Impact of chronic kidney disease on postoperative outcomes following craniotomy for aneurysmal subarachnoid hemorrhage.

PURPOSE: Aneurysmal subarachnoid hemorrhage is an urgent manner in which blood accumulates in the subarachnoid region. Presence of chronic kidney disease (CKD) is often a predictor of adverse outcomes perioperatively. Patients with CKD may have different perioperative outcomes during surgery for aneurysmal subarachnoid hemorrhage compared to patients without CKD, and we sought to retrospectively examine the effect of CKD on perioperative outcomes in aSAH patients receiving surgery. METHODS: Patients undergoing craniotomy for aneurysmal subarachnoid hemorrhage were analyzed using the ACS-NSQIP

Transcatheter Pulmonary Valve Replacement With a Novel Valve: Medium-Term Results From a Single Center Study.

BACKGROUND: Transcatheter pulmonary valve replacement (tPVR) has emerged as a promising management strategy for dysfunctional right ventricular outflow tract (RVOT). However, the management of chronic pulmonary regurgitation (PR) in native or patched RVOT remains challenging with the vast variation in morphology, leading to the demand for novel self-expandable valves. OBJECTIVE: To report procedural performance and medium-term follow-up data of the Med Zenith PT-Valve for tPVR in native or patched RVOT from a single-center feasibility trial. METHODS: Fifteen patients with PR were recruited f

Ultrasound Images That Speak: Assessing the Therapeutic Decision in the Emergency Department Regarding the Risk-Benefit Ratio of Systemic Thrombolysis in Intermediate-High-Risk Pulmonary Embolism-A Case Report.

Background: The management of acute pulmonary embolism (PE) in the Emergency Department (ED) remains challenging, particularly in hemodynamically and respiratory stable patients with minimal symptoms. Diagnostic and therapeutic difficulties are further compounded when the condition is complicated by a mobile right atrial (RA) thrombus, representing an extreme-risk phenotype. Case Presentation: We report the case of a 65-year-old male with a single known venous thromboembolism risk factor-chronic venous insufficiency-who presented to the ED following a transient episode of severe dyspnea at hom

Caecal volvulus: A rare cause of bowel obstruction.

INTRODUCTION: Caecal volvulus is an uncommon but significant cause of intestinal obstruction.1,2 Due to its nonspecific clinical presentation, diagnosis can be challenging, and delays may result in bowel ischemia or perforation. Early recognition and prompt surgical intervention are crucial to achieving favorable outcomes. CASE PRESENTATION: A 66-year-old male with a history of sacral chordoma treated by sacrectomy and radiotherapy presented with abdominal distension, pain, and signs of intestinal obstruction. His medical background included multiple abdominal surgeries, chronic renal insuffi

[The 513th case: acute respiratory failure after bilateral lung transplantation].

A 67-year-old male had undergone bilateral lung transplantation for chronic obstructive pulmonary disease 11 months before the current presentation. He was admitted with a 5-day history of cough with sputum, and a 2-day history of fever. Computed tomography (CT) of the chest revealed rapidly progressive bilateral diffuse "ground glass" opacities. Despite anti-infective therapy and methylprednisolone pulse therapy, his condition deteriorated, necessitating endotracheal intubation with mechanical ventilation and veno-venous extracorporeal membrane oxygenation (V-V ECMO) for life support. Combined strategy of transcatheter edge-to-edge repair of mitral valve and percutaneous mechanical circulatory support device to treat high-risk patients with severe mitral regurgitation and cardiogenic shock.

BACKGROUND: Acute mitral regurgitation (MR) associated with cardiogenic shock (CS) is a medical emergency with excessive in-hospital mortality. The vast majority of these patients are turned down for corrective surgery. The combined strategy of transcatheter edge-to-edge repair of mitral valves and percutaneous mechanical circulatory support devices to treat high-risk patients with severe mitral regurgitation and cardiogenic shock has not been evaluated in randomized controlled trials. AIMS: In this multicenter registry we aimed to assess the outcomes (all-cause mortality, other important sec

Prevalence and prognostic role of atrial fibrillation in chronic severe aortic regurgitation.

BACKGROUND: Atrial fibrillation (AF) impacts management decisions in valvular heart disease but its significance in patients with chronic aortic regurgitation (AR) is not well established. OBJECTIVES: To evaluate the prevalence and prognostic role of AF in patients with chronic, haemodynamically significant AR. METHODS: Adults with chronic ≥moderate-to-severe AR, from March 2004 to April 2019, were retrospectively identified. Those with acute AR, moderate or worse other valve disease or prior valve surgery were excluded. Patients with AF were identified based on 12-lead ECGs, 1-lead ECGs (sh

Ventriculo-arterial coupling in pulmonary regurgitation following transannular patch repair of pulmonary stenosis.

Pulmonary regurgitation is a common consequence following the repair of tetralogy of Fallot and can lead to heart failure. Early detection of right ventricular dysfunction remains challenging, and current clinical markers have limited predictive value to identify which patients are at risk for heart failure and require interventions. This study aimed to investigate the potential of ventriculo-arterial coupling as a marker of early right ventricular dysfunction in a porcine model of chronic pulmonary regurgitation following transannular patch repair of neonatal pulmonary stenosis. Neonatal swin

Mid-Term Outcomes of Valve-Sparing Repair With Intraoperative Balloon Dilation in Tetralogy of Fallot.

BackgroundLate sequelae associated with chronic severe pulmonary regurgitation (PR) following transannular patch (TAP) repair of tetralogy of Fallot (TOF) have driven the use of alternative approaches. This study investigates mid-term pulmonic valve (PV) durability in TOF patients who underwent valve-sparing repair with intraoperative balloon dilation (VS-IBD).MethodsBetween 2010 and 2022, 139 TOF patients underwent VS-IBD (n = 66) or TAP (n = 73) repair at a single institution. Patients who underwent VS repair without IBD were excluded. Baseline differences were balanced by inverse probabilit

CERAS: covered endovascular reconstruction of the aorta and side branches. Description of the technique of revascularizing more than two visceral branches in patients with a coral reef aorta.

Coral reef aorta represents a distinct vascular pathology characterized by large calcified exophytic plaques that grow into the aortic lumen typically at the level of the visceral and renal arteries. In most cases, these arteries are also heavily calcified and stenosed or occluded. Symptoms vary depending on the affected artery and differ from renovascular hypertension, and renal insufficiency to cardiac decompensation and chronic limb threatening ischemia. This report describes two patients with a coral reef aorta and stenosed aortic side branches. Both patients had experienced episodes of pu

Prevalence of comorbidities and their impact on prognosis among patients with rifampicin-resistant tuberculosis: a multicenter retrospective cohort study in China.

Comorbidities are a significant factor affecting the prognosis of patients with rifampicin-resistant tuberculosis (RR-TB). We evaluated the prevalence of comorbidities and their prognostic effects in a Chinese RR-TB cohort. In this study, we reviewed the clinical data of RR-TB patients who started anti-tuberculosis treatment in China between May 2018 and April 2020 by conducting a multicenter cohort study. A log-binomial regression model was used to analyze the relationship between comorbidities and the treatment outcomes of RR-TB. The burden of comorbidities among RR-TB patients in China was

Our 10-Year Experience with Atrial Myxomas: Is Concurrent Valve Intervention Really Warranted?

INTRODUCTION: Primary cardiac myxomas are rare tumors. Concurrent valvular lesion is a common finding on evaluation which is thought to be due to annular dilatation secondary to tumor movement across the valve, functional obstruction across the valve, and severe pulmonary hypertension secondary to chronic obstruction. A common belief among surgeons is that excision of myxoma leads to abatement of symptoms, and further valve intervention may not be warranted. METHODS: A 10-year retrospective descriptive study was designed to analyze patients who underwent excision of cardiac myxoma at our cent

The Impact of Medical Comorbidities on Cough Improvement Following Superior Laryngeal Block.

INTRODUCTION: Chronic refractory cough is defined as cough lasting greater than 8 weeks and with an unclear etiology. Blockade of the internal branch superior laryngeal nerve (iSLN) has been shown to be safe and effective in the treatment of chronic cough. It remains unknown, however, if underlying comorbidities impact patient response to iSLN blockade. METHODS: A total of 44 patients aged 18 years and older were seen at our institution's Laryngology clinics between 2019 and 2022 and treated with iSLN blockade. Patient demographics, comorbidities, and pre- and post-treatment cough severity in
