Patient-ventilator asynchrony is highly prevalent during invasive mechanical ventilation, yet its detection at the bedside remains limited. Conventional waveform inspection is intermittent, operator-dependent, and insufficient to capture the complexity and temporal variability of patient-ventilator interaction. Automated systems based on advanced signal processing and artificial intelligence represent a paradigm shift, enabling continuous, objective, and scalable detection of asynchrony. Recent approaches highlight the value of entropy-based metrics to quantify the irregularity of ventilatory

BACKGROUND: Malignant central airway obstruction (MCAO) is a common complication of lung cancer. Cross-sectional imaging is the best method of screening for this complication, but prior data indicate that such obstruction is frequently underreported on radiology reports.
METHODS: A cohort with known MCAO was selected and radiology reports were reviewed. Data on scan indication, airway obstruction severity intravenous contrast use were tracked, as were relevant dates. Data on diagnostic and therapeutic bronchoscopy were also tracked.
RESULTS: The frequency of missed MCAO across mild and sever

BACKGROUND: Perry disease is a rare autosomal dominant inherited neurodegenerative disorder caused by cytoskeleton-associated protein glycine-rich (CAP-Gly) domain-related variants in the DCTN1 gene, with characteristic TDP-43 pathology. The typical manifestations are parkinsonism, psychiatric symptoms, weight loss, and central hypoventilation.
OBJECTIVE: The aim of the study was to delineate the genotypic and phenotypic spectrum of Perry disease in a Chinese parkinsonism cohort.
METHODS: We screened the DCTN1 CAP-Gly domain-related variants in 932 Chinese parkinsonism patients using next-ge

RATIONALE: Obesity hypoventilation syndrome (OHS) is treated with noninvasive ventilation (NIV) that is titrated during polysomnography. Auto-adjusted NIV could obviate the need for polysomnographic titration, thereby reducing costs and delays in care. However, non-inferiority long-term clinical trials comparing auto-adjusted NIV with manually-adjusted NIV are lacking.
OBJECTIVES: To determine the comparative effectiveness of automatic vs manual NIV modality in OHS.
METHODS: In this multicenter, blinded, parallel group, non-inferiority and cost-effectiveness trial, we randomly assigned treat

Management of acute Type I respiratory failure (RF) in patients with neuromuscular disorders (NMD) and myopathies poses a clinical challenge in the acute care setting. There is a lack of studies which looked into the effectiveness of High flow nasal cannula (HFNC) in acutely ill NMD patients with Type I RF. This single centre, retrospective case-series study was conducted at the RICU of a tertiary care institute in the state of Rajasthan, India, between 1 January 2020 and 31 March 2025 .All patients with a diagnosis of NMD who were admitted in RICU with Type I respiratory failure were sequenti

BACKGROUND: Respiratory failure (RF) is the leading cause of death in chronic obstructive pulmonary disease (COPD), yet reliable biomarkers for early risk stratification remain unclear. Circulating C-reactive protein (CRP) reflects systemic inflammation, but its prognostic value for incident RF in COPD is controversial.
METHODS: A total of 38 933 patients from the UK Biobank with the ratio of Forced Expiratory Volume in 1 second to Forced Vital Capacity (FEV1/FVC) < 0.70 but without RF at baseline were included, and a maximum of 17.87 years of follow-up was conducted. Participants were divide

BACKGROUND: The superficial cervical plexus block (SCPB) is routinely employed for clavicle surgery due to the dominant innervation of the surgical field by the supraclavicular nerves (SN). However, evidence directly comparing the blocking effect of different SCPB approaches-particularly the novel modified longitudinal intermediate cervical plexus block (MU)-on SN coverage and phrenic nerve safety remains limited. This randomized, double-blind, controlled trial in healthy volunteers aimed to evaluate the efficacy of MU relative to landmark-based and ultrasound-guided intermediate approaches in

AIMS/BACKGROUND: Hypoxemia and respiratory failure are common among critically ill patients, and high-flow nasal cannula (HFNC) therapy has been increasingly utilized as a non-invasive respiratory support modality. However, a subset of patients eventually requires mechanical ventilation (MV), and predicting this transition remains challenging. This study aimed to develop and validate nomograms that predict the risk of MV among patients undergoing HFNC therapy.
METHODS: A retrospective cohort study was conducted using the publicly available Multiparameter Intelligent Monitoring in Intensive Ca

Critically ill, immunocompromised individuals are vulnerable to both common and uncommon etiologies of respiratory infection. Identifying the etiology of infection has therapeutic, prognostic, and public health implications. In some circumstances, immunocompromised patients may be empirically treated with excessively broad antibiotics but ultimately be diagnosed with a common cause of pneumonia. In other circumstances, immunocompromised patients may have a non-specific clinical presentation for severe respiratory failure and be diagnosed with a rare etiology of infection. Because this patient

Buprenorphine has long been recognized as a mu opioid agonist with a distinctive and intricate pharmacological profile. It is a partial agonist at the mu opioid receptor, an antagonist at the kappa and delta opioid receptors, and an agonist at the nociception opioid receptor. Similar to other mu agonists such as morphine and fentanyl, buprenorphine can produce side effects, including tolerance, physical dependence, respiratory depression, and addiction. The mu opioid receptor gene, OPRM1, undergoes extensive alternative splicing, generating an array of splice variants or isoforms, which are co

BACKGROUND: Silicosis is a chronic lung disease caused by inhaling crystalline silica dust that, despite safety advances, remains widespread, especially in developing countries. Respirable silica particles reach the alveoli, triggering inflammation and fibrosis development. Severe cases may progress to respiratory failure, underscoring the importance of identifying risk factors.
METHODS: We retrospectively assessed male patients diagnosed with pulmonary silicosis, with or without respiratory failure. All data were obtained at the time of diagnosis.
RESULTS: Respiratory failure was present in

PURPOSE: Obesity hypoventilation syndrome (OHS) frequently coexists with obstructive sleep apnea (OSA) and is linked to lung function impairment and poor outcomes. However, its underlying pathology remains unclear. This study compares lung function and endotypic traits in obese OSA patients with and without OHS. METHODS: We prospectively collected polysomnographic data from 186 obese patients with OSA who underwent lung function and blood gas tests. Eighteen patients were identified as having OHS, as defined by body mass index ≥ 30 kg/m² with daytime hypercapnia (PaCO₂ ≥ 45 mmHg). Endotypic tr

OBJECTIVES: Acute hypoxemic respiratory failure is a leading cause of death and disability in critically ill children. Pulmonary capillary barrier dysfunction, in large part, drives severity. We aimed to define how blood and tracheobronchial lavage (TBAL) fluid differentially affect pulmonary capillary barrier function and, as a second exploratory aim, identify protein mediators.
DESIGN: We performed a secondary analysis in April and May of 2024 of blood and TBAL samples from a previously completed observational cohort study of mechanically ventilated children collected from October 2018 to F

Diaphragmatic dysfunction, whether unilateral or bilateral, partial or complete, is often an overlooked cause of dyspnea, sleep disturbances, and, in more advanced stages, chronic alveolar hypoventilation. It may result from injury to the phrenic nerve, the diaphragm muscle itself, or central nervous system pathways. The diagnosis is frequently suspected when an elevated hemidiaphragm is incidentally observed on chest imaging. A comprehensive assessment combining clinical examination, pulmonary function tests, arterial blood gas analysis, and targeted imaging is essential to confirm the dysfun
