Clinical characteristics, risk factors, and prognostic modeling for poor outcomes in children with influenza-associated encephalopathy: A retrospective cohort study.

Influenza-associated encephalopathy (IAE) in children is a rare but severe complication associated with high morbidity and mortality. Timely identification of high-risk cases remains challenging due to variable clinical presentations and limited pediatric-specific evidence. This retrospective cohort study included 198 children diagnosed with IAE between 2015 and 2022 in a pediatric intensive care unit. Patients were categorized by in-hospital prognosis, defined as either poor (including death, coma, or neurological sequelae at discharge) or favorable outcome. Clinical and laboratory parameters

Case Report: A rare pediatric case of hepatitis B virus infection with acute disseminated encephalomyelitis/Guillain-Barré syndrome spectrum diseases.

A 12-year-old girl with convulsions was incidentally found to be infected with hepatitis B virus(HBV), with HBVdeoxyribonucleic acid (DNA) and HBsAg positivity in the cerebrospinal fluid. The patient experienced frequent convulsions and consciousness disorders, and positive anti-GQ1b/anti-GD1b antibodies were detected in the cerebrospinal fluid. Acute disseminated encephalomyelitis(ADEM)/Guillain-Barré syndrome (GBS) spectrum disorders were observed on brain and spinal cord magnetic resonance imaging, indicating a high likelihood of HBV-related ADEM+/GBS spectrum disorders. After treatment wit

T1w/T2w Ratio Identifies the Basolateral Amygdala as a Preferential Target in Autoimmune Limbic Encephalitis.

BACKGROUND AND PURPOSE: The amygdala plays a key role in the pathophysiology of autoimmune limbic encephalitis (ALE), contributing to epileptic seizures and neuropsychiatric symptoms. While no study has examined microstructural changes in individual amygdala nuclei in ALE, we used the T1-weighted/T2-weighted (T1w/T2w) ratio to explore amygdalar pathology and its associations with clinical manifestations, including epilepsy and neuropsychiatric symptoms. METHODS: This single-center study examined 57 patients diagnosed with ALE and 16 healthy controls (HC). Patients underwent a comprehensive as

Mimics and Diagnostic Pitfalls of Anti-Adenylate Kinase 5 Limbic Encephalitis.

BACKGROUND: The clinical understanding of limbic encephalitis associated with antibodies against adenylate kinase 5 (AK5) remains limited. Misinterpretation of antibody test results may lead to diagnostic errors and inappropriate management. We aim to assess the frequency of anti-AK5 encephalitis overdiagnosis and identify common diagnostic pitfalls. METHODS: Cases of confirmed and mimicking anti-AK5 limbic encephalitis from January 2021 to July 2024 using established criteria for autoimmune encephalitis (AE) were reviewed. AK5 mimics were defined as patients initially suspected of AE with a

Type I IFN autoantibodies underlie chikungunya live-attenuated vaccine encephalitis.

Human autoantibodies neutralizing type I interferons (IFNs) have emerged as strong, common, and global determinants of a growing number of severe viral diseases, including hypoxemic viral pneumonia, arboviral encephalitis, and adverse reaction to the live-attenuated yellow fever virus (YFV) vaccine. Chikungunya virus (CHIKV) is a growing global health concern that the live-attenuated vaccine VLA1553 (IXCHIQ®) was developed to address. In 2025, five unrelated adults (aged 82 to 88) on the island of La Réunion (France) developed severe reactions postvaccination; two died. The three patients with

Comparative analysis of PRV-induced neuropathology and blood-brain barrier disruption identifies C57BL/6 as the most susceptible mouse model.

Pseudorabies virus (PRV), an emerging zoonotic alphaherpesvirus, can cause severe central nervous system (CNS) injury in non-natural hosts. Although mice are widely used for PRV neuropathogenesis studies, strain-dependent differences remain incompletely characterized. In this study, we established both intranasal and intramuscular PRV infection models in three commonly used mouse strains, KM, BALB/c, and C57BL/6, and performed comparative evaluations at early fixed post-infection timepoints as well as at moribund stages. After intranasal infection, C57BL/6 mice exhibited the highest absolute L

Gut metabolite indole-3-acetic acid aggravates neuropsychiatric lupus via the AHR/STAT3 pathway in microglia.

Neuropsychiatric systemic lupus erythematosus (NPSLE) is a severe complication of systemic lupus erythematosus (SLE). Despite its high morbidity, the exact pathogenesis of NPSLE remains poorly understood, and effective therapeutic options remain unavailable. Here we show gut bacterium Lactobacillus reuteri (L. reuteri) and its metabolite indole-3-acetic acid (IAA) play key roles in the progression of NPSLE. L. reuteri and IAA induce behavioral deficits, microglial activation, pro-inflammatory cytokine secretion, neuronal loss, and blood brain barrier (BBB) disruption in female lupus-prone mice

Safety and efficacy of adjunct dexamethasone in adults with herpes simplex virus encephalitis in the UK (DexEnceph): a multicentre, observer-blind, randomised, phase 3, controlled trial.

BACKGROUND: Herpes simplex virus (HSV) encephalitis is characterised by inflammation and swelling of the brain, resulting in death or severe neurocognitive sequelae. HSV encephalitis is treated with the antiviral aciclovir but still has substantial mortality and morbidity. Adjunct corticosteroids are sometimes used, but whether they improve the outcome is unclear. We aimed to establish the safety and efficacy of adjunct corticosteroids in HSV encephalitis. METHODS: In this multicentre, observer-blind, randomised, phase 3 trial, adults with HSV encephalitis admitted to 53 hospitals in the UK w

Placental Extracellular Vesicles Exhibit Reduced Neurogenic Potential Linked to Changes in Their miRNA Landscape Upon HCMV Infection.

Extracellular vesicles (EVs) are key mediators of maternal-foetal communication, regulating placental function and foetal development through the transfer of bioactive molecules. Although placental EVs play a crucial role in placental function during pregnancy, their contribution to foetal development, notably foetal brain, remains poorly understood. Human cytomegalovirus (HCMV) is the most common virus transmitted in utero and a leading cause of infectious brain malformations. Although certain central nervous system lesions caused by HCMV are explained, the neuropathogenesis of congenital inf

Case Commentary: Intraventricular polymyxin B-small steps, big questions.

Carbapenem-resistant gram-negative ventriculitis is a life-threatening infection with limited treatment options. H. Jiang, Y. Hu, J. Cai, J. Zhang, et al. (Antimicrob Agents Chemother 70:e00943-25, 2026, https://doi.org/10.1128/aac.00943-25) describe two patients with this condition who were successfully treated with intraventricular polymyxin B. Their report highlights uneven antibiotic distribution within the ventricles, the influence of cerebrospinal fluid drainage on intraventricular drug concentrations, and cure with intraventricular polymyxin B alone. These findings raise important quest

Pathogenesis and Research Models of Acute Influenza-Associated Encephalitis/Encephalopathy: An Update.

Influenza-associated encephalitis/encephalopathy (IAE) is a severe neurological complication characterized by central nervous system dysfunction and structural damage following influenza virus infection. Predominantly affecting infants and young children, IAE exhibits its highest incidence in those under five years of age. Key clinical manifestations of IAE include acute seizures, sudden high fever, and impaired consciousness, frequently progressing to coma. Neuroimaging, particularly magnetic resonance imaging (MRI), often reveals multifocal brain lesions involving multiple brain regions, inc

Risk factors for acute kidney injury associated with intravenous acyclovir in older adults: ACICLOAGED study.

BACKGROUND: Intravenous acyclovir is the recommended treatment for suspected viral meningoencephalitis, but it may induce acute kidney injury. Evidence in very old adults is limited, particularly regarding the role of frailty in renal vulnerability. METHODS: We conducted a retrospective cohort study of adults aged 75 years or more who received intravenous acyclovir for suspected meningoencephalitis at a single university hospital between November 2021 and December 2024. Frailty was assessed using the 9-point Clinical Frailty Scale, with frailty defined as a score of 4 or higher. The primary o

Pseudorabies virus TK protein antagonizes alpha interferon response by interfering with the JAK1-STAT1 interaction.

Pseudorabies virus (PRV), a member of the Alphaherpesvirinae subfamily, is the causative agent of Aujeszky's disease, which severely affects swine health and poses a potential zoonotic risk. PRV can evade the type I interferon (IFN-I)-mediated antiviral response, thus enabling persistent infection, yet the molecular basis for this immune evasion remains unclear. Here, we identify a novel role for thymidine kinase (TK), a key PRV virulence factor, in suppressing IFN-I signaling. Ectopic expression of TK markedly inhibited IFNα-induced transcription and expression of interferon-stimulated genes

A multicentre evaluation of metagenomic sequencing for pathogen detection in central nervous system infections.

OBJECTIVES: Metagenomic next-generation sequencing (mNGS) is a promising tool for diagnosing central nervous system infections. However, the low-biomass nature of cerebrospinal fluid (CSF) increases susceptibility to contamination and host-background interference, potentially compromising accuracy. This study aimed to evaluate CSF mNGS performance across multiple laboratories and to identify key factors influencing detection accuracy. METHODS: A reference panel of 15 CSF samples was designed to evaluate CSF mNGS performance across laboratories, including 3 replicate samples, 5 serial concentr

Distinct mechanisms of ER stress-mediated autophagy induction by high- and low-virulence pseudorabies virus strains.

Pseudorabies virus (PRV) is one of the most significant pathogens threatening the swine industry, and its infection has caused substantial economic losses to pig farming worldwide. This study utilized two PRV strains isolated by our laboratory: the GXLB-2015 strain, a natural recombinant between a PRV variant strain and the Bartha-K61 vaccine strain, which exhibits stronger virulence, and the GXGG-2016 strain, a classical genotype II PRV strain with a natural 69-amino acid deletion in its TK gene, which demonstrates weak virulence and is non-pathogenic in mice. Focusing on endoplasmic reticulu

Inborn errors of autophagy underlying severe viral infections in humans.

Inborn errors of immunity can underlie susceptibility to severe viral infection in humans. and the majority relate to defective induction of or response to antiviral type I interferon (IFN). However there is increasing awareness of defects in other cellular processes, that can predispose to severe infectious disease. Recently, defects in autophagy-related genes or -processes have been demonstrated to predispose to life-threatening viral diseases, including defects in autophagy-related genes in patients with herpes simplex virus and varicella zoster virus infections in the central nervous syste
