing. METHODS: From April 1994 through December 1996, 199 patients (age, 36 to 93 years) underwent minimally invasive coronary artery bypass grafting through minithoracotomy, subxiphoid, and lateral thoracotomy incisions, with internal mammary artery, gastroepiploic artery, and composite grafts placed using local coronary artery occlusion. RESULTS: The conversion rate to sternotomy was 7% (14/199). Preoperative risk factors included unstable angina (n = 83), reoperative coronary artery bypass grafting (

Lung retransplantation: institutional report on a series of twenty patients.

BACKGROUND AND METHODS: Between 1986 and 1995, 124 isolated lung and 29 combined heart-lung transplantations were performed at our institution. Twenty of these procedures were retransplantations. Four different types of reoperations were performed: ipsilateral single lung retransplantation (n = 3), single lung retransplantation after bilateral or heart-lung transplantation (n = 7), bilateral retransplantation after bilateral lung transplantation (n = 5), and bilateral retransplantation after single lung transplantation (n = 5). Nine patients underwent retransplantation while still in the inten

Acute respiratory failure due to tracheobronchial involvement in Crohn's disease.

We present a case of tracheobronchial involvement in Crohn's disease, associated with respiratory failure leading to mechanical ventilation. Bronchoscopy revealed small, diffuse, whitish granulations and erythematous mucosa. Histopathology disclosed ulcerative bronchitis and noncaseating tuberculoid granuloma. The patient improved under a 6 week oral corticosteroid treatment followed by inhaled corticotherapy, without relapse of pulmonary symptoms.

Single lung transplantation for pulmonary lymphangiomyomatosis. Unexpected need for extracorporeal circulation.

The present case describes an acute respiratory-related hemodynamic failure during a single left lung transplantation in a 32-year-old woman suffering from end-stage pulmonary lymphangiomyomatosis. During the first 5 min of single right lung ventilation, a progressive increase in airway pressure and decrease in tidal volume associated with a decrease in arterial pressure and Spo2 occurred that were successfully countered by reventilation of the left lung. Proper positioning of the double-lumen tube was confirmed with a fiberoptic bronchoscope. Despite deliberate hypoventilation, within a few r

[Acute mitral valve insufficiency].

This type of disease has benefited considerably, over recent decades, from progress in ultrasound technique and new methods of medical and surgical treatment. The aetiologies of mitral incompetence can be classified into 4 categories according to their mechanism: 1. Mutilating valve lesions, usually secondary to bacterial endocarditis, but sometimes secondary to trauma (percutaneous valvuloplasty). 2. Rupture of chordae tendineae, either spontaneous or bacterial, in a context of pre-existing valvular heart disease, usually degenerative. 3. Papillary muscle lesions, usually corresponding to rup

Aortic root replacement with a pulmonary autograft.

A series of 42 consecutive patients with exclusively aortic root replacement using the pulmonary autograft is presented. The mean age at operation was 19.3 years (range 0.3-41.4). Two patients died in hospital (4.8%; 70% CL: 0.0-8.2). This mortality was not related to the autograft procedure. The mean follow-up time was 30 months (range 3-70; SD: 20). Late mortality consisted of two patients; in one of these severe autograft failure occurred due to chronic juvenile rheumatoid arthritis. The estimated survival rate at 4 years was 88.8% (70% CL: 83.3-94.5). Morbidity involved three patients. One

[Concept for therapy of heart failure in heart surgery].

Perioperative deterioration of the circulatory performance of patients undergoing heart surgery ranges from transitory impairment in cardiac output by deterioration of the compensation range of the oxygen transport system to manifest circulatory failure without previous myocardial damage and the acute decompensation of pre-existing chronic heart failure. On the basis of the current state of knowledge in this field, a concept for rational staged treatment should be based on the different myocardial beta-adrenoceptor conditions related to the type and stage of the individual underlying heart dis

Aortic valve replacement with cryopreserved pulmonary allograft.

Experimental as well as clinical data support the finding of using the pulmonary valve as a systemic semilunar valve. From January 1990 to December 1992, 89 patients received pulmonary cryopreserved allografts as aortic valve replacement. Sixteen subjects suffering from native or prosthetic valve endocarditis were included. Age ranged from 10 to 74 years. Sixty-five patients received isolated aortic valve replacement and 24 patients required additional surgical measures, such as coronary artery bypass graft, mitral valve reconstruction, replacement of the ascending aorta, supravalvular patch a

[Heart and lung transplantation in acute organ failure].

Heart and lung transplantation represent a realistic therapeutical option in endstage cardiac and pulmonary diseases. Transplantation for acute organ failure, however, is limited due to the scarcity of suitable donors with respect to the shortage of time. Aside from pharmacological means a number of mechanical assist devices are available for bridging of those patients in acute myocardial or respiratory failure. Indications for acute organ transplantation are primary graft failure following heart or lung transplantation and acute myocardial insufficiency due to extensive myocardial infarction

[Lung and heart-lung transplantation: indications, complications and prognosis].

Lung- and heart-lung transplantation has become a serious therapeutic option for terminally ill patients with chronic pulmonary failure. Due to the still rather high risk of acute rejection, severe pulmonary or systemic infection, and the relatively demanding postoperative care, this treatment can only be considered for patients where inspite exhausted conservative options for therapy quality of life and prognosis are not acceptable. Not only due to lack of donors single-lung transplantation has developed to be the favorite treatment for patients with interstitial lung disease (idiopathic, sar

Postoperative acute respiratory failure following thoracotomy in a patient with Charcot-Marie-Tooth disease.

We present the case of a patient with Charcot-Marie-Tooth (CMT) disease who required prolonged ventilatory support following a thoracotomy due to respiratory muscle weakness. Although CMT was traditionally believed to affect only skeletal muscles, recent evidence suggests that respiratory involvement is relatively common, even in the absence of pulmonary symptoms. Assessment of respiratory muscle strength using measurements of vital capacity and negative inspiratory force is helpful in evaluating pulmonary reserve in patients with CMT.
